INTENSE PULSED LIGHT

Technology

Intense Pulse Light, or IPL is a new and exciting way to improve and rejuvenate the skin. Unlike a laser, which emits one specific wavelength of light, IPL emits a broad spectrum of light with each pulse. The broad spectrum of light in each pulse allows us to treat a variety of skin imperfections at the same time. The state of the art machine we use is the Lumenis One, one of the most advanced IPL units produced. Treatments are quick and effective with very little downtime.

What is possible

The most common application of IPL is a skin rejuvenation technique known as PhotoFacial . This treatment provides an average of 70 percent improvement in the signs of sun damage (redness, brown spots, etc). There may also be some improvement in wrinkles, pore size and flushing. An entire zone, such as the face, is treated during each treatment The sessions are scheduled every 3 to 4 weeks. Usually the treated skin stays red for 1-3 days and there may be a little bruising. Brown spots that have been treated usually flake off within 5-7 days. Most people can go back to work right away.

The Fat Transfer Procedure
At Vita MedSpa Fat Transfer is performed by Board Certified Physician Karina Bibicheff, M.D. and is an in-office, outpatient procedure, under local sedation. During the procedure topical anesthetic is applied to the location where the fat will be removed and the treated area where the fat will be injected. The fat cells are harvested from the donor site through liposuction to be processed by Dr. Bibicheff before they are injected into the treated area. It takes at least three days for the first tiny blood vessels (capillaries) to reach and supply the newly-transplanted fat cells with a blood supply. During this time, a percent of the transferred fat cells will die. For this reason, Dr. Karina Bibicheff will usually overfill the treated area. More than one treatment may be required to achieve the best results. Each time the procedure is repeated, more fat accumulates in the treated area. Deep Skin Brightening System

Melasma is a common skin disorder that consist in a dark skin discoloration that appears on sun-exposed areas of the face: cheeks, forehead, nose, upper lip and chin. Though it can affect anyone, women with brownish skin tones are at greatest risk.

It is believed that Melasma is caused by over stimulation of pigment producing cells (melanocytes), causing them to produce more melanin on the sun exposed areas. Often referred as the “Mask of Pregnancy”, Melasma is often associated with the female hormones estrogen and progesterone and is especially common in pregnant women, women who are taking birth control pills and women under hormone replacement therapy. Sun exposure is also a strong risk factor that exacerbate the Melasma.

Although Melasma doesn’t cause any other symptoms than the dark skin discoloration, it is a great cosmetic concern for the millions of women who suffer from this skin condition, making them feel embarrassed and unattractive. Using make-up and concealers has been a popular option to “cover” the problem, but thanks to the advances of technology and medicine now you can prevent, treat and stop the progression of Melasma.

2. Hydroquinone: considered one of the most effective and strong inhibitors of melanin production, meaning that it prevents skin from making the substance responsible for skin color. Hydroquinone does not bleach the skin but lightens it.

3. Vitamin C: it has been shown clinically to possess brightening and skin soothing properties. Hydroxyacids enhance penetration and accelerate the exfoliation of hyperpigmented cells.

5. Kojic Acid: it has been successfully used to lighten pigment spots and skin discoloration and considered as an inhibitor for melanin. Kojic acid can produce excellent effects in fighting age spots, pregnancy marks, as well as general skin pigmentation disorders.

The excessive pigment distribution caused by the Melasma varies from patient to patient and can be either superficial or at deeper layers of the skin. This affects the response the patient will have to the treatment. To find out if you are a good candidate for the procedure and what results to expect, it is necessary to schedule a consultation with the doctor in order to evaluate the skin for a proper diagnostic and treatment. During consultation, it is very important to be honest with the doctor and mention all products that you have been using or use at present time. If you continuously use lightening agents, especially Hydroquinone without any breaks every 2 to 3 months you might get more darkening of your skin, called acquired ochronosis, and unfortunately can be permanent.

Even when treated successfully, Melasma may occur. It only takes a small amount of sunlight to worsen the Melasma and it is vital to use daily sunscreen with high SPF all year around as well as wearing sun protective clothing to help prevent it and control its progression. Preventing and treating Melasma can be challenging, it needs a lot of patience and dedication and it may be necessary to do some changes in your lifestyle and habits. For example, if you like to walk on the beach or gardening, it is better to do it early morning when the sun is less strong and without forgetting to wear sunscreen and a wide-brimmed hat.

Remember that by minimizing your daily sun exposure you will help prevent further darkening of existing Melasma spots as well as a formation of new patches. Be good to yourself, use sunscreen every day!

