MCST 3601 – Powerful New Drive Electronics for Stepper Motors

Searching the multitude of available programmable motion controllers for one that is ideal for micro stepper motors with phase currents of sometimes less than 100 mA was once a real challenge. The new MCST 3601 series, developed from the cooperation between FAULHABER and Trinamic GmbH (the motion controller specialist for stepper motors), offers the solution. This controller is compact and flexible, and is suited to fit perfectly with the full range of FAULHABER micro stepper motors.

The series, which is equipped with a micro controller and a driver, can precisely regulate phase currents from 10 mA to 1.1 A. At the same time, it can be operated with a supply voltage that can vary between 9 and 36 VDC. Thanks to simple and fast communication with a PC via USB 2.0, all types of movements can be programmed using the available software. The ten inputs and outputs also allow external variables to be taken into consideration. Since the controller is designed as a test electronics system, users can adopt the predefined architecture according to their requirements and integrate the components in the platform of their specific industrial application.

Furthermore, the MCST 3601 series can actuate stepper motors in micro stepping mode. In this operating mode, up to 256 micro steps can be programmed per full step. This allows the resolution of the controlled rotary or linear movement to be increased considerably and/or the vibrations caused by the motor to be reduced. This capability is an excellent fit for optics applications where resolutions are often in the micro meter range.

Medtech Takes Pharma into Drug Delivery Telemanagement; Partnership is Key to Success

Information technology for drug delivery telemanagement and the business of Pharma are also becoming inextricably interwoven, with medical technology, or medtech, taking the industry well beyond NDA approvals and filling scripts. As if drug development and delivery were not already sufficiently complicated, new combination products and telemanaged combination products make the processes even more so.

Within and around Pharma there are many stakeholders who can partner to make drug delivery telemanagement a success. Some of these may not have always been obvious. These stakeholders / potential partners have a wide range of interests and include a wide range of people and organizations. Many entities and individuals are stakeholders in multiple categories – sometimes they can even be perceived or actual adversaries.

Q&A:

1. Has the Drug Delivery Ecosystem evolved? What does it look like today?

I can only comment relative to the Over the Counter (OTC) pharma business- which represents $119 billion in worldwide annual sales, and $29.5 billion in U. S. annual sales according to the Nicholas Hall 2014 DB6 database. Oral drug delivery is the vast majority of the business in the U. S., with more than 80%; topical products (dermatology) and nasal delivery represent the most of the remainder of the business. Medical devices are a very small portion of the OTC business. Injectables are not relevant among OTC's- they are utilized for pharma products and animal health. Although there is a shift toward new forms among oral OTC's- long acting medicines, better tasting forms (gummies, drinks, chewables- especially among vitamins and supplements), and traditional tablets are still a large part of the OTC business.

2. What is the next big thing in Drug Delivery as we move into 2016?

In the OTC space, we see a continued development of better tasting forms, although this will be regulated as it relates to certain OTC categories where overuse can be harmful to consumers. Intellectual Property (IP) will be crucial to new brand introductions and brand extensions, as novel items will be promoted more aggressively. Medical devices may continue to be more of a part of the OTC landscape (e. g. Icy Hot, primarily a topical pain reliever, introduced a tems device for pain relief in late 2014).

3. What are some of the newest and most evolving technologies you have your eyes on? And what’s next in connectivity?

It's all about technologies which will appeal to consumers to provide a better solution to their healthcare needs, and increase compliance

Over time, OTC's usage, especially in important maintenance areas (e. g. low dose aspirin and its daily use for heart healthy benefits) will be tracked on smart phones and other devices frequently followed by consumers. Apps and reminder messages will also likely evolve more and more as it relates to OTC usage of maintenance items (vitamins, fiber supplements, probiotics, etc.), as well as items crucial to their well being.

4. When do you start to look at utilizing drug delivery systems?

DD systems are evaluated for OTC's in advance of a new product launch, when considering brand extensions, as lifecycle management additions for Rx to OTC switches, and as a way to appeal to new users or improve compliance for existing OTC products. The incorporation of new forms within OTC's can sometimes be done quickly (1-2 years prior to launch), especially in light of the monograph and DSHEA regulatory approval routes. However, in the case of NDA's,( e. g. sustained release orals, dermatology products with enhanced penetration capabilities, etc.), the planning in advance of launch can sometimes be five years or more.

