Irrefutable evidence has already proven that smoking is bad for your heart, lungs, and overall health. But did you know that it’s also horrible for your skin? Smoking can significantly age the appearance of your skin. Quitting smoking may not only keep you alive and healthy, but it can also help you maintain a more youthful appearance as you get older. If you have already made the healthy choice to quit smoking, but your skin still looks worse for the wear, skin care treatments can rejuvenate your appearance and restore a fresh, healthy glow.

Effects of Smoking on the Skin

Smoking can lead to any or all of the following issues for your skin health:

Bags Under Your Eyes: Due to nightly nicotine withdrawals affecting your quality of sleep, smoking can lead to chronic puffiness under the eyes.

Increased Risk of Skin Cancer: In addition to increasing your risk of lung, esophageal, throat, and mouth cancer, smoking also raises your likelihood of developing skin cancer.

Premature Aging: Smokers generally look older than people their same age because smoking accelerates the aging process. Chemicals and free radicals cause the destruction of elastin and collagen and lead to sagging skin, rough texture, and wrinkles.

Higher Risk of Psoriasis: Your risk of developing this scaly autoimmune skin condition goes up significantly if you smoke.

Worse Scarring: Surgeons require you to quit smoking before and after surgery because smoking narrows the blood vessels and limits oxygen flow to the skin. A smoker’s body will take longer to heal after surgery or injury and will develop a larger and darker scar.

Uneven Coloring: Chemicals in tobacco smoke cause uneven skin coloring and deplete necessary oxygen levels in the skin. This may cause a smoker’s skin to develop a gray or yellow tone.

Wrinkles: Smoking slows blood and oxygen supply, which denies the skin what it needs to stay smooth and healthy. Additionally, repeated puckering of the muscles around the mouth and lips combined with squinting from cigarette smoke can increase the rate at which wrinkles form around the mouth and eyes.

Treatments for Damaged Skin

If you have already seen some of the detrimental effects of smoking on your own skin, Dr. Spies can rejuvenate you with various treatments that may provide long-lasting results. For instance, a photofacial can use intense pulsed light to correct many signs of aging and improve your skin tone and clarity. Microneedling with platelet-rich plasma (PRP) can create controlled micro-injuries to stimulate collagen production and enhance your skin’s health. Medical-grade skin care treatments and peels can improve skin tone, soften skin texture, and hydrate damaged dry skin. Erbium laser resurfacing can stimulate cellular regeneration and renew the skin to soften fine lines and other signs of aging. Ultherapy® is like a nonsurgical facelift that lifts and tightens loose skin to create a youthful look. Whatever your skin needs, Dr. Spies can recommend customized skin care treatments to brighten your appearance and correct the consequences of smoking.

However, losing weight and losing fat are two different things. Trying to get rid of unwanted fat through weight loss is possible, but it is often difficult and unreliable. With liposuction, you can destroy fat, spot reduce, and tone your contours according to your desires. Liposuction permanently eliminates unwanted fat from specific, stubborn areas to redefine your body contours and give you a more flattering body shape.

How Weight Loss Works

Weight loss is when your overall body weight goes down (when the number on the scale decreases). Although this can seem like a victory, it doesn’t account for what type of mass has decreased. Your body weight measures not only fat but also your muscles, bones, organs, fluids, and other parts. Losing or gaining a few pounds can be due to changes in bodily fluids and waste product. For instance, you may weigh yourself before and after a run and discover that you’ve lost weight, but it was only because of lost fluid due to sweating. Likewise, doing the same thing after a bathroom trip might show a difference because you got rid of bodily waste. On the other hand, if you adopt a new exercise regime and a healthier diet plan, you may lose fat and gain muscle in such proportions that even after a few months, the number on the scale doesn’t change at all. Not seeing any weight loss, in this case, can be discouraging, but it is still something to be proud of because you have indeed burned excess fat.

