<p>From time to time, medical transcriptionists are suspicious about voice recognition software over taking their jobs. Even today we get tons of questions about this and so we thought it is better to address this issue.</p>
<h2><span style="color: #3533bd;">What the heck is voice recognition?</span></h2>
<p>For those of you who are not familiar with voice recognition technologies, let us give a brief introduction. In today&#8217;s medical transcription world, there are two kinds of transcribers &#8211; One who transcribes video to text and predominantly called as medical transcriber or medical transcriptionist. The other is the guy who edits it. It is the responsibility of the editor to quality check and make sure that it is good to be delivered to clients. Now there was a period when software burst happened and software came in the market stating that they will be able to provide good transcription. However most of them gave 40 to 60% accuracy. None of the software was able to produce 100% accurate result which may be impossible with the current technology framework.</p>
<h2><span style="color: #3533bd;">Voice Recognition is there for a while:</span></h2>
<p>You may wonder, but voice recognition technologies are there for a while right from 2006. Multiple companies have tried to provide the best transcription that can be used for medical transcription. But till today there is no one stops solution for this. There is no software that can provide 100% accuracy results when compared to human transcribers.</p>
<h2><span style="color: #3533bd;">Will MT business slow down because of software domination?</span></h2>
<p>Since its inception, there were multiple questions. Right now situations for MT are so clear and people are still outsourcing to humans to interpret their audio. But in future there may be slight slip in terms of relying humans for transcription. However the editing part of the transcription business will be always there as software reaching 100% accuracy is very much unlikely to happen.</p>
<h3><span style="color: #3533bd;">Few disadvantages of voice recognition:</span></h3>
<ol start="1">
<li>Well, it is not accurate. A well spoken audio file (which is difficult by itself), can be translated in the accuracy range of 40% to 60%.</li>
<li>This low quality transcription has to be proof read and changes have to be made.</li>
<li>Voice recognition does not recognize all slang of a doctor.</li>
<li>Voice recognition needs crystal clear audio.</li>
</ol>
<p>With all this challenges we are sure that Medical transcription is still a blooming industry with tons of job offers with high quality pay. We still see people taken up <a href="http://www.themtassist.com/medical-transcription-course/">MT course</a> which is really a great sign for this industry.</p>

Most of the folks say that this headset is very unique, simple and more importantly cheap. The cost is just $14.24 at the time of writing this article.</p>
<p>One of the user stated that this headsets creates a sore feeling over the ear. This is common for most of the headsets and can be ignored. <p>It is a process of updating the patient&#8217;s record with the diagnosis that was done and transferring this information to the insurance company in the electronic format. Medical billing and coding also involves the follow-up process that are involved in getting the right money to be reimbursed for the treatment. It is a multi-step process involving various stakeholders. The typical time frame for this process can be any where between few weeks to months depending upon the amount of investigation that is needed. Here are the steps involved in this process.</p>
<h2><span style="color: #3533bd;">Patient Medical Record and Audio Recording:</span></h2>

The medical record has the entire history of the claimed disease. It starts with the incident details, patient name, diagnosis and sensitive details like patient contact number, social security ID, insurance policy number etc.,</p>
<h2><span style="color: #3533bd;">Coding starts:</span></h2>
<p>Now the real job of the coder starts here. The medical coder starts listening to the conversation audio of the interaction between doctor and patient, reads patient records and analyzes the situation. After understanding the situation and analyzing the problem, the coder converts this into the standardized ICD-9-CM format. These are numeric codes that are being standardized internationally.</p>
<h2><span style="color: #3533bd;">Transfer to Payer:</span></h2>
<p>The payer is typically the patient&#8217;s insurance company. Now here on the medical billers job starts. He or she transfers this ICD-9-CM format codes to the insurance company electronically. Now as per the HIPAA (Health Insurance Portability and Accountability Act) rules the medical biller has to stick with multiple set off rules. One of the most important rules is to stick with electronic format when it comes to transfer of information.</p>
<p>Now the first form of interaction from the medical biller to the payer is called as ANSI 837. Here ANSI stands for American National Standards Institute. This has a large chunk of patient and incident details that are being coded by the medical coder.</p>

<h2><span style="color: #3533bd;">X12-997 and X12-835 Transactions:</span></h2>
<p>Subsequent to the receipt of the ANSI 837, the payer will issue X12-997 acknowledgement. It is just to inform the biller and the hospital that the claim has been received. Upon inspection and verification, the payer finally send X12-835 transaction which shows a detailed information on the claim.</p>
<p>In all this process of interaction and follow-up with the payer, medical biller is involved.</p>

The job of a medical transcriptionist is to convert the dictated text to proper medical transcription report. <p><strong>Past Surgical History: </strong>Bladder suspension, Appendectomy, Hysterectomy.</p>

<p>Verapamil 120 mg per day, Atenolol 50mg per day, Celexa 40 mg per day, one baby aspirin per day.</p>
<p><strong>Social History: </strong></p>
<p>Tobacco Abuse – Positive</p>

<p>Coronary artery disease – Positive</p>

<p>Sinus rhythm devoid of clear abnormalities in ST segment.</p>

<p>Patient denies night sweats, chest pain, fevers, diabetes mellitus and cough. 10 point system review is negative.</p>

<p><strong>Heart</strong> :  Regular rhythm, S1, S2 diminished. Systolic murmur in right sterna border.</p>
<p><strong>Lymphatics:</strong> Post Amputation, Negative</p>
<p>&nbsp;</p>
<p><strong>Abdomen</strong>: Non-distended, Non-tender and Soft. There is no rebound, masses, guarding, ascites, hepatosplenomegaly, ascites, hernias.</p>
<p><strong>Genitalia</strong>: Normal</p>
<p><strong> </strong></p>
<p><strong>Rectal</strong> : Positive stool</p>
<p><strong>Chest</strong> : No rales, wheezes, rhonchi and Percussion dullness.</p>
<p><strong> </strong></p>
<p>She is not suffering from acute distress. Respiratory exams are clear for auscultation and neck veins aren’t distended. Cardiac exam reveals regular with systolic murmur and S1 and S2 are diminished. Soft and non-tender abdomen with palpation.  GU exam does not reveal costovertebral tenderness and neurological examination is not-focal.</p>
<p><strong>Impression:</strong></p>
<p>Chest pain with irregular features and is not related with exertion and radiation is present all over her body and there is no strong explanation for this. Though it represents arrhythmia, non-cardiac or cardiac problems will be clear.</p>
<p>Mitral Valve prolapse:</p>
<p>There is no clear evidence and it has been previously documented. There was’nt any murmur with this examination.</p>
<p><strong>Recommendations:</strong></p>
<p>The patient has symptoms related to palpitations, preserved exercise tolerance and irregular chest pain and there is no clear explanation for chest pain. Apart from that, basic metabolic reaction like lipid, C-reactive proteins and BMP has been checked. I have asked her to follow up after 6 weeks. The patient will require gastro copy and colonoscopy for viewing his cardiopulmonary and amputation status. He will require twenty four hour admit on careful observation and IV fluids.</p>
