Declaration of Independence

This paper will explain the basic ideas contained in the Declaration of Independence, the impact of the Declaration upon the American War of Independence, and the reasons the Declaration was considered a "radical document". These themes will be discussed and analyzed to demonstrate the premise of the Declaration of Independence, as well as to explain why and how it worked within the time it was written in.

A Brief analysis of the Declaration of Independence

By understanding the framework of time that the Declaration of Independence was written in, we can see its impact upon the peoples of early America and how they dealt with this famous writing. This paper will explain the basic ideas contained in the Declaration of Independence, the impact of the Declaration upon the American War of Independence, and the reasons the Declaration was considered a "radical document". These themes will be discussed and analyzed to demonstrate the premise of the Declaration of Independence, as well as to explain why and how it worked within the time it was written in.

When Thomas Jefferson wrote the Declaration of Independence, he foresaw the coming of great changes within the times that he lived in. The Declaration embodied many of the ideas that separated the colonies from England and thus began the process of creating a new country out of the fray.

One of the ideas that Jefferson wrote about was the freedom of religion. This was a core idea that made the basis of a republican society work, via separation of church and state. This caused many schisms in the many faiths that were already implanted in the colonies. Moreover, it took a long time to separate the church from the state because of the old foundations set by England. Jefferson set the trend for freedom in this writing to give everyone a fair chance to be “whomever they wanted to be regardless of race, creed or religion” (Zinn p.77).

Another idea entrenched in the Declaration of Independence is that men should not be subject to the hierarchy that was the foundation for a monarchical government -- such as the one that England had used unsuccessfully with the colonists in the new world. A king could do as he pleased and thus wreaked havoc within the colonies by taxing without representation. This started the whole “windfall of rebellion by the colonists from the North to the South” (Maier p.236). Jefferson wrote that all people were created equal -- and this was the great democracy that he and others envisioned in their time. Jefferson believed that the English form of governing was no longer the acceptable way for a human to be treated and this was the basis for him to ink the Declaration against primogeniture and other forms of “special” treatment for Lords who governed the colonies so poorly through the influence of King George the III.

The Declaration of Independence inspired the American Revolution because it rebelled against the harsh treatment that many colonists underwent due to the unpredictable behavior of the English monarchy. “Riots and protest were started and sometimes ended in blood-shed” (Maier p.45) and many people began to assemble militias and guerrilla styled armies. Many wealthy merchants and farmers realized that they could better govern their own people in a rational manner, rather than utilizing the sometimes haughty and foolish measures of the British.

Predominantly, these were the reactions of the common people, but the new congress did debate some of what Jefferson had written and “saw his criticisms of King George somewhat too much” (Stein p.112). There were some who believed the document was the unreasonable result of the hot temper that Jefferson wrote with at certain times. This document, even though some powerful men of the colonies thought it reactionary, was in many ways a radical document because it reintroduced the old Greek form of Republican government and set the tone for the later documents that would make the ideas law. It was a great split from the Old World and cut the chain from England as a ruling body that was attached to early America.

In conclusion, we find that the idea of men being created equal is the main focus of the document, and thus separated England from America during the late 18^th century. Even though some considered it to be a radical text, ultimately, the Declaration of Independence is a great document.