Attempts to control weight loss to achieve fat loss can be tricky and complicated. Creating an appropriate calorie deficit doesn’t just mean cutting calories; if you simply eat less, your body may conserve energy and burn muscle instead of fat. Starving yourself can lead to a plateau or even weight gain as your body struggles to function on diminished energy levels. Certain food items can assist or hinder weight loss efforts as well. Attempting to lose weight too quickly can lead to substantial decreases in muscle mass, which is why bariatric surgery patients are encouraged to consume a lot of protein.

How Fat Loss Works

Fat loss is when excess fat is either removed from the body or burned by the body for fuel. The typical goal with weight loss is to get the body to burn fat. But even when successful, this does not always turn out how you’d expect or desire. Weight loss can sometimes have the unwanted effect of taking fat from where you want it (such as the breasts) and leaving fat where you don’t want it (such as the abdomen). The challenge of eliminating fat from precise areas and not others is called spot reduction, and it is impossible to achieve through dieting and exercise.

Achieve Fat Loss With Liposuction

Achieving fat loss is possible through liposuction. Your surgeon will insert a thin, hollow tube called a cannula into the areas of your body that have stubborn fat. With the cannula inserted, the surgeon will gently suction out the excess fat cells. This leads to a permanent reduction in fat in the treated area, resulting in a slimmer shape. By targeting stubborn regions directly, liposuction can spot reduce and produce fat loss. If your weight loss efforts fail to yield your desired results, liposuction can successfully sculpt your body contours and give you the flattering figure you seek.

Tummy tuck surgery is among the most commonly performed cosmetic surgeries and provides stunning results. The benefits achieved through a tummy tuck are what patients dream of having—smooth, sculpted, and firm midsections with long-lasting results. Unfortunately, while the results are ideal, the recovery period is not as glamorous. Drains are placed under the skin to remove excess fluid and aid the healing process, and they remain inserted for one to three weeks after surgery. These drains can be unsightly and irritating; however, they may soon be a thing of the past. TissuGlu® Surgical Adhesive now offers a proven alternative to these uncomfortable drains, and it is the only FDA-approved surgical adhesive for drain-free tummy tucks.

The Downsides of Drains

To the dismay of most, drains have always been placed after tummy tuck surgery because fluid accumulation is one of the most common problems experienced during recovery. Although it is necessary to monitor and collect excess fluid, drains bring with them a list of potential complications, including that they:

are painful

are difficult to hide under clothing

require constant maintenance

put you at risk for infection

may cause additional scarring

What Is TissuGlu®?

TissuGlu® is a safe, synthetic surgical adhesive that forms a bond between the layers of tissue to aid the healing process without the need for drains. Because it reduces the areas where excess fluid can collect, the fluid is instead naturally absorbed by your body, thus allowing for a quicker and less painful recovery. Unlike drains, TissuGlu® is non-invasive, and the adhesive will safely eliminate itself from your body over time.

A New Kind of Recovery

With TissuGlu®, gone are the days of having to wait over a week to shower, drive, and wear what you want. TissuGlu® allows you to take control of your recovery. Without the presence of painful drains, you can:

return to your daily routine sooner

forget the annoyance of having to empty the drains multiple times a day

bypass the risk of drain-related infection

avoid additional scarring

After your surgery with TissuGlu®, you may feel healed and like your usual self immediately; however, it is essential that you follow all of your doctor’s post-surgical recommendations for your best results!

The vast majority of women enjoy years of satisfaction with the feel and appearance of their breasts. However, every woman does have a risk of an implant problem that may require attention. Capsular contracture is an example of one of the complications that may occur after breast augmentation. Breast revision surgery can correct capsular contracture and restore a natural, beautiful appearance to the breasts.

What Is Capsular Contracture?

When a foreign substance (such as a breast implant) is introduced into the body, the body forms a scar capsule around it. Capsular contracture is when this scar capsule tightens or constricts around the breast implant. This can cause discomfort, hardness, and a visible deformity of the breast.

