<p>Dr. Long: Same thing, before the teeth come out.</p>
<p>Charlie: Before they come out. (meaning come out from the gums into the mouth, not fall out for the tooth fairy)</p>
<p>Dr. Long: Yes.</p>
<p>Charlie: So we get used to it, get the child used to it.</p>
<p>Dr. Long: Yes, because by the time the teeth…</p>
<p>Charlie: And we’re not talking about with a toothbrush?</p>
<p>Dr. Long: It could be…</p>
<p>Charlie: It could be?</p>
<p>Dr. Long: If there are no teeth then obviously the cloth and the finger.</p>
<p>Charlie: Okay.</p>
<p>Dr. Long: And once the teeth…</p>
<p>Charlie: Clean hands as well.</p>
<p>Dr. Long: Yes, clean hands. Once the teeth are out then you continue with the cloth and introduce the floss and the toothbrush.</p>
<p>Charlie: A soft toothbrush.</p>
<p>Dr. Long: Yes and the reason why we want to brush or clean the gums before the teeth come out is also getting used to going in there and really for the child to get used to having somebody brush his teeth and putting something in his mouth.</p>
<p>Charlie: And get mommy and daddy used to going in there to do it.</p>
<p>Dr. Long: Yes because it’s not easy for sure it’s very difficult. It’s a struggle but if we’re consistent.</p>
<p>Charlie: I’ve found with our grandchild and again, I’m a new granddad so my wife has always been the one to give the bath when Beckett stays overnight with us, so I’m in there helping and I take him with a towel. Well the first time he had it he screamed his head off.</p>
<p>Dr. Long: Sure.</p>
<p>Charlie: The second and third one, he loved it. So, this is reminding me that once they get used to it, they like what’s going on here.</p>
<p>Dr. Long: And they tell you how to do it.</p>
<p>Charlie: Yeah</p>
<p>Dr. Long: If you’re not doing it right…</p>
<p>Charlie: They let you know.</p>
<p>Dr. Long: They remind you that you missed a spot.</p>
<p>Charlie: So we need to start brushing before those teeth come in.</p>
<p>Dr. Marc: Here we are: When Should I Start <strong>Brushing My Baby&#8217;s Teeth</strong>? Dr. Julie we’ve talked about the importance of starting right away, there are so many things for a new mom to have to know, is there any kind of benchmark or trick of the trade to suggest as to when it make sense to start actually introducing the brush as opposed, we started with the wash cloth wiping the gums and then the teeth, how does a new mom know okay now it’s time to buy the toothbrush?</p>
<p>Dr. Julie: I would say when you clean the teeth that the cloth is not reaching all the way you want to reach.</p>
<p>Dr. Marc: Okay.</p>
<p>Dr. Julie: When you feel comfortable enough using the toothbrush. For sure if you’re not used to it, mom’s can be reluctant to sticking something hard…</p>
<p>Dr. Marc: Right because you’re finger at least you have…</p>
<p>Dr. Julie: Yes you have the full control.</p>
<p>Dr. Marc: You have tactile feedback.</p>
<p>Dr. Julie: That’s why the program is there for her as well. We’re going to teach you and show you how to do it. We have certain tricks and techniques and things that we’ve developed through time that we use on all children but there’s no…</p>
<p>Dr. Marc: So there’s no hard and fast rule.</p>
<p>Dr. Julie: No, but the minute they start by the age of two or three when they have most of their baby teeth in they should start using a toothbrush because there is so much you can achieve with the cloth and your finger. Floss as early as possible that you’re comfortable using it.</p>
<p>Dr. Marc: Even flossing as early as possible.</p>
<p>Dr. Julie: Yes. Again it’s all picking up good…</p>
<p>Dr. Marc: Habits.</p>
<p>Dr. Julie: Yes, good habits.</p>
<p>Dr. Marc: Routines.</p>
<p>Dr. Julie: They’ll get used to it and often, my kids won’t to forget to floss their teeth, they’ll say daddy you didn’t floss my teeth.</p>
<p>Dr. Marc: That’s when you know that you have instilled good habits.</p>
<p>Dr. Julie: Yes.</p>
<p>Dr. Marc: And I know that in your program you use the twos as a good memory device as well as the development, so at two teeth the child is coming in. When you have the mom bring in the child at two teeth are you in the office showing them how to use a toothbrush at that point?</p>
<p>Dr. Julie: Yes we can do it with a toothbrush; we can do it with a cloth as well, whatever they feel more comfortable.</p>
<p>Dr. Marc: So that may be another incentive for really bringing the child in at two is that you could actually help teach them how to make that transition.</p>