SlimLipo or SmartLipo?

While SmartLipo and SlimLipo appear to be similar procedures, there are crucial differences that render SlimLipo a far superior procedure.

SmartLipo utilizes a stainless steel "wand” laser energy that is stiff, it can't turn corners and is limited. The two wavelengths used are 1064 nanometers and 1320 nanometers. Both laser wavelengths are non-specific for fatty tissue and are absorbed by water, generating a fair amount of heat to the tissues. The heat generated at the laser tip is significant, disrupts the fat cells and shortens the collagen fibers. This is particularly important when operating areas like the neck where the skin is very delicate and it puts the patient at risk as it can result in tissue burns. SmartLipo has been modified numerous times to improve the results and some doctors are still using the old machines that don’t have the advanced technology necessary to give you the expected results.

SlimLipo uses a flexible wand made out of silicone fibers and can turn corners. It uses 924 nanometers and 975 nanometers wavelengths that are specifically absorbed by fatty tissue. 924 actually "melt" the fat and results in liquefying the fatty tissue with much less energy than SmartLipo. 975 NM is more specifically absorbed by collagen fibers and results in more specific tightening without using excessive heat. The laser wand delivers energy to a much wider area and therefore "melts" many more cells with each pass, and requires less energy to do the job. Slimlipo is only available at select medical centers around the country since only a handful of surgeons are adequately trained to perform the procedure.

Bottom line: Slimlipo is the next generation of laser assisted liposuction and is superior in its results, ease of use and patient safety.

SlimLipo vs. Traditional Liposuction

Traditional liposuction uses general anesthesia, leaves larger scars, causes more bleeding and more post-treatment pain, and requires a long recovery time. Patients would have significant discomfort, swelling and bruising after their procedure and depending upon their skin elasticity there would be some skin tightening and in many cases the skin does not retract at all.

SlimLipo uses local anesthesia, few tiny incisions are made, and patients have minimal blood loss, bruising, swelling and discomfort then patients who have had traditional liposuction. As we get older, our hand get older as well. When you understand the aging process you may be able to delay and prevent age related conditions and certain body changes. There two main processes of aging intrinsic and extrinsic. The intrinsic is what you have got from your parents, that your genes. Its also known as the natural aging process, so we can not change that. There is as well extrinsic aging or number of external or environmental factors that often act together with normal aging process to prematurely age our skin. La laserlipólisis es menos agresiva que otros métodos más tradicionales e incorpora varias ventajas respecto a ésta, como un postoperatorio menos doloroso y más rápido y menor tiempo de baja para los pacientes. Sin embargo, no está indicada en todos los casos, sino sólo para zonas concretas del cuerpo. La liposucción láser se realiza generalmente bajo anestesia local o peridural, dependiendo ésta de que dependerá de la zona a tratar. La laserlipólisis puede durar de una a tres (o más) horas que variarán en función de la zona y de la cantidad de grasa que se vaya a extraer. Al no ser un tratamiento tan invasivo para el paciente, los dolores del postoperatorio serán menores que los de una liposucción tradicional. Lo mismo ocurre con la inflamación, ya que al no manipular las áreas con tanta fuerza, las zonas afectadas se hincharán menos.

Desventajas de la liposucción con láser.

Entre las desventajas de la liposucción láser está que si se desea realizar una lipoescultura, la grasa extraída con la técnica del láser no se podrá injertar de nuevo en otras partes del cuerpo.

Stem Cells From Fat: Is It the Next Wave of Healing? Or Just a Marketing Scheme?

Most Americans are not terribly fond of their fat, but what if you could put that spare tire to work -- healing wrinkles, increasing breast size or even speeding the recovery of a knee injury?
Too good to be true? Not necessarily.
For 20 years cosmetic and plastic surgeons have used fat grafts taken from their patients' own "supply" to restore fullness and decrease wrinkles in the face, but only recently have medical researchers begun investigating whether the adult stem cells found in the fat tissue could be used for healing on a wider level.
"Fat is naturally rich in adult stem cells ... and these stem cells have regenerative capabilities," says Dr. Peter Rubin, associate professor of Plastic Surgery at the University of Pittsburgh and a clinical researcher of fat stem cells.