Post-mortem Care

Post-mortem Care Procedure:
1. Check the physician’s order. Check the chart for the physician’s certification of the death.
2. Ask permission and explain to the significant others that you are going to perform post mortem care. Ask the relatives if they wish to stay while doing the procedure.
3. Place “NO VISITOR-CHECK AT NURSE’S STATION” – sign to door. Provide privacy. If in the ward, draw curtains.
4. Do handwashing.
5. Place body in a supine position in bed flat (Align body in natural anatomical position with body on back, arms along side and palms turned toward thighs – poor alignment may cause irregular body formations/deformities as a result of rigor mortis.)
6. Place pillow under head to prevent deep, reddish-purple discoloration around sides of the face, earlobes and neck.
7. Prepares necessary equipments. Pour lukewarm water into the basin.
8. Don gloves.
9. Remove jewelries and personal items. Return these to the family in the presence of a witness. Do not remove engagement or wedding ring unless specified by an order or according to hospital policy.
10. Replace soiled linen and straighten top linen.
11. Close patient’s eyelids by applying light pressure downward with the fingertips. If eyelids will not remain closed on their own, initially, you can place wet gauze pads over closed eyes until it remain closed on their own. Put dentures if any. Then close the patient’s mouth.
12. Remove all equipments, tubes or clamp all drains according to hospital policy. Remove IV and other tubes unless autopsy is to take place.
13. Dip washcloth in basin with lukewarm water. Then wrap it around your hand to make a mitten. Using a circular strokes, wet forehead, eyes, cheeks, nose, mouth and ears. Using same technique, soap the areas mentioned then wash with lukewarm water, using same technique.
14. Get cotton balls and put them on patient’s closed eyes, nares and outer ear. You may secure it with each tape lengthwise depending on hospital policy.
15. Place a small rolled towel into chin to prevent the lower jaw from sagging.
16. Drape the patient with a blanket, then remove the top sheet underneath.
17. Remove patient’s clothes carefully as to not expose any body parts.
18. Perform bed bath. Expose farther arm, place bath towel under it. Using circular motion, wet, soap, rinse and dry thoroughly. Expose the other arm, do the same procedure. Using the same technique, wet, soap and rinse the chest, abdomen, sides and pubis. Turn patient towards you. Place the towel lengthwise under the patient’s back, down to the buttocks. Wet, soap and rinse the back and buttocks thoroughly. Expose farther leg. Place the towel under it. Wet, soap and rinse thigh, leg, groin and foot. Do the same technique to the leg nearer you. Finish the bath doing the same too the patient’s genitalia.
19. Put diaper, underwear and patient’s new clothes.
20. Remove bath blanket.
21. Give time to the family to view the patient, encouraging them to say goodbye through touching and talking. Give time not to rush the family or relatives.
22. Then, secure ankles and wrists. Cross right ankle over left, place disposable pads between ankles and secure with gauze roller bandage or according to hospital policy. Cross right wrist over left, placing on the chest, place disposable pads between wrists and secure with gauze roller bandage.
23. Attach two body tags, DA Form (DEATH TAG). Tie death tag to the great toe and left wrist. (Do not tie the tags so tight as to cause severe skin discoloration.
24. Cover and secure the patient with the shroud. Secure it with safety pins and secure another nametag on the shroud.
25. Transport patient to the morgue. Lift the patient, taking special care for the head and limbs and transfer onto the cart from the morgue. The head or the cart should be elevated in a continuing effort to prevent liver mortis.

Nursing leadership and management

Effective management leadership in nursing is not a career that is suitable for everyone or for every nurse. Leadership positions not only require additional sacrifices on your behalf but it also requires someone who is a critical thinker, in other words, somebody who can work effectively under pressure and who is able to resolve problems within the workplace quickly and efficiently and without conflict. Nurses who assume leadership and management roles also need to be creative thinkers. Not only is their work demanding of them, but they too have to oversee that everyone else is fulfilling their duties and that patient care is of the best. Within a position of leadership, one should be a role model and a positive influence on the nurses within the hospital as well as on the image of the hospital as a whole. Therefore, effective management leadership in nursing does not mean just barely surviving the job, it actually means taking it full force and making it flourish!

A nurse leader may be a nurse manager, responsible for one nursing unit, or a nurse executive, responsible for all in-patient nursing units. Rather than choosing just one leadership style, an outstanding nurse leader typically uses multiple styles depending on the situations she's facing.

Ensuring the safety of everyone who comes into contact with health services is one of the most important challenges facing health care today.
Patient safety is a new healthcare discipline that emphasizes the reporting, analysis, and prevention of medical error that often leads to adverse healthcare events. The frequency and magnitude of avoidable adverse patient events was not well known until the 1990s, when multiple countries reported staggering numbers of patients harmed and killed by medical errors. Recognizing that healthcare errors impact 1 in every 10 patients around the world, the World Health Organization calls patient safety an endemic concernIndeed, patient safety has emerged as a distinct healthcare discipline supported by an immature yet developing scientific framework. There is a significant transdisciplinary body of theoretical and research literature that informs the science of patient safety.^[2] The resulting patient safety knowledge continually informs improvement efforts such as: applying lessons learned from business and industry, adopting innovative technologies, educating providers and consumers, enhancing error reporting systems, and developing new economic incentives.

