Charlie: I’m here with dentist Dr. Long Vo, we’re talking about the 2×4 Early Prevention Dental Health Program and I have a feeling we’re coming up to the second one because the question here is, For the First Dental Appointment: When should I take my baby to the dentist for the first time?

Dr. Long: Now, two months old is the answer.

Charlie: Two months old.

Charlie: So we’ve already talked second trimester in an earlier question, now we’re talking two months here after the child has been born. Is that when the First Dental Appointment should be scheduled?

Dr. Long: At that stage it’s important because that’s where we can take a quick look and see if anything is wrong in the mouth, such as the tongue being tied down and there’s not much that we do really for the baby just a quick check up but the appointment is really for mommy. To get mommy back on track because during pregnancy your diet may change, you’re not as strong and sometimes we don’t brush and floss as often as we’re supposed to and things can get forgotten. Now it’s just an appointment to make sure that mommy is healthy as well and that’s a good time for x-rays because mom hasn’t had x-rays for over a year perhaps. If there are any small problems we can detect them early and fix them early before they get bigger because if mommy is healthy and strong she can take better care of baby.

Charlie: So at two months after the child is born we’re going to have a quick look at the baby to make sure that there’s no problem in here and we’re also going to refresh things with mom and get her back into her brain also a check up for her to make sure that things are going okay.

Dr. Long: Yes. So the First Dental Appointment after the child is born is at two months.

Dr. Marc: Dr. Julie at two months after the birth of your child are things starting to settle down a little bit so that you can regain some sanity and some focus and can start thinking about getting your health back on track? Is that part of it? Is that why the First Dental Appointment is scheduled at two months?

Dr. Julie: Hopefully. It always varies during pregnancy, my first pregnancy it took me more than two months, my second pregnancy after the first day I needed to get out and get things back on track again. It definitely varies but usually by two months things have settled a bit. You get used to somewhat of a routine but what’s important here is also that we provide mommy with the right information and what to do with baby’s mouth.

Dr. Marc: Right because baby obviously has no teeth yet.

Dr. Julie: There are no teeth yet.

Dr. Marc: It would be easy to not think that there’s anything at all to do with them.

Dr. Julie: Yes you want to start taking care of the teeth much longer after they come in but also a lot of mommy’s don’t know when to start looking at the baby’s teeth. It’s like prenatal classes you give prenatal classes before the mom…

Dr. Marc: Prenatal, yes exactly.

Dr. Julie: Prenatal…

Dr. Marc: This is pre-tooth classes.

Dr. Julie: This is pre-tooth…

Dr. Marc: Pre-dental?

Dr. Julie: Pre-dental classes.

Dr. Marc: Okay.

Dr. Julie: To give them the tools and information to take care of the baby’s teeth when baby arrives.

Dr. Marc: Great.

Charlie: This is a difficult one, I know when my daughter first had our first grandchild and this one, this question that comes to us is: My baby can’t latch on during breast feeding, what can I do?

Dr. Long: That’s a very…

Charlie: It’s a sensitive one I know for a lot of mothers, especially those who are breast feeding.

Dr. Long: Now obviously if your child cannot latch on, it’s important for your child to get the nutrition and the food properly, so if they’re not latching on you want to make sure it isn’t affecting your child’s growth. By seeing a dentist or even a doctor at that early stage we can determine if the tongue is tied down to the floor of the mouth and if it is, if it’s an emergency and it needs to be dealt with at that time if it affects the nutrition of your child then perhaps we have to intervene and do the procedure, if there’s other ways of feeding and we can keep an eye on it until a stage where the child will be strong enough for us to go ahead with the procedure and go ahead and free that tongue. It’s not only the breastfeeding, it affects the speech and all that, it’s the future right. It’s important that as long as the nutrition is there, it’s okay we have some time but if your child is unable to feed and doesn’t get the nutrition for development because at that stage the brain is developing, the body’s developing. Nutrition is so important at that stage. If your child doesn’t latch on, it is a priority; we need to find a way to feed your child.

Charlie: What you’re raising, to me anyway as a new granddad watching my daughter go through this, is that it could be something with the baby, right? The baby who is breast feeding.