<p>Dr. Julie: They have to be comfortable. We can clean the baby’s teeth when they come in but the teeth need to be cleaned every day.</p>
<p>Dr. Marc: Right.</p>
<p>Dr. Julie: So mommy or daddy whoever’s in charge of doing it, they need to be comfortable doing it because they will be providing that care for the baby every single day.</p>

<p>Dr. Long: Flossing is definitely important. You have to floss your child teeth.</p>
<p>Charlie: Get between and get all of that…</p>
<p>Dr. Long: Yes, so whether it’s only two or twenty teeth, it’s the same. Flossing reaches where the toothbrush cannot reach and more important than that is, the baby teeth are practice for the adult teeth so it’s practice for the parents to take of the baby’s teeth but also the baby will learn by feel how it’s supposed to feel when they’re being floss and when their adult teeth come in and they’re able to do it themselves, then they know how it feels and that way it’s a lifelong skill and absolutely we need to remove that bacteria that doesn’t belong there.</p>
<p>Charlie: It doesn’t matter how many teeth, start flossing now, get them in the habit of doing it, take away that bacteria, dislocate the plaque.</p>
<p>Dr. Long: Yes.</p>
<p>Dr. Marc: Just to be clear and it may sound like a silly question, but people have silly questions, we talked about dental care before teeth show up, is there anything that you would do with floss before a tooth has erupted or is that purely a treatment for a tooth.</p>
<p>Dr. Julie: You need teeth to floss.</p>
<p>Dr. Marc: You don’t floss your gums.</p>
<p>Dr. Julie: You may practice on yourself to be skillful in using the floss. But the purpose of the floss is to reach where the toothbrush cannot.</p>
<p>Charlie: Between the teeth.</p>
<p>Dr. Marc: Between the teeth. With baby teeth sometimes there are spaces so it’s probably more forgiving. Some kids, like my children, all their baby teeth were tight so we had no choice we absolutely had to floss or else it wouldn’t be forgiving. They would have had a lot of cavities between their teeth if we didn’t floss them. Again, it’s just instilling those good habits early on. My daughter, what she likes to do, she likes to floss herself. She’ll take a piece of floss and I’m flossing for her then she’ll hold my fingers and direct me to flossing her teeth. When I see that it makes me very proud because I know that hopefully from now and on flossing will always be part of her oral hygiene habits.</p>
<p>Dr. Marc: I guess it’s a good sign if your daughter is correcting the dentist on how to floss her own teeth.</p>
<p>Dr. Julie: Yes.</p>
<p>Dr. Marc: That’s a good sign, job well done.</p>

<p>Charlie: This sort of sounds like a disease the way this question is asked: <strong>What Are Symptoms Of Teething</strong>? and what can I do about it? Isn’t this when we go to the liquor cabinet, get a little something and we put on the gums?</p>
<p>Dr. Long: No.</p>
<p>Charlie: That’s not the way we’re supposed to do it, okay. I haven’t gotten to that yet with my grandson so I won’t do that.</p>
<p>Dr. Long: Teething is basically when the baby tooth is trying to break through the skin and come into the mouth.</p>
<p>Charlie: Okay.</p>
<p>Dr. Long: The symptoms are a little bit of swelling of the gums, a bit of blanching of the gum because the tooth is forcing its way through the gum&#8230; it can be a little bit painful the child may start crying, avoid food.</p>
<p>Charlie: A lot of saliva?</p>
<p>Dr. Long: A lot of saliva and putting stuff in their mouth and having a tendency to bite on hard surfaces just to help that tooth cut the gum so it can come out. That’s why cleaning baby’s gum is a good way to stimulate it and speed it up because having the tooth erupting through the gum is not very pleasant so if we can massage the gum and push it down on the tooth and help cut it faster. It heals very, very well. I often brush my own kid’s teeth and have seen a little bit of blood on the cloth and my child is not crying and to me it’s like wow that’s a good thing because now the tooth is erupting and my child won’t need to go through the painful stage. Fever is not really common but sometimes there’s that misconception that maybe the fever may be due to teething. Basically if there are any symptoms that are before three days of the eruption of the tooth and then sometimes after three days after the tooth has come in those are really related to the eruption but any symptoms that are before that but there are not teeth coming in, it may be something else and you may want to consult with your physician. If you have a question we’re here to help. Just give us a call, drop in and we’ll take a look and we’ll see. But it’s a natural process.</p>
