The goal of this course is to address current challenges faced by health professionals in delivering consistent health care to all populations worldwide. The student participants will focus on five major areas, each of which will introduce a number of associated 21-century issues:&lt;/p&gt;  &lt;p&gt;   &lt;br /&gt;• Challenges in Global Health addresses the major public health challenges in developing countries and their remedies    &lt;br /&gt;• Health Policy and public health examines the major principles of public health and the effect of different cultures on its implementation    &lt;br /&gt;• Tropical Medicine reviews the more common tropical infectious disease and how they are treated in areas with limited resources    &lt;br /&gt;• Vaccinology explores the difficulties in making and distributing vaccines in the third world    &lt;br /&gt;• International development and health aims to look at the broader issues and challenges to health in those countries struggling with modernization in a global economy.&lt;/p&gt;  &lt;p&gt;   &lt;br /&gt;This course was taught for the first time during the summer of 2011. In reality, most RNA transcription in the genome is non-coding and recent work from our lab and many others has documented these transcripts are by no means &#39;transcriptional dark matter&#39;, as has been alleged. Rather, they are highly functional, involved in many key processes in development and in cancer biology. Some, like XIST, are vital to X-chromosome inactivation. Others, like HOTAIR, control key processes in metastasis, like epithelial-mesenchymal transition in breast cancer. In the course of our studies of high-risk childhood cancer, we have encountered a plethora of ncRNAs that appear to dictate diagnosis and prognosis. We have studied one transcript in Ewing&#39;s sarcoma in particular, AK057037, and have determined it is involved in normal brain development and is a key determinant of the metastatic phenotype. Novel and multi-functional nanodevices will be capable of detecting cancer at its earliest stages, pinpointing its location within the body, delivering anticancer drugs specifically to malignant cells, and determining if these drugs are effective. Functionalized nanoparticles would deliver multiple therapeutic agents to tumor sites in order to simultaneously attack multiple points in the pathways involved in cancer. Such nano-therapeutics are expected to increase the efficacy of drugs while dramatically reducing potential side effects. &lt;i&gt;I&lt;em&gt;n vivo&lt;/em&gt;&lt;/i&gt; biosensors would have the capability of detecting tumors and metastatic lesions that are far smaller than those detectable using current, conventional technologies. The NCI formed the Alliance for Nanotechnology in Cancer to pursue applied nanotechnologies for cancer detection, therapy, and prevention with an aim to achieve clinical translational stage of these technologies towards culmination of the program.&amp;#160; This presentation will describe the details behind the organization and science and technology of the Alliance.&lt;/p&gt;  &lt;p&gt;&lt;b&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p&gt;&lt;b&gt;Dr. Piotr Grodzinski&lt;a name=&quot;PiotrGrodzinski&quot;&gt;&lt;/a&gt; &lt;/b&gt;is a Director of NCI Alliance for Nanotechnology in Cancer at the National Cancer Institute in Bethesda, Maryland. I have traveled and done medical volunteer work on 14 trips to Port-au-Prince over the past 25 years and I wanted to bring the depth and richness of this experience to others. Defects in any of these processes can lead to neural tube defects (NTDs). To identify the genes involved in this complicated process, our lab has undertaken ENU mutagenesis and forward genetic screens in mice. Over the years we have identified a large number of novel genes that are critical for neural tube closure and determined their mechanisms of action. Examples from some of our recent work will be given.&lt;/p&gt;  &lt;p&gt;&amp;#160;&lt;/p&gt;  &lt;p&gt;The etiology of NTDs is complex and involves both genetic and environmental factors. Mouse NTD models have enormous potential to aid in understanding the genetics underlying responsiveness or non-responsiveness to environmental factors. The most effective environmental influence in decreasing the risk for NTDs in humans is folic acid (FA) fortification. However, only a few mouse models have been tested for FA responsiveness and very little is known of the mechanisms underlying FA action or the extent to which molecular pathway information can be applied to determine which mutations may be environmentally sensitive. The Niswander lab is exploring the relationship between genes and responsiveness to FA supplementation using mouse models that are unrelated to the FA metabolism pathway. Although FA supplementation is clearly efficacious in reducing human and mouse NTD incidence, contrary to expectations, we found that some mouse genetic mutants respond in a surprisingly negative way to FA supplementation, showing an increased incidence of NTDs in homozygous mutants, occurrence of NTDs in heterozygous embryos and embryonic lethality prior to NT closure. Furthermore, we found that, for the same genetic allele, NTD risk can depend on the length of FA exposure. Our unexpected findings highlight the need to understand how FA influences NT closure and the mechanisms and genetics underlying the response to FA supplementation.&lt;/p&gt;  &lt;p&gt;&amp;#160;&lt;/p&gt;  &lt;p&gt;Neural tube morphogenesis is highly dynamic, yet has largely been studied in static images. Our lab has coupled the ability to culture the mouse embryo during the process of neural tube closure with confocal imaging technology and fluorescent reporter lines to provide a dynamic view of NT closure in a living mammalian embryo. This allows a real-time visualization of cell movements and behaviors during NT morphogenesis in the mouse. The 650-bed hospital is the major teaching affiliate for the UC Davis School of Medicine. The Hospital serves an area of tremendous diversity in both cultural and socioeconomic background. It is a Level 1 Trauma Center, the only Burn Center in Northern California, the only Children’s hospital in Northern California, the only Transplant Center in Northern California, and the only such institution in inland Northern California.
