<p>The American Society of Plastic Surgeons has just released their <a href="http://www.plasticsurgery.org/News-and-Resources/14-Million-Cosmetic-Plastic-Surgery-Procedures-Performed-in-2012.html">2012 annual statistics</a> reporting the number and variety of plastic surgery procedures that were performed last year. The report, a virtual &#8220;state of the union&#8221; for plastic surgery profession, provides us with several interesting insights into trends in plastic surgery for the year.</p>
<p>“The fact that total U.S. cosmetic procedures were up last year is not surprising given that the economy is beginning to bounce back and these procedures are high on many people’s list of things to do with the extra income they are seeing.” says <a href="http://www.drwigoda.com/about/">Dr. Paul Wigoda</a>. <p dir="ltr">Overall, cosmetic procedures in total were up a whopping 5%! The most popular procedures by volume were breast augmentation with 286,274 performed in 2012, followed by nose reshaping (242,684), eyelid surgery (204,015), liposuction (202,128), facelift (126,320) and tummy tuck (106,628).</p>
<h1 dir="ltr">Faces Lifting!</h1>
<p dir="ltr">The biggest bump last year came in the form of facial rejuvenation procedures.</p>

A recent study on Tummy Tuck and weight loss confirms that weight gain after surgery is not typical and that most patients not only lose weight but maintain the weight loss. The study by ASPS Member Surgeon Dr. Rex Edward Moulton-Barrett of Alameda Hospital and colleagues just released in the February issue of Plastic and Reconstructive Surgery, reveals that undergoing abdominoplasty (&#8220;Tummy Tuck&#8221;) may lead to significant and lasting weight loss for many patients.</p>
<p dir="ltr">“We’ve always seen outstanding long-term results with our<a href="http://www.drwigoda.com/procedures/tummy-tuck/"> abdominoplasty</a> (or tummy tuck) patients but this new study proves what we have suspected for years. This is very exciting news,” says<a href="http://www.drwigoda.com/about/"> Dr. Paul Wigoda</a> of Fort Lauderdale.</p>
<h1 dir="ltr"><strong>The Tummy Tuck Study</strong></h1>
<p dir="ltr">The Tummy Tuck study included 20 women and the average amount of abdominal tissue resected was approximately five pounds. A year later, the Tummy Tuck  study found that 14 of those women, a large majority, kept the weight off and had even achieved additional weight loss. In fact, their weight loss was greater than the weight of the tissue resected. Tummy Tuck patients with a preoperative body mass index (BMI) greater than or equal to 24.5, borderline overweight, maintained long-term weight loss after one year. In fact, their weight decreased an average of 4.5% of their original BMI. (Note that a normal BMI is between 18.5 and 25, between 25 and 30 is classified as overweight, and over 30 is obese).</p>
<p dir="ltr">The tummy tuck study found that long-term weight loss was more likely for women whose initial BMI was 24.5 or greater, that is, just under the borderline for overweight. Of the 14 women with sustained weight loss, just one had an initial BMI of less than 24.5. In addition, it was found that long-term weight loss was more likely for women with a greater amount of excess abdominal tissue removed at abdominoplasty. Twelve of the 14 women who enjoyed long-term weight loss had more than 4.5 pounds of tissue resected.</p>
<h1 dir="ltr"><strong>Dr. Wigoda Shares it How it Works</strong></h1>
<p dir="ltr">The tummy tuck study points to Increased satiety or the “feeling of being full” as a important contributor to long-term weight loss. Three-fourths of women reported an increased feeling of satiety, either after eating or throughout the day, after undergoing their abdominoplasty. &#8220;We hypothesize that the increased satiety [experienced by] our patients is related to changes in the neuroendocrine system,&#8221; said Dr. Moulton-Barrett and his co-authors. The tummy tuck  theory is that removing fat cells from the abdomen could lead to reduced levels of hormones affecting appetite, which are secreted by fatty tissues.</p>

