Lashfood for Thicker, Longer Eyelashes

After years of selling Revitalash, we were recently approached by the folks bringing in its direct competition, Lashfood. Both products are effective for enhancing natural eyelash growth, producing thicker, longer, darker eyelashes. One major drawback with Revitalash was the cost. At $238+GST, even with each tube lasting over 6 months, most found it a tad too costly. We did have some repeat buyers though, in that they saw great results and came back to buy more for their friends and relatives.

Doing my due diligence, I searched out this new product online, and found nothing but rave reviews. This is in contrast to Revitalash which did have some issues regarding irritation. Certainly, the original Lashfood conditioner which is the first product to be brought into Singapore, is specifically compounded to be safe even for the most sensitive eyes and even for those wearing contact lenses.

With regards to ingredients, all the components are natural, so no chemicals will be introduced anywhere near the eye area. The names are familiar, with copper, Arginine, Panthenol and Biotin being the key ingredients, all having good evidence of reversing hair loss. The product can even be applied on eyebrows, should there be any sparse areas or just general thinning.

The product is applied like an eye liner, twice daily initially. Results are seen after about 4-8 weeks. Pictures of the dramatic thickening effects can be seen on the Lashfood website.

Each tube costs $190 (price inclusive of GST). The distributors have offered our clients a 10% introductory discount for the month of June 2010, following which the price reverts back to normal.

The Pain of Hair Loss

With a whole series of cancellations this afternoon, it was the ideal opportunity to slip across the road for a quick haircut. I made the very difficult decision of abandoning the hairdresser I'd followed around for almost a decade a month back. She had, in that time, moved from Wisma Atria to Harbourfront and a few months ago to The Sail. I visited her once at the new premise, but I hated the drive there, even though it was much closer than Harbourfront. I had gotten so lost on a relatively quiet Saturday afternoon. By the time I'd driven around the CBD 5 times, I was a frazzled mess! And I was so traumatised by it that by the time I was desperate for another haircut a good six months or more later (the joys of a pony tail), I could no longer remember how to get there!

So I made the difficult decision to switch. I was very concerned as all the haircuts I'd had before finding her were awful. But as I'd gotten friendly with the folks at Perfect Style (our old UE Square neighbours), and I had seen the work of William, I decided to take the plunge & chopped my locks off a month back. And as with all shorter hairstyles, the inconvenience of regular trims surfaced. Hence I was glad to just be able to slip across for a tidy up.

William is the bggest gossip I know. He's certainly entertaining & knows everything about everyone! Today, he commented that I was lucky to have naturally straight & oily hair. It saved me a fortune in treatments! My response was I would love thicker, more luscious hair, but have been too busy to try out our new hair treatment. He asked what hair treatment, so I showed him our results from the Divine MedSpa website.

His jaws dropped!

Poor William has a small thinning patch in his crown. He'd seen his GP about it years ago, and was dismissed with normal male pattern balding. As a hair stylist, he was sometimes even embarassed by his clients for not having perfect hair. And it certainly stopped him from recommending tonics to prevent hair loss. Anyway, his interest was piqued when I told him of our experiences with the treatment, and he was almost ready to run across the road for his first treatment.

It as certainly an eye-opening conversation for me. Having been dealing primarily with skin anti-aging, acne & slimming for so many years now, I never really stopped to think of the impact of hair loss. I was just dubious of expensive treatments at Yun Nan and Beijing 101. All that ridiculous hair analysis for what??? Hair loss is hair loss when seen with the naked eye or magnified 100x. And results are only worth talking about when the growth can be seen with the naked eye. And our clients have certainly been happy with their own results, and have had many commenting on their luscious locks.

Anyway, I'm hoping William will slip across for his treatments soon! I think he'll have fabulous results!

Painless Bikini IPL Permanent Hair Removal?

Where smooth armpits have been "norm" for years, having no hair or a neater "bush" has only been in fashion very recently. Waxing and shaving have been the norm in this area, but the trend has been strongly moving towards permanent hair removal, be it by IPL or laser.

We've certainly been zapping quite a few groins, particularly after our move to Robertson Walk a year ago. The interest was mostly amongst the expat community intially, but the trend has caught on with local Singaporeans in the last couple of months.

And with that change in client profile, we've noticed that what is a slightly uncomfortable procedure in Caucasians can be extremely uncomfortable amongst Asians, especially if the skin is strongly pigmented. This isn't terribly surprising since the energy emitted is targeted at melanin in hair, but is also present in dark skin.

As I'm incredibly pain averse, I re-acquainted myself with my Entonox (laughing gas) cylinder during a recent fractional RF trial. I have 3 cylinders which have been sitting idle for the last few years, after we stopped mesotherapy.

I then realised that this would be an ideal way of controlling the discomfort of Bikini IPL Permanent Hair Removal treatments. So we contacted a few gas supply companies to enquire on the current re-filling rates. It has always been expensive, but I nearly fell off my chair at how much the refills now cost, plus the turnaround time was a good 6-8 weeks!