5. What are your thoughts on how innovation is going to be pressured as a result of changes in the site of care?

OTC's will continue to benefit from increased self selection and more products switching from prescription to OTC. Innovation needs to focus on changing trends in the home and workplace, the aging and more active population, and greater use of self care, especially maintenance self care to insure a more healthy and improved lifestyle.

Applying the EpiPen Case Study to Implementation of Smart Medication Telemanagement (SMT)

The EpiPen was the groundbreaking civilian auto injector. It is still a notably successful product. There were many barriers to its introduction and eventual success (see recent articles in Fortune and Businessweek). Many are the same barriers for smart medication telemanagement–which can be overcome by using some of the same strategies and tactics that resulted in the EpiPen's success.

Biotech and specialty pharma (often costly, injectable, sensitive products) are the great hope for a better life for many patients. These products are also a focus of many healthcare stakeholders. There are parallels with the EpiPen. Smart medication telemanagement systems allow patients to improve outcomes and provide great value for all stakeholders.

Implementing telemedicine technologies in smart medication telemanagement is especially critical and parenteral product stakeholders have incentives to lead in the efforts.

Despite the fact that there was a known opportunity, there was a perception that EpiPen was just another new complicated thing. Similarly, some still find the concept of providing smart medication management too new. Some want to concentrate only on the same old initiatives used for years. Many of those don't work well in t he changing healthcare environment. Arguably, smart medication telemanagement can help solve many current and coming problems.

Some barriers, issues and threats, as well as observations on possibilities to deal with or overcome them are discussed in this poster to be presented at a session during The Universe of Pre-filled Syringes & Injection Devices conference being held November 3-4, 2015.

Capsugel Acquired New Drug Delivery Technology from MIPS

Researchers at the Monash Institute of Pharmaceutical Sciences (MIPS) developed a novel drug delivery technology - it has been acquired by Capsugel, a global leader in innovative dose-form development.

Capsugel has acquired the intellectual property pertaining to proprietary Ionic Liquid Technology developed by researchers at MIPS led by Professors Chris Porter and Peter Scammells. The novel technology uses lipid-like counter-ions to transform crystalline drugs into ionic liquids. This leads to significant increases in drug solubility in lipid-based liquid, semi-solid and multiparticulate formulations.

New Delivery Method to Assist Parkinson's Patients

AbbVie's new Parkinson's drug DUOPA™ (carbidopa and levodopa) was recently approved by the FDA and is delivered straight into the patient's small intestine over a 16 hour period using an infusion pump and procedurally placed tube.

"The FDA approval of Duopa is another significant milestone for AbbVie's pipeline," AbbVie Chief Scientific Officer Dr. Michael Severino said in a statement. "This advancement is important for patients with advanced Parkinson's disease and their care teams, as it provides a new therapeutic option to help manage motor symptoms."

The oral delivery of levodopa/carbidopa is notorious for unpredictable absorption and this set off the search for new delivery methods - hence DUOPA and its percutaneous endoscopic gastrostomy procedure with jejunal extension, that delivers the drug into the small intestine by bypassing the stomach.

However, this approval is coming off the heels of the FDA approval of Impax Laboratories' RYTARY which contains both immediate and extended release beads of carbidopa and levodopa. Learn more about RYTARY here.

Tiny Sensors May Soon Monitor Seniors’ Medicines From Inside

Ever been lost on a new trail on a hike? Or confused between north and south in a new city? Or after a certain age, unsure if you really took that anti-cholesterol pill last night, or was it the blood pressure pill? They kind-of look the same.

Only about 50 percent of patients take their medications as prescribed. And, according to the Centers for Disease Control and Prevention, in 2010 almost 40 percent of adults older than 65 were taking five or more prescriptions a day.

With families sometimes far away and many older people unable to afford personal caregivers, companies have searched for a technological solution to monitoring medicine.

Forget armband monitors like Fitbit, the newest body monitors are as tiny as BBs. These so-called nanomeds, miniscule sensors embedded in a placebo pill that you swallow, set up shop in your gut. As they slowly work their way through your system, these “ingestibles” – which are actually not digested – are switched on by contact with saliva and/or gastric juices. The signal is picked up by another sensor which looks like a Band-Aid and is worn on your chest.

This system records medicine intake as well as other measures, such as heart rate. The information shows up on your smartphone or tablet, via Bluetooth and can automatically go to your doctors, family members or caregivers, with your permission.

The Food and Drug Administration approved these devices in 2012, but they’re not on the open market yet. They’re still being tested in pilot projects, including with England’s National Health Service.