<p>Charlie: Do you ever recommend anything like Oragel or anything like that or do we want to stay away from that?</p>
<p>Dr. Long: I’m the type of guy that avoids medication as much as possible but of course if there’s any doubt and you think that your child is going through a painful stage then consult with your physician and perhaps a little medication can help alleviate the discomfort. Remember it’s a natural process that will happen no matter what, so whether they’re in pain or not it’s going to pass and it’s the way teeth erupt and it’s just natural.</p>
<p>Dr. Julie responds to: <i>What Are Symptoms Of Teething</i>?</p>
<p>Dr. Marc: Dr. Julie you were kind of enough to give me a little cheat sheet here so I’m going to use my cheat sheet. There are some interesting ones here. Other sort of signs and symptoms that teething may be happening or about to happen. One that I would have never actually thought of is rubbing of the ears as well as cheeks?</p>
<p>Dr. Julie: It’s irritating to them. So it’s itchy. The kids try to do something. They grab the ear, they grab the cheek.</p>
<p>Dr. Marc: I see.</p>
<p>Dr. Julie: They try to take away that discomfort from their jaw.</p>
<p>Dr. Marc: They’re just reacting.</p>
<p>Dr. Julie: Yes, they’re reacting to it</p>
<p>Dr. Marc: Okay.</p>
<p>Dr. Julie: Generally speaking it shouldn’t be persistent symptoms if it’s something that you’ve noticed that baby’s been going through for more than a couple weeks just make sure that is not another underlying condition that they may have.</p>
<p>Dr. Marc: Something like refusing food would be an important one to pay attention to.</p>
<p>Dr. Julie: Yes refusing food is they’re not comfortable, they’re itchy, the area is tender they may not have as much appetite. Again just make sure there are no other medical conditions that they could have. That’s not related to the teeth. Remember teething is a natural process the symptoms should not be severe.</p>
<p>Dr. Marc: And they should not last a long, long time.</p>
<p>Dr. Julie: They should last a long time, so if they have a persistent fever, consult your physician to make sure they’re not sick or have any illnesses. If you suspect teething, again, make sure you wipe those gums and you inspect the mouth. Do you see the swelling area where the teeth will come in. If everything looks fine just make sure you rule out any other causes for those symptoms.</p>

My daughter likes it to be a little cooler. I’m still used to testing it here so I can’t feel it but this question here, I don’t know how I will do this when he starts to get a couple teeth and he’s on the soft food. It says: <strong>Why Can’t I Share Food With My Baby</strong>? If I’m warming any of that food I would think I would taste it a little bit. Why would I not share food? Tell me.</p>

<p>Dr. Long: Obviously transmission of bacteria is possible from mommy’s saliva to your baby. There are different ways of testing food for example, just take a new spoon every time. There will be more dishes to be done but…</p>
<p>Charlie: worth it for the child.</p>
<p>Dr. Long: It’s totally worth it. With dishwashers it just makes things easier. That’s the main thing. We don’t want to give bacteria any chance to travel from mommy to baby. Even if you do it and if you have good hygiene you’re still reducing that chance. We understand that sometimes you’re at the mall and the pacifier falls on the ground what do you do? You don’t have a water source. Mommy just puts it in her mouth and gives it to baby. Which is usually done and it’s a common thing, I’ve seen it many, many times. That’s why mommy’s hygiene is also important.</p>
<p>Dr. Julie responds to: <i>Why Can’t I Share Food With My Baby</i>?</p>
<p>Dr. Marc: Dr. Julie what else do we need to know about this or have we covered it?</p>
