It is considered an anachronistic remnant of a dubious medical ideology that psychoanalysis has largely renounced. However, while analysts today avoid surgical analogies, their patients continue to produce surgical fantasies about analytic treatment. This fact alone requires a serious consideration of the meanings that surgical metaphors have for them. A second reason for reconsidering the role of the surgical metaphor, from the analyst’s perspective, lies in its creative revival by W.R. Bion. Disregarding the shift away from surgical analogizing. Bion employed the metaphor to vividly portray various aspects of the analytic situation and the patient’s experience of them. A brief historical overview of the surgical metaphor in psychoanalysis is provided, followed by an account of the reasons for its demise and by a review of the criticisms that continue to be levelled at it. Bion’s use of surgical metaphors toward the end of his life is then explored. Finally, illustrations are given of the various ways in which patients use spontaneously surgical metaphors to depict transference and the analytic process. Though the analyst should not deliberately adopt surgical metaphors, it is important to remain open to these transference portrayals.&lt;div class="feedflare"&gt;

The idea of an archaic psyche is proposed and conceptualized as non repressed unconscious, originated by the projective traumatic attributions of the parental unconscious and looking for expression and representation through enactment within the different contexts where psychic reality is worked through. It is considered that the idea of a psyche, expressing and representing through repetition in dramatic act, is the logical consequence of a model of an open psychic subject generating psychological meaning in the encounter with the inter-subjective other, no more view as a contingent object. The Author argues that the Freudian notion of agieren supposes an implicit change in the theory of psychoanalytic listening as transference is no longer considered the displacement between intra-psychic representations but an inter-subjective phenomenon linked to counter-transference. Thus transference is considered exceeding both the model of the first drive theory as well the first Freudian topography. It is postulated that the instrumental use of regression in the analytic process implies that the setting, containing the scenic representation of certain archaic intrapsychic configuration of the patient registered as not-verbally-thought-psychic-gestures, creates the conditions for a new form of representation compatible with Ego’s verbal thinking, apart from repetition in act and liable to be resignified infinitely.&lt;div class="feedflare"&gt;

The organization of her False Self supports this complaisant and symptomatically homosexual behaviour. Early separation anxiety and intense anxiety towards strangers appear to form the matrix of avoidance as well as phobic behaviour towards the Other, male and paternal figures in the "primal scene". The question of pathological mourning, suffering and pain within the patient is also dealt with due to the death of the patient's father a short time after coming back into her life.&lt;div class="feedflare"&gt;
