Sixth Annual Hip Joint Course, Baltimore

The 2011 Hip Joint Course was an extremely successful event with 180 attendees from 19 countries and 25 states in the U.S. The conference brought together the world leaders in hip joint preservation and reconstruction for a combination of didactic presentations, focused discussions, and hands-on cadaveric labs. Our faculty lectured on topics such as new surgical approaches, bearing surfaces, standard and resurfacing hip arthroplasty options, treatment options for femoroacetabular impingement, and avoiding complications. Participants attended two afternoon cadaveric lab sessions that featured expert-led, hands-on training in arthroscopy of the hip, anterior approach with and without navigation, resurfacing and preoperative templating magnification and digital planning.

Limb Reconstruction Fellowship Program, Baltimore

The Limb Reconstruction Fellowship Program of the ICLL provides postgraduate medical education in a unique orthopedic surgical subspecialty that includes the in-depth study, prevention, and treatment of limb length discrepancies, nonunions, malunions, and congenital and posttraumatic limb deformities by medical, physical, and surgical methods. Specifically, the Program is based on but not limited to techniques derived from the science of distraction osteogenesis. Methodologies used in the Program are not limited to any specific device or apparatus but rather encompass the full gamut of orthopedic fixation materials, including plates, intramedullary rods, and monolateral and circular fixators in the study and subsequent treatment and correction of these deformities. Fellowship opportunities are typically granted for periods of 6 months to 2 years.

The Limb Reconstruction Fellowship Program is centered at the Rubin Institute for Advanced Orthopedics at Sinai Hospital of Baltimore. The Institute provides substantial support resources for this Program, including but not limited to outpatient, surgical, inpatient, imaging, laboratory, rehabilitation, and research resources. Operating rooms are provided, with equipment and staff to support the adult and pediatric reconstructive activities of this Fellowship Program. Research activities may take place at the Institute.

Partial knee arthroplasty (PKA) has enjoyed a renewed interest in the last decade. Isolated medial, lateral, and patellofemoral arthroplasty are all becoming the norm for the treatment of compartmental disease. Additionally, bicompartmental arthroplasty has been introduced in an effort to preserve the normal ligamentous structures in appropriate knees. A combination of improved results over previous experiences, newer devices such as mobile bearing PKA and newer patellofemoral replacement designs, and improved implantation techniques including minimally invasive alternatives have driven this excitement. Debate continues to surround, however, the appropriate indications for PKA, particularly involving the patellofemoral joint.

This continuing medical education event will focus on the contemporary issues in PKA, featuring didactic lectures and interactive panel discussions. The indications, contraindications, and outcomes of medial, lateral, patellofemoral, and bicompartmental replacements will be reviewed by a faculty of worldwide experts.

In conjunction with this program will be discussions that center around post-operative care such as PONV, DVT prevention, blood loss, and pain management.

TARGET AUDIENCE

The focus of this program will be the contemporary issues in PKA. The indications, contraindications, and outcomes of medial, lateral, patellofemoral, and bicompartmental replacements will be reviewed.

Compare and evaluate implant choices for the partial knee arthroplasty (PKA) patient based on patient type, design qualities, and reported patient outcomes.
Evaluate patient selection criteria for knee revision surgery.
Identify the options for blood and VTE risk management in the orthopaedic patient.
Evaluate and compare the different options for wound closure and infection management.
Review and discuss current options for pain management and other supportive care strategies

Dr. Kenneth Proefrock Dr. Ayham Al-Ayoubi

Dr. Kenneth J. Proefrock is a Practicing Physician, International Speaker, and Chemist. He has been using PRP (Platelet Rich Plasma), Growth Matrices (SCGM), and Autologous Stem Cell Therapies (ASCT) for the past decade treating musculoskeletal injuries, autoimmune conditions like Lupus and Multiple Sclerosis, degenerative conditions like osteoarthritis, Parkinson's Syndrome and ALS (Amyotrophic Laterals Sclerosis), and chronic viral conditions (including Epstein-Barr, Cytomegalovirus, and Herpes viruses). Dr. Ayham Al-Ayoubi received his MD in 1990, and was awarded his MS in ENT, Head and Neck Surgery in 1994. He obtained a Diploma in Laryngology & Otology from the Royal College of Surgeons of England in 1998. A member of The European Academy of Facial Plastic Surgery, a Fellow of The European Society of Aesthetic Surgery and an Associate Member of The British Association of Cosmetic Doctors. He is also a member of The American Academy of Otolaryngology - Head and Neck Surgery, and specializes in Facial Plastic Surgery, facial reconstruction, and neck surgery

