Why We Look Older In Winter

Dry skin shows the lines of age more. Except for those with extremely oily skin, as the cold, dry days of Winter come, we must make modifications to our daily home skin care routine to accommodate for the moisture that the dry air pulls from our skin. We must not only contend with the cold, dry air outside, but being inside with forced hot air heating dehydrates skin even further. We loose moisture through our full body surface area. In the summer months, perspiration makes us more aware of how quickly we are losing moisture. In winter, moisture is actually pulled from our skin without us realizing it.

It is a common misconception that we can hydrate our skin by drinking more water. While water is very important for our bodies’ function, oral hydration does very little to moisturize skin. Skin must be moisturized topically. It is important that your moisturizer penetrate your skin appropriately, instead of sitting on the skin’s surface. Products that sit on the surface not only prevent the moisture from penetrating to the depth of your skin where it can be utilized, but will also clog your pores, causing enlarged pores and breakouts.

Another problem that must be dealt with in winter is an increase in the number of dead skin cells left on the surface of our skin, giving it a dull, aged appearance. Just as dry skin shows fines lines more, a build up of dead skin cells actually increase the depth of fine lines. It always amazes me how the medical community expands the use of products that began with very limited use. For Instance, a machine that was developed and used only for separating neurons in a surgical procedure, can now be used to remove moles without bleeding or scarring. A procedure that was developed for treatment of facial skin cancer is now used to treat sun damage, cure acne, and give skin a more youthful appearance.
When Botox cosmetic was first introduced to the medical aesthetic community, it was approved and used only for the frown lines between the brows. Doctors then experimented with it on the forehead and crow’s feet but it was always considered a treatment for the upper face only. Any area below the nose was thought to be a danger zone. But of course we continued to experiment and now realize that so much can be done, safely with Botox in the lower face. It can be used to stop the drag on the corners of the mouth that create the appearance of an eternal frown. It can stop the down turn of the tip of the nose that comes with age. It can stop the “golf ball” appearance of the chin that also comes with age.
Perhaps the most innovative and exciting new use for Botox is for the treatment of Rosacea. The red flush of rosacea that often remains even after laser treatments can now be treated with Botox. With often very small doses of Botox, the flushed appearance on the nose or cheeks resolves and stays gone for the 3 to 5 month period that Botox is effective.
When having Botox treatments, it is important to be aware of how many units are being used for your treatment area. Treatments will vary in price, efficacy, and longevity based on how concentrated the Botox solution is. If you are always comparing the number of units used as opposed to volume or area treated, you will never be disappointed with your result.

If you are like me, you are a bit skeptical when new technology is launched. Just over a year and a half ago, the FDA approved laser lipolysis as a new technology available to the general public. This is a laser that melts fat, giving you results similar to traditional liposuction without the recovery time, or the cost associated with the old method. I will admit, I was skeptical. I went to an informational conference to learn more about it. It was not until I went to some hands on training that I was convinced. This is truly one of those miraculous breakthroughs with very little risk, very little discomfort and absolutely incredible results!

At Serenity Medical Spa we now have many patients who are more than a year out from their procedure and the results are amazing and permanent! I was one of the first physicians in this area to be trained on this procedure and the results we are seeing are fantastic. We have patients who have lost more than four inches around their waist, two to four inches around each of their thighs, two inches around their upper arms! All with a one-time office based procedure with no general anesthesia and only one day of downtime! Saggy, Baggy Eyes

In the past, as the area below your eyes sagged or sunk, the only answer was to have a lower eyelid lift. The excess skin and the fat pad below it were removed. The problem that arose over time is that as we age, we need that fat pad to maintain fullness in our face. So if it is removed in our forties or fifties, by the time we get to our sixties, our eyes look sunken and tired.
With the advances in laser technology, we can now make a definitive change in the lower lid without the risk of surgery or this negative future outcome. We start with a laser procedure that will peel away the fine lines and wrinkles of the lower lid as well as tighten the fibers deeper in the skin. There is a three to five day recovery period during which an ointment is worn on the skin to allow it to heal in a moist environment. After that, the skin is smooth and well healed. The effected skin may be slightly pinker than your natural skin tone at this point, but make up can now be worn to cover it. At your one week follow up appointment, we will inject some Restylane, a filler made of hyalluronic acid, into the area just below your tear duct. This enhances the smoothness and creates fullness below the eye that is more reflective of a youthful appearance. In some cases we will recommend Botox for the frown lines between the brows as a final step in smoothing out the stern look of the eye area.

Finally Skin Tightening That Works!

For many years there have been various methods of skin tightening available that have shown little to no results. Common brand names are Thermage, Affirm, and Titan. While the technology has always intrigued me, the process is known to be quite painful, expensive and result in little to no visible improvement in as much as 70% of the patients treated. I have always thought that this was a procedure that, once perfected, would be phenomenal. Finally, a company has perfected it!
The procedure is called Pelleve and uses a constant delivery of radiofrequency energy to the deeper layers of the skin to denature the collagen fibers. This turns the tissue into an almost gelatinous substance that is then chilled, causing instant contraction! The tissues then stay contracted! The best analogy is that of cooking the protein of an egg white. The egg has changed from a gelatinous raw egg, into the firm white of a cooked egg. The egg does not then become “uncooked” after it cools!
The skin temperature is closely monitored during the procedure such that the appropriate temperature for collagen denaturization is achieved. This target temperature is below the threshold for pain, so this procedure feels like a warm massaging of the skin! This is one of the major difference between this procedure and the older methods of skin tightening. Previously a set dose of radio energy was delivered resulting in either not enough heating, therefore no tightening, or too much heating, therefore visible retractions in the skin. Adding to the benefits of this remarkable procedure: the results are visible immediately, there is no necessary recovery time, patients can go back to normal activity and their daily skin care regimen immediately.
Though results will be seen immediately with one treatment, it is recommended to do a series of three treatments, at intervals of 4 weeks. This allows for the first treatment to “take up the slack” in the skin, then achieving the maximum tightness in the subsequent treatments. After that initial series, touch ups are done at 15 to 20 month intervals.

Get Rid of that double chin!

The chin and neck have long been a difficult area to treat cosmetically. As we lose volume in our face, the skin will frequently sag below our chin giving a loose, aged appearance. One should take care with prevention. Any topical treatment you might use on your face should be applied to your chin as well, in an upward sweeping motion.

In the past, we were limited to neck lifts or traditional liposuction in this area to achieve any noticeable difference. These procedures are quite invasive and carry all the risks associated with general anesthesia. Once again, technology has come up with a new advance that is minimally invasive for this hard to treat area. Using the New ProLipo laser, we can now melt and remove the excess fat that has accumulated in that area. The advantage of the ProLipo laser over the better known Smart Lipo laser is that ProLipo will also treat with a second wavelength that will tighten the skin. The entire procedure is done while you are awake, using local anesthesia and no discomfort. You can return to normal activity the next day wearing a wrap to support the area. After two to three days, any minor swelling has resolved and you are able to see an immediate result from the removal of fat. Over the next several weeks the tightening results become more and more apparent, as the skin around your chin becomes more adherent to your jaw line.

This procedure can also be used on other, difficult to manage body areas, such as arms, abdomen, flanks, saddlebags, thighs and buttocks. Any area with excess fat that has not responded to diet and exercise, responds wonderfully to this state of the art technology