Dr. Long: Yes.

Charlie: Which is where you’re raising it from, that dental standpoint and that it may be something with the woman. My daughter happened to use a fairly inexpensive breast shield it went on top and the baby could then nurse directly from that and with my daughter it worked really, really well for her breast feeding.

Dr. Long: Yes.

Charlie: So there is another part to that?

Dr. Long: Yes.

Dr. Marc: And Dr. Julie anything else we should know or to add to this breast feeding issue here?

Dr. Julie: Latching is always an eternal constant issue when you become a mom for the first time. I remember there used to be nursing clinics, they would be teaching us proper technique but again just to have awareness there can sometimes be a condition related to the mouth that’s preventing the baby to latch properly.

Dr. Marc: So it’s important to get it checked?

Dr. Julie: Just have it checked. Make sure you consult your doctors and your physician because the most important thing is the baby. Is the baby gaining enough nutrition? If the baby does, we have time to look into to it, but just to bring awareness about it because I know my son didn’t latch properly the first couple days of breast feeding, they thought he might have a tongue issue related to that. The nurse didn’t know that we were dentists but I could see it clearly that the tongue was perfectly normal.

Dr. Marc: You had the advantage there.

Dr. Julie: The professional will know about this as well. If any doubts consult a physician and consult your dentist.

Charlie: So a question from a new mom about your 2×4 Early Prevention Dental Health Program. I breastfeed my baby, can breast milk cause decay?

Dr. Long: Okay so there has been some research done on that that shows that breast milk is very unlikely to cause decay. Now the problem is because now some mothers exclusively breastfeed their child they think that they don’t need to brush their teeth because it’s all natural and it’s not supposed to cause decay. I’ve researched it and it shows that it doesn’t cause decay; therefore I don’t need to brush my child’s gums or my child’s teeth. That’s where the misconception is because whether or not breast milk can cause decay, the truth is we will introduce sugar from the exterior into our children’s diet especially when they’re starting to eat solids and it’s always important to remove the bacteria.

Charlie: Of course a lot of moms are breastfeeding longer and so they’re going to have other things introduced besides the breast milk the later they go with that, but by itself it doesn’t cause…

Dr. Long: Probably not.

Charlie: Okay. So when a woman asks, Can Breast Milk Cause Tooth Decay, we can tell her by itself, No.

Dr. Long: Even if, let’s say it doesn’t, it’s still not practical because in real life bacteria or sugars will enter the diet for sure.

Charlie: You and your wife, not only as parents, but as professionals, do recommend breastfeeding.

Dr. Long: Absolutely, yes. That is the healthiest way for your child; it’s the most natural food for them.

Dr. Marc: OK, so: Can Breast Milk Cause Tooth Decay? Dr. Julie I know there are a lot of issues for a lot of people; this can get a lot of intensity here, the whole issue of breastfeeding, not breastfeeding. Anything else to add to that that’s important for us to know?

Dr. Julie: Breastfeeding or not breastfeeding is a very personal choice and I will never tell someone what they should do. But in terms of, is breast milk causing decay? We should think about cavities, about what we can do in terms of diet and oral hygiene, that’s what we can control the most so good oral hygiene practice is always the way to go. Start early on, keep those teeth clean and healthy, provide your child a balanced diet, a healthy diet, and if you do that then you have all the keys to keep them healthy. It doesn’t matter if you’re breastfeeding, you’re not breastfeeding, formula or you’re doing both or both or half and half, and it’s all a matter of keeping good oral hygiene.

Dr. Marc: So it’s picking the path that works for you and then really focusing on the health of yourself and the child.

Dr. Julie: Yes.

Dr. Marc: Okay.

Charlie: Again a question from a pregnant woman on our topic of New Mom and the Dentist: Why is it important to see the dentist after giving birth? After giving birth that sounds like we have two people we’re talking about now but certainly for the moms, what are we saying?

Dr. Long: You may think that now the baby’s out of my body I don’t need to control my diet or my hygiene anymore but bacteria can be transmitted to the baby. Same thing with prevention, the knowledge simply to avoid sharing food with the baby, kissing directly right on the mouth, that can transmit bacteria. That’s where that appointment, the two month appointment is important. It’s mostly for mommy for her to gain that knowledge because there’s no point in talking about it too early because it’s not relevant to her and she may forget.

