Walking the 5K for Breast Cancer Awareness today was Nurse Paula, Seena, Stephanie, Kristin and Laury.

Third Neurotoxin Enters U.S. Aesthetic Market

On July 21, 2011 the FDA (Food and Drug Administration) approved yet another neuromodulator drug for the treatment of glabellar lines (between the eyebrows) in adult patients called incobotulinumtoxinA.

IncobotulinumtoxinA is manufactured by Merz Aesthetics and will be marketed under the name of Xeomin. It’s the third botulinum toxin type A to enter the marketplace following the original neurotoxin, Botox and most recently Dysport. Xeomin is expected to be available to physicians in the United States in Spring 2012 and is the only botulinum toxin currently approved in the United States that doesn’t require refrigeration prior to being reconstituted.

Because many patients "price shop" by calling various clinics inquiring as to the “price per unit”, it is important to note that Botox, Dysport and Xeomin differ in their level of potency. Therefore, their measurement of “units” cannot be compared equally.

While Botox continues to dominate the market, it will be interesting to see how much market share these new competitors will capture.

FDA's New Law on Sunscreen Labeling

The FDA has recently approved a new labeling law when it comes to the verbiage placed on sunscreen bottles. We've all seen "Waterproof", "Sunblock", "Sweat Resistant"... well, now sunscreen manufacturers will be scrambling to prove what they claim.

Sunscreen products sold on U.S. shelves will soon be required to follow a standardized labeling system that specifies which products offer the best protection and bans the terms "sunblock" and "waterproof." Under the new system, only sunscreen products that protect against both ultraviolet B (UVB) rays and ultraviolet A (UVA) rays, and have an SPF of 15 or higher, will qualify to be labeled as "Broad Spectrum."

Sunscreens that meet the new test for broad spectrum protection and also are SPF 15 or above can for the first time include the statement: "Using as directed reduces the risk of early skin aging and skin cancer when used with other sun protection measures". All sunscreen products that do not meet the FDA's new standards will be required to include a warning on the label saying it has not been shown to prevent skin cancer or early skin aging.

The new labeling laws should be effective by next summer, but may begin to appear on the shelves earlier than that. Additionally, the FDA is also banning use of the word "sunblock" by sunscreen manufacturers, because it gives a false impression of the protection offered by the product. Also, no sunscreen product will be allowed to carry an SPF value higher than 50 because there is not enough data to prove that sunscreens with SPF values higher than 50 provide greater protection.

The FDA is also banning the use of the words "waterproof" and "sweat-proof" and requiring that sunscreen products claiming to be water-resistant must indicate how long the sunscreen remains effective when exposed to water. To really put it to the manufacturers, the FDA is also requiring that sunscreen products can no longer be described as offering protection for two hours or longer. Drug fact boxes must also be present on all labels.

Dr. Young Wins Award at THE Aesthetic Show in Las Vegas

On Saturday evening, June 4th, Dr. Thomas Young won an international award in the category of "Best Overall Body Makeover" at the "2011 THE Aesthetic Show" held at the Aria Hotel in Las Vegas.

Seven awards were presented that evening in different categories. The awards show was emceed by Dr. Andrew Ordon from "The Doctors" television show. Distinguished judges comprised of Hollywood's top aesthetic physicians and key members of the aesthetic media including Dr. Will Kirby, Dr. Jason Diamond and Dr. David Matlock from "Dr. 90210", Dr. Ava Shamban, Dr. Alexander Rivkin, Dr. Mark Berman, and Keith Veseleny of American Health & Beauty.

The before and after photos that won the award for Dr. Young was his work on our very own Jodi from "The Jodi Project"!

Understanding Laser Hair Removal

Did you ever wonder why it takes so many session of laser hair removal treatments to reduce the amount of hair growth?

Did you also ever wonder why you only need a few treatments for one area of the body and several for another?

And, finally, did you wonder why laser hair removal treatments don’t provide permanent results?

To answer all of these questions you first need to know that hair follicles have three growth phases, the anagen phase, the catagen phase, and the telogen phase. Each hair follicle is independent from one another, meaning they all aren’t going through the same phase at the same time.

The anagen phase is the active growth phase. The length of the phase determines the length of the hair and the depth of the follicle. Scalp hairs may remain in the anagen for up to 3 years.

As the hair follicle begins to enter a dormant (slowed down) state of growth, it is called the catagen phase. The catagen phase is a brief transitional phase of regression which lasts a few weeks.

The telogen phase, or resting phase, can last for many months and is dependent upon the number of actively growing hairs in a body area (i.e., the scalp has a shorter telogen phase than the chest). In this state, the bulb of the hair has reduced to one-third its normal size.

As the anagen phase resumes activity, the hair follicle lengthens and the bulb penetrates deeper into the skin which causes the displacement of the old hair shaft. Hair follicles are mostly in the anagen phase, but are independent of one another which results in the shedding of individuals hairs rather than an entire area.

Technically, most laser hair removal devices operate the same way when performing a hair removal procedure. The technician performing the treatment needs to have the required skills and training to set the parameters on the device (joules, milliseconds, pulse, etc.). These parameters should be set per individual person according to their thorough skin analysis and history of previously performed laser treatments, sun exposure, medications, skin disorders/diseases, etc.

