Tasks include preparation of unit dose medications, outpatient prescriptions, intravenous admixture and stock orders. Tasks will also include responsibilities for technical training, supervisory responsibility over Pharmacy Technician Trainees and Registered Pharmacy Technicians, participation in department goals and projects and requires successful experience or performance as a Registered Pharmacy Technician.<br><br>These tasks are designed to assist the Licensed Pharmacist in the preparation and delivery of pharmaceutical supplies and require the authorized review and check of a licensed Pharmacist.<br><br>PRINCIPAL DUTIES AND RESPONSIBILITIES<br>1. Train new Pharmacy Technician Trainee and Registered Pharmacy Technician staff.<br>2. In concert with Pharmacy Managers, develops and conducts selected Quality Assurance audits.<br>3. Participates in both Departmental and Hospital based Quality Improvement projects and group teams.<br>4. Assists Pharmacy Managers in compiling Department specific productivity indicators and workload data.<br>5. Provides direction and guidance to Pharmacy Technician Trainees and Registered Pharmacy Technicians in following department policy and procedure when necessary.<br>6. Performs all required pharmaceutical dosage calculations necessary to prepare extemporaneous dosage forms under the direct supervision of a Pharmacist.<br>7. Assist in the dispensing of medications to Nursing units for immediate patient use using bar-code scanning to ensure accurate dispensing of mediations.<br>8. Perform hourly rounds to Nursing units to deliver medications and return unused medications to Pharmacy to be credited in accordance with the regulations of the Board of Registration in Pharmacy.  All deliveries and returns employ bar-code and scanning technology.<br>9. Prepares sterile product admixtures (chemotherapy, parenteral nutrition, neonatal dilutions, etc.) under the direct supervision of a Pharmacist using the principals of aseptic technique and following the procedures for a clean room environment.<br>10. Provide computer operation support for the Pharmacy and other hospital departments by printing reports, labels and forms and medication requests.<br>11. Assist in dispensing and maintaining appropriate records for investigational medications.<br>12. Inventories, replenishes and maintains the organization of medications and other ancillary supplies in stored in all Pharmacy areas, including the Omnicell cabinets.<br>13. Receives, checks, and stores pharmaceutical supplies and medications that have been ordered from pharmaceutical manufacturers and drug wholesalers.<br>14. Fills medication supply requisitions for the operating room, emergency department, ambulatory clinics, Omnicell cabinets and nursing units.<br>15. Performs monthly floor inspections of nursing units as assigned.<br>16. Performs pricing and charging of stock orders, Omnicell shrinkage, and patient and clinic requisitions.<br>17. Assembles medications and type labels for ambulatory prescriptions.<br>18. Prepares discharge prescriptions.<br>19. Assists in the distribution of controlled substances and alcohol, and maintains required records.<br>20. Assists in the maintenance of laboratory, computer and other equipment.<br>21. Answers the phone, provides information and directs calls to the appropriate Pharmacy areas.<br>22. Maintains clean and organized work areas.<br>23. Performs delivery duties as required using bar-code and scanning technology.<br>24. Carries out duties and responsibilities assigned by Pharmacists or Technical Services Coordinator.<br>25. Performs medications expiration date checking as assigned.<br>26. High school diploma or equivalent.<br>2. Must be at least 18 years of age.<br>3. Two years previous hospital experience as a Registered Pharmacy Technician or equivalent experience at BWH.  Successful completion of a Pharmacy technician training program and two years Registered Pharmacy Technician experience at another healthcare facility or pharmacy is required.<br>4. Excellent command of medical terminology, medical abbreviations and computer technology is required.<br>5. Trained and satisfactorily performed in all appropriate Registered Pharmacy Technician positions of the Pharmacy department or is recommended by the majority of the Pharmacy management for promotion to the position.<br>6.  Must be a registered Pharmacy Technician with the Massachusetts Board of Pharmacy or a registered Pharmacy Intern with the Massachusetts Board of Pharmacy.<br>7. Must maintain the title Certified Pharmacy Technician with a nationally recognized certificate from an accredited certifying organization or be a registered Pharmacy Intern with the Massachusetts Board of Pharmacy.<br>8. Must complete annual Continuing Education credits as required by the certifying body to maintain Certification.<br>9. Must complete annual BWH Pharmacy Technician Competency Assessment Exam.<br>10. Directs and coordinates the delivery of all pharmacy services on the assigned shift, without direct supervision.<br>2. Responsible for the interpretation and implementation of all departmental policies and procedures on the assigned shift.<br>4. Interprets, clarifies and evaluates all medication orders for completeness and appropriateness.  Communicates with physicians and nurses on medication order discrepancies, possible drug interactions or stability problems. Assures that patient medication profiles contain appropriate medication information and clinical data.<br>5. Provides prospective evaluation and monitoring of patient drug therapy. Recommend appropriate medication doses and adjustments as indicated by patient conditions. Identifies, documents, resolves, and works to prevent drug therapy related problems.<br>6. Actively participate in the development and performance of drug use evaluations and other drug therapy monitoring initiatives.<br>7. Monitor drug therapy to prevent contraindications, duplicate therapy, drug interactions and allergic reactions. Reports all adverse drug reactions to pharmacy leadership and appropriate hospital committees.<br>8. Documents and reports all clinical interventions, adverse drug events and drug therapy profile reviews.<br>9. Provide information and consultation to physicians, nurses and other healthcare professionals, regarding appropriate drug therapy. Give advice on drug interactions, side effects, incompatibilities, allergies and drug administration.<br>10. Supervise and verify the accuracy of all work completed by supportive personnel assigned to the area.<br>11. Work in the sterile products area as needed, insuring the accuracy and sterility of all compounded products.<br>12. Responsible for submitting on time, assigned monthly inspection reports and equipment maintenance logs as needed.<br>13. Attend and participate in daily medical and multi-disciplinary rounds. Participate in specialty rounds as needed and as time allows.<br>14. Attend and participate in journal club review, in-services, and case studies for afternoon pharmacist meetings.<br>15. Assist and contribute to the orientation and training of new personnel as assigned.<br>16. Insure the proper maintenance of appropriate records for controlled substances and investigational drugs as required.<br>17. Prepare and dispense ambulatory prescriptions as needed.<br>18. Provide discharge medication and drug history counseling to patients when appropriate.<br>19. Formulate and compound special dosage forms not commercially available.<br>20. Attend, participate in, and conduct continuing education programs and conferences to keep current with developments in the profession and to maintain license.<br>21. Maintain pharmaceutical care expertise through membership and participation in professional organizations and continuing education programs.<br>22. Maintain expertise in the departmental policies and procedures.<br>23. Complete annual performance appraisal on support personnel, including Pharmacy students.<br>24. Serve as a preceptor for Pharmacy interns and externs with the Massachusetts Board of Registration in Pharmacy.<br>25. Maintain skill and competency level to comply with departmental and hospital Quality Assurance standards.<br>26. Participate in department quality improvement efforts.<br>27. Participate in the training of medical, nursing and allied health professionals.<br>28. Supports patient care and drug therapy research by ensuring study protocol medication order compliance.<br>29. Serve as a preceptor for clinical pharmacy externship students.<br>30. Assists in and provides feedback for the annual performance appraisal of Staff Pharmacist I pharmacists.<br>31. Participates with the supervisor in personnel management, including any counseling processes.<br>32. Provides staff coverage, when needed, in areas of the department that the individual has been appropriately trained in.<br>33. Participates in pharmacokinetic dosing and monitoring of selected medications and recommends dose adjustments and laboratory monitoring as required meeting patient needs. Must be credentialed to perform this service.<br>34.