Charlie: Remind her of things.

Dr. Long: Right so at that appointment, that’s where we show mommy how to clean baby’s gums and most mothers don’t know that you have to do that but the benefit of wiping the gums is that we’re removing bacteria, that’s the number one reason, number two, that slight pressure, we’re stimulating the gums and teething. Also taking care of mommy’s teeth is important because if mommy’s teeth are healthy there is less chance of transmitting and plus for example if mommy wasn’t healthy because of pain or what not she would not be physically fit to take care of her baby because it’s very demanding. You’re losing sleep, waking up for nursing and all that and for the baby it’s important because we can detect small problems such as tissue problems or the tongue can sometimes be tied down to the floor of the mouth and it can affect…

Charlie: It seems to me as well you recommend no x-rays for about a year. Is that for mom? For baby?

Dr. Long: No, it’s for both because obviously during pregnancy we avoid x-rays. Now after when the two months appointment it’s a good time for x-rays because for now it’s been over a year that mommy hasn’t had x-rays.

Charlie: Oh I see.

Dr. Long: And things can develop because of changes in the diet and the hygiene. It’s like when you have braces and they come off, it’s a good time to come in for x-rays because little things can develop and we can detect them and fix them early. If there is a cavity that develops into a bigger problem mommy is not at her best and she won’t be able to take care of baby.

Charlie: I guess the short answer as well here is, let’s get mom back on track to where she would have been had she stayed right on that dental health program.

Dr. Long: For sure. Yes.

Charlie: Very good.

Dr. Marc: Dr. Julie it sounds like there is so much that you have to take into consideration that it can be overwhelming so this notion of getting back on track makes a lot of sense. Anything to add to that?

Dr. Julie: Well mommies always know to take care of herself last so that’s what we have it there. It a reminder for her to take care of her teeth so that she can also take care of her baby. When baby is born actually, baby doesn’t have any bacteria causing cavities in her mouth and they acquire that later on and who’s the first person that they get it from? It’s mommy.

Dr. Marc: Okay, so mom taking care of any bacteria she has is actually about preventing illness in her child?

Dr. Julie: This is how her baby get the bacteria most of the time so we want to make sure that her mouth is in top health as well because as much as we tell the moms not to share the food, not to lick the spoon.

Dr. Marc: It happens.

Dr. Julie: It happens. It happens. We kiss them, we kiss them on the lips, close to the lips and we do pass it on.

Dr. Marc: We’re not focused on bacteria at that point.

Dr. Julie: Yes. That’s why it’s so important. By taking care of herself again will take care of baby.

Dr. Marc: Well, that’s the bottom line. Right now.

Charlie: Dr. Long I think we covered part of this. I want to make sure that we’re real clear on this because we’ve already covered about food, now I really want to cover here on this question: What medications should I avoid during my pregnancy?

Dr. Long: All medication really.

Charlie: They should just stop taking them? They should consult their doctor?

Dr. Long: Yes. That’s where it’s important, there’s no one size fits all answer. It depends on the situation and if there are any health issues then obviously we have to weigh the benefit of avoiding the problem and the risk of taking medication. That’s why it’s important to see your doctor and your dentist and we can then decide if it’s worth the risk taking that medication. Any medication, such as alcohol…

Charlie: I wouldn’t call it a medication, more like a drug.

Dr. Long: Yes, drug related that goes in your system really affects your baby.

Charlie: I know that so many of these medications say right on them that if you’re pregnant, going to be pregnant, if you’re nursing, don’t take them so my daughter, when she was expecting our grandson, would be on the phone right away with the doctor, so we want to encourage that.

Dr. Long: Yes.

Charlie: We don’t want them to put that off. We want to encourage them to check in with their doctor and their dentist if they think it’s related to dental heath.

Dr. Long: Yes, we’re here to help so any questions that you have give us a call, drop by, part of the program we don’t charge for any consulting service because we do believe that prevention should be made available to anybody.

Charlie: And that’s the 2×4 Prevention Dental Health Program?