But how much to charge? At a knee-jerk $50/session, there would likely be few takers. We had to try to work out as close to the real cost of refill as possible, then re-assess once we started using the cylinders in practice.

Fortunately, one of the cylinders was labelled with the gas volume. And that's where the Anaesthetist in me emerged. Based on the average volume in one breath and number of breaths per minute, we were able to work out the refill cost per 10 minute use. And we came up with the figure of $20. So that works out to just an extra $100 for each 5 session package. Hopefully that will just about cover our cost. We will have to keep a close eye on the treatment time though, and charge in 10 minute blocks. Just in case we meet someone who forces us to pause after every 3 shots. Then a 10 minute treatment can drag on for half an hour.

I've also been asked about numbing cream as an option. Honestly, I don't know how effective that would be. But if anyone is keen on that as an alternative, we can supply it at $10/session.

So painless bikini IPL permanent hair reduction... is it possible? Maybe not painless, but certainly tolerable. And we offer these pain management options at a very nominal cost.

Visagel for $600/syringe

Our suppliers are currently running an offer on Visagel dermal fillers at the moment, which brings our base cost down significantly. Rather than pocket the difference, we have decided to pass the savings onto our clients. So until the promotional stocks run out, Visagel costs $600/syringe. Each syringe contains 1 ml of filler, which is often more than needed for one treatment. If there are any remnants, we will store it for you, and subsequent top-ups will just cost $50 each time.

We have been using Visagel for over 2 years now, and are very comfortable with the product, and happy with the results and safety profile. It's a German product and is CE certified.

The alternative filler option available is Juvederm, which costs $800/syringe and each syringe contains 0.8 ml of product.

We brought in 20 boxes of Visagel at promotional price (10 boxes each of 16 and 24), so anticipate this offer will not last long. Delivery was made yesterday, and the first box used this afternoon, with a couple more booked in for tomorrow. We do not know how much longer our suppliers will be running their promotion for.

Visagel 24 is used for nose augmentation and filling out the nasolabial folds. Visagel 16 is used for lip plumping and filling in hollow tear troughs. Once the promo stock runs out, the price will revert back to $800/syringe.

Thank God for Entonox!

I'm not at all ashamed to admit to having zero tolerance for discomfort. Vanity takes over, but I do everything to make my treatments comfortable.

This afternoon, I had the opportunity to try out E Matrix personally. Manufactured by Syneron, this device uses fractional RF to coagulate the skin and stimulate collagen production. The treatment can be used for rejuvenation, but it's primary indication is for acne scars and wrinkles.

The before/after photographs shown were AMAZING! It's a shame we're not allowed to put up such photos on Singapore medical websites now. All doubts about whether the treatment works or not were cleared when I saw them.

So I bravely volunteered my face again. The rep had tried the medium intensity program on himself with no pain relief, but that was a bit too ambitious for me. So I applied a thick layer of numbing cream and waited...

After my lips went quite numb, we started. Even with the cream on, it felt like hot pricks. He reduced the intensity from medium to low, but it was still too much for chicken shit me! So I jumped off the couch and dug out my old Entonox cylinder. Three puffs later, I was high as a kite and happy for him to continue.

We bought the Entonox equipment when we opened the clinic almost 4 years ago. It's pretty much just gathering dust these days, cos most people are much better with pain than me! But it really was such a relief to be able to use it for this procedure. Phew!

As for my face, it's a bit red & swollen. It was quite sore for the first couple of hours, but much better now. I can't wait to see the results!

Rejuvenating Simply Aesthetic

The winds of change finally shifted towards Simply Aesthetic last week. With the departure of Joey and Candy within days of each other, the energy has changed considerably. Julie has finally settled the spa sufficiently to start focusing her attention on the clinic. And I've been very blessed in being able to persuade an old friend to come in to help organise the place medically.

I first met Celia in 2001 when I went to Cambridge to sub-specialise in Day Surgery. A fellow Singaporean, she was the Sister in charge of the transplant theatre. She also happened to live just around the corner from me! Although we tried to keep in touch after my return to Singapore, the inevitable happened and we lost contact with each other.

Out of the blue, I received a Facebook friend request from her. So I accepted and we sent a few messages to each other. Her comment was that I looked so different she'd never recognise me!

At the end of last year, she announced that she was taking early retirement, and heading home to Singapore. We had planned on meeting up, but never got round to contacting each other till a couple of weeks ago. I realised I needed someone strong and organised to come in to set the procedures right. Without a sensible leader in the clinic, things were spiralling out of control. Previously, we had either Mery or Dyna co-ordinating everything. Since they left, it seemed they took common sense with them too! So we had to come up with procedures & operational policies for everything. Decency & integrity seemed to have disappeared too, and I'd been robbed blind by some of the staff that had left!

