Fluoroquinolones: Classification and Therapeutic Uses

Fluoroquinolones are antibiotics that act by inhibiting DNA gyrase and inhibition of DNA type IV topoisomerase. Specific drug is intended for specific conditions. Before taking any of the quinolones, advice of a highly qualified health professional is needed. These medicines can cure certain bacterial infections but can also produce side effects and adverse effects. Proper use is recommended to avoid unwanted effects.

Fluoroquinolones are bactericidal agents for growing bacteria. It is related to nalidixic acid. The mechanism of action of this popular class of antibiotics is by inhibiting DNA gyrase and inhibition of DNA type IV topoisomerase. They basically interfere with DNA replication. These agents are highly active against Enterobacteriaceae, Pseudomonas aeruginosa, Salmonella bacteremia, Shigella, Neisseria, Haemophilus influenzae, Chlamydia, Mycobacterium tuberculosis, Chlamydophila, Moraxella catarrhalis, Mycoplasma pneumoniae, Legionella pneumophila and Campylobacter.

These fluoroquinolones are classified by generation based on their antimicrobial activities. Brand names of these agents may vary depending on country.

First-generation agents are not often used today. They are indicated for uncomplicated lower urinary tract infections given through oral administration for 7 days. They have restrained gram-negative reactions and inconsequential systemic distribution. Patients suffering from liver disease should take these medicines with precaution.

Nalidixic acid

*Reported side effects with this drug are cranial nerve palsy, metabolic acidosis and toxic psychosis.

Second-generation agents are very active in treating diseases caused by aerobic gram-negative bacilli but have narrow gram-positive activity.

Class I

Class I agents of this generation are indicated for urinary tract infections.

*Patients should definitely avoid driving or skilled activities when taking Norfloxacin as it prejudice performance with the said tasks. 400 mg of this drug should be given twice a day for 7 – 10 days. If in case the urinary tract infection is uncomplicated, Norfloxacin is given for 3 days only. Chronic relapsing of UTI may occur; in this case 400mg twice daily of Norfloxacin is given up to 12 weeks. If suppression of the infection is acceptable then the dosage may reduce to 400 mg once a day for 4 weeks.

Class II

Class II agents are used to target infections such as complicated urinary tract infections, Nosocomial infections, Gastroenteritis, Prostatitis, and sexually transmitted diseases. Class I of these agents are given orally while Class II can be given orally or intravenously.

*Effects of Ciprofloxacin are enhanced by alcohol.

Third-generation agents have a wider gram-negative and atypical intracellular activity. They also have enhanced reaction against gram-positive spectrum. They gave a higher serum concentration making it more effective against bacteria like Enterobacteriaceae, Streptococci and other atypical pathogens. They have similar indication as the Class II of second generation antibiotics except that they can be given to hospitalized or non-hospitalized patients with community-acquired pneumonia. These can also be given in patients suffering from exacerbated chronic bronchitis in which patients are not responding to other antibacterial agents. These agents are administered orally and intravenously.

Moxifloxacin
Levofloxacin

Some side effects that can be seen with the use of Levofloxacin are hypoglycaemia, flatulence, constipation, sometimes tachycardia and very rarely pneumonitis. This medication reacts adequately in acute sinusitis with 500 mg daily oral dose for 10-14 days. For some complicated UTI, 250 mg daily by intravenous infusion is given and may be increased in severe infections.

Sparfloxacin
Gatifloxacin

*Gatifloxacin was withdrawn due to increased frequency of hypoglycaemia and hyperglycemia.

Fourth-generation antibiotic inhibits both DNA gyrase and topoisomerase which enables the slowing down of resistance development. It is consider in treating intra-abdominal infections. This antibiotic can be provided through oral and intravenous administration.

Trovafloxacin

*Trovafloxacin was withdrawn due to the risk of hepatic toxicity.

There are some precautions to watch out in using quinolones. Among the occasional adverse effects that may arise are headache, dizziness, insomnia, diarrhea, nausea, dyspepsia, hypoglycaemia, tendonitis, photosensitivity and effects on central nervous system. In some cases, rashes, hallucinations, tremor, leukopenia, and urticarial may occur to patients taking these antibiotics. Quinolones are contraindicated in patients with a history of tendon disorders related to quinolone use. Drug dosage forms of antibiotics may differ considering the site of infection and the age of the patient to take the medication.

