SSS Form R3 – Contribution List

For instruction, please refer to the pdf file from SSS official website.

The worksheet “R3” is protected. You can write/input only in the following items/captions:

EMPLOYER ID NUMBER

REGISTERED EMPLOYER NAME

TEL. NO. and ADDRESS

At page 1, item no. 4 under the section “NAME OF MEMBER” there is a text box with text

Drag the text box up or down accordingly.

You can check/uncheck the check boxes in the section “ADJUSTMENT TYPE” even in page one only. The checkboxes in other pages will likewise be checked or unchecked. If you want to use this form for household employer, unprotect the sheet, under the section “TYPE OF EMPLOYER” > “Regular” option, remove the fill (Right click > Format Shape…) of the oval graphic object then apply fill in the oval provided for the “Household” option. To unprotect the worksheet/s (MS Office Excel 2007), on the Review tab, in the Changes group, click Unprotect Sheet.

In the worksheet “Data” (also protected), just copy your records in the table provided therein. Fill-in the necessary information i.e. Quarter Ending; TR/SBR No.; Date Paid and Amount Paid. You can delete the contents of the cells but don’t move, insert or delete columns and/or rows. Formulas in the R3 forms are linked to this table.

A maximum of 225 records equivalent to 15 pages of the R3 are allowed in this “USER-FRIENDLY FORM”.

Adding and deleting record/s: First, unprotect the worksheet “Data”. Type the new employee/s’ information at the blank row of the table immediately following the last record. Select cell D5, under the column field “Surname”, then sort ascending. In deleting separated employee/s, replace the employees' surname by "zz" (with out the quotation mark), select cell D5, sort ascending, at the bottom, delete the record/s with surname zz (Warning: delete record or cell, not row).

Description

This return/declaration shall be filed in triplicate by the following taxpayers;

1. A VAT-registered person; and

2. A person required to register as a VAT taxpayer but failed to register.

This return/declaration must be filed by the aforementioned taxpayers for as long as the VAT registration has not yet been cancelled, even if there is no taxable transaction during the month or the aggregate sales/receipts for any 12-month period did not exceed the P1,500,000.00 threshold.

The worksheet for page1 is protected but without password. You cannot select cells that are locked for protection like those with calculations and other unused cells. The purpose is to prevent accidental deletion of cells with calculations. At the right side of the form are unprotected cells with outside border. The purpose of which is to serve as a calculator. Included of course in unprotected cells are cells or items that require user’s input. To unprotect the worksheet/s, on the Review tab, in the Changes group, click Unprotect Sheet.

Likewise, the column and row headers are not shown. To show/hide the column and row headers go to Office Button > Excel Options > Advanced, under the Display options for this worksheet, check or uncheck the Show row and column headers checkbox.

I didn’t put so much formula in page2, only the totals. I didn’t practice tax for the last 4 years so I’m not sure with the proper calculations with those items in page 2. Of course I have an idea but I prefer to leave it to the VAT experts. Page 2 worksheet is not protected.

BIR Form No. 2316 - Certificate of Compensation Payment / Tax Withheld For Compensation Payment With or Without Tax Withheld

Description

A Certificate to be accomplished and issued to each employee receiving salaries, wages and other forms or remuneration by each employer indicating therein the total amount paid and the taxes withheld therefrom during the calendar year.

This Certificate in turn should be attached to the Annual Income Tax Return (BIR Form 1700 - for individuals receiving purely compensation income, or BIR Form 1701 for individuals with mixed income).

Filing Date

To be issued to payee on or before January 31 of the succeeding year in which the compensation was paid, or in cases where there is termination of employment, it is issued on the same day the last payment of wages is made.

At the right side of the upper right corner of the form is a big number in red. That number is the “LOOK UP VALUE” in VLookUp function I used in this form. That “LOOK UP VALUE” is also the value in the field/column “SEQ. NO” of your annual alphalist. This number can be changed or click the “UP ARROW” to increment the value by one (1) except for the last record.

The format of the alphalists, especially the fields/colums’ arrangement must be strictly unchanged. Avoid moving or deleting columns and rows except if you are capable of editing the calculations involved or creating your own formula. Inserting column is also prohibited. Just copy/paste your existing data in the provided alphalist worksheets. The maximum row number in VLOOKUP function is 200. If your record is beyond the scope of “ROW200”, a simple trick is to insert row/s on or before row200 and the referred row in the formula will be adjusted accordingly.