While most patients in my practice have the implant placed under the muscle, there are cases where over the muscle may be beneficial,” says <a href="http://www.drwigoda.com/">Dr. Paul Wigoda</a>.</strong></p>
<p dir="ltr">
<p>Take a moment to look at the diagram below to see the size and location of the muscles that make up your chest wall. You will notice that the pectoralis major muscle is a large fan-shaped muscle that does not cover the lower, outer aspect of your chest wall. Generally in breast augmentation surgery the pectoralis major muscle will be detached from its lower origin along the ribs and the implant will be placed under it leaving only about half the implant covered by the muscle. The bigger the implant, the less of it that is covered by the muscle.</p>
<p><a href="http://blog.drwigoda.com/wp-content/uploads/2013/04/Screen-Shot-2013-04-08-at-6.02.13-PM.png"><img class="aligncenter size-full wp-image-1144" title="Screen Shot 2013-04-08 at 6.02.13 PM" src="http://blog.drwigoda.com/wp-content/uploads/2013/04/Screen-Shot-2013-04-08-at-6.02.13-PM.png" alt="" width="500" height="377" /></a></p>
<p>The pectoralis minor muscle is much smaller and lies under the outer edge of the pectoralis major. The serratus muscle lies along the sides of your chest wall. The main sensory nerves to your breast come up between the slips of this muscle to enter the back of your breast tissue and from there to the rest of your breast. Generally surgeons place the implants on top of the pectoralis minor and serratus muscles, but below the pectoralis major muscles.</p>
<p>There are pro’s and con’s to having your implants placed over or below the muscle. Here are some things to consider:</p>
<h3 dir="ltr">Over the Muscle (Subglandular) Breast Implant Placement:</h3>
<ul>
<li dir="ltr">
<p dir="ltr">Pros: When textured implants are used, the capsular contracture rate is only 5–10%. If you have a moderate amount of breast tissue (a B cup or larger), this can be an excellent choice and produce very natural-looking breasts that age very well. Also, recovery is usually easier than when the implants are placed in a submuscular position.</p>
</li>
<li dir="ltr">
<p dir="ltr">Cons: The use of smooth implants in a subglandular position have an increased rate of capsular contracture compared to other options. Additionally, when textured implants are used in thinner women with small breasts rippling may be visible in the upper and inner areas. It is also easier to see the upper edges of the implants when they are placed over the muscle.</p>
</li>
</ul>
<h3 dir="ltr">Under the Muscle (Subpectoral) Breast Implant Placement:</h3>
<ul>
<li dir="ltr">
<p dir="ltr">Pros: Using smooth implants under the pectoralis muscles reduces the capsular contracture rate to less than 10%. In thinner women, the added bulk of the muscle over the upper and inner aspects of the implants will make the edges less visible and the curve of the breast more natural. When textured implants are used, the rippling in the areas under the muscle is usually invisible.</p>
</li>
<li dir="ltr">
<p dir="ltr">Cons: The recovery is typically thought to be more uncomfortable. However, with <a href="http://www.painlessbreastimplants.com/">Dr. Wigoda’s painless breast implant procedure</a>, patients most often have implants placed below the muscle and have little or no pain. It can take several months to see your end results because the implant tends to start out high before it settles into its final position, lower on the chest. When the pectoralis muscle is contracted during exercise or in some daily movements, the muscle may flatten the implants and push them outward. This can change the shape of the breast. However, this does not happen in all patients. Occasionally, an indentation over the implant can be seen along the edge of the muscle but again, this is not typical. When there is a moderate amount of breast tissue that is lax or becomes lax in time, it may sag below the position of the implant that is held in place by the muscle.</p>
</li>
</ul>