Coping With Insomnia

Do you find it hard to go to sleep at night? Are you having trouble keeping asleep? Do you wake up very early in the morning? Oh dear, you might be suffering from insomia.

If you are experiencing any of this symptoms, do not ignore it and see your doctor for help.

According to studies, people experiencing symptoms of this serious sleep disorder can be associated to high mortality risk from cardiovascular disease. That doesn't sound good. So it's better to resolve the issue rather than waiting for worst to come.

Here are few tips that can possibly give us a good night sleep.

1. Drink a glass of milk before going to bed.

2. Have a cup of Chamomile tea. It will help you relax.

3. Take some food supplement like melatonin.

4. Do not exercise or do extrenious activities few hours before retiring.

5. Remove computer and television from your bedroom. Start thinking that your bed is only for sleeping and sex.

6. Listen to a soothing music before going to bed.

7. Do not watch high energy movies, action, thriller, or horror movies at night.

8. Do not eat heavy at night. Make sure to eat 3 hours or so before hitting the sack.

9. Avoid drinking caffeinated drinks in the afternoon and evening. It will most certainty keeps you awake.

You can always searched for solutions to solve your sleeping problems. But then again, it's imperative to set an appointment with your doctor to discuss about your symptoms and before taking any food supplements or medication even if it's over-the-counter drugs.

Possible Causes of Backaches

Backache can be caused by different things, situations and conditions. To have one is total nightmare. Yes, there are pain relievers that you can count on for faster way out of agony. But always relying on them will not eliminate the problem. If real cause is not address properly, it can worsen day by day.

Lack of exercise
Wrong posture when sitting or standing
Unhealthy diet
Wrong way of picking up things or carrying stuffs
Smoking
Sitting for a long period of time
Too soft or too hard mattress
Sleeping positions
Stress and anxiety
Obesity

Taking medicines to relieve from terrible back pain is okay for one or two days. Always consider going to your doctor before taking more medicines. In such way, you'll be sure of the root cause of the backache.

Staying Away from Backaches

As I said most causes are stated above. If you don't want to suffer from this, there are plenty of ways to take care of ourselves. Working to correct or avoid the possible causes will definitely help as in the long run.

Exercising is always the best way to stay away from any disease.
Keeping a balance and healthy diet is next in line. Always find ways to unwind and de-stress.
Sleep well in a very good mattress. You can find foam mattress that can help you on this.

Mahalaga na maging handa tayong mga pharmacist na sagutin ang mga katanungan ng ating pasyente. Una, karapatan nilang malaman ang mga bagay-bagay patungkol sa kanilang kalusugan at sa gamot na kanilang iinumin o gagamitin. Pangalawa, bilang Pharmacist, responsibilidad natin na maglaan ng impormasyon sa ating pasyente.

Ano-ano nga ba ang karaniwang tinatanong sa ating mga Pharmacists?

1. May generic brand ba ang gamot na ito?

Hindi lahat ng tao ay kayang bumili ng mamahaling gamot. Kaya nga may mga generic brands para makapaglaan tayo ng lunas sa presyong magaan sa bulsa. Siguraduhin lamang na magkatulad ang dosage strength ng gamot.

2. May iba pa bang uri ang gamot (different dosage form or flavor) na ito na maaari kong inumin?

Ito ay kadalasang tinatanong ng mga taong may problema sa paglunok o may maselan na panlasa.

3. Marami akong iniinom na gamot, bawal ba na pagsabay-sabayin ko sila?

Dapat maging bihasa tayo sa drug interactions. May mga libro na maaari nating maging gabay. Siguraduhin lang na basahin ang mga libro na iyon ng hindi ka nakikta ng pasyente.

4. Ano ang mga epekto ng gamot na ito?

Maaring tinutukoy dito ang indication o side effects ng gamot. Liwanagin mabuti ang nais niyang malaman.