What Does Capsular Contracture Feel Like?

Capsular contracture is measured by the Baker scale (Grades I, II, III, or IV) in terms of severity. Grade I may not feel any different than a normal breast, but Grades II, III, and IV may feel hard or firm. Grade IV may be quite hard and unnatural. Discomfort or pain is more likely to be present in Grade III or Grade IV capsular contracture.

What Does Capsular Contracture Look Like?

Low-grade capsular contracture (Grade I on the Baker scale) may look no different from a normal breast. Grade II also tends to look generally normal but feels a bit firm. Grade III capsular contracture will begin to look abnormal and have some firmness under the breast tissue. The breast may appear constricted, forced into an unnatural shape, or fixed in one spot on the chest wall (instead of relaxed and malleable like a normal breast). Grade IV capsular contracture (the severest grade) will have the most noticeable distortion.

What Causes Capsular Contracture in Breast Implants?

Capsular contracture occurs for largely unknown reasons. I tell my patients that there is no blood test or physical exam finding that gives them a higher risk. Even more perplexing is that most contractures only occur on one breast even though both breasts have the same implants.

What Helps Prevent Capsular Contracture?

Capsular contracture is difficult to predict or prevent. You may reduce your risk of developing it by selecting an experienced and board-certified plastic surgeon for your breast augmentation procedure. Lower occurrence rates are seen when the implant is placed under the chest muscle instead of just under the breast tissue, using an incision at the breast crease (inframammary approach) instead of around the areola, as well as avoiding smoking or taking medicines that thin the blood. Following your doctor’s recommendations during recovery is the best way to minimize your risk of post-surgical complications.

What Are the Chances of Getting Capsular Contracture?

One scientific literature review indicated that the capsular contracture rate affects 10.6 percent of patients. Since 2011, the risk in my patient population ranges from two to five percent. The risk does vary depending on the implant you choose. The new ‘cohesive’ silicone gel implants lower the risk as do textured silicone implants. Also, your chance of getting capsular contracture will be higher if you have already had capsular contracture in the past. Your likelihood will be lower if you follow the recommendations for prevention and if you are a naturally good healer.

When Is Capsular Contracture Most Likely to Occur?

Capsular contracture can occur at any point after breast augmentation and without any warning. Most cases are likely to take place within the first two years after breast augmentation.

Can You Treat Capsular Contracture Without Surgery?

Patients with mild cases of capsular contracture (Grade I or possibly Grade II) may not need corrective surgery. If there is no visible distortion and you are not bothered by your capsular contracture, you may choose to ignore it. In regard to nonsurgical treatment for capsular contracture, it is thought that some substances such as anti-inflammatory medications or vitamin E supplements may help soften the scar capsule. Patients should discuss their options with their doctor before taking any medications or supplements. Most women with capsular contracture (especially Grade III or Grade IV) will likely need to undergo breast revision surgery.

How Is Capsular Contracture Treated?

Capsular contracture treatment is labeled as a ‘breast revision surgery.’ These are secondary procedures that can correct a number of complications after breast augmentation. Specifically, your surgeon can either release the tight scar tissue (capsulotomy) or remove the scar tissue (capsulectomy), both softening the breast. Usually, at the same time, a new implant is placed. These options will resolve capsular contracture and improve the appearance and feel of your breasts.

Saggy, shapeless breasts are the enemy of alluring, feminine beauty. If your breasts have begun to sag, there are several things you can do to maintain a youthful appearance. These steps include breast lift exercises, wearing the right bra, and considering breast lift surgery if your breast sagging is more severe.

Do Breast Lift Exercises

Breast sagging occurs when the connective tissues of the breasts become weak and lax. Strengthening and toning this area of the body can firm and lift sagging breasts. Some examples of good breast lifting exercises include:

Chest Presses: While lying on your back, hold a dumbbell in each hand and bend your elbows until your hands are at the level of your chest. Slowly straighten your arms so that your hands with the dumbbells in them are in the air directly above you, then retract your arms to their starting position, with your hands nearly touching your chest. That is one rep. Repeat eight to 15 times for one set, and do multiple sets if desired.