During a laser hair removal procedure, you should not wear dark clothing near the area to be treated because the pigment (coloring) in the clothing could potentially be absorbed by the laser light and could result in a burn. You also should not use any body washes, lotions, cosmetics, or any skin medications the day of treatment.

With some lasers, like an IPL (Intense Pulsed Light), you may want to have a topical anesthetic applied prior to your procedure. Other lasers, like the SopranoXL diode laser, operate differently and a topical anesthetic is not required.

People with darker skin types or suntanned skin could experience a “paradoxal response”. What this means is the area of skin that was treated with the laser or light therapy may stimulate more hair to grow instead of reducing growth. People with dark or ethnic skin types should be spot tested a month before proceeding with a full treatment.

A spot test, or patch test, should be performed on the intended treatment area. Small white sheets of paper with perforated holes are sometimes used for spot testing patients. In the spot test procedure, the test paper is placed over the area to be treated and one firing of the laser or IPL device is applied over the hole in the paper. After waiting 10 to 15 minutes, the area is checked for unusual redness swelling, bruising, or any other undesirable responses. If the area emits a slight reddish hue with no other abnormalities, the device settings are optimal. Some devices, IPL in particular, require a second pass over the intended treatment area.

You may experience a mild burning sensation and redness following your hair removal procedure. This is can be expected and can be relieved by applying ice packs and an oral anti-inflammatory or pain medication if you have no other medical conditions that would prohibit you from taking a medication. You should avoid any exposure to ultraviolet light (sun, tanning beds/booths) after your procedure and a sunscreen with an SPF 20 UVA and UVB protectant should be applied to the skin prior to leaving the office especially during sunny days.

Sometimes people may experience pigment changes such as lightening or darkening of the skin that could last for weeks or possibly months.

You should know that an actively growing hair follicle is the only follicle which will respond to laser and light therapy. In the weeks following your procedure, the portion of the hair which remains under your skin will come out. You may mistake this action as the hair growing again when it is really just falling out from the root. This is especially more noticeable in areas of the body which has thicker or more coarse hair such as the armpits and genitalia. After a week following your procedure, you can use a mild loofa or exfoliating sponge during showering to aid in the expulsion of the hairs.

No laser or IPL device can destroy all of the hair follicles especially after only one procedure. Some hairs are in their dormant state, some have been placed into a prolonged dormant state due to the procedure effects, and some have been transformed into vellus hairs. Therefore, several treatments are required at monthly intervals for an optimal outcome. Additionally, a maintenance procedure performed at yearly intervals is recommended.

Contraindications for laser or IPL hair removal include:

People who have had previous laser treatments and have experienced a prolonged healing process.
People who are prone to skin discolorations.
People with grey, white or red hair.
Pregnant people.
People with suntanned skin (except for treatment with the long-pulsed Nd:YAG laser). It is recommended that the patient let the suntan fade before treatment.
People taking any photosensitizing medication (i.e., Accutane, tetracycline, or Retin-A). These medications should be stopped months prior to treatment. Retin-A can be stopped weeks prior to treatment.
People with a history of Herpes Simplex Labialis (can treat with prophylactic antiviral medications prior to treatment).
People with excess hair should have the underlying problem corrected beforehand by their primary care physician. Such conditions can be caused by hormonal abnormalities and certain medications including minoxidil, steroids, oral contraceptives, and some immunosuppressive drugs.

Core Physicians - Real or Fabricated?

Aesthetic procedures are on the rise in the United States and with that are more and more clinics and medical spa openings. Like any business, there is competition. With competition, there are claims of excellence. With claims of excellence, there is criticism of others.

While this may be fine for politics, or department stores comparing brands of lipsticks or perfumes, it should have no place in the medical aesthetic arena, or any medical arena for that matter. As much of a pollyanna as I am, I have realized that the business that is cosmetic medicine is not sheltered from such drama. One of the criticisms I have always been confused by is when one aesthetic physician deems themselves to be a "core" physician, thus scrutinizing others who are not. I searched the Internet, but could not find any definition as to what a "core" aesthetic physician is and why they are better than "non-cores". I had always thought that experience and training is what made a physician specialized. Not particular degrees or specialties of medicine.

So, I set out to find out where the term "core" physician came into existence by writing an editorial blog for MedicalSpaMD called "Understanding Core Physicians". I thought it would elicit heated discussions among the specialties, but surprisingly it did not.

One person did answer with clarity and unbiased perception. The answer was what I had thought all along.

So, if you are seeking out a physician to perform a certain aesthetic procedure and you are either told that the physician is a "core" physician, or read on their website that a physician is a "core" physician, ask them to define it for you and where the terminology came from. And if you are looking for a physician to perform a certain aesthetic procedure, simply ask them who they trained with, how much experience they have, and how many successful procedures they have actually performed themselves.

In following the spirit of "The Jodi Project" where we feature real patients from the Lehigh Valley, our next project involved twin sisters who have been trying to undo what genetics had done to their thighs. They both religiously tried diet and exercise, they're young, they eat healthy, but no matter what they did their inner and outer thigh pockets of fat would not resolve.

We asked the girls if they would share their story with others and they selflessly said "yes". We featured "The Twins" in the April 2011 edition of Lehigh Valley Style.