<p>Dr. Julie: Just keep in mind whatever you do will affect your baby so keep things as clean as possible. People don’t often think that they will transmit the bacteria to the baby. Again, just practice good oral hygiene and just keep in mind that whatever you share with the baby you make sure you don’t transmit those bacteria to them so don’t share the food, don’t lick the spoon, keep toys as clean as possible and keep yourself in good oral health.</p>
<p>Dr. Marc: Just to be clear if I’m hearing your right. It’s not just that we’re saying that if you’re sick or have a cold or you know that you’re ill, so even if you’re feeling healthy is it still dangerous to transmit bacteria?</p>
<p>Dr. Julie: Generally speaking the same way you learn how to wash your hands, whether you’re sick or you’re not sick you still wash your hands before you eat. Wash your hands before you prepare food. The same rules apply here; you don’t wait until you’re sick so that you don’t touch baby’s food, but just do it on a regular basis. It’s just good practice&#8230;</p>
<p>Dr. Marc: I could have bacteria in my mouth that isn’t making me sick that might actually lead to the child developing gum disease or cavities or something like that?</p>
<p>Dr. Julie: Prevention is the key. It doesn’t hurt, it may not be as practical sometimes, but you feel like okay let’s just test the food quickly, but prevention is the key. Try your best to minimize the amount of bacteria you transmit to your baby.</p>
<p>Dr. Marc: Okay.</p>

<p>Dr. Long: Yes, absolutely. Children learn what they live.</p>
<p>Charlie: They learn what they live.</p>
<p>Dr. Long: If they see their parents taking good care of their teeth, they look up to us as role models they want to be like daddy, they want to be like mommy. That’s why we have all these little toys like shopping carts…</p>
<p>Charlie: Right.</p>
<p>Dr. Long: Miniature kitchen sets and kids want to copy their parents. So that’s number one. Number two is if the parent’s are healthy then they will not transmit those bacteria to their kids. When it comes the time to brush their baby’s teeth you can always say, look daddy is brushing his teeth so now it’s your turn and it becomes a good habit and it becomes normal. It’s part of the culture in the house.</p>
<p>Charlie: Too funny you say that. I still remember this one time when my daughter was there watching me shave and I was putting all the lather on and she wanted it too. We have this great photograph of her with the lather. She did not go on to be a shaver at that point, but in any case she did, as you were saying, model what was going on by the parents and her taking care of her own teeth because mom and I focused on that enough to have her know that it was something really important.</p>
<p>Dr. Long: This is beyond the dentistry; this is simply about giving your child the best head start possible. They learn by seeing what you do if they can see that this is what you’re supposed to do…</p>
<p>Charlie: Right.</p>
<p>Dr. Long: They’ll keep doing it and that’s how they stay healthy.</p>
<p>Dr. Julie Dao replies to: <i>Why Must Mom Take Care Of Her Own Teeth</i>?</p>
<p>Dr. Marc: Dr. Julie you’ve mentioned a couple of times that one of the hard things about being a new mom especially is this tendency to be so focused on the kids and have so much to do to focus on the kids that you end up neglecting yourself. Wouldn’t you say that also runs the risk that you’re setting that example for your kids that you’re not taking care of yourself? Is that part of the important of being a good role model?</p>
<p>Dr. Julie: Yes, we always put that on our shoulders to be a good role model for our children. We tend to not think about the dental part but definitely that would be a component of it, setting the good example for them. Also keeping yourself healthy so that you can take care of your children. My daughter once in a while will remind me if she sees me getting ready to go to bed saying, mommy did you brush your teeth? Did you floss?</p>
<p>Dr. Marc: great.</p>
<p>Dr. Julie: They know that we’re about to brush their teeth and floss for them that its bed time.</p>
<p>Dr. Marc: Fair is fair.</p>
<p>Dr. Julie: I find it very cute to see whenever she asks me about that. You want to be the best role model we tend to forget the dental part, but that’s what the program is there for. To set up those guidelines and to be reminders of what stage we need to keep up with.</p>

