Responsibilities include:
1. Provide long-term, intensive Clinical Social Work Services to patients and families to promote optimal participation in health-care decision-making and maximize satisfaction with and adherence to agreed-upon treatment plans. Services include assessment, counseling for adjustment to illness, crisis intervention, community agency liaison and coordination services to patients and families; assessment and referral for child abuse, domestic violence, vulnerable adult protection, and other services. Participation in interdisciplinary team meetings and education and research activities related to these patients and families. Work with a team of other health care providers in assuring coordination across the continuum of care. Provide special services for specific patient/family groups such as support groups, educational or informational programs as needed. Liaison with home health care and rehabilitation resources as well as funding sources for these resources.

2. Participation in Social Work Department meetings (includes staff meetings, work team meetings and supervision).

3. Other required administrative tasks (includes submission of required data, participation on department or interdisciplinary team committee or initiative).