5. May mga pagkain o inumin ba na dapat iwasan habang umiinom ako ng gamot na ito?

May mga gamot na bawal inumin kasabay ng gatas. May pagkakataon naman na dapat may laman ang sikmura bago uminom ng isang partikular na gamot.

6. Paano ko ba iinumin ang gamot na ito?

Hindi lahat ng gamot ay pare-pareho ang pag-inom. Halimbawa na lang ang mga gamot na sublingual kung tawagin. Kung alam natin na ito ay inilalagay sa ilalim ng dila, merong mangilan-ngilan na hindi.

7. Kailan ko dapat inumin ang gamot na ito?

Kadalasan nakasulat na ang mga detalyeng ito sa prescription. Kailangan na lamang natin itong ipaliwanag sa paraang madaling tandaan.

8. Anu-ano ang mga dapat kong iwasan habang umiinom ng gamot na ito?

Iba-iba ang epekto ng gamot sa tao. May mga gamot na nagdudulot ng pagkahilo kaya't makabubuti na umiwas sa mga gawaing nangangailangan ng pagiging alerto.

9. Ano ang gagawin ko kung nakalimutan kong inumin ang aking gamot?

Dalawa ang kadalasang sagot sa katanungang ito.

Kung nakalimutang uminom ng gamot sa nakalaan na oras, inumin ang susunod na dose sa karaniwang oras at kung ano lang ang nakasulat sa prescription. Huwag uminom ng higit sa karaniwan.
Kung nakalimutan uminom ng gamot sa nakalaan na oras, laktawan na ito at magpatuloy sa karaniwang schedule.

10. Wala bang panganib ang gamot na ito kung ako ay buntis o nagpapasuso?

May mga gamot na bawal inumin ng taong buntis o nagpapasuso dahil hindi magiging ligtas sa dinadalang bata o sa sanggol.

11. Sensitibo ba ang gamot na ito sa liwanag, init o lamig? Saan o Paano ko itatago ang gamot na ito?

Tamang paraan ng pagtago ay kailangan upang mapanatili ang bisa ng gamot.

12. Ano ang gagawin ko kung ako ay maubusan ng gamot?

Kung Senior Citizen ang pasyente mo, nakasulat na sa SC book kung may balanse pa ang prescription niya. Sa ganitong pagkakataon, malalaman mo kung maari ka pang mag-refill o hindi na. Kung wala ng balanse, sabihin lamang na kailangan niya nang bumalik sa doktor para sa follow-up checkup. Huwang mag-dispense ng walang tamang prescription. Maaaring hindi na angkop ang gamot noon sa kondisyon ngayon.

I've read in different books and magazines that Yoga has lots of health benefits. Knowing its existence for 5,000 years, I will not doubt that. I heard a lot of stories too that can verify the claims.

Yoga when done in proper ways can help improve flexibility, strength, posture, and breathing. Series of yoga poses must be done in order to achieve each of these benefits. 'Asanas' is a style of yoga to achieve a more flexible body while 'Astanga and Yoga Power' are the most common style to improve strength. If both flexibility and strength are achieved, posture will definitely follow as well as proper breathing.

People who are stress can also benefit from yoga because it can help to become more calm. There are plenty of biochemical responses happening during yoga and even after yoga. Meaning to say, the hormones in our bodies reacts actively in yoga. The dopamine, norepinephrine and epinephrine levels were decreased making the person more calm. And other more reactions.

Others also said that yoga is so beneficial in different medical conditions like asthma, insomia, arthritis, back pain and even multiple sclerosis. There are some studies to prove these.

Before starting your yoga, you should consult your doctor. There experts on yoga that you can approach to help you too. Make sure to wear proper clothes and use thick yoga mat. This is for your own protection and comfort too.

Basic Skin Care

I am not a dermatologist but I know how to take care of my skin. I don't get pimples except when my period is about to come. But that is tolerable as it's only one or two on the forehead.

We all know that there are plenty of good ways to take care of our skin. You may have your own tricks too. Allow me to remind or share with you the following basic skin care.

Drink at least 2 liters of water a day. Our body uses up water in all our activities, if not replenish, our skin will show dryness or sometimes other skin problems like pimples.