<p>Charlie: Those of us who are parents, and I know that includes both of us, realize that no two children are the same. I think this question comes from a rather frustrated mom at this point: <strong>My Child Won’t Let Me Brush His Teeth</strong>, what should I do?</p>
<p>Dr. Long: Obviously, be consistent is the most important.</p>
<p>Charlie: Keep doing it.</p>
<p>Dr. Long: Yes, just like changing diapers, just like bathing, just like getting your husband to take out the trash, be consistent, that’s all and in the end that’s how people learn. When we feel like we don’t have a choice we just have to do it and the important thing here is to be gentle and not to put any force, not to pin down the child and make it a negative experience. You just simply let him know that we are going to clean your teeth whether you like it or not and if you don’t succeed today, try again tomorrow and tomorrow and every day and never give up. That’s very important.</p>
<p>Charlie: I know right before a nap would be a terrific time to do that but if we find in that that regularity that the child doesn’t cooperate then, it’s earlier in the day and you go in with a clean hand to help with the gums or to do the teeth, at least we get into the habit of that and the child gets to know what’s going on, right?</p>
<p>Dr. Long: Yes.</p>
<p>Charlie: Okay.</p>
<p>Dr. Julie Replies to: <i>My Child Won’t Let Me Brush His Teeth</i></p>
<p>Dr. Marc: Dr. Julie I would imagine, I don’t know if this happened with your own kids but certainly your patients kids, you’re going to have situations where somebody is just not a happy camper no matter how much you’ve tried to build the relationship so what do you do? How do you get that relationship in a more positive frame?</p>
<p>Dr. Julie: That’s why early prevention is so important, if you wait until they’re much older and they resist it’s going to be much harder for you to try to convince them. Fresh new baby’s teeth, do that very early on so they get used to it because that’s all they could remember I’ve always had…</p>
<p>Dr. Marc: They didn’t know enough to argue.</p>
<p>Dr. Julie: They didn’t but definitely brushing doesn’t have to be… Brushing the baby’s teeth doesn’t have to be a painful or traumatizing experience for the kid. Sometimes with the child it’ll be easier for one parent to do it than the other. See what works best for you but its repetition. Be constant about it, with kids, it’s like sleep training, they resist, they resist but eventually they’ll learn.</p>
<p>Dr. Marc: They’ll learn the pattern.</p>
<p>Dr. Julie: They’ll learn the pattern.</p>
<p>Dr. Marc: I think it’s great that you’re setting this pattern so early because you’re making it smooth and routine. I was joking that they don’t know to argue but they really don’t know to argue.</p>
<p>Dr. Julie: They don’t.</p>
<p>Dr. Marc: It’s just normal and one of the things you’re doing that’s a great community service. By doing these things early, because sometimes its mom or dad’s anxiety that makes the child resist, you’re actually helping mom and dad get comfortable so early on that they’re not then making their own child get nervous or anxious or rebel.</p>
<p>Dr. Julie: Eventually if they don’t let you brush their teeth and they do get a cavity how will we be able to fix that cavity for them?</p>
<p>Dr. Marc: Right.</p>
<p>Dr. Julie: So the brushing is really the basis of everything. Try to keep those teeth healthy so we can avoid future more complicated treatment on the child.</p>

<p>Charlie: I’ve often said to my own children: <strong>We see what we look for.</strong> Have you ever gone into a room and looked for something that wasn’t where it was supposed to be and so you leave and then twenty minutes later you find it and it was right next to something else? That’s because we weren’t looking for it there. I feel for this mother here who says: &#8220;<strong>I HATE The Dentist</strong>! I hate going to the dentist,&#8221; so of course her whole attitude, the whole thing that she is looking at, is that she hates going to the dentist. &#8220;I don’t think that my child will like it either. Okay. Why scare him at such a young age?&#8221; Because we’re talking about coming in so early. I’d like to reverse that around and hope that mother can see something new from this whole program that we’ve talked about. But I’ll put that question over to you because I think I know your answer already. <strong>Why take them at such a young age?</strong></p>