So Celia started earlier this week. She's written out the policies and is now organising the chaos the old staff left behind. Julie has removed anything that is remotely scary for anxious patients in our treatment rooms. Jam, our spa receptionist has kindly donated sarongs, to match the bed skirtings, which have finally arrived. The rooms have now a much softer feel, quite different from the minimalistic clinical feel it had before. Every corner is being cleaned, and all unnecessary clutter tidied away.

Staff-wise, MJ is learning to cope with being the only trained Clinic Assistant in place. Marigold, another nurse from the Philippines is finding her feet. Aubrey's pass has just been approved (finally!) and she arrives next week. We have also just interviewed Jam's friend, who is keen on coming in as our Intern.

With the new furnishings in the treatment rooms, the walls suddenly look very bare. So I'm in the process of ordering some prints for the walls. I've chosen a pretty floral theme with frangipannis. These should be up by the middle of next week.

In addition to our new takeaway menus (designed a few weeks ago), we will soon have some posters outside our front door, so it's not so easy to walk past without realising we exist.

All in all, it feels really good to finally move towards the next phase with a new team, and two fantastic women helping me. After years of struggling on my own, it's such a relief to finally have assistance, and I'm truly blessed as both Julie and Celia are power houses on their own right.

XL Hair Update

We often take our crowning glory for granted, but premature hair loss is becoming a very common problem, in women as well as men.

As a result, we've gone through all kinds of hair loss treatments over the past few years, mostly with disappointing results. The brochures and data are always impressive, the science irrefutable, but the results not impressive. Actually, only mesotherapy worked. All other topicals were a waste of space. We went through Minoxidil, Floxia Hair and Ballvic Serum & Shampoo. We don't stock any of these now. In fact, we got the distributor to take back virtually the entire Floxia range as we really didn't think they were as effective as the other medical skincare brands.

So when I was introduced to XL Hair, I was skeptical. If not for samples being available, I may not have even tried the treatment. So when the hair growth was unrefutable after just a month of weekly treatments, I was very impressed!

We've since treated 2 men and 2 ladies. One lady was not compliant with the treatment schedule at all, but just based on the home spray alone, has now grown a very luscious head of hair that is the envy of all around her. The thinning area which occupied her entire crown previously, is barely visible now!

The most recent gentleman, still in the middle of his treatment program, has started sprouting baby hairs in the receding temple area, which has been totally bald for many years now. We had not held out any hope for this area, the prime aim of treatment being to stop the hair loss in the crown area, which was thinning out very quickly. That seems to be responding well too!

So I could't be happier to offer this new XL Hair hair loss treatment at Simply Aesthetic. Finally, we have something that is as effective as mesotherapy, without the discomfort. Infact, most describe it as a very soothing treatment! And yes, I'm drooling... My hair's not thinning by any stretch of the imagination, but I'm lusting after luscious locks too!

From iPico to iEllios

We've been waiting for the Perfector upgrade for over a year now. It's been a long wait, with many changes in design and technology over time.

We had been promised delivery in mid 2009, then late 2009, then early 2010. But Dr Weiss is determined to create the perfect rejuvenation machine, so has continued to modify the prototype.

The latest news is that the machines will finally go into production in end April. There's over 120 orders before me, so it is likely to be mid year at the earliest.

What is most interesting, is how the device has changed, from looking like the current Perfector, to a new-age clear blue case, so the inner workings are visible, to one that looks like a Mac.

They've also decided to produce a cheaper device with some Pico functions (now named the iPico), and iEllios, the ultimate rejuvenation machine.

As far as I'm aware, I'm getting the iEllios. I'll be really upset if they pull a fast one on me now. Not my fault the name has been changed!!! And I've not parked my deposit with them for a year to be passed off with an inferior machine. No siree!!!

Ion Magnum 2: Now at Simply Aesthetic

It's here! It's here! After weeks of waiting for the latest upgrade to roll off the production line, our Ion Magnum 2 has arrived!

Actually, it arrived yesterday, but thanks to some sticky beak nose customs officers, the machine arrived with blown fuses. That set off a major meltdown with me last night, but fortunately Cirsten pointed us in the right direction, so a simple fuse replacement got it working!

I dashed back to the clinic after my SNEC list this afternoon, eager to try it out for myself. The last time I tried the original Magnum was 3-4 months ago, and it was not nice. I take my hat off to my patients who have had their entire body treated at high intensity.

So does the machine live up to its promise of being comfortable?

Treating the tummy alone, it certainly seems to be. The contractions are certainly smoother and less painful than before. There's still some prickling involved, but nowhere near as much as before. As I was in a rush, I didn't do the standard measurements/photos. My main concerns were whether the machine was working and whether it was more comfortable. A big sigh of relief yes to both. Phew!

The New Ion Magnum's En Route!

Patience is a virtue. That's what I've been led to believe. But sometimes, I wonder how much stress is caused by waiting and waiting and waiting...

We've been talking about the Ion Magnum upgrade for months now. Much negotiation happened behind the scenes to finalize a price which was just about within our reach. The money was sent across in early January, then everything ground to a halt! The machine we were supposed to get went for further upgrading, got trapped in UK Customs forever, and we were left stranded!