<p>The location of your incision is one of the many options you will discuss with Dr. Wigoda during your preoperative consultation. Each incision location option has its pro’s and con’s and there is not one location that is considered best for everyone. Here are the options:</p>
<p>1) Inframammary Incision: Inframammary incisions are placed underneath the breast just slightly above where the breast meets the rib cage. Many surgeons like this incision location as it allows them easy access to create an implant pocket above or under the muscle. This direct access allows Dr. Wigoda to use very small incisions with saline implants, generally as small as 1 to 1.5 inches wide for saline implants, slightly larger incisions for silicone. The scar is typically not visible when standing up as the breast will cover it. This is the most common incision site used.</p>
<p>2) Transumbilical Incision or TUBA: A Transumbilical incision is a C or J shaped incision placed in the folds of your belly button. A small camera is used to negotiated the surgery and very small instruments are then inserted to create a path to the breasts. Tissue expanders are inserted to create the implant pocket. Using TUBA incision, implants can be placed over or under the muscle. While TUBA incisions have the advantage of there being no scarring on the breast area, there are many potential disadvantages. Only saline implants can be used if TUBA is selected as silicone implants will not fit through the very small incision. <p>3) Transaxillary Incision (Armpit): These incisions are created in the armpits and are generally 1 to 2 inches long. A short path is then created to the breasts where the implants will be placed. While there is no scar on the breast, a scar will be visible in the armpit with the arm is raised. There is also a risk of injury to a sensory nerve that can cause numbness on the inner arm.</p>
<p>4) Periareolar Incisions: The Periareolar incision is created around the areola. This incision is commonly used if other cosmetic breast procedures such as breast lift or areola reduction are being performed in conjunction with breast augmentation. Contrary to popular belief the periareolar incision does not necessarily increase the risk of losing sensation in the nipple. There is potentially a greater risk of infection as bacteria is found in milk ducts and often these ducts are cut during the procedure. If you plan to breastfeed after your surgery should make sure to discuss this with Dr. Wigoda because periareolar incisions may not be right for you.</p>

<p><strong></strong>Breast implants come in two shapes: round and teardrop. Which is right for you? Well, it depends. Here are some considerations to help you decide.</p>

</strong></strong>Teardrop Implants: Teardrop implants also known as “anatomical” are contoured and shaped just like a natural breast. It takes a skilled and experienced plastic surgeon to ensure they are properly placed. Dr. Wigoda has extensive experience with teardrop implants. Anatomical implants are always textured to minimize their movement and decrease the risk of the implant rotating.</p>
<p dir="ltr">Round Implants: Round implants will give you that “Victoria Secret” look. They less natural looking than teardrop shaped implants but can deliver a higher impact result. Round implants are currently the most common implants as they give women the most lift, fullness and cleavage many of us are looking for. Also, certain types of round implants will enhance the forward projection of your breasts. Keep in mind that women with large implants often report that others ‘know’ they have had breast augmentation. For most women, these days, this is not a concern. It all depends on you!</p>

<p><strong>Smooth or Textured?</strong></p>
<p>Breast implants are available in two surface choices: smooth and textured. Smooth breast implants feel like the outside of a balloon. Textured implants feel somewhat like sandpaper. Which is right for you? Below we will discuss some considerations to help you make your choice.</p>
<p>Smooth Breast Implants: Smooth breast implants are used in most breast augmentation procedures. Smooth implants move freely within the breast pocket and have thinner shells making them softer than textured implants. Smooth implants may be less likely to ripple than textured implants. Other benefits of smooth implants include lower cost and increased implant longevity.</p>

<p dir="ltr">Textured Breast Implants: Textured implants have been designed to limit the risk of capsular contracture, an implant complication involving scar tissue growing around the implant. However this claim has not been conclusively proven. However, textured implants do create cohesion between the implant and surrounding tissue reducing the risk of the implant moving. Textured breast implants have been associated with a higher rate of leaking and breast rippling than smooth implants; however, textured implants have also been shown to decrease the risk of displacement.</p>

<h3><span style="font-size: 1.17em; font-weight: normal;">Small, Moderate or Large?</span></h3>

Diameter is the distance across the base of the implant, that is the surface that rests against the chest wall. Projection is the distance the implant protrudes from the chest wall. Implants are available with a small amount of projection (low profile) , a moderate amount of projection (moderate profile) and a large amount of projection (high profile).</p>
<p>When deciding which profile is right for you, your age is a factor. If you are middle-aged (or older) or if the confidentiality of your procedure is important to you, you may want to chose implants with a small projection as these give the most natural appearance.</p>
<p>If you are younger, you may prefer a more aggressive take-off at the upper part of the breast, so moderate plus profile implants may be right for you. Moderate plus profile implants will give you a more dramatic impact and are currently “in vogue”.  Yes, they will look less “natural” but these days there is no stigma attached to having undergone breast augmentation so a completely “natural” look is not as much a concern as it may have been years ago. Also, the natural shape of a your natural breast is a factor here.  If your natural breast has very little or no superior (upper) volume, you might benefit more from implants that provide more superior volume.  Implants with more projection do this.</p>
<p>If you have narrow chest you are also a good candidate for implants with moderate plus profile. To achieve a significant amount of volume, you may want to choose implants with more projection but less diameter. Finally, implants with more projection tend to ripple less than those with less projection. If you are very thin you are at greater risk for rippling and should consider moderate plus as your profile choice.</p>