<p>Dr. Long: The reason why the mother was probably scared of the dentist, or hates the dentist, is the same reason why she has to take her child to the dentist at the young age to prevent the child from hating the dentist. </p>
<p>Charlie: Right.</p>
<p>Dr. Long: Normally, people hate the dentist because they had a bad experience.</p>
<p>Charlie: And they started so late and there was a problem and they had to drill or we had to pull or we had to…</p>
<p>Dr. Long: Yes, for sure and that’s where we get those nightmare stories. And we want to teach people that prevention is so much easier, so much more pleasant, and less expensive and it’s really the way to prevent transferring that fear of the dentist to their children. So even though you had a bad experience and you’re reluctant to go yourself you may transfer that fear onto your kid and say I don’t want my kid to go through that but the same reason if you take your child on a regular basis and we detect small things and they’re fixed without the needle without numbing, without pain, it’s pleasant, it doesn’t cost too much and that way it becomes pleasant for your child. Perhaps for yourself if you can see that, you know what, you’re already on the good path for good dental health then things are pretty good and they’re not that bad and perhaps you can change your own mindset about it. If let’s say if you’re unable to change, at least give your child that chance to experience the good side of dentistry. It doesn’t have to be negative what so ever.</p>
<p>Charlie: Yes, let’s turn this completely around because I know from my daughter, my younger daughter, she was afraid of dogs most of her life because mom reacted to a dog once coming up when my daughter was really young. We then happened to be trick or treating one time, we of course cleaned her teeth very well after the trick or treating Dr. Long, and she met this dog from a neighbor and she just loved the dog and she loved dogs from then on because that woman’s horizon expanded it for my daughter she could now see how to love the dog. So I, as a dad not as the dentist, but as a dad, plead with the mother who gave us this question. Please go early on so that the good set of habits, the good horizon, can be seen.</p>
<p>Dr. Long: As parents we have that power to break that legacy so it doesn’t have to be that way. </p>

<p>Dr. Marc: Absolutely, Dr. Julie it just occurs to me sitting here, and I can’t help it because I’m a psychologist, that there’s a whole other layer to this whole prevention program by bringing these children in with their mom’s early setting all these good habits, you’re actually preventing a kind of self fulfilling prophecy about what may have been bad experiences that someone else had growing up because they didn’t have the kind of program offered to them that you’re offering to your community, where you’re setting these good habits, so you’re actually preventing the next generation from having preconceived notions that dentistry is a painful thing that I have to fear.</p>
<p>Dr. Julie: It doesn’t have to be because if you have the tools and the knowledge and you know what to do to keep your teeth healthy then you’ll never have to experience negative appointments at the dentist. That’s why it’s so important to see those children early on and teach those children early on that dentistry is not scary and it doesn’t have to be scary and it’s fun and it’s just about taking care of your teeth so you can be healthy as well.</p>
<p>Dr. Marc: What’s awesome about that too is in the process of that you may actually break that pattern for the mom or the dad for their bad experience because they’re seeing that it doesn’t have to be that way.</p>
<p>Dr. Julie: Yes, hopefully for them as well because the more they fear, again, the more they will tend to neglect the dental health.</p>
<p>Dr. Marc: They’ll postpone.</p>
<p>Dr. Julie: The more they’ll postpone then the more real complication may occur.</p>
<p>Dr. Marc: Things becoming more advanced by the time they come in.</p>

I think we’ve come to the fourth one of these twos. <strong>Why Take My Baby To The Dentist</strong>? Why do I need to take my baby to the dentist at two years old? We already had the second trimester, two months, two teeth, and now two years old. <i>Why Take My Baby To The Dentist</i> then?</p>
<p>Dr. Long: At that time is when the molars are coming out.</p>
<p>Charlie: Ah.</p>