Well, not completely, but the old machine is starting to rebel from overwork now, so we're ready for the upgrade in more ways than one.

FINALLY, I received the e-mail with our tracking number this morning. Assuming no further delays (and I hope to God they didn't put a box of chocolates in it again this time!) we should have the new machine with us on Thursday.

I'm hoping to be the first on this new baby, but not sure. Some days, appointments are back to back! I'm going to try though!

For those wondering if there'll be a price increase, the aswer is yes. No choice lah, our prices are half of what another Singapore aesthetic clinic charges. With the mega package, half the treatments are full body 60 minutes. The other clinic charges double for single area, less than 30 minutes for first generation machine. The upgrade has cost me twice as much as I was prepared to pay. But our prices won't increase till the new machine arrives, so come on over these few days if you wish to avail the current price.

New Acne Zapper Device

When I went to Cosmoprof in Hong Kong last November, the only item that really caught my attention was a handheld acne management device. It's not much thicker than a standard lipstick and promised to clear up acne spots within 24 hours.

The device is called Clean and is made in Korea. I spoke briefly to the guy in charge, but they were clearly not after individual clinics or spas. The minimum order volume was way more than I could ever hope to sell.

As I was visiting the fair with Andy from Ocean Health, I dragged him along to see it. It would be a great addition to their TDF acne skincare range. As with all skincare, however good it is, breakthrough spots will occasionally appear. This was ideal to clear it up quickly.

So how does it work?

The device emits heat and blue light. Both act to neutralise the acne causing bacteria. In fact, Zeno, which retails at about $400 works on heat alone.

This device is reusable, rechargeable and has no consumables. It should last for 2 years, and is charged through a USB cable.

The burning question in everyone's mind was "does it work"?

So Andy persuaded the Korean guy to sell him a few pieces for trial. Over the past few months, the tests that have gone on have shown great results. Spots have been clearing up in just 1-2 days, without any scars.

I sold one to a Bangladeshi patient, who brought it home & shared it with her relatives. She reports good response.

So we've started selling this device in the clinic. It's just $250+GST for now. Ocean Health is still trying to figure out the best price point for this, and this price is likely to increase in the near future. Hair Loss Treatment to Reduce Hair Fall

Many years ago, when we first started with mesotherapy, one of the cocktails was for stimulating hair growth. We tried it on one of Ivan's friends, and were amazed when we saw baby hairs growing after just 3 treatments. This is a young man who'd been thinning since his teens, and his hairline was receding pretty significantly.

By the end of the first course of treatments, there was a lot more hair than what he began with.

But mesotherapy was expensive. The products were costly, and it was a pretty time consuming process too. I really don't know how uncomfortable it was. I used a meso gun to inject the cocktail into the scalp. It felt like I was running a sewing machine over his head. There was a little bleeding too. He was very brave though, and endured it all very well.

Fast forward to 2010. Mesotherapy's been banned for years now. Hair lasers and hair transplantation are very much in. Hair loss, particularly in the young, is getting way too common. Blame it all on stress and junk food!

I was given a few samples of XL Hair to try out a few months ago. Our test case was a young man, still in NS. His hair was thinning out really badly. He'd been quoted thousands by a hair speciality centre, which was way out of his budget. For some reason, his girlfriend called us.

We started the protocol on him, not knowing what results we'd see. To our amazement, his hair started to grow back about a month after we started treatment. And it was a simply jetting of a hair tonic with 47 active ingredients into his scalp, followed by a massage. He did have to use a home serum 3 times a week too. But after 8 weeks, he was done!

This time, there was none of the discomfort or bleeding associated with mesotherapy. No drugs involved, just primarily vitamins, amino acids and minerals. Pure nourishing of the hair follicles. Good for male hair loss as well as female hair loss.

After waiting a few more months, stocks of the product are finally in. This may be the cheapest hair loss treatment in Singapore. I could charge much more, but I really don't see the point. I believe in fair prices, after all, this is likely to require maintenance treatments over time. And $150 per treatment is certainly worthwhile to maintain a healthy head of hair.

End of Liposuction at Simply Aesthetic

It is with great sadness that I announce the end of Waterjet Liposuction at Simply Aesthetic. Our final operation will be on Monday, 9th February.

This decision was a tough one to make. There were many factors, including the lack of demand for a relatively more expensive liposuction technique. Few seemed to appreciate the safety and recovery benefits associated with WAL. The recent liposuction death has reduced demand even further.

Most importantly, however, is that my personal health has not been optimal recently. I have had spasms in my left trapezius muscle behind my shoulder on and off for the past few months. There is some discomfort almost every day, which is usually tolerable. However, at times, the pain moves to my left arm and elbow. When this happens, the entire arm can go numb and weak. I can still just about cope with small liposuction procedures, but certainly not big ones.

With the unpredictability of this dysfunction, plus the negative market sentiments, we finally made the very difficult decision to discontinue liposuction services, at least for the time being.

