<p>A Case of Cataract<br />

<p>One day as I entered the clinic I found a little white-haired woman waiting patiently to be treated. I had not seen her before, and did not know what her trouble was. The usual crowd of patients was waiting for Dr. Bates and myself, so when he said to me, &#8220;See what you can do for this woman,&#8221; I did not ask any questions, for I knew that whatever the condition of her eyes relaxation would help her.</p>
<p>I placed her four feet from the test card, at which distance she read the forty line (read by the eye with normal vision at forty feet), and told her how to rest her eyes by palming and how to avoid staring by shifting from one side of a letter to another. These practices helped her so much that before she left she was able to read the thirty line.</p>

<p>Later I learned that she had first seen Dr. Bates in March, 1919, and that she had incipient cataract of both eyes. In October. 1916. she had visited another dispensary where an operation was advised when the cataracts were ripe. I also learned that in spite of her seventy-three years she worked hard every day for her living, being employed in an orphan asylum where she mended the children&#8217;s clothes. The fact that she was very deaf I saw for myself, of course, at the first interview, for I had to scream to make her hear. Her courage and cheerfulness under circumstances that might have daunted the bravest spirit were amazing. Her .face was always radiant with smiles, and she was so witty, and so appreciative of everything that was done ,for her, that each one of her visits to the clinic was a pleasure to me.</p>
<p>&#8220;I have so much to be thankful for,&#8221; she said one day. &#8220;I know I will see all right again. They ate waiting to operate at the abler dispensary, and I am waiting to fool them.&#8221;</p>
<p>The orphanage is about two miles from the clinic, and often she walks the entire distance rather than bother waiting for a car. She insists after these feats that she isn&#8217;t a bit tired. One day there were no cars running;and the walking was so bad that a friend urged her not to go out unless she was prepared to swim. She came just as usual, however. Why should she stay in, she asked, because other people were afraid to go out. She wasn&#8217;t tired either, and she hadn&#8217;t even got her feet wet. She just dodged the snowdrifts.</p>
<p>Most patients frown when they cannot see a letter, but my little cataract patient smiles instead and remarks cheerfully, &#8220;That&#8217;s the time you got me.&#8221;</p>
<p>One day she did not do as well as usual, and I found that the people in the place where she worked had been saying unpleasant things. I told her she must try not to let things of this sort disturb her, because that made her strain and made the cataracts worse.</p>
<p>&#8220;Well,&#8221; she said, &#8220;it is mighty hard not to worry; but I&#8217;ll try not to.&#8221;</p>
<p>At a recent visit she explained that she wouldn&#8217;t he able to do very well because she hadn&#8217;t had time to practice.</p>
<p>&#8220;Never mind,&#8221; I said. &#8220;Just do as well as you can.&#8221; Without her knowing it I placed her two feet farther from the card .than usual. Then I told her to palm, and after a short time I pointed to a small letter on the bottom line and asked her if she could see it. She recognized it immediately. Then I pointed to another, but she was so eager to see it that she tried too hard and failed. She closed her eyes for a few minutes without palming, and when she opened them she read the whole line. I then told her that she was two feet farther away from the card than she usually was. She was very happy about this and said, &#8220;That&#8217;s the time you fooled me.&#8221;</p>
<p>She has since become able to read the bottom line at ten feet, and one day she read&#8217; it at eleven feet, without knowing it and without having done any practicing at home. On sunshiny days she can read the &#8220;W. H. Bates, M.D.&#8221; on Dr. Bates&#8217; card, and for over a month she has done all her sewing without glasses. There is no doubt that she is going to fool them at the other dispensary.</p>
<p>Along with the improvement in her eyes has gone a considerable improvement in her hearing. Noises in her ears which she describes as a &#8220;ringing and a singing&#8221; are promptly relieved by palming, and she says that the relief, which at first was only temporary, is now becoming more constant. She also says that she hears conversation better than she used to.</p>

<p><span style="font-family: Arial;">Pencils, notebooks, crayons, chalk. It’s that time again:  summer has packed its bags, and you’re filling your children’s backpacks with  all the supplies they’ll need to succeed. But have you left out the most  important school tool?</span></p>
<p>According to All About Vision, more than 80  percent of what a child learns in school is presented visually, so following  good vision practices for your kids can make a huge difference in their academic  performance.</p>

<p><strong>Warning Signs<br />

First and foremost, you need to pay  attention to the signs that your child may have a vision impairment. A new  national survey, reported by U.S. News &amp; World Report, shows that more than  20 percent of kids aged 12 to 17 have trouble seeing the classroom chalkboard.  And about 25 percent of children aged 6 to 11 have a vision impairment strong  enough for them to wear prescription glasses. But more than 66 percent of kids  under the age of 6 have never had their eyes examined by an eye doctor, the  survey found. And little kids don’t know how to tell you they’re having problems  seeing; they just squint or sit right up in front of the TV.</p>
<p>These last  two signs—along with frequent eye rubbing, tilting the head to see better,  excessive tearing, light sensitivity, using a finger to follow along while  reading, complaining of headaches or tired eyes, and closing one eye to read,  watch TV, or see better—could indicate that your child has a vision problem. If  your child exhibits the above signs, schedule an appointment with your eye  doctor.<br />
<strong><br />
Vision Screening: How and When</strong></p>
<p>But don’t wait until  something goes wrong to see the eye doctor. And don’t rely on the school nurse  or your pediatrician to monitor your child’s vision, as some pediatricians don’t  perform vision screenings, and school screenings are designed to alert parents  to the possibility of a visual problem, not take the place of a visit to an  expert eye care practitioner. Even if your child exhibits no symptoms of visual  problems, he or she should have an eye exam by the age of 6 months, then again  at age 3, according to the American Optometric Association. Having a complete  eye exam before your child enters school allows for enough time to catch and  correct any problems while the visual system is developing.<br />