<p>Dr. Long: Dealing with two teeth is pretty straight forward. They’re right in front.</p>
<p>Charlie: We see them easily.</p>
<p>Dr. Long: No problem, now the back teeth come in and it’s like: wow what do I do? It’s all the way in the back, that’s when baby can bite and some kids are not cooperating. Between the two teeth to two years it’s a long time and if we are consistent by that time if we can succeed to instill that good habit then it’s smooth. But if it hasn’t been done it’s another chance for the dentist to help mommy succeed and that’s the goal because everybody’s different and for one child it could only take about a week before they get used to it, other children they just won’t have it and the parents may give up halfway. If we see them at two years old and then we see that there are any small issues, we can ask,  &#8220;well are you brushing the teeth?&#8221; If you are, then good, if not then we can help. What is it that is so difficult and we will help you overcome that barrier and that way we can make sure that the babies are healthy. At that stage, because the molars are in, we’re starting to get teeth contacting other teeth and there’s more chance of bacteria.</p>
<p>Charlie: And food being caught in there. So you’ve made it real easy with this <strong><em>2&#215;4 Early Prevention Dental Health Program</em></strong>. This would be the fourth one of those; we have the second trimester, two months, two teeth, and now two years.</p>
<p>Dr. Julie responds to: <u>Why Take My Baby To The Dentist</u>?</p>
<p>Dr. Marc: Dr. Julie we’ve arrived at two years. Is it the terrible twos when it comes to dentistry as well or not so much?</p>
<p>Dr. Julie: With kids it’s always the terrible something. It’s the terrible twos, the terrible threes, and the terrible four. But at two chances are the kids haven’t had any major dental problems yet and we can also make their first visit a happy one. If the child is cooperative enough we can then perform a first cleaning to them so that a visit to the dentist is not something to fear, it’s not scary.</p>
<p>Dr. Marc: It’s not painful.</p>
<p>Dr. Julie: It’s just something normal. </p>
<p>Dr. Marc: Right.</p>
<p>Dr. Julie: That’s the purpose of the second year visit as well. We want to project to them that dentistry is okay. They don’t need to fear anything and if we can make oral hygiene a good habit hopefully they will never have to fear either.</p>
<p>Dr. Marc: Right.</p>
<p>Dr. Julie: So the two year is very important, if there are any problems that may have started. If a cavity has started we can catch it very early on. Also treatment of those cavities can be easier.</p>
<p>Dr. Marc: Sure because catching it early is a less invasive procedure than if it becomes advanced and they wait until the child is three or four.</p>
<p>Dr. Julie: That’s the whole purpose of all this prevention if you treat a cavity small enough more than likely we will not be required to give them any freezing as well, making it as pain free as possible to the child. If they’re not cooperative then we’ll work with them but again, at that age it’s important that we meet with them.</p>
<p>Dr. Marc: Build the relationship. So it’s not critical that you intervene right there but just bringing them in any way you’re having a chance to interact with them and make them comfortable.</p>
<p>Dr. Julie: Yes, but if mommy has been successful in brushing the teeth and taking care of baby’s teeth chances are the kid may be very receptive to receiving treatment. Because they’ve been used to having someone look at their teeth or work on the teeth or brush the teeth so their first visit is just something very simple and it could be fun as well.</p>

<p>Dr. Long: Yes.</p>
<p>Charlie: Why do you say it’s an import time for mommies to take their children to the dentist?</p>
<p>Dr. Long: So those two first teeth are there and now you need to learn to take care of them because taking care of two teeth is much easier than taking care of twenty teeth. That’s the time when we should start to learn. Prevention is about learning to do something before, to prevent the problem. We don’t want to buy a car and then get our license, we want to get the license first, learn how to drive the car and then we buy the car. Same thing, we want to know how to take care of two teeth first before all twenty teeth come out. It’s much easier and we are teaching the child that something will be done to clean their teeth every night and we’ll be consistent about it and once all the other teeth erupt it becomes a good pattern, a good behavior, and it’s so much more fluid because the problem that we find is that the first appointment is a major appointment such as removing a loose tooth…</p>
