However, remaining in a therapeutic community for the recommended duration, which various for each individual, is often a struggle.</p>

It can also be an important element of the self-reflection you learn in 12-step recovery.</p>
<p>While it is not a cure for clinical depression, nor will it magically solve problematic situations in your life, being consciously grateful can set the tone for your daily living. This gratitude is focused on what already exists in your life. It is a practice that allows you to unearth the good that is already there, and place it at the forefront of your mind.</p>

Oprah has suggested many times that <a title="Oprah.com: The Power of Gratitude" href="http://www.oprah.com/spirit/The-Power-of-Gratitude" target="_blank">a gratitude journal </a>is the way to go, and we think it&#8217;s a great place to start.</p>
<p>Initially, it may take some effort to think of things you are grateful for, but remember, nothing is too insignificant for your gratefulness. Your list may even look something like this:</p>
<blockquote><p>I am grateful for my health, for the means to enjoy a cup of coffee this morning, for a good conversation with my sister, for shoes that fit, for a second chance to live well, for a bed to sleep in &#8230;</p></blockquote>
<p>By setting a tone of gratefulness and appreciation, you may find that you are less distracted and distressed by the things in life that are trivial, fleeting or materialistic.  For example, realizing how fortunate you are to have a roof over your head, keeps the fact that you don&#8217;t own a fancy home in its proper perspective. This gratefulness spills over into your attitude toward friends, toward your job, toward family and toward many aspects of life.</p>

			<content:encoded><![CDATA[<p>The idea that a glass of wine&#8211;or an ounce or two of liquor&#8211;will reduce a person&#8217;s feelings of stress and tension is probably as old as alcohol itself. It would be easy to blame Hollywood, with the abundance of television and movie images of everyone from desperate housewives to anxiety-ridden lawyers downing alcohol at the end of their busy days, but the truth is they are just portraying the cultural norm in our society. &#8220;Unwinding&#8221; has become almost synonymous with drinking for a lot of people.</p>
<p>But are we really unwinding with a glass of alcohol? Is drinking really a solution to stress?<img class="alignright" title="alcoholism-glass" src="http://mrg.bz/tK6pR9" alt="" width="168" height="211" /> Or are we unintentionally building greater levels of anxiety and tension for ourselves?</p>
<p>While scientists have found that a small amount of alcohol&#8211;very low quantities&#8211;may reduce the physical and mental responses to stressors in some individuals, it will not for an alcoholic. In fact, for many people it reduces the body&#8217;s ability to cope with stress. For many people, drinking alcohol actually induces stress by stimulating the very same hormones that are released by the body in times of anxiety. What&#8217;s more, according to the <a title="NIAAA Web Site" href="http://www.niaaa.nih.gov/" target="_blank">National Institute on Alcohol Abuse and Alcoholism (NIAAA)</a>, chronic, daily exposure to alcohol can cause an<a title="NIAA Fact Sheet on Stress and Alcohol" href="http://www.harmonyhc.com/display_article.asp?article=3440" target="_blank"> increase in adrenaline and other stress-related hormones</a> for virtually everyone.</p>
<p>In other words, drinking can create increasing feelings of stress and tension in the body and brain over the long haul. We may believe that our life is becoming more stressful and our situation more difficult to handle, when in fact our perception of reality has become tainted. Our body has simply become more prone to the acute sensations of anxiety because of our own alcohol consumption.</p>
<p>It is important to unwind when we experience stress, but it&#8217;s also vital to do so in healthy ways that are not going to trigger increasing amounts of stress. Drinking socially or privately to remedy feelings of stress can, in fact, be the worst way to unwind.</p>