<p>Some other healthy vision habits to instill in kids are getting  them used to wearing sunglasses. They need sun protection just as much as adults  do—maybe even more, since they (hopefully) spend more time playing outdoors in  direct sunlight. Which leads to what may be the biggest kid problem of our  modern age: getting them away from the TV, computer, and video games. Yes, they  may protest at first, but setting reasonable limits on indoor activities like  these can not only help their physical development (and help prevent such  pervasive modern diseases as childhood diabetes, which can lead to serious  vision problems, even blindness), but also their vision.</p>
<p>Though heredity  seems to play a significant role in the development of myopia in childhood, All  About Vision notes that some research suggests eye strain—specifically computer  eye strain—may also be involved. Some researchers think that fatigue caused by excessive  focusing can lead to changes within the eye that cause myopia. And experts agree  that focusing on images on a computer screen causes greater eye fatigue than  reading print in a book or magazine. So have your child follow the same  computer-use rules that Rebuild Your Vision has taught you, particularly the  10-10-10 rule: every 10 minutes your child should take his or her eyes off the  computer and look at an object at least 10 feet away for at least 10  seconds.</p>
<p>And when you finally do get them outside, remember to provide  protective eyewear for sports activities. Prevent Blindness America estimates  that about 40,000 sports-related eye injuries each year in the U.S. are severe  enough to require emergency room care—90 percent of these injuries could easily  be prevented by wearing protective eyewear.</p>

<p>Herman Snellen, a Dutch ophthalmologist, designed the first eye chart, which measures visual acuity. Sounds complicated, but it&#8217;s not, really.  Referred to as the Snellen Chart, it was developed in 1862 and is still used today. One of those things that just didn&#8217;t need any changes through the years. He set the baseline of measurement at 20 feet. (The approximate distance at which light rays do not require our eyes to bend the rays to focus them upon the retina.)</p>

<p>Snellen set this standard of 20/20 vision to represent &#8220;normal sight,&#8221;.  Or,it is what a person with normal vision could see at a distance of 20 feet.</p>
<p>This means that a person with 20/40 vision must be 20 feet away to read what he or she should be able to read at 40 feet; a person with 20/85 vision must be 20 feet away to read what he or she should be able to read at 85 feet away, and so on.</p>
<p>There is another chart that measures near vision (nearsighted).</p>

Both Pepsi and DreamWorks Animation ran 3-D commercials during the game; last year saw the release of a 3-D Journey to the Center of the Earth; this spring, the 3-D stop-animation movie Coraline is playing in theaters.</p>

<p><strong>But why do you need those goofy oversized glasses? And how does 3-D work?</strong><br />
Unlike the eyes of the space aliens featured in some 3-D features, our two eyes are not positioned together, but lie approximately 3 inches apart. This separation causes each eye to see the world slightly differently. The brain fuses these two views together and uses them to calculate distance, creating our sense of depth.</p>

<p>3-D glasses work similarly, feeding different images into your eyes (thus the two different-colored lenses: one red, one blue). In a theater, the movie screen actually displays two images, and the glasses cause one of the images to enter one eye and the second image to enter the other.</p>
<p>It may surprise you to learn that the ability to visually perceive three-dimensional space, called “binocular depth perception,” can be achieved without external aid. In fact, this ability is a way to measure parts of our vision: if you can successfully combine separate images from two eyes into one three-dimensional image in your brain, your binocular vision is working well. Both eyes are working as a team.</p>
<p>Yet not all people have binocular depth perception. According to the Optometrists Network, up to 12 percent of the population has some type of binocular vision disability: the two most commonly known types are amblyopia and strabismus.</p>
<p><strong>Amblyopia</strong>, from the Greek word amblys, meaning “blunt” or “dull,” is a condition in which one eye has lowered visual acuity and/or poor muscle control. Your eyes may be straight, but one eye doesn’t send the proper information to the brain, so your brain can&#8217;t perceive binocular depth. Amblyopia—more commonly known as “lazy eye”—affects approximately 2 or 3 out of every 100 children in the United States.</p>
<p><strong>Conversely, strabismus</strong>, from the Greek word for “squint,” is a visual defect in which you may see clearly, but your eyes don&#8217;t line up—the two eyes point in different directions. One eye may turn in, out, up, or down, while the other eye looks straight ahead. That’s why strabismus is more commonly known as “walleye” or “wandering eye.” Strabismus affects approximately 4 out of every 100 children in the United States.</p>
<p>These conditions are not always easy to diagnose. With strabismus, particularly, the eye misalignments may not obvious to the untrained observer. And it used to be that doctors thought lazy eye could only be treated in childhood, up to about 8 years old. Currently, treatment is given at any age.</p>
<p>So next time you’re watching a 3-D production, give your eyes a screening, too. You can test your binocular depth perception by first covering and then uncovering one eye: if the image floats out from the screen, your eyes are working in tandem. If the image remains flat, you may have a vision problem, so consult your ophthalmologist. And if you see any space monsters coming for you, duck.</p>