<p>Charlie: Some parents wait too long don’t they?</p>
<p>Dr. Long: It’s not their fault because nobody told them and that’s the purpose of this program. We are trying to intercept all these problems as early as possible. That two teeth appointment is important because it’s something that’s different. There were no teeth and now there are two teeth. We want to make sure that mommy and the parents know how to take care of those two teeth very well and when the other twenty come out then we’re ready to go. Baby is used to it, mommy and daddy are used to it and it’s just so much smoother.</p>
<p>Charlie: And at this point mommy can show what she’s been doing to clean the teeth for you to either praise her or help her learn a little better way.</p>
<p>Dr. Long: Yes.</p>
<p>Charlie: And the nice thing about going into an appointment at this point is, this is not the time that a child goes and gets hurt. There’s not a cavity where there has to be a drill going on. He starts to like the dentist, wants to go to the dentist, wants to take care of the teeth properly and it’s a big help that way too.</p>
<p>Dr. Long: Yes, at this stage they may not be aware too much of the dentist and that’s why it’s also a good appointment for mommy. That’s when we can give all that information because it’s also about the diet of the child and what to avoid as well. How to brush and what to avoid and to make sure the baby is healthy.</p>
<p>Charlie: So, two teeth that’s an important time.</p>
<p>Dr. Long: Yes.</p>
<p>Dr. Julie Responds To <i>Why Take My Baby To The Dentist</i>?</p>
<p>Dr. Marc: Dr. Julie one of the things I really love about your program is you really are not only building good habits you’re also building your relationship that’s based on <em>positive</em>, rather than <em>scary</em> things or hurtful things. I hadn’t really thought it through that way but as I’m hearing this it really strikes me how important that is. I’m hearing two teeth but I’m just kind of curious, those first teeth when they erupt, is it usually two? Or is it common to have one tooth or three teeth all at once? Is two…</p>
<p>Dr. Julie: You may see one at first but they usually come in pairs.</p>
<p>Dr. Marc: Pretty quick? Okay.</p>
<p>Dr. Julie: Mostly the bottom will be the first ones to come in. Sometimes maybe one, sometimes maybe two, usually it’s to give the mom or the parents a chance to show them in person because it’s one thing to say, do this and do that…</p>
<p>Dr. Marc: Right to read it on a label or on a sheet…</p>
<p>Dr. Julie: But the reality is that let’s say the child is not behavior or it’s not allowing, they actually have to do it to be comfortable as well because a lot of parents may be reluctant to put something in the baby’s mouth. They haven’t started wiping the gums just for them to have a feel and be comfortable with that. That’s the best way to show them, it’s much easier to try to do that on a six or seven months baby as opposed to when they’re much older and the fight back with the arms and legs swinging.</p>
<p>Dr. Marc: Clearly it’s much easier and more positive and experience than when they’re coming in for the first time dealing with a problem instead preventions.</p>
<p>Dr. Julie: Yes, when the first two teeth have first erupted, there are no problems yet.</p>
<p>Dr. Marc: Right.</p>
<p>Dr. Julie: No cavities yet so this is why it’s so important to start it very early on.</p>
<p>Dr. Marc: Does anyone manage to do that? Get a cavity in their first two teeth before any other teeth. Somebody’s tried really hard to eat nothing but sugar? Could it be done?</p>
<p>Dr. Julie: No…</p>
<p>Dr. Marc: We wouldn’t encourage the competition.</p>
<p>Dr. Julie: We wouldn’t encourage. Definitely not, but bring them in the minute you start to see the two teeth. We recommend they come in so bring them in so we catch that, they’re still healthy and we can avoid so many problems and also remind the parents about certain types of diet, about certain foods that make the child more prone to decay. It’s a good opportunity to review that with the parents as well.</p>