Push Ups: Face the ground and put your hands on the ground, slightly wider than shoulder-width apart. Straighten your arms. Extend your legs behind you, keeping them straight, and set your feet shoulder-width apart or closer (even touching). Imagine your body is a straight line extending from your head to your heels. Maintain this line while bending your elbows to a 90-degree angle or smaller, keeping them tucked close to the body. After a slight pause at the bottom, push your arms back up to your starting position. That is one rep. Do eight to 15 reps for one set, then repeat as desired.

Suspension Bands: Perform strength training exercises such as bicep curls, presses, chest/arm flys, etc. using suspension bands instead of free weights.

Each of these exercises works the pectoral muscles, which will strengthen their support of the breast tissue. With stronger pectoral muscles, the breasts tend to be more naturally lifted.

Wear the Right Bra

Wearing the right bra begins with knowing which bra is right for you. Get professionally sized for both your sports bras and your everyday bras, as you want the best fit for each of your preferred activities. It’s a good idea to be sized once every year or so (more often if you experience weight fluctuations or pregnancy), as your breasts will change over time. Remember that a quality bra may be expensive, but the better support your breasts have, the better off they will be. Be sure to have several bras that you cycle through and never wear them multiple days in a row, as this can overextend the elastic. Replace your bras every six months to one year, especially if you only have a few.

Consider Getting a Breast Lift

The above resolutions can help reduce breast sagging, but their impact will be relatively minor. For a more dramatic improvement, consider getting a breast lift. A breast lift is a surgical procedure that can significantly improve the position, shape, and youthful perkiness of the breasts. If your breasts are noticeably sagging and the above measures are insufficient, this surgery can help. A breast lift repositions the breast tissues higher on the chest, removes excess skin, and reshapes the breasts so that they are firm, round, and lifted. This surgery can restore a lifted, youthful breast appearance that is flattering and feminine. Many women even get breast implants with their breast lift to further enhance breast fullness, firmness, shape, size, and position.

Depending on your needs, one of these two procedures could be the perfect way to help you achieve your desired body shape. The following pros and cons of CoolSculpting® and liposuction can help you decide which treatment is best for you.

CoolSculpting®

CoolSculpting® is a nonsurgical procedure that removes fat from the body over a period of time. During the treatment, controlled cooling technology freezes the fat cells. Over time, the body flushes the destroyed fat cells away. Results are unnoticeable at first but improve steadily, with the final results being seen several months after the treatment. The results are not as complete as liposuction, but CoolSculpting® is a good choice for someone who does not want to go through a surgical procedure or is not a candidate for surgery/anesthesia.

Pros:

Permanent fat removal

Targeted body sculpting (but with limitations)

Nonsurgical (no incisions)

No downtime

Relaxing treatment with minimal pain

Side effects are usually minimal

Cons:

Results take a while

Treatment areas are limited to the abdomen, love handles, hips, thighs, and neck

Results are subtle (may require multiple treatments)

Liposuction

Liposuction is a surgical procedure that removes fat from the body for immediate results. During the treatment, fat cells are suctioned directly out of the body. The results are instant and improve steadily as the body contours smooth themselves out, with the final results viewable just a few months after treatment. This method is able to remove more fat cells than any other nonsurgical method.

Pros:

Permanent fat removal

Targeted body sculpting (anywhere on the body)

Results are immediately noticeable

Scarring is usually minimal to nonexistent (only requires small incisions)

Very high safety record

Cons:

A surgical procedure that requires an anesthetic

Recovery takes several days to several weeks depending on how many areas are involved

Can cause pain or discomfort

For both CoolSculpting® and liposuction, patients should have realistic expectations and be at or near their goal weight before receiving treatment. Fat removal is permanent with both treatments, and a healthy lifestyle is the best way to maintain the results.