Platelet Rich Plasma (PRP)
As the first practical application of tissue engineering, this concentrated source of autologous platelets, contains and releases (through degranulation) at least seven different growth factors (cytokines) that stimulate bone and soft tissue healing. VFN will demonstrate why this popular clinical tool is being used for several types of medical treatments, including wound healing in surgery, tendonitis, cardiac care, cartilage regeneration, disc regeneration, dental health, dermatology, cosmetic surgery, and sports medicine.
In this course you will learn how to increase the survival of your autologous stem cells using PRP in combination with Stabilized Crosslinked Growth Factors located within the Stem Cell Growth Factor Matrix.This process has an advantage of directly stimulating or initiating the healing pathways at a greatly accelerated rate. Mesenchymal Stem Cells (MSCs)
These pluripotent cells are found in multiple human adult tissues including bone marrow, synovial tissue, adipose tissue, and the peripheral blood. Since they are derived from the mesoderm, they have been shown to differentiate into bone, cartilage, muscle, and adipose tissue. MSCs from embryonic sources have shown promise scientifically while creating significant controversy, and as a result, many researchers have focused on adult stem cells, or stem cells isolated from adult humans that can be transplanted into damaged tissue.
Because of their multi-potent capabilities, mesenchymal stem cell (MSC) lineages have been used successfully to regenerate articular cartilage and bone. Recent research demonstrates that articular cartilage may be able to be repaired via percutaneous introduction of mesenchymal stem cells (MSC’s).
Surprisingly, the most commonly used source of MSC’s is still currently the bone marrow aspirate...

PRP is employed as a matrix graft, often referred to as an autologous tissue graft. This PRP matrix is defined as a "tissue graft incorporating autologous growth factors and/or autologous undifferentiated cells in a cellular matrix where design depends on the receptor site and tissue of regeneration." (Crane D, Everts PAM. Practical Pain Management. 2008; Jan/Feb. 12-26)

The reason PRP is so useful is that platelets, a normal blood cell that aids in clotting, contain multiple growth facotrs that stimulate tissue growth. In particular, PRP stimulates the growth of Collagen which is the main component of connective tissue such as tendons and cartilage. These growth factors include transforming growth factor beta (TGF-B), fibroblast growth factor, platelet-derived growth factor (PDGF), epidermal growth factor (EGF), connective tissue growth factor, and vascular endothelial growth factor (VEGF).

These growth factors recruit undifferentiated cells to the site of injury and stimulate new tissue growth. Another constituent of platelets, stromal cell derived factor I alpha causes newly recruited cells to adhere to the area.

In addition, when used with Stem Cells harvested from the patient's bone marrow, adipose, or blood stream, PRP messages the stem cells to multiply quickly. This inflammatory response is a major driver of the appropriate healing response.

An important consideration is that PRP needs to be prepared in a way to ensure a maximal amount of platelets along with a high concentration of growth factors and active stem cells. Obviously, the more growth factors that can delivered with the stem cells to the site of injury, the more likely tissue healing takes place. "We have found that Crosslinking a Matrix of Stabilized Hyaluronate (a base connective tissue material) with PRP, and growth factors, can tremendously expedite the healing process."

This regenerative approach is in strong contrast to the conventional medical approach to healing injuries which has been to use non-steroidal anti-inflammatory drugs as well as steroid injections, which, while reducing inflammation, also markedly impede the healing process and produce less durable tissue that tends to break down more easily over time.

PRP is often used in conjunction with percutaneous tenotomy. This is a procedure where there is purposeful needle irritation of the affected area using ultrasound guidance under which PRP can then be injected right into the specific sight of injury or in need of healing.

The use of diagnostic ultrasound has revolutionized the field of musculoskeletal medicine in allowing tissue healing procedure like tenotomy which often prevent the need for open surgical procedures with their risks, complications, expenses, and extensive recovery times.

The aforementioned comments and statements have not been evaluated by the FDA. The Stem Cell Growth Matrix is not an approved drug nor device, but rather a compounded item. 3rd POSI-POSNA workshops with emphasis on surgical techniques. Pre-conference POSI-POSNA workshops are conducted on 22nd January 2010 and Post-Conference POSI-POSNA workshops will be conducted on 24th January 2010 afternoon to 25th January 2010 noon. Post-conference is limited (limited Delegates only) to the practicing orthopaedician who wanted a much focused meeting. Pre and Post – conference workshops are given preference to POSI, TCOS and OSSAP Members. Conference registration is a must for Workshop Registration. Delegates are requested to collect their full registration package including that of the conference at the time of workshop. No extra ID Badges and coupons will be issued later. Any lost coupons or badges will be charged as per actuals.