Eat healthy. Add more fruits, vegetables, lean meats and whole grains in your diet. These foods are rich in vitamins and minerals essential in keeping your skin glowing.

Smoking cigarettes won't help. Stop is you are, don't dare if you're not. Smoking contributes in narrowing your tiny blood vessel leaving your skin dry. Not to mention the other diseases connecting to cigarette smoking.

Always protect your skin from the sun. Use products that can protect you against the harmful rays of the sun. Products with SPF 15 or more are recommended. If you are out swimming, bring with you your sun lotion and re-apply every two hours.

Wearing make-up is okay. Just make sure you use the brand that you trust and brand that won't give you skin problem. Always try new product in small portion of your skin before thoroughly applying them. Also, you should keep your brushes and sponges clean and fresh all the time. Wash them regularly or replace them when possible. Dirt accumulated on your make-up applicators can also create skin problems.

Wash you face at least twice a day. Use soap that is best for you. If you have sensitive skin, make sure to buy fragrance-free or soap-free products. The more scent, the more chance your skin gets irritated. Remove make-up before going to bed. Leaving them on you face will only make your skins clogged by make-up particles.

Change your pillowcase regularly. Even when we are sleeping our skin produce oil which is left in the pillowcase. Plus, all the dirt from our hair will also sleep there. I'm sure you don't want them to get all that into your skin. Only worse could happen. Besides, it's always great to sleep on clean pillow and linens.

Handle your stress well. Yes, stress can make your skin ugly. To avoid that, keep everything in order. Organize the things you need to do so you don't worry much of the things undone. Exercise daily as this is a very good tool to release your stress. Learn to unwind and take it easy.

These are just few skin care tips that I can share with you. Remember, maintain good hygiene, eat and drink properly and exercise regularly to keep your skin healthy. If despite the extra care you are giving to your skin and there's still breakout of pimples, you may want to consult a doctor to check you out. It maybe caused by other things like hormones.

INDIGESTION

Other Names: Dyspepsia, Stomach Upset, Non-Acid Dyspepsia, Upset Stomach

Indigestion occurs when the acid in stomach flows back up to the esophagus. This may also occur when stomach is inflamed or irritated. Commonly, the symptoms of dyspepsia are aggravated after eating. Symptoms may start from the upper gastrointestinal tract (stomach and first part small intestine). Indigestion generates soreness which is hard to identify where it is coming from but is normally sited between the navel and the breast-bone.

Symptoms of Indigestion:

Upper abdominal pain (pain, fullness and discomfort)
Nausea (feeling sick)
Abdominal bloating
Abdominal swelling (distention)
Flatulence, belching and burping
Loss of appetite
Heartburn (Gastro-oesophageal reflux disease/GORD)

Causes of Indigestion:

Indigestion can be due to different factors such as lifestyle and medical conditions. These factors set off the symptoms of indigestion.

Stress, depression and anxiety
Excess Acid in the stomach
Overweight or obese

Treatment of Indigestion:

There are many things we can do by ourselves in order to minimize the symptoms of indigestion, this include stopping smoking, reducing levels of stress, not eating much and fast, avoiding fatty foods, coffee, and alcohol, and even losing excess weight.

On the other hand, there are plenty of available over-the-counter and prescription only medicines, which help in treating dyspepsia. Rarely, surgery is suggested to a patient who doesn’t show improvements after taking medicines or if patient do not like to depend on proton inhibitor for long period of time.

OTC Preparations

Antacids (acts by neutralising acid in stomach)
H2 Blockers (works by reducing production of stomach acid)

Proton Pump Inhibitor (acts by stopping the production of stomach acid) (maximum use of four weeks)

POM

Long term use of proton inhibitor will call for the need of a prescription. In a long run, if this medicine still won’t give the benefit you are expecting, doctors may consider the use of another drug.

Prokinetics (acts by by-passing the food easily through a stomach which causes reduction of the symptoms)

- domperidone

- metoclopramide

*In case of Helicobacter Pylori infectivity, doctors may recommend two different antibiotic and proton pump inhibitor to kill the bacteria for seven days.