Dr. Long: Yes.

Charlie: Great.

Dr. Marc: Dr. Julie clearly everything is a balancing act, this is a very delicate developmental time but clearly we’re not saying that people should just stop taking medications just because they’re pregnant so what would you advise in terms of the pacing of that?

Dr. Julie: If any doubt just consult your doctor and ask us if there is something that you’re taking for your teeth and you’re not sure. Ask us whether if it’s safe to take while you’re pregnant. But again it’s best to prevent, if we can, the problem arising than to try to treat it afterwards with medication.

Dr. Marc: Right.

Dr. Julie: That’s why we recommend that you come in and get your teeth check and prevent problems.

Dr. Marc: Before they even develop, yeah, absolutely.

A woman who’s expecting writes this question. Does what I eat affect my baby?

Dr. Long: Definitely because everything that we eat, any medication, any drugs, does pass through the blood, through the placenta to the baby. That’s why it’s so important, early prevention and the first two parts of the program is really for mommy because baby is so young we do very little but for mommy in terms of getting that knowledge is important such as there are drugs to avoid, such as alcohol or smoking it can really harm your baby. Some antibiotics such as Tetracycline can cause permanent staining of the teeth, the adult teeth of the baby. The teeth start developing at six weeks.

Charlie: Six weeks.

Dr. Long: So anything really that you do, that goes in your body, affects your baby. Such as too much fluoride as well can cause teeth to be discolored and become weakened.

Charlie: Certainly I know when my daughter was pregnant, anything she looked at she had to think about twice, it had to be cooked meats, it couldn’t be lunch meats, so food is really, really important here as well as the medications. There are certain things that need to be avoided.

Dr. Long: Yes.

Dr. Marc: So clearly we know the old saying you are what you eat, I imagine that has to be literally doubly true for a pregnant mom because you’re eating for tow. Anything else we really need to be aware of when it comes to diet and the affect it has? I didn’t realize that teeth developed that early.

Dr. Julie: Yes. Yes. …and people will know about that. Definitely when you’re pregnant you want to have a good balanced diet and as a mother you have to make the decision about what kind of food you want to eat. There are all these sorts of things to follow, what to eat, what not to eat…

Dr. Marc: A million books and advice.

Dr. Julie: Right, it’s what you’re comfortable with and what you want to do, but you want a balanced diet to be as healthy as possible because again, people don’t know that they baby’s teeth are being developed while the baby is in the womb still so keep that in mind because…

Dr. Marc: Because you don’t think of that when a baby is born, obviously there are no teeth there so you don’t think; you’re not really thing about it.

Dr. Julie: You don’t see the teeth until later on, but they’re there.

Dr. Marc: Right.

Dr. Julie: They’re already there.

Dr. Marc: At six weeks, I mean that’s amazing.

Dr. Julie: Yes.

Dr. Marc: Yeah. Okay.

Charlie: We’re going down our questions here for the 2×4 Early Prevention Dental Health Program and this one is: Do my teeth lose calcium during pregnancy? Does that make me more prone to decay?

Dr. Long: Okay at that stage the teeth are fully developed, they’re in the mouth so nothing can really affect the calcium inside the teeth.

Charlie: In the mom we’re talking about.

Dr. Long: Yes, in the mom. So the answer to her question, Do my teeth lose calcium during pregnancy, is NO. Now because the question arises because pregnant women get cavities often and really the problem is because they are not able to take care of themselves as they would if they weren’t pregnant.

Charlie: Because they’re taking care of the child and thinking of the child.

Dr. Long: Yes, or just too tired, too exhausted. Sometimes, I remember my wife when she was pregnant, she would sometimes fall asleep and I felt bad waking her up and I would just let her sleep. It’s just one of those things that can make you more prone to decay so it’s not just a question of your teeth being more weakened from lack of calcium or anything like that, it’s just the diet may change from all these cravings, also the hygiene is not as good because you’re just exhausted so the important thing is to simply remove the bacteria. If there are no bacteria, there’s less chance of decay.

Charlie: You always told me that you were a dad first and a dentist second so I guess letting mom sleep like that’s a really good thing.