Research is under way around the globe to explore how these regenerative capabilities can be used to heal, he says, from reconstructing the face after traumatic injury to rebuilding breast tissue for breast cancer patients after a mastectomy.But this technology leads a bit of a double life.Its medical applications are still in the research phase, but some cosmetic surgeons are allegedly putting this technology to aesthetic use on their patients.Services such as stem cell face lifts or even vaginal rejuvenation promise revolutionary results with the help of the regenerative power of stem cells. Given that this technology is not yet backed by clinical data or FDA approval, however, the benefits of such procedures are questionable.

"Stem cells are a buzz word now. Some of these procedures are driven by a profit motive and some are driven by science. If it's profit-motivated, you have to take it with a grain of salt for sure," says Dr. Felmont Eaves, plastic surgeon and president-elect of the American Society for Aesthetic Plastic Surgery.

Fat to Make You Pretty

"The blood supply to a graft is very important, so since stem cells release growth factors, the concept is that if you increase stem cell density, then you can preserve greater fat retention," says Dr. Rubin.
The problem is, the machine used to make the stem cell-enriched fat is not FDA-approved, Rubin says, it's only approved for research purposes.That doesn't mean that such procedures won't work or that they're necessarily unsafe, Rubin says. "It's unlikely that these cells will cause harm [and] there are really great applications that can come out of this, but we need to be careful and deliberate in how we use these therapies," he says.

Dr. Mark Berman, president-elect of the American Academy of Cosmetic Surgery, has become an authority on the use of fat for cosmetic regeneration of tissue, and he says that most of the stem cell benefit occurs without specially concentrating stem cells in the fat.While he's used concentrated stem cell fat in one woman to treat a chronically injured knee, "this is early stuff," he says. The "stem cell buzz" for things like wrinkle treatment, he says, is "misleading."

"Most doctors in the U.S. [probably all] that claim they are doing 'stem cell' fat grafting to lift eyelid and facial tissues are in reality using simple fat transfer," Berman says.
"There are very few, if any doctors in the U.S., set up to actually [concentrate stem cells]. What you'll find, doctors who say they're doing these stem cell fat grafts are not performing this step and are rather telling you that the fat is loaded with stem cells and so they don't need to do it. It's a bit disingenuous."
Both Eaves and Berman say that using stem cell for publicity casts a shadow on simple fat grafting, which they believe is a highly effect procedure in its own right.
"In the mainstream, we're seeing that after twenty years working with fat, it's a very powerful tool," Eaves says. "It can be done with safety and efficacy, [but] with the stem cell buzz, it's kind of a wild west out there," Eaves says. "People make claims and hire a press agent and they get into the news and in the public eye it's validated, but the validation really isn't there."

The Future of Fat Fixes

Plastic surgeons are "extremely enthusiastic about the role of fat stem cells in plastic surgery," Eaves says, but Berman says that most likely, this technology will be "even more important in other therapeutic uses." Researchers abroad have used fat stem cells to close bowel cistules in patients with inflammatory bowel conditions and to regenerate bone for facial reconstruction after cancer, among other applications, Rubin says. In his own research, he is working on breast reconstruction for cancer patients as well as facial reconstruction for those in the military with facial injuries.

"This is a promising field," he says, but one that needs to be researched to ensure safety and long-term positive results. "Especially in the area of breast reconstruction, you have to be careful that those cells won't impact adjacent breast cancer cells and possibly cause cancer recurrence."Plastic surgeon Eaves echoes this caution for those dabbling in aesthetic therapies."The message we would like to say is that patients is to really do their homework and make sure the procedure is a safe one," Eaves says. "Don't be suckered into a glitzy ad; find out if there's any data behind it."

Botox approved for elbow, wrist, finger spasms

WASHINGTON - The Food and Drug Administration said Tuesday it approved Allergan's botulinum-based drug Botox to treat spasms of the elbow, wrist and fingers.

Botox is most famous for it's ability to smooth frown lines on aging foreheads, but the drug has long been used to treat neck spasms, eye muscle disorders and excessive underarm sweating.

Under the latest FDA indication, Allergan will be able to market Botox as a treatment for adults with spasticity in the flexor muscles of the elbow, wrist, and fingers.

"Muscles affected by spasticity have increased stiffness and tightness, which may lead to pain, difficulties with hygiene and other activities of daily living, and may affect how a patient looks," said Dr. Russell Katz, director of the FDA's division of neurology products.

Allergan, which is based in Irvine, Calif., is studying Botox for a variety of uses, including treating migraine headaches.

Botox works by blocking the connections between nerves and muscle, temporarily paralyzing the spastic muscle. The drug is a purified form of botulinum, one of the most toxic substances in the world.