*If indigestion persists for more than two weeks, it should be examined by a doctor as it could be a indication of a serious medical condition such as a stomach ulcer.

Preventing Chapped Lips

Dry chapped lips can be caused by many things. But the most common ones are excessive exposure to the sun, winter season, biting lips and even licking your lips. The loss of moisture on lips causes the skin to break leaving it flaky and dry. In worse cases, chapped lips can lead to a more serious conditions especially when expose to foreign bodies that can cause allergies or infection.

Here are several ways to prevent having chapped lips:

1. Drink the right amount of water everyday.

2. After brushing your teeth, washing your face or taking a bath (when your lips are still wet), apply a good amount of petroleum jelly or lip balm.

3. Take Multivitamins daily as lack of essential vitamins like vitamin B complex and Iron can lead to dry lips. If you are not sure what to have, consult your local pharmacist.

4. When brushing your teeth, make sure to gently brush your lips too in order to remove dead skin cells.

5. Avoid biting or licking your lips, both actions can in fact loss moisture of your lips.

6. Use lip balm properly. Applying too much of it can also make your lips dry. Lip balm is use to seal the moisture and not to make your lips moist, unless the product you are using contains vitamins you need. Otherwise, using lip balm can possibly worsen the condition in the long run because you are not permitting water to enter your lips.

7. Another good trick is to pat your lips first with dry towel or wet wipes before applying lip balm. This is the same trick before applying lipstick on your lips.

8. Always use a good quality lip products equipped with SPF 15 or more. Some lipstick dyes can contribute to dryness of your lips.

9. Make it a habit every night to apply petroleum jelly or lip balm on your lips.

10. If you are smoking cigarettes or any kind of tobacco, stop as they can also make your lips dry.

11. If chapped lips are giving you a lot of discomfort for long time, then go to your doctor to check it out. It can be caused by other condition or disease.

Traditionally, we go to places to unwind and clear our minds from worries or anything that keeps us stress for a day or week. That is still a good idea.

Different people are expose in different kinds of stressful events or circumstances. As we allow those things to occupy our minds, our bodies tend to react on them and keeps us down. We surely do not want that to happen.

In our work, for example, there are situations that we cannot avoid. These situations can either be good or bad. For good, well things can be brighter ahead of us. But when there are bad instances, everything else can fall apart. It is just right to face every situation with clear mind and appropriate actions.

Facing different kinds of stress can sometimes be challenging. But with the right move, you will be alright.

For me there are few things that we can do to fight stress. First, live a healthy and happy life by eating healthy and exercise regularly. Second, live one day at a time. You cannot solve all problems in a day. Third, every problem has a solution. Giving up is not an option. Just focus and don't worry of not getting the solution right the first time. Fourth, include music in your life. Music is a known aid in releasing stress in our lives. Listen to a song you adore so much. Play an instrument like drums, guitar, violin, saxophone or anything else you want. Finally, always find time to meditate, pray and love. These will rejuvenate us from inside to outside.

We are created above all things. We have a God that is bigger than any of our problems. DO NOT let stress get you. Instead, remember that YOU GOT THIS.

Have a Blessed Holy Week

It's the time of the year when all people are busy for all different reasons. Some are busy in remembering the main reason of Holy Week and that is the sacrifice Jesus did to save us from our sins. Reenactments, passions, stations of the Cross, "Bisita Iglesia", and other more rituals and traditions in different part of the country.

Some of us is using these holidays to unwind and just be quiet in a family get-away. Some of us it's there just to be with friends and have fun in their own ways, mostly swimming. Whatever it is, don't forget to say a prayer to our heavenly Father for the great thing He did for us, sending us His son to die for us so we can live and enjoy His blessings. We can also sing our hearts with praises and thanksgiving to honour Jesus in our lives. If you can play instruments, do so.

Have a Blessed Holy Week, Be safe and God be with us all.

Oath Taking of March 2017 Pharmacists Licensure Examination Passers

Professional Regulation Commission (PRC) announced that the Oath Taking of March 2017 Pharmacists Licensure Examination Passers will be held on 20th of April 2017, 1:00 p.m. at Reception Hall, PICC Complex, Pasay City.

