master_w="384" master_h="384"/&gt;&lt;/div&gt;For many years, Domhnall was one of Quora&amp;#039;s most influential writers and leaders. His contribution to the community was immediately apparent from the beginning. He touched the lives of many people on Quora and built up a large collection of not just followers, but friends. &lt;br /&gt;&lt;br /&gt;He passed away peacefully on November 14, 2015, at St. James Hospital Dublin. He is survived by his partner, two beautiful daughters, his mother, father and sister. &lt;br /&gt;&lt;br /&gt;Domhnall impressed everyone who interacted with him with his sharp intelligence, compassionate heart, dry humour and yes his epic rants. Some of his more notable contributions are but are not limited to: &lt;br /&gt;&lt;br /&gt;&lt;ul&gt;&lt;li&gt;He was a three time Top Writer. He earned this for his funny and sharp contributions to Quora.&lt;/li&gt;&lt;li&gt;He was an admin for two years, dedicating much of his free time to help make Quora a safer place for everyone. He enjoyed the discussions with the other admins very much and it is here where he made some of his best Quora friends. &lt;/li&gt;&lt;li&gt;He greatly enjoyed going to meetups and even organised one in London for Christmas 2012. Last year we also had the pleasure of him flying to New York, despite hating to fly, for the Top Writers New York meet-up in 2014. He enjoyed meeting his fellow Quora writers so much and it was his dream to attend another event like this. Not Google+ notifications. &lt;i&gt;&lt;u&gt;Quora notifications.&lt;/u&gt;&lt;/i&gt; For the love of all that is good and holy, EMRs need a real-time notification system that&amp;#039;s actually fast/useful/good. Quora has the best implementation I&amp;#039;ve ever seen. Right now, I have to manually check every patient I&amp;#039;m following to check if new stuff has come in. This is an absurd waste of time, and is a detriment to good patient care.&lt;br /&gt;&lt;br /&gt;&lt;b&gt;6) Automatic Logs&lt;/b&gt;&lt;br /&gt;Automatically tracking changes for text boxes would be amazing. The current implementations I&amp;#039;ve seen are clunky, and don&amp;#039;t show you exactly what was changed--you have to hunt it down yourself. So, for instance, if you don&amp;#039;t catch that your radiologist slipped in &amp;quot;No films available for comparison&amp;quot; and then page them asking to compare to old films, you look like a colossal pillock (this is, of course, a purely theoretical example that totally didn&amp;#039;t happen to me last week). Put together with notifications, logs are a pretty dang powerful tool.&lt;br /&gt;&lt;br /&gt;&lt;b&gt;7) Topics and tagging&lt;/b&gt;&lt;br /&gt;Topics would essentially translate over to medical conditions. Tag patients with them, tag notes with them, tag tag tag tag tag like mad. This would be so amazing for research purposes. Think about it--all you&amp;#039;d have to do is click on a topic, and...bam. All the patients who&amp;#039;ve ever been tagged with, say, &amp;quot;high blood pressure&amp;quot; come up. All the recent ones show up in the feed. The physicians who treat the most of it would come up where the &amp;quot;Top Answerers&amp;quot; is now.&lt;br /&gt;&lt;br /&gt;&lt;b&gt;8) Central server&lt;/b&gt;&lt;br /&gt;I can access the questions/answers/posts of someone thousands of miles away. It doesn&amp;#039;t matter where I am. I&amp;#039;m not limited to answers from Texas, or Silicon Valley, or New York based on my geographic location when I access Quora. This isn&amp;#039;t the case with EMRs.&lt;br /&gt;&lt;br /&gt;Right now, even if I&amp;#039;m using the exact same EMR as another hospital a 5 min walk away, I can&amp;#039;t access that hospital&amp;#039;s records, because the two locations are running on separate servers. This is idiotic.&lt;br /&gt;&lt;br /&gt;&lt;i&gt;Example: Patient comes into the ER with a badly broken bone. Bone is X-rayed, assessed, and it&amp;#039;s determined that patient needs to be transferred to another hospital for better care. Because the two hospitals are not on the same server and records aren&amp;#039;t shared, the patient has to undergo another set of X-rays once transferred because the second hospital needs another set for their own records, exposing them to another dose of radiation. Let us suppose this is a very small child. Do you really want to gamble whether that extra dose will affect his/her cancer risk 30-40 years later?&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;We will have to deal with security issues regarding this, but realistically, banks have had to deal with similar issues for a long time and they seem to be doing an ok job. I defer to security experts for best practices, but this is something that really needs to be done.&lt;br /&gt;&lt;br /&gt;&lt;i&gt;Example: Patient comes in with very, very rare syndrome (&amp;lt;1 in 100,000 live births in the U.S.). The expert on that syndrome is the person it&amp;#039;s named after, and he practices in another academic center 2500 miles away. I call him, he agrees to consult and offer his expertise to assist the team with the case. We both log in to QuoraEMR; he goes to my profile and sees the patients I&amp;#039;m following; I give him the name and he follows this patient as well. He now has access to all of that patient&amp;#039;s records/labs/imaging/etc even though he&amp;#039;s 5 states away, and is able to follow along far more effectively than just through e-mail, which is insecure anyways.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;&lt;b&gt;9) Inbox&lt;/b&gt;&lt;br /&gt;An easy messaging system, please. A prominently placed one, like on Quora.