Although liposuction is great for contouring, our recent results with Ion Magnum has shown that tremendous results can be obtained withoit surgical risk and downtime. The current machine is uncomfortable, but we have ordered the upgraded Magnum 2 which promises to be very comfortable. That should be arriving in the next few weeks.

Personally, I too have lost 4kg in the past month, with simple dietary changes (no starvation), oral supplementation and Thermojet sessions 2-3 times most weeks. You can read more about it in my personal blog. I'll be happy to share more during consultation.

Inevitable Price Increases

Despite our costly move to Robertson Walk, we've always tried to maintain our fair prices, particularly for popular treatments such as IPL and chemical peels.

Unfortunately, we're not only in the position of having higher overheads (rental, utilities and staff), but our suppliers have also increased the cost of our consumables and skincare products quite significantly over the last few months. For some items, the increase has been over 100%!

We therefore have had to increase the price of some of our popular treatments in line with the increase in our costs to deliver the treatments. It's unfortunately inevitable. We're keeping the costs as low as physically possible, and creating packages in order to create more value.

As such, we apologise that any quotations given previously may now be out of date. Please do check with the clinic staff (call 6732 5527) to confirm current pricing.

On the bright side, Simply Aesthetic now as a new manager who will oversee operations. Julie Lee comes from a spa background, and has many ideas on how to truly pamper each of our patients whenever the opportunity arises. So watch this space to find out the details as they unfold! Upgrading Ion Magnum

The Ion Magnum has become the most valuable asset that we own. The results we have had for some of the more persistent patients have been quite amazing. However, the number of patients who are unable to have adequate treatments because of treatment discomfort is also significant.

I had always thought of it as a "no pain, no gain" situation, plus "you reap what you sow". Afterall, even normal exercising can be uncomfortable and results are proportional to effort put in. However, I recently received an e-mail from Ion Genius, the distributors of Ion Magnum. It was primarily to inform us that the cost of the machine had increased yet again, but there was a small one-liner mention about thev latest version being very comfortable. Intrigued, I decided to find out more, and most importantly, if it was possible to upgrade mine.

After a lot of e-mailing back & forth to the Ion Genius team, we have finally come to a compromise. The upgraded machine will be ready sometime in February. They are including all the latest modifications, so we move from the earliest version to the latest. Treatments should be more comfortable. Resistance reduced, so results should be better. Personally, I'd love to get up high on the intensity scale & not feel too much.

As the upgrading comes with a hefty price tag (of double my budget, but hey, I'll do whatever is best for my patients), so once the new machine arrives, the price will have to go up. Sadly, my credit card bill will be here before the machine arrives. We are already offering the treatments at about half the price of another Singapore aesthetic clinic, but the new prices will still not be that high. I believe in fair prices and affordability long term. Clients who have expired packages will have to renew at prevailing rates once the new machine arrives. Those on existing packages will continue on the lower current prices as long as they maintain a balance, ie renew before expiry.

Sculptra Liquid Face Lifting

I'd heard of Sculptra many years ago, but other than it being a type of filler for large volume facial filling, never thought much more about it. However, when I saw a video presentation about it at a recent aesthetic conference, my interest was piqued. As an added bonus, as part of the launch of the product in Singapore, they were offering to train interested doctors for free. Their trainer was scheduled to fly in from Australia to give personal tuition to each interested doctor.

It was our turn today!

We had three excited (albeit a little nervous) volunteers lined up for the treatment. One had a full liquid face lift, going from temples to upper cheeks to naso-labial folds to jowls. The second had filling of her hollow tear troughs and below her acne scars. The last, sadly, was breastfeeding, so was postponed till January.

The technique is pretty straightforward. Each treatment could be done in 10 minutes once perfected. We didn't really take much longer, just wasted a lot of time joking around with each other. The volume increase is seen straight away, so the response from both volunteers was "wow!". Pin point bruising is possible, but this resolves quickly. The "swelling" lasts a couple of days, then the body slowly builds new natural collagen over the next ten months. A top-up (if required) is done a month later. The final results last 2 years.

So why do I want to learn Sculptra? We've used machines for face lifting for years now. It works well for some, not so well for others. The less adequate results are usually with people with low cheek volumes. Even surgical face lifting would not give great results with such folks, as the skin might be tight, but the features not youthful.

So what does Sculptra do? By stimulating new collagen production, it adds volume to the face. Three vials are required on average, but very thin faces may need 10! The skin quality also improves. And by injecting in specific traction lines, a lifting effect can be created.

I was quite concerned about the safety of the product. There are some scary stories online, afterall! So I quizzed them in detail. As a company, Sanofi looks at all the problems seriously, and concluded that the problems occurred with wrong technique and/or bad practice. Early on, they diluted the product with very little water, so the product set into hard rocks. Now, it is injected extremely dilute, and carefully massaged into the face to ensure even spread. Tear trough reconstruction can be safely approached by injecting upwards from the mid cheek. The lumps seen are from depot injections directly through the eye muscles into very thin skin. The product also had to be diluted the day before to ensure complete dissolution. That is a relief, as hollow tear troughs & flat cheeks are extremely common and result in premature ageing.