Again, Congratulations to 2,426 Pharmacist Licensure Examination passers. I am very sure that you, your parents, and professors are all playing trumpets for the success you achieved. Well done and may you all succeed to whatever field of practice you choose to embark on in the future. May God bless you all.

Now that the winter is very near, having enough information about different diseases that occur during this season is something we should do. This is the best way to keep ourselves from catching one. Prevention is the best cure as they said.

During winter season, if possibly staying inside our houses is the not a bad idea. Just make sure that you have all need and you have something to make yourselves busy. If you have kids, be equipped with numerous ideas for their activities. Learning new things is one including musical instruments like keyboard, violin, guitar and the like. In cold nights, cuddling under a warm blanket with a great movie to watch and popcorn on the side. If you then choose to walk outside, make sure to wear appropriate clothes and shoes. Wear scarves and gloves too.

Toddler's Development

Toodlers grow so fast. At one point they are climbing up the chair and then suddenly they are in the virtue of communicating with you. That all start by using words to connote the things they see. Followed by two-word sentence and everything else follow so quickly.

Monitoring their growth both cognitive and language development is one primary task of parents. Involving our tots in different activities indoor and outdoor will encourage those developments in a more rewarding manner. Among these activities are the following:

playing with legos or wooden blocks
allowing them to play with toys that involve their sense of touch
introduce music to them by singing nursery rhymes, playing instruments like piano for toddlers
reading stories with them
take toddlers for a walk in a park or even just around the village
teach them to organize, starting with their toys
teach them new words by labeling things around your house

These are just few things that can help in your toddler's development. I'm sure you can come up with dozen more. Just keep in mind that we, parents, should also be involved in those activities that will lead us to develop our relationship with them.

Guitar and Rose by Scoop and Snap

In lieu to my recent travel in Ireland, I happened to see the glimpse of the lovely culture of Irish people. One thing that surface more is their absolute love of music. It's like having guitar center hour in every popular street of Dublin. The food and people are equally amazing. To top it all, the green pasteur made me fall down to my knees.

If you are to ask me, how's the Pharmacy Practice here? It is far more different back home. If you are used to easy dispensing of medicine, you will be shocked on the difference. Let me elaborate that on my next post so stay tune.

Pharmacist Licensure Examination June 2016 Results

We would like to Congratulate the 1,406 of 2,784 examinees who passed the June 2016 Pharmacist Licensure Examination in Manila, Baguio, Cebu and Davao given by the Board of Pharmacy under the Professional Regulation Commission (PRC) Philippines.

The Professional Regulation Commission (PRC) released the results 2 days after the examination was given. The PRC would like to remind the successful examinees that the registration for the issuance of Professional Identification Card (ID) and Certificate of Registration will be done on-line at PRC ONLINE starting on June 27 to June 29, 2016. Carefully follow instructions set for online registrationUpon registration on-line, successful examinees are then required to bring the following at the PRC Office:

duly accomplished Oath Form or Panunumpa ng Propesyonal
2 pieces passport size picture (colored with white background and complete name tag)
1 piece 1” x 1” picture (colored with white background and complete name tag)
2 sets of documentary stamps

All 1,406 successful examinees should personally register and sign in the Roster of Registered Professionals.

The oathtaking ceremony date and venue will be announced later.

Keeping Warm on Cold Weather

Nowadays, phone apps allow us to know the current weather. It is in our advantage as we can plan ahead what to wear and we can prepare even more for extreme conditions.

In a very cold weather, we can control the temperature by wearing appropriate clothing. Thin warm layers are better than wearing a bug chunky clothing. For a more Cozy Winters , accessories like gloves, scarfs and hats are important.

Drinking hot drinks and eating hot meal as well as eating regularly can keep our energy high during winter.

Maintaing a good 18 degrees celcius temperature at home can make us more comfortable.

Keep warm, stay active at home and enjoy a very good cuddles with your loveones.

I love to travel. The thing that I don't want to happen is to get sick during certain trip. Long before I became a mother, I always pack my bag with medicines that can be of great ease when something goes wrong. Few items are added to my list for my baby, our little tag along.