			<content:encoded><![CDATA[<p dir="ltr">Recovery from alcoholism or drug abuse, or both, is one of life&#8217;s real challenges. No surprise there &#8211; recovery is hard, hard work.</p>
<p dir="ltr">But addicted persons have other challenges to their recovery beyond the initial hurdle of getting clean or sober. One of those challenges is cross addiction.</p>
<p dir="ltr">In the interview below, Louise Beirne, RN, a Certified Substance Abuse Counselor at Mt. Regis Center, talks about what cross addiction is, how cross addiction can happen, what to watch for when going to the doctor or hospital, and, how important it is that individuals in recovery take full responsibility for their recovery.</p>
<p dir="ltr">&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;&#8212;</p>
<p dir="ltr"><strong>Q:  Louise, what is cross addiction and what kinds of substances are involved?</strong></p>
<p dir="ltr">LB: Cross addiction is an addiction to two or more drugs that are in two different categories.</p>
<p dir="ltr"><strong>Q: Can you explain? </strong></p>
<p dir="ltr">LB: Sure, here&#8217;s what I mean by categories. There  are central nervous system depressants, substances like alcohol, benzodiazepines, and barbiturates. That&#8217;s one category. And, there are central nervous system stimulants, things like cocaine, methamphetamines, Ritalin, and, this might surprise you, nicotine and caffeine. That&#8217;s the second category. So if you have someone who&#8217;s addicted to alcohol and to cocaine both, then that&#8217;s what we call a cross addiction. The person is addicted to two different substances from two distinctly different categories.</p>
<p dir="ltr"><strong>Q. So a person that is addicted to alcohol and barbiturates would not be considered to have a cross addiction? </strong></p>
<p dir="ltr">LB: Exactly right. That would come under a different classification &#8211; poly-drug dependent, or poly-drug addiction.</p>
<p dir="ltr"><strong>Q. How do people become cross addicted? What happens?</strong></p>
<p dir="ltr">LB: It can be as simple as being introduced to a new drug. One person might have an affinity for a class of drugs &#8211; he or she might favor barbiturates, alcohol or other types of drugs that are depressants, over stimulants like cocaine. The problem arises when, for example, alcohol use might impact a person&#8217;s judgment. So all of a sudden you have a person with a drinking problem and one night after a few drinks they start to take cocaine. Then they&#8217;re well on the way to a cross addiction.</p>
<p dir="ltr"><strong>Q.  How does this play out in treatment?</strong></p>
<p dir="ltr">LB: Well, we see people who are in treatment and manage to stay clean for a while. What can often happen is that they relapse on alcohol first, and again, like I just mentioned, a few drinks can lead to a set of bad decisions because judgment is affected. So the alcohol is a trigger to another event &#8211; that can be cocaine or methamphetamine or some other kind of stimulant type of drug.</p>
<p dir="ltr"><strong>Q. Is it more difficult for a person to recover if they are cross-addicted? </strong></p>
<p dir="ltr">LB: Not necessarily. They do run more of a risk because they are addicted to that many more drugs. But if someone can get clear of one substance, they can get clear of two. Everything depends on how much they want to recover.</p>
<p dir="ltr"><strong>Q. Are there medical situations &#8211; for instance a surgery &#8211; where a person in recovery can find themselves addicted to some kind of pain killer almost without knowing it? Is this a problem for cross addicted individuals? </strong></p>
<p dir="ltr">LB: Yes, absolutely. Consider a person who is in recovery for both alcohol and methamphetamines. After surgery, this person&#8217;s physician, not being aware that her patient is a recovering alcoholic, prescribes a pain killer that is highly addictive and falls in the same category as alcohol. <em>(OxyContin and Percocet ).</em>The problem<em> </em>here is that the body doesn&#8217;t know the difference between the alcohol that used to be consumed and the new pain-killing drug, because <em>both substances are in the same category &#8211; they&#8217;re depressants. </em>And we might be headed for that very same scenario we just talked about &#8211; a relapse into alcohol abuse and impaired judgment which can lead to all sorts of unwelcome problems. <em> </em></p>
<p dir="ltr"><strong>Q: So the person is addicted again?</strong></p>
<p dir="ltr">LB:  There&#8217;s a real danger of that, yes. It&#8217;s especially tricky if the physician is unaware of her patients&#8217; medical and drug use history. Things become even more complicated if the patient is taken off the drug too quickly. The patient has to be titrated down daily, which is just another way of saying that withdrawal must be very carefully calibrated.</p>
<p dir="ltr"><strong>Q. So what should a patient do?</strong></p>
<p dir="ltr">LB: Any person in recovery &#8211; no matter how long ago it was that you got clean or sober &#8211; must take extra precautions when prescribed medications by a physician. Always tell your physician about your addictions and your recovery process. Be specific and detailed. Ask that you be given the least addictive drugs possible. Get your support systems in place if they are not already in place for you.</p>
<p dir="ltr"><strong>Q. Anything else?</strong></p>
<p dir="ltr">LB: Yes. Give your medication to a family member or sponsor and make sure they understand that you are to be given this medication as &#8220;needed&#8221; and not as &#8220;wanted&#8221;.</p>
<p dir="ltr"><strong>Q. What about detox? Is it more difficult to detox when you are cross-addicted? </strong></p>
<p dir="ltr">LB: Well, it&#8217;s important to know that depressants are dangerous to detox from on your own, without medical supervision. We see elevated blood pressure, pulse rate, there&#8217;s a tendency for hallucinations and seizures, that sort of thing. So detox from those substances really should take place in a medical or clinical setting.</p>
<p dir="ltr"><strong>Q: Any parting advice? </strong></p>
<p dir="ltr">LB: Of course! We like to tell people that they are responsible for their own recovery. That means you have to make sure you tell your doctor what your story is. No one else is going to do it. It&#8217;s very disheartening to see people lose ground due to a prescribed medication. It&#8217;s so unnecessary. We&#8217;ve seen people work so hard on their recovery and we know that it&#8217;s difficult work. We don&#8217;t want to see anyone go through this anymore than they have to.</p>