So I'm very happy to finally have a volumizing treatment available. Well, once we order the product anyway... But that won't be too far in the future.

Urticaria!

Urticaria is a skin condition that can afflict people for many months (to years) at a time. Itchy lumps appear out of nowhere & move from one area to another, causing havoc with work, sleep and social activities. It's not a condition aesthetic doctors deal with, but a topic dear to my heart as I have suffered on & off with this for decades! So when the opportunity arose this week for me to attend a lecture by a world expert from Germany, I jumped at the chance!

Turns out, there can be many causes of chronic urticaria, from infections (teeth, tonsils, gut), to environmental factors (hot, cold, exercise, etc), food intolerances and an inherent tendency towards the condition. And to complicate matters, an affected individual may have 2 causes, or require 2 sets of conditions to co-exist before developing urticaria!

I know I had food intolerance issues: the ALCAT test proved my excessive ingestion of tomatoes caused my incessant itching two years ago. The latest bout seems to have settled down, and I have yet to find a trigger other than "stress".

Which really brings me to treatment. Stopping the cause is ideal, but locating the cause may not be easy. The aim ultimately is for cure and/or stopping all symptoms. Without the cause, we are left with a series of pharmacological interventions. Starting with a non-sedating antihistamine at standard dose, then quadrupling it to maximum dose for another month. If this is insufficient, then other fancy drugs get added.

Anyway, the anti-histamines have been ordered. I'm desperately awaiting their arrival. Not so much for the skin now as for my allergic rhinitis. The joys of being atopic (tendency to allergic rhinitis, urticaria, eczema & asthma)...

How this is related to standard aesthetic practice? By controlling urticaria, we stop skin trauma from excessive scratching. And scratches can cause severe post-inflammatory hyperpigmentation (PIH). PIH is relatively easy to treat on the face (eg after acne), but seems much more stubborn on the body, particularly legs.

Melasma Treatment Options

I've been told that Melasma is very common amongst the Asian population. But in my 3 years of Aesthetic practice, I've hardly seen a single case! So although I was always intrigued by how to treat this very common condition, I've never had much opportunity to try it out.

Melasma, also known as Chloasma and "mask of pregnancy" is a hereditary condition that causes the deposition of pigmentation in the face. The brown discolouration has indistinct edges and predominantly affects both cheeks, athough the forehead ad chin can be affected too. It is predominantly seen in women, worsened by hormonal stimulation (pregnancy, oral contraceptive pill) and sun exposure. The pigmentation can be at various leves, from superficial to relatively deep.

The treatment options have been relatively limited, with creams containing Hydroquinone (such as Triluma) being first line. Although it is known that lasers do not improve the condition, it seems people continue to persist trying!

When we obtained the Nano Perfector last year, the treatment of Measma was one of the draws. Apparently, relatively recent onset Melasma could be reversed quite quickly with this device. We have yet to try this. Upon discussion with Dr Xanya Weiss recently, she now says that it can cause a lightening, but it didn't sound hugely dramatic. Perhaps it's as well we've not had any patients to treat in this way?

I'd also been hearing about the success our Thai friends have been havng with soft plasma peeling for Melasma. So when the opportunity came about for me to tag along with our local distributor for training, I jumped on it!

The treatment protocol for plasma peeling Melasma came about because the doctors were treating Melasma with lasers. What started off as superficial Melasma was recurring as deep Melasma. So from a light brown, it was turning very dark. Naturally patients and doctors were distressed, so they started to look for alternative options.

The protocol now involves rather aggressive plasma peeling with iontophoresis and the occasional glycolic peel. The skincare regime has also been documented, including exact quantities for best results. Downtime is minimal.

I met one lady who has been treated for the past year. She started off wth significant melasma over both cheeks. Now it is barely visible and her skin glows. There is still a light hit of the underlying condition, but she is happily continuing treatments in hope that it will one day be gone.

We are also bringing in Estelan, a program that is modified from Cosmelan. This starts off wth the application of a cream in clinic. This is left on for 4 hours, so washed off at home. Then twice daily application of Estelan 2 continues the treatment for the next few months. There is some peeing and redness after the initial peel, but by all accounts the results are pretty decent and provides rejuvenation & lifting as well as de-pigmentation. This is perfect for the person who is able to tolerate a few days of downtime and doesn't want the inconvenience of regular trips to the clinic for pasma peeling.

With the introduction of these two services, it's interesting that I am starting to see some patients with Melasma now. So I'm really looking forward to assessing for mysef which of these is more superior in the long term.

Surprisingly Effective Puffy Eyes Treatment

I saw my friend Ivan for the first time in ages yesterday. I was horrified to find huge eyebags under his eyes! I don't remember him ever having any problems in that area before. The skin also looked a little dark and water-logged. I was just about to convince him into a little bit of Botox when I remembered that I had a new eyebag treatment.