May is when spring begins to turn into summer. It is also Skin Cancer Awareness Month. Both of these reasons are a perfect reminder to take the time to focus on ways to keep your skin healthy and prevent skin cancer. With the following tips and expert skin care products and treatments, you can maintain healthy skin, protect your skin from the sun, and lower your risk of developing skin cancer.

What Is Skin Cancer?

Skin cancer is when abnormal skin cells grow at uncontrolled rates, typically because of UV radiation from the sun. Unrepaired DNA damage to skin cells can trigger mutations that lead to rapid multiplication. These genetic defects can form malignant tumors that threaten your health and even your life.

Skin Care Prevention Tips

Tip 1: Reduce Sun Exposure and Wear Sunscreen

Ultraviolet rays from the sun are the primary cause of skin cancer, so you should reduce sun exposure as much as possible. Wear broad-spectrum (UVA and UVB) SPF 15 or higher sunscreen on a daily basis. During outdoor activities (especially at the pool or beach), wear broad-spectrum SPF 30 or higher sunscreen and reapply at least every two hours. Stick to the shade during the hottest parts of the day, and avoid burning. Wear sunglasses, a hat, and clothing that shields your skin when you spend extended hours outdoors.

Tip 2: Avoid Tanning

Avoid tanning and tanning booths. Since UV rays are a known carcinogen, both are proven to increase the risk of cancer.

Tip 3: Examine Your Skin

Examine your skin from head to toe once every month. Get a professional skin exam from your personal physician once a year. These habits can catch suspicious and precancerous skin lesions early on so that they can be removed and protect you from developing skin cancer.

Skin Care Treatments and Products

Keeping your skin healthy isn’t just about preventing skin cancer. By maintaining healthy skin, you can retain a more youthful appearance. The following products and treatments offered at our practice can revitalize your skin, treat sun damage, and protect against future skin damage.

EltaMD®: broad-spectrum sunscreen protects the skin against harmful UVA and UVB rays as a defense against skin cancer and premature aging.

Intraceuticals: skin care products protect against dehydration that is caused by sun damage and aging.

The endoscopic brow lift (also called an endoscopic forehead lift) is a procedure that has advanced beyond traditional brow lift techniques to produce high-quality results without a painful recovery and associated scarring.

What It Does

Brow lift surgery differs from facelift surgery and eyelid surgery in area of focus as well as technique. Facelift surgery rejuvenates the mid and lower face, while eyelid surgery rejuvenates the eyelids and the area directly around the eyes. The brow lift, on the other hand, elevates the brows and smooths out fine lines and wrinkles that appear on the forehead or between the brows. This creates a more youthful and rested appearance. Often, brow lift surgery is combined with eyelid surgery and/or facelift surgery to rejuvenate the whole face.

How the Procedure Works

For the endoscopic brow lift, the patient is placed under general anesthesia. The procedure requires only a few very small incisions hidden behind the hairline. Through these small incisions, Dr. Spies can perform the brow lift procedure endoscopically without risking visible scars or hair loss. Through one incision, Dr. Spies inserts an endoscope, which is a small tool with a tiny camera attached to a monitor to give the surgeon a clear view of the underlying tissues. Then, Dr. Spies inserts surgical instruments to lift and alter the brow and forehead tissues. Dr. Spies removes tissue where necessary and repositions the remaining tissue to create a smoother and more lifted brow and forehead. The procedure takes careful finesse and usually lasts less than two hours. Because of the use of an endoscope, recovery is far milder than after traditional brow lift surgery. Patients can achieve worthwhile and rewarding results with a more youthful and rejuvenated appearance by undergoing endoscopic brow lift surgery. Results typically last for many years.

The endoscopic brow lift improves the position and appearance of the brows and forehead to rejuvenate the upper face. Dr. Robert Spies performs endoscopic brow lift surgery and other plastic surgery procedures to minimize signs of aging for patients from all walks of life.