Proper use of powerful, sophisticated meds aimed at keeping the elderly active and out of institutional care, Bill Satariano of the UC Berkeley’s School of Public Health believes, will depend increasingly on these “indigestible chips.”

How has the evolution of modern medicines evolved to bring in a wide diversity of therapeutic modalities?

Molecular biology techniques and high-speed sequencing methods have been the primary drivers. These methods have allowed us to better understand the molecular basis of health and disease. That we now can more fully appreciate these events provides us the opportunity to design medicines that can address selected targets more specifically.

The types of medicines we can now imagine have also evolved from the traditional small molecules that have driven the development of a robust pharmaceutical industry. Protein and peptide therapeutics have now become over 50% of the product pipelines for the major pharmaceutical companies. It is anticipated that nucleic acid-based medicines will be the focus of the next evolutionary step.

What are these modalities? What challenges do they present to the science and technology?

While proteins and peptides are currently leading the way in changing the face of modern medicine, the future seems to be headed toward utilizing novel and unexpected findings that have defined a complex and elaborate role of RNA in controlling cell function. Sequencing of the human genome has demonstrated that only a small fraction of cells’ DNA is transcribed into RNA that is then translated into proteins and peptides via the classical pathway. It is now clear that most of the genome elaborates RNA that has a variety of non-classical actions in health and disease.

Proteins, peptides, and RNA-based molecules all present similar challenges of delivery. Such molecules, due to their physicochemical properties, are labile and excluded from moving efficiently across biological barriers of the body. While some of these molecules can be delivered successfully by injection to overcome these challenges, optimal benefit of these highly potent therapeutic agents often requires overcoming systemic and cellular barriers that limit their patient-friendly delivery for chronic therapy. Thus, the main challenge of all of these is their delivery into the body and into specific cells to optimize their actions while limiting potential side effects.

Is Silent Innovation Driving Patient-Centric Medicine Forward?

Scientists tend not to look backwards but sometimes it is an interesting exercise to do so if only to confirm how much science has advanced in a relatively short time.

When I left university with the degree in pharmaceutical science and pharmacy in 1989, the first proton pump inhibitor was about to be launched, the Cytochrome P450 System had just been discovered and Kary Mullis was four years away from receiving the Nobel Prize for the development of the polymerase chain reaction (PCR) method. At this point, only a few people were envisioning high-throughput screening and combinatorial chemistry as a major tool for drug discovery or that the human genome would be sequenced by the beginning of the new century. During the past decade we have progressed in not only DNA sequencing but in understanding the DNA, the "omics," Single nucleotide polymorphism (SNPs) and disease networks. While personalized medicine is still considered as being somewhere in the future by many scientists and healthcare professionals, it is already the reality for life-threatening diseases like cancer while making silent inroads into the treatment of many other diseases.

Scientists who finished their thesis in the mid-90s will recall the huge and heavy volumes of scientific journals in the library and the cumbersome process of making hard copies to reference. Today, literature research can be made at any time, easily, from a laptop with immediate access to download the full publication. The Internet has revolutionized access to scientific data. Thanks to the tangible value and ease of use, this information technology progress and its applications were quickly adopted by our society. But what about digital health applications? Young people like their wireless activity trackers to share their heart rates when exercising within their social network. But implementing digital monitoring of various clinical parameters and adherence into daily treatment practice still encounters concerns regarding data protection and potential misuse of digital clinical data as well as genomics data. However, once the benefit of digital health becomes evident for high risk patients or older people with mobility issues who cannot easily visit a healthcare professional, this innovation will be implemented. Moreover, wouldn´t it be interesting and motivating for a patient to track the positive clinical effects when taking the prescribed drugs using their smartphone? Some healthcare professionals might reply that every patient is different and not everyone can cope with technology or wants to know about the clinical parameters. This is a fair remark and we have to recognize that different patient populations have different needs. We are starting to recognize that patients play a critical role in achieving positive therapeutic outcomes and that the “one-size-fits-all” product concept no longer fits. What if we develop products that precisely meet patients’ needs and can be used intuitively, as intended? Wouldn´t the patient perceive this as a similar “innovation” to the smartphone?

Looking forward, pharmacogenomics and patient centricity are becoming integral to the design of innovative drug delivery technologies and resulting drug products. Innovation will come from integrated and flexible drug delivery systems tailored to patients with regard to their personalized therapy and specific needs.