The new eye mask, in conjunction with Aesthetic Dermal's Meso-K Arnica Solution is primarily used for reducing dark eye circles. The Vitamin K helps to strengthen the blood vessels in the eye area. Leakage in weak blood vessels causes the deposition of pigmentation in the delicate under-eye area, hence dark eye circles.

The treatment is pretty straightforward: 3 drops of Meso-K Arnica solution is massaged under each eye. The mask is applied and moistened with toner. It is left on for 15 minutes before the mask is massaged into the under-eye area.

One hiccup: when the 15 minutes was up, I was the only one free! And I can't massage to save my life, let alone do any fancy lymphatic drainage movements. So poor old Ivan had me rubbing this stuff into his face, until Cha took over. Now Cha's a nurse by training, so although more gentle with facial massage, equally clueless about lymphatic drainage.

After about 10 minutes, the sheet mask turned sticky goop was pretty much all absorbed. We had to wipe off some of the residue so Ivan could open his eyes and see, but otherwise it went pretty smoothly for our little play.

Ivan's eyes looked brighter straightaway. Less puffy. This was even more obvious half an hour later. I have no idea how long the results will last. He didn't get a bottle of Meso-K Arnica to continue the treatment at home. So his results won't be maximised by any means. I guess if he likes it, he could purchase a full kit for home use. These are available by pre-order only as we don't hold any stock at the moment.

I'd say that this was a successful trial of a new product. The dark circles were certainly less apparent, and the puffiness totally gone. Bye bye eyebags! This service will be available at both Simply Aesthetic & Divine MedSpa once we finalise the price.

Last Few Weeks of Nano Perfector

The Nano Perfector has been a fantastic addition to my huge family of machines. It has been my favourite for face lifting and general rejuvenation. The response has mostly been positive. Instant results, painless, relatively long lasting... What's not to love?

But sadly, we'll be giving up this machine in just over a month's time. It'll be an upgrade to an even more powerful machine, that is currently still in the production phase. We've been waiting for this machine for the last 6 months. But they have delayed the production till they were absolutely 100% satisfied with the new machine.

And new machine it is! It started off as a new card within the Perfector casing. Now the engine is housed within a transparent casing, with snazzy LED display. There are 300 programs to choose from, and by all accounts the results are reven more remarkable than the Perfector. How could I refuse???

If everything goes to plan, the new machine will be delivered in early September. The price will increase, to cover the cost of the upgrade. However existing clients will continue to enjoy the current price as long as their packages have not lapsed. The increase will be at least 20% above current prices. We have a number of patients who have pre-purchased their treatments at the current super-low Perfector prices and are waiting (very impatiently) for the new model to come in. Us too!

We've got Ultrasonic Cavitation!

Ultrasound has been used for some years for fat melting. The technology has evolved significantly in the last year or so, with the development of cavitation.

When ultrasound is applied to liquids, compression and decompression waves are created. In the decompression phase, the negative pressure generated produces bubbles within the liquid. These expand and enlarge until the compression phase begins. This compresses the bubbles until they implode. Heat is generated in the implosion process and the fat cell membranes rupture. The fat gets released and is taken into the lymphatic system for metabolism.

This amounts to the targeted destruction of fat cells (ie body contouring) without the need for surgery. This is ideal for small pockets of fat, where long-lasting results are desired without undergoing liposuction. The trade-off is a few treatments, but the lack of downtime makes this feasible.

I was just talking about this to one of the reps last week. When I got back from SNEC this afternoon, I found a box in our pantry... It's arrived!!! Talk about fast! Hopefully the training will be as swift. I believe I have the first unit of this device in Singapore. There's plenty of excess fat amongst us to test this out. It's pretty exciting stuff!

Simply Aesthetic has moved to Robertson Walk

After many months of planning, renovation, inspections and running back and forth, we have finally settled into our new home at Robertson Walk. We moved all our things over last Thursday, then abandoned the girls for a short holiday, so they could tidy up/organise stocks in peace without any interference from me!

I have to say that I am very relieved to have moved in at last. The last month of running back & forth was getting quite frustrating, especially since the new clinic was in a much nicer space than the old one! But the wait is finally over, the licence is ready and we can unveil the beautiful new facilities.

We now have 3 good sized treatment rooms, two with natural sunlight (frosted glass, of course!). The restrooms are still outside, but just next to the clinic (so no more dashing through the shopping mall to get to the ladies!). Parking is even more convenient. The car park entrance is in the lane between Park Hotel Clarke Quay and Robertson Walk. There's ample parking and is very straightforward compared to UE Square, where people often got lost! And getting lost generally should not be an issue anymore: no more confusion between UE Square and United Square!