Dr. Long: Yes.

Dr. Marc: Good guy isn’t he? Is he a good guy?

Dr. Julie: Yes, he’s a good guy.

Dr. Marc: Good.

Dr. Julie: I would get upset at him at that time for waking me up but definitely appreciate that he did because even though I’m a dentist, you’re just as guilty. You’re tired, you’re pregnant and the last thing you think of is I have to brush my teeth, I have to floss. But yes, we go through so many changes while you’re pregnant, there’s so many things we need to keep track of, I tried to remember that I have to take care of myself so I can take care of my family as well.

Dr. Marc: I guess that’s why you guys recommend a certain kind of pacing so the second trimester is the prenatal care…

Dr. Julie: Yes.

Dr. Marc: And then you’re giving the mom a break for at least two months to get settled, get oriented, get a little sleep but then at two months you’re recommending that they come back in to get back on track.

Dr. Julie: Definitely.

Dr. Marc: Okay. Great.

Charlie: Here’s a question that I know is on the minds of a lot of pregnant women.
Charlie: Here’s a question that I know is on the minds of a lot of pregnant women. I know my daughter when she was pregnant, my first grandchild he’s only about 10 weeks old at this point, my daughter would be so careful at this point with everything she would eat and everything that she would do. So this question here is: Is It Safe To See My Dentist When I’m Pregnant?

Dr. Long: Obviously since you’re pregnant there are things we need to avoid but if dental work needs to be done, we take all the precautions to avoid, for example x-rays or any elective work that needs to be done, we do avoid it, but prevention is always minimally invasive. The goal here is to prevent something that could affect your baby. Sometimes there’s no choice, if you have an abscess it needs to be treated, sometimes even x-rays need to be taken. We have to weigh the benefits versus the risk of getting treatment done and that is when prevention is even more important. We absolutely need to avoid being in a situation where we have no choice.

Charlie: And clearly even if you think you might be pregnant you need to mention this to your dentist to take extra precautions if some work is needed at that time.

Dr. Long: Yes.

Charlie: To make sure that it is safe.

Dr. Long: Yes.

Dr. Marc: So the question that we are looking at is: Is It Safe To See My Dentist When I’m Pregnant? I guess the alternative question is, is it safe not to get dental work when you’re pregnant?

Dr. Julie: I think again, weighing the risk versus the benefit. Most dental treatment will be safe, most noninvasive ones will be safe and we do take the precaution in applying the treatment to make it as safe as possible for a pregnant woman. If you don’t get the treatment and it leads to a worse complication like abscess, you need a root canal, you start requiring more invasive treatment and this is when it becomes less safe. We make it as safe as possible but try to prevent things before they become worse is always the best road to take.

Dr. Marc: That’s the importance of why you’ve helped me understand that second trimester is so important because you’re preventing a problem from becoming more manifest in that third trimester where it’s going to be much more difficult.

Dr. Julie: Look at it this way, it’s best to get a filling done than a root canal.

Dr. Marc: Right, because I might need anesthesia at that point and it’s going to be very complicated.

Dr. Julie: Mostly if you’re pregnant it becomes more important that you avoid all these more major treatment but if it gets to the point where you need it, you should get it done to avoid infection because infection can affect your overall health and the health of the baby as well.

Dr. Marc: Great.

Charlie: Why is it important to see the dentist when I’m pregnant? That’s the question written here.

Charlie: Why is it important to see the dentist when I’m pregnant? That’s the question written here.

Dr. Long: Well like I said, when you’re pregnant the baby and mommy are really one. They are connected right? So if mommy takes care of herself they she is directly taking care of baby.

Charlie: Yes.

Dr. Long: Because they’re one and we discovered, and most people don’t know this, we discovered that gum disease will affect the baby and sometimes babies are born earlier than they’re supposed to be and they’re much smaller as well. That’s a reason why taking care of your gums before baby is born is important and this is not something that is difficult to take care of. It’s a little bit of knowledge and to see a professional because sometimes when we’re pregnant things change like our diet and we just don’t have the energy to take care of ourselves the way we would have if we weren’t pregnant. That’s where little things can be taken care of before they affect your pregnancy and your baby.

