Making Braces Hip

To make braces more acceptable and fun for young people, manufacturers have made brightly colored elastics. These are the tiny rubber bands that hold the wires to the braces. Children can choose elastics with their school colors or a holiday color scheme, such as orange and black for Halloween.

Choosing the color of the elastics allows patients "to feel that they are more involved in their treatment," Dr. Cangialosi says.

Still Take Getting Used To

Braces today tend to be less uncomfortable and less visible than they used to be. But they still take some getting used to. Food can get caught around brackets and in wires, and flossing and brushing can take more time. After adjustments sometimes the teeth may be a little sore. Tooth discomfort can be controlled by taking a pain reliever, such as ibuprofen (Advil, Tylenol and others) or aspirin if necessary. The use of lighter and more flexible wires has greatly lessened the amount of soreness or discomfort during treatment.

As more and more children get braces, schoolyard nicknames such as "tin grin" and "metal mouth" are heard less often. Because treatment has become more socially acceptable, embarrassment may be less of a concern.

Beyond Braces

Orthodontic treatment in young children is known as interceptive orthodontics. Intervention may begin as early as age 6 or 7. At this age, teeth are still developing. The jaw is still growing. That means certain conditions, such as crowding, may be easier to address.

Before permanent teeth have come in, it may be possible to help teeth to erupt (emerge through the gums) into better positions. It's common, for example, for the dental arch to be too small to fit all of the teeth. A few decades ago, the solution for crowding was almost always to extract some of the permanent teeth to make space. Then fixed braces were used to position the teeth properly.

Early intervention takes advantage of the fact that a child's jaw is still growing. For example, a device called a palatal expander may be used to expand the child's upper dental arch. Once the arch is the proper size, there's a better chance that the adult teeth will emerge in better position. Sometimes teeth still may be crowded after all of them have erupted. In such cases, some permanent teeth may still have to be extracted to make room to align the teeth properly.

So-called early treatment also may be useful when the dental arches and jaws are not in the correct position. Functional appliances may fix or improve these problems. More treatment usually is needed later on, but it may be shorter and less involved.

"It is important to note that children who receive interceptive orthodontics generally still need braces or other orthodontic appliances later," Dr. Cangialosi says. "However, this early treatment may shorten and simplify future treatment in selected cases." This is commonly known as two-phase treatment.

It is important to note that early treatment does not apply to all orthodontic problems. However, it may help in certain cases.

Two conditions that require early intervention are cross-bites and protruding front teeth. A cross-bite can cause the jaws to grow unevenly. Front teeth that stick out may be fractured or injured in an accident, such as a fall.

Last December, the new CEO of a firm we have been doing business with for over ten years requested a meeting to address these questions. It was an excellent opportunity to describe how the small business insurance market (2-50 employees) works in California.

After that meeting, I realized that every business owner and CFO could use a refresher on plan pricing and broker compensation. Here are the answers to these common questions:

No, you would not get a lower price for the same medical product from another broker, by going directly to the insurer, or by purchasing insurance coverage online. Likewise, Shargel & Co. cannot get a lower price for you than another broker.

Each insurance carrier files “standard” rates with the one of two California regulatory organizations, the Department of Managed Health Care or the Department of Insurance.

Small businesses are protected by California’s small business health reform, AB 1672. Your premiums may be no higher than 10% above standard rates, regardless of medical claims. Pricing can be no lower than 10% below standard.

Oftentimes, insurance will charge smaller groups the highest allowable rate and give discounts to larger companies. Our commission is built into these rates. Every customer pays them whether they use a broker, buy direct, etc. Some brokers charge extra fees for services performed. Shargel does not.

Last week, both Congress and the California legislature gave business some long awaited good news regarding tax reporting, tax relief, and the health reform law. We have put together the following list of the 5 most significant health reform tax related changes to date.

1. New 1099 Reporting Requirement Eliminated
The biggest news is what you don’t have to do this year. Congress eliminated the onerous new 1099 reporting requirement that would have required businesses to provide 1099’s for all products purchased over $600. President Obama’s signature is expected shortly.

Last July, I attended a meeting in Washington, D.C. called by the White House Director of Health Reform to discuss this provision of the Affordable Care Act. It was clear then that the administration would support repealing this requirement. Ten months later, it has finally been done.

2. No CA Income Tax due on Adult Child’s Health Insurance Premiums
Last week, California’s legislature passed and Governor Brown signed (AB) 36 which exempts employees from paying taxes on employer-paid health insurance for their young adult children’s coverage (until the year they turn 27). This law conforms California income tax law with the federal income tax law provision of the Affordable Care Act.

For information on what employers need to do to correct W-2’s and employees need to do to file for this adjustment, go to the Franchise Tax Board.

3. Self-Employed Receive One Time Tax Benefit
Self-employed individuals may receive the greatest tax benefit this year. They can deduct the amount of their insurance premiums from their adjusted gross income before calculating self-employment tax.

4. Small Business Health Insurance Tax Credit
Remember there are tax credits available to the smallest businesses and not-for-profits with low wage employees to help pay the cost of health insurance. Business owners’ salary is excluded from the calculation. Use the calculator on our website.

5. W-2 Reporting Requirement for Health Insurance Delayed until 2013
The ACA requirement that the amount a business pays for employer-sponsored health coverage be included on an employee’s W-2 has been postponed for businesses with less than 250 employees. See IRS Notice 2011-28.

Economic Stimulus or Cost Control? The Health Care Conflict

Four hundred business people with conflicting priorities came to the recent San Francisco Business Times program on the future of health care.