The simplest definition of a health care error is a preventable adverse effect of care, whether or not it is evident or harmful to the patient. Errors have been, in part, attributed to:

Human Factors

Variations in healthcare provider training & experience,fatigue,depression and burnout.
Diverse patients, unfamiliar settings, time pressures.
Failure to acknowledge the prevalence and seriousness of medical errors.

Medical complexity

Complicated technologies, powerful drugs.
Intensive care, prolonged hospital stay.

System failures

Poor communication, unclear lines of authority of physicians, nurses, and other care providers
Complications increase as patient to nurse staffing ratio increases.
Disconnected reporting systems within a hospital: fragmented systems in which numerous hand-offs of patients results in lack of coordination and errors.
Drug names that look alike or sound alike.
The impression that action is being taken by other groups within the institution.
Reliance on automated systems to prevent error.
Inadequate systems to share information about errors hamper analysis of contributory causes and improvement strategies.
Cost-cutting measures by hospitals in response to reimbursement cutbacks.
Environment and design factors. In emergencies, patient care may be rendered in areas poorly suited for safe monitoring. The American Institute of Architects has identified concerns for the safe design and construction of health care facilities.
Infrastructure failure. According to the WHO, 50% of medical equipment in developing countries is only partly usable due to lack of skilled operators or parts. As a result, diagnostic procedures or treatments cannot be performed, leading to substandard treatment.

The Joint Commission's Annual Report on Quality and Safety 2007 found that inadequate communication between healthcare providers, or between providers and the patient and family members, was the root cause of over half the serious adverse events in accredited hospitals.^]Other leading causes included inadequate assessment of the patient's condition, and poor leadership or training.

Common misconceptions about adverse events are:

"'Bad apples' or incompetent health care providers are a common cause." Many of the errors are normal human slips or lapses, and not the result of poor judgment or recklessness.
"High risk procedures or medical specialties are responsible for most avoidable adverse events". Although some mistakes, such as in surgery, are easier to notice, errors occur in all levels of care.Even though complex procedures entail more risk, adverse outcomes are not usually due to error, but to the severity of the condition being treated.However, USP has reported that medication errors during the course of a surgical procedure are three times more likely to cause harm to a patient than those occurring in other types of hospital care.
"If a patient experiences an adverse event during the process of care, an error has occurred". Most medical care entails some level of risk, and there can be complications or side effects, even unforeseen ones, from the underlying condition or from the treatment itself.

It is a pervasive developmental disorder characterized by the development of multiple deficits after a period of normal functioning. It occurs exclusively for girls, is rare, and persists throughout life.

· Extrapyramidal Symptoms – EPS are caused by antipsychotic treatment and need to be monitored/evaluated for early intervention.

Akinesia – rigidity and bradykinesia

Schizophrenia

In 1908, Eugen Bleuler, a Swiss psychiatrist, introduced the term schizophrenia, which replaced the term dementia praecox, used bu Emil Kraepelin (1896). Kraepelin viewed this disorder as a deteriorating organic disease. Bleuler viewed it as a serious disruption of the mind, a splitting of the mind.

It is the study of distance zones between people during communication.

Four Distance Zones:

1. Intimate zones – 0-18 inches between people. This amount of space is comfortable for parents with young children, people who mutually desire personal contact, or people whispering. Invasion of this intimate zone by anyone else is threatening and produces anxiety.

2. Personal zone – 19-36 inches. This distance is comfortable between family and friend who are talking.

3. Social zone – 4-12 feet. This distance is acceptable for communication in social, work and business settings.

4. Public zone – 13-25 feet. This is acceptable distance between a speaker and an audience, small groups and other informal functions.

Florence Nightingale

Born in Florence, Italy on May 12, 1820

She chose not to marry, to pursue her life's ministry of social action, which she described as "mankind creating mankind."

Before her intervention nursing was often regarded, at least in England, as 'a menial employment needing neither study nor intelligence' .