<p>If you’ve undergone a mastectomy high profile implants could be right for you because you  have no projection to start with. Also, if you like the look of round, projecting breasts with upper pole fullness (high cleavage) high profile could be the wise choice. Every plastic surgeon has a certain aesthetic style they prefer (re: some aim for the most natural looking shape and size possible while other find the ‘high and round’ look more attractive), but at the end of the day, it’s all up to you!</p>

<p>We want to increase our size and/or have our breasts more proportionate to the rest of our bodies. Some women worry about going too big, while others worry about not going big enough. Having realistic expectations regarding size is imperative. Here are some ways you can figure out what size is right for you.</p>
<h3><strong><strong>A Picture’s Worth 1,000 Words</strong></strong></h3>
<p>When consulting with a plastic surgeon, it helps to bring pictures depicting how you want to look after surgery. Also, bring photos of what you consider too small or too large as this will help your surgeon immensely. Celebrity magazines are a great resource for photos of women with various size breasts. Keep in mind that everyone&#8217;s anatomy is different, so you will not look exactly like the photos you show your surgeon; however, these photos will give the surgeon a very good idea of the SIZE you want to be. You may have a good idea of what cup size you would like but photos can further inform your surgeon of your desired results.</p>
<h3>Dr. Wigoda Talks About The Rice Test</h3>

<p><strong><strong></strong></strong>Surgeons generally do not talk in &#8220;cup&#8221; sizes they talk in cc’s. So &#8220;trying on for size&#8221; in cc’s prior to your <a title="Plastic Surgery Consultation" href="http://www.drwigoda.com/">plastic surgery consultation</a> is a very good idea. And, it’s fun! If you&#8217;re very flat-chested, almost anything may seem big but you’ll be amazed at how quickly you adjust to your new, larger size. The Rice Test will give you an idea of what you&#8217;ll like to look like after your surgery, as well as a rough idea of how many cc&#8217;s it may take get that result. It&#8217;s not 100% accurate, but it&#8217;s pretty close.</p>
<h3 dir="ltr">How to Perform the Rice Test</h3>
<p dir="ltr">Use knee-high hose or cut-off pantyhose, and, using rice (or oatmeal or grits), fill them with the desired amount of cc&#8217;s. Once you have them filled, try them on under a sports bra.  Here are some conversions below:</p>
<ul>
<li dir="ltr">1 cup = 236cc</li>
<li dir="ltr">3/4 cup = 177cc</li>
<li dir="ltr">2/3 cup = 156cc</li>
<li dir="ltr">1/2 cup = 118cc</li>
<li dir="ltr">1/3 cup = 78cc</li>
<li dir="ltr">1/4 cup = 59cc</li>
<li dir="ltr">1/8 cup = 30cc</li>
</ul>
<p>Some guidelines: An average of 189 cc is needed to go up one bra cup size. Increasing an A cup to a C cup generally requires a total of 391 cc, or 196 cc per cup. Moving from a B cup to a D cup generally requires a total of 448 cc, or 224 cc per cup. The largest change, an A cup increasing to a D cup, generally requires 437cc, or 145 cc per cup.</p>
<p>The amounts above can be used in different combinations to achieve the cc amount you desire. It&#8217;s important to remember that you should not solely rely on cc amounts of friends whose size you like. 400cc can look completely different on different women. This is due to several things, including, but not limited to: amount of breast tissue, shape of the chest wall, deformities of the chest wall (such as pectus excavatum and pectus carinatum), and weight, just to name a few. Basically, your existing breast tissue, and anatomy dictate how many cc&#8217;s you will need. Therefore, you should do the rice test to give you an idea of how many cc&#8217;s you may need to get the size you want. Keep in mind the &#8220;rice test&#8221; is just an estimate, it is not an exact science!</p>