Many were interested in the enormous business opportunities that the UCSF and California Pacific hospital construction projects would bring to their companies. Others wanted to know what was being done to control health care costs and their companies’ Health Insurance premiums.

That is the conflict. Health care facilities construction is one of the key economic drivers in San Francisco. We have seen many of our clients refocusing their business development efforts to the health care field. The companies who have won contracts are prospering.

However, these expensive new hospitals, designed for seismic safety and greater efficiency, will be paid for by our taxes and insurance premiums. It was all too apparent that neither Mark Laret (CEO, UCSF Medical Center) nor Warren Browner (CEO, California Pacific Medical Center) were focused on cost control. Laret described new robots which will prepare and deliver medications to hospital patients, thus eliminating human error.

In contrast, Blue Shield’s COO Paul Markovich declared, “We are obsessed with costs.” Bill Kramer, Executive Director for National Health Policy, Pacific Business Group on Health and Steve McDermott, CEO, Hill Physicians, also spoke convincingly of the need for major overhaul in our health care system.

What do they see as the future? Coordination of care seems to be the current buzzword. Markovich proudly described a collaboration with CalPERS, Mercy Hospital, and Hill Physicians designed to both reduce cost and improve quality of care. We are hopeful that the annual savings of $400 per person is just the beginning of the project’s impact.

During the question period, I asked how the Small Business Health Insurance Exchanges would deliver the much anticipated premium savings. The members of the panel shook their heads. Kramer said the exchanges have been “oversold.” The exchanges are likely to be the major source of insurance for lower income Californians, people with the most health problems and the least ability to pay. If the panel is correct, then the savings will come from tax credits for the smallest, lowest wage businesses and the subsidies to their employees—not from any reduction in insurance costs.

(CNN) -- Severe thunderstorms ripped through central Oklahoma Monday triggering vicious flash floods that left 136 people injured, roads and cars submerged, and thousands without power, authorities said.

None of those injured required hospitalization, according to Michelann Ooten, information officer for the Oklahoma Department of Emergency Management.

Meanwhile, thunderstorms continued to roll across the southwestern and south-central part of the state Monday evening, sparing Oklahoma City more rain that would have likely worsened the flooding there.

A state of emergency has been declared in 59 of Oklahoma's 77 counties, at the request of Gov. Brad Henry, who is out of town. Lt. Gov. Jari Askins, who approved the emergency paperwork, commended the state's emergency crews, saying, "Once again, our heroic responders have demonstrated the Oklahoma Standard."

The National Weather Service said that almost 10 inches of rain fell between 2 a.m. and 11 a.m. Monday. A flash-flood watch was extended across central Oklahoma through 7 a.m. CT (8 a.m. ET) Tuesday.

CNN meteorologist Jacqui Jeras said the damage Monday morning was caused by "backbuilding thunderstorms," a series of heavy rain cores without intermittent periods of let-up.

The American Red Cross opened a center for any victims displaced by the storm at an elementary school in nearby Edmond, according the Ooten.

According to Oklahoma Gas & Electric's website, close to 1,500 customers were still without power by 8 p.m. Monday, including 1,300 in the Oklahoma City metro area.

It's a "bad situation throughout the entire county," said Oklahoma County Sheriff's Office spokesman Mark Myers.

Some washed-out Oklahoma City metro highways and ramps remained closed Monday night, including parts of Interstate 40, according to the Oklahoma Department of Transportation. Department officials urged drivers to be alert and warned them to avoid driving through water-covered roads.

The Oklahoma Highway Patrol reported getting about 180 calls related to the flash flooding. Troopers worked nearly 30 crashes, with a total of six injuries.

According to Ooten, several state buildings in the metro area were damaged in the deluge. Those affected agencies either closed or reduced their services because of the flooding.

Standing flood waters also can be dangerous, the Oklahoma State Department of Health warned. They urged residents to avoid the water as it can contain snakes, insects, sharp objects, oil, gasoline, and in some case raw sewage.

Glynda Chu, a police spokeswoman in nearby Edmond, said it was the worst flooding that city has seen in 25 years.

CNN iReporter Jennifer Oliver, who lives in Edmond, says there's normally a little creek with a 20-foot waterfall behind her house, but by late morning, she couldn't even make it out. "Never seen anything quite like that," Oliver said.

KOCO broadcast pictures of a car submerged almost to its windows. Dive teams navigated the raging waters to get to the scene and then rescued the car's occupants by boat. CNN affiliate KFOR showed another water rescue, with crews ferrying people to safety.

It's a dangerous situation for rescue crews as well. CNN affiliate KWTV showed gripping video of a woman trapped in a tree, struggling through the rushing water in search of stronger branches to hold. Oklahoma City Fire Battalion Chief Kirk Wright said the first rescue boat sent her way got stuck, stranding the three would-be rescuers. Another team set off for the scene and returned safely with all four people.

Oklahoma City Fire Department Deputy Chief Marc Woodard estimated that there were 20 to 30 rescues of people stranded in cars, homes or businesses Monday morning.

CNN iReporter Sarah Burris, who lives on the northwest side of Oklahoma City, said leaving her neighborhood wasn't even an option, with every way out under water. Burris said backlogged drains were regurgitating water into the streets.

The National Weather Service was issuing flood warnings across the breadbasket region Monday. Most of Nebraska and southeastern Kansas were under advisories Monday afternoon.

The Weather Service said 3 inches of rain had fallen in both Tulsa, Oklahoma, and Kansas City, Missouri, which experienced flash floods Monday morning. But Morris said the Kansas City overflow had affected only low-lying areas and didn't look like a threat to homes or businesses.