In her early twenties Florence began visiting hospitals. In Egypt she met some of the Sisters of St Vincent de Paul who were nursing in Alexandria and recognised the value of their discipline, organisation and training.

In August 1853 she started work herself as the superintendent of the recently-opened Hospital for Invalid Gentlewomen in London.

At the request of the British government, she agreed to go as Superintendent of Female Nurses in Turkey to organize medical care for the British soldiers injured or ill in Scutari during the Crimean War (1854-56). She took thirty-eight nurses with her, eighteen of them Roman Catholic or Anglican nuns. In 1856 Florence Nightingale returned to England as a national heroine. She had been deeply shocked by the lack of hygiene and elementary care that the men received in the British Army.

While in the Crimea she became ill with Crimean Fever and was invalid from 1858-1888 due to her debilitating and excruciating symptoms. An innovator and a driving force for public health reform, she brought about significant changes in the health of British soldiers and established the first secular School of Nursing. All of these achievements were accomplished by her as an invalid at home suffering severely from an illness acquired in service to others

National Center for Mental Health (NCMH) is leading by example when it comes to maintaining good hand hygiene and preventing the spread of infections.

On 13th of May 2011, every staff of different pavilions had joined the Department of Health ‘Clean Hands Every Time’ campaign. NCMH supported this campaign which aims to raise the awareness of every individual for preventing the spread of MRSA in sporting facilities, as well as reinforcing the messages given regularly by the Infection Control Team in Noble’s Hospital to promote effective hand hygiene.
There are three main messages in this campaign:
• Clean your hands frequently: Thoroughly washing the hands with soap and water for 15 to 20 seconds - rubbing to get lots of bubbles all over the hands to loosen and remove the oils and germs, followed by a good rinse and then drying properly. Moisturizing your hands regularly is also very important for both men and women.
• Remember when to wash: After you use the toilet, change a nappy, touch an animal, blow your nose, cough or sneeze, or handle rubbish. It is also important to wash before preparing or eating food, treating wounds or giving medicine, and tending to a sick or injured person.
• ‘Catch it, Bin it, Kill it’: You can reduce the spread of germs by covering your nose and mouth with a tissue whenever you sneeze or cough. If you don’t have a tissue, coughing or sneezing into your arm rather than into your hands is preferred.

NCMH has set May 13, 2011, Friday, 10:00 a.m as a day to implement a health teaching which focused on hand washing to demonstrate the major commitment to making patient safety a daily reality. Nurses and other healthcare professionals believe that handwashing is a major priority to prevent the spread of infection in hospital.

Pavilion 5 supported the hospital activity. Pavilion 5 staffs and nurses conducted health teachings on proper handwashing to patients and nursing affiliates. Posters and lectures were given to emphasize the importance of washing hands. After the health teaching, we conducted a returned demonstration to check if they can do handwashing properly.

Importance of Handwashing

Hand washing is an important hygienic habit to stay hale and hearty. It not only helps in preventing germs and bacteria, but also helps in maintaining a good healthy life free from diseases. We all come in contact with multitude of germs and bacteria in our day to day living. These micro organisms are present all around us, on the door knobs, water taps, light switches, railings and yes, even on your mouse! While it is impossible to live without touching these objects, it is imperative to be cautious to stay disinfected. Touching the eyes, nose, ears or any other body part and eating food with the infected hand are common ways by which we unknowingly inject the germs and bacteria in our body, thereby making ourselves vulnerable to diseases. The knowledge and importance of washing hands would help in staying free from germs and bacteria.

The most important reason of hand washing is to prevent getting infected with dangerous aliments. People who don’t wash their hands regularly are at the higher risk of getting infected with diseases like cold or flu. Eating without washing the hands often causes gastrointestinal troubles and diarrhea, which can spread within the family. In some cases, improper or no hand washing can cause pneumonia in elderly people and those who are facing chronic diseases. As such, washing hands is prototypical for staying in the pink of health. Remember, prevention is always better than cure. Hence, always clean your hands and stay germ free. By indulging in this easy activity, you would not only stay fit but also avoid unnecessary medical bills, as there would be fewer chances of you getting sick.