Dr. Marc: So Dr. Julie I have to imagine that this is a bit of a new concept for a lot of people. I think a lot of people don’t think about, they’re clearly thinking about their child when they’re pregnant, but not necessarily thinking about the dental side of things. So, how do we raise that consciousness in pregnant women here and what’s important about that?

Dr. Julie: Research has found connection between dental health and overall health as well and when you’re pregnant you always want to make sure you’re healthy, you’re eating well, you’re avoiding things that could harm the baby, but people don’t necessarily think about what I do with my teeth will affect the baby as well. We want to raise awareness in that regard so we want to encourage the pregnant woman to come in and make sure that the teeth are okay because any kind of infection can affect the health of your baby, can affect your health and the health of your baby as a consequence. You we really encourage the pregnant woman to come in and have a look and it should be part of their routine.

Dr. Marc: The maintenance of their health and really focusing on caring for this unborn child, it’s great I wouldn’t have thought about it so I think we’re really helping people out by letting them know.

Charlie: I’m here with dentist Dr. Long Vo and a question to us and I think I know how you’re going to answer this first part because of what we’ve talked about, but the second part: So should a pregnant woman see a dentist and when?

Dr. Long: Should a Pregnant Mom See The Dentist? Pregnant women should definitely see their dentist. Obviously there’s a link between mommy and baby so what effects mommy, affects the baby. When should she come in, that’s the more difficult question to answer? During the first trimester of pregnancy there are so many changes going on mommy is excited, she’s scared, she’s confused, all of these hormonal changes, changes in her body. Your body is trying to adapt and mommy’s adapting and she’s uncomfortable and it’s not the best time to come in for a dental appointment and the child is so underdeveloped that it’s very important to stay away from any dental work at that stage. We want to make sure that baby is strong enough that why the second trimester that’s where the baby is more developed and mommy is more comfortable, she’s more used to her body and adapting to everything and that’s when she should come in and see the dentist. The third trimester, obviously it’s more difficult to move, there’s more weight and laying down for a long time for an appointment is not the best thing having all that weight on you and you’re body is simply getting ready to welcome the baby and there’s more important things to take care of at that point. So, the second trimester is definitely the best time to come in to visit and see your dentist.

Charlie: So, second trimester, that’s part of this 2×4 Early Prevention Dental Health Program.

Dr. Long: Yes. That’s the first.

Charlie: That’s the first time a pregnant woman should see the dentist: the second trimester.

Dr. Marc: So Dr. Julie, you’ve actually had a couple of second trimesters.

Dr. Julie: Yes.

Dr. Marc: From a mom’s point of view as well as a dentist, what makes this second trimester more important than the first or the third?

Dr. Julie: Again, you want to have enough time if we catch anything, it’s to prevent problems that can cause complications towards the end of the pregnancy. By the time you’re pregnant, everything goes pretty fast but you also never have the time for yourself. Towards the end of the pregnancy you want to avoid infection or needing major dental treatment because that’s not the best time. This is a time you need to focus on the arrival of your baby. That’s why the second trimester is so important because if you wait so long and there’s need for treatment you’re not in the best position to receive it.

Dr. Marc: Right.

Dr. Julie: The first trimester is always the most diligent part. When you’re pregnant…

Dr. Marc: You don’t want to disrupt things.

Dr. Julie: Definitely, the baby’s developing all of its major organs. When you’re pregnant when you pass the first trimester that’s when you can start to take a bit of a breather. There’s always all of these changes always happening in your body but it’s very important and we want to remind all future moms that they should come in and have your teeth looked after because you want to prevent complications and also once you have a baby you’re so busy.

Dr. Marc: Right. You’re not going to have time at that point.

Dr. Julie: You won’t be able to come back again.

Dr. Marc: You’ll be worried about getting some sleep, let alone going off to the dentist.

Dr. Julie: That’s not the time either. Let’s say you need a root canal, now you have a young infant that’s not the best time to get that done and you want to prevent complications.

Dr. Marc: So it really gives you a chance that things are stable enough but not so advanced that you’re getting into other things.

Dr. Julie: Definitely.

Dr. Marc: Great. Thanks.