For those who are not familiar with Robertson Walk, it is a beautiful development at the junction of Mohamed Sultan Road (section opposite Kim Yam Rd) and Unity Street. It is largely F&B and the restaurants are distributed around a courtyard which is pretty happening at night, especially at weekends. There are some services on the second floor, including the DBS Arts Theatre! Our frontage is just off the corner facing the bridge, next to Divine MedSpa.

It is lovely to see sunlight all the time. UE Square was so depressingly grey, both inside & out! The Singapore River is also a stone's throw away. I'm looking forward to my strolls there when things are quiet in the clinic. So appointments by booking only please! It won't be a pretty sight if I have to run back to the clinic to see you!

Judging the Harper's Bazaar Beauty Awards 2009

I received an exciting e-mail from Alli Sim, Harper's Beauty Editor a couple of weeks ago. She wanted to know if I woud be keen on being on the judging panel of Harper's Bazaar's Beauty Awards 2009! She warned me that it would be an intensive month of trying out a variety of the newest products for 90 categories! It was naturally an offer that I could not refuse! Which female would?!!! My brief was to give the medical take on which products were best in each category.

The products arrived yesterday. I had a phone call from my very excited 13 year old daughter (and self-elected co-judge), exclaiming that she was "traumatised" at the number of products to try! Fortunately she changed the term to "overwhelmed". Hmmmmmm................

I didn't get home till after 10pm, and all hopes of a decent "early night" were dashed immediately. Six of the largest plastic containers from IKEA were waiting for me. In them was a myriad of products, spanning cosmetics, skincare, hair products, nail care, scrubs, body washes, perfumes, even 2 eyelash curlers! My jaw dropped as it looked like over 1000 items to try in just one month! No wonder Alli said her hair almost dropped out with trying out the hair products last year!

My daughter was bouncing off the wall with glee, proclaiming she now officially has more makeup than any of her friends. The dogs were going crazy trying to get into the boxes. All I could think of was "Oh My God!" and "Thank God I have help!". My daughter is definitely better with makeup than I am. Her beady eyes were already on the eyelash curlers and she'd tried on one of the nail polishes already. One of my friends has extremely dry skin and is desperate for a tan, so anything vaguely oily and in the self tan range will be palmed off to her. The scrubs and hand/foot spa things will be brought into Divine MedSpa for the girls to try on me. Nothing like an expert opinion!

I managed to try a shampoo and conditoner last night. A common high street brand that my daughter has purchased before. Cannot make it. Neither could the face wash I tried. I was intrigued by another sample which had a strange plastic tool and capsule of cream. I have to say that it was the tool that really grabbed my attention! After fiddling about, pulling & twisting it, it broke! Then I realised that the fancy "tool" was nothing but a spatula to scrape out the cream. What a joke!

At this point, I have to say that I gave up on trialling the products & decided to try something "simple": choose the lipstick with the most compelling design. The age gap then surfaced with the little princess going for something hip and the old mother insisting on something classy. The real judge won that battle and we decided on a lipstick with a beautiful shape and unique matt black finish. Settled one item out of 90.

Tonight's task will be an attempt at sorting out the products into the correct categories, choosing some to bring on holiday with me this weekend, and packing the ones for the spa. Then slowly trying out the makeup one by one. If I can find eye makeup that is resistant to my compulsive rubbing, that'll be a miracle!

New Clinic Opening Hours

With immediate effect, we will be changing our opening hours to the following:

Weekdays: 11.00 to 20.00
Saturdays: 10.00 to 15.00
Sundays: 14.00 to 17.00

We'll be Closed on Tuesdays and Public Holidays.

As before, consultations are by appointment only, although we will try to see walk-ins.

We should be moving to Robertson Walk by the end of next week. Please check this blog for the exact date and closures for move. We'll also be revamping our website, so keep an eye out for this!

Simply Aesthetic at Robertson Walk

This project has kept me well occupied for the last few months. We have been searching for a suitable new home since November last year. We're happy to finally show the photos of the new location of Simply Aesthetic... Robertson Walk.

This first photo is a pre-renovation view of the clinic side of the unit. The French doors gives it a bright, airy feel. The glass will be frosted, so no privacy issues here! We will have one consultation room and 3 treatment rooms in this section of the unit. Plus a washroom for the desperate!

Robertson Walk has a very laid back atmosphere during the day time. Incredibly private, very European. From the balcony outside our main door, we can catch a glimpse of the Singapore River in the distance. It's just a short stroll away, so fantastic for a mid-day de-stress stroll. Wonder if I'll be motivated enough to go jogging?!!!

The main square at Robertson Walk comes to life at night. Lots of excellent restaurants surround the area. The DBS Repatory Theatre faces us from across the square.

The clock tower is directly above our unit. The open French door that is just visible will be our main doorway. The clinic will be on the left side and the spa on the right.

Renovations begin on 11th May. We are expecting to open the spa by mid-June, but MOH licensing will delay the clinic move till late June. Fortunately, Robertson Walk is just next to UE Square, so disruption will be minimal. The couple of months will be busy but exciting. Our private label line (organic, no less!) is coming together.
