Identifying Factors For Success of Online Health Promotion Programs

Background: Online health communication has the potential to reach large audiences, with the additional advantages that it can be operational at all times and that the costs per visitor are low. Furthermore, research shows that Internet-delivered interventions can be effective in changing health behaviors. However, exposure to Internet-delivered health-communication programs is generally low. Research investigating predictors of exposure is needed to be able to effectively disseminate online interventions.

Objective: In the present study, the authors used a longitudinal design with the aim of identifying demographic, psychological, and behavioral predictors of visiting, using, and revisiting an online program promoting physical activity in the general population.

Methods: A webpage was created providing the public with information about health and healthy behavior. The website included a “physical activity check,” which consisted of a physical activity computer-tailoring expert system where visitors could check whether their physical activity levels were in line with recommendations. Visitors who consented to participate in the present study (n = 489) filled in a questionnaire that assessed demographics, mode of recruitment, current physical activity levels, and health motivation. Immediately after, participants received tailored feedback concerning their current physical activity levels and completed a questionnaire assessing affective and cognitive user experience, attitude toward being sufficiently physically active, and intention to be sufficiently physically active. Three months later, participants received an email inviting them once more to check whether their physical activity level had changed.

Results: Analyses of visiting showed that more women (67.5%) than men (32.5%) visited the program. With regard to continued use, native Dutch participants and participants with a strong motivation to be healthy were most likely to continue usage of the program. With regard to revisiting, older participants and highly educated participants were more likely to revisit the program after three months. In addition, positive affective user experience predicted revisiting.

Conclusions: The results suggest that online interventions could specifically target men, young people, immigrant groups, people with a low education, and people with a weak health motivation to increase exposure to these interventions. Furthermore, eliciting positive feelings in visitors may contribute to higher usage rates.

Screening for Subclinical Atherosclerosis - Is It Useful?

Atherosclerosis (also known as arteriosclerotic vascular disease or ASVD) is a condition in which an artery wall thickens as the result of a build-up of fatty materials such as cholesterol.

"Screening asymptomatic individuals for subclinical atherosclerosis has the potential to improve cardiovascular disease prevention strategies, but any screening program must yet be proven to prevent actual cardiovascular disease, an editorial viewpoint and commentary suggest."

In the Journal of the American College of Cardiology, Prediman Shah (Cedars-Sinai Medical Center, Los Angeles, California) explains that screening is central to the primary prevention of CVD.

In a related commentary, Michael Lauer (National Heart, Lung, and Blood Institute, Bethesda, Maryland, USA) argues that undertaking such screening has disadvantages, such as over diagnosis, which are often overlooked.

I have enjoyed watching the tele-serials ER as well as Law and Order: SVU ,so i was excited to stumble onto this video on TedMed.

Neal Baer is a political science graduate, a pediatrician ,television writer and producer. He's written numerous episodes of ER and Law & Order: Special Victims Unit. He explains how most of his stories are derived from actual patient circumstances. His story-writing and Film industry experience has given him a unique perspective on Physician skills.

Listening to him, i realized how true what he says is. Interviewing a patient well and then putting it down on paper logically is so very important. Also,television and movies can really be a good health information platform, especially in a movie crazy country like India!

Neal Baer demonstrates the connection between Hollywood and medicine in this enlightening and entertaining talk!

Dr. Seyi Oyesola points out that common, survivable ailments and injuries -- burns, trauma, heart attacks -- kill thousands of Africans each year because basic medical care can be so hard to get. Having spent his early medical career in Africa, he understands the difficulties faced in providing care in under-developed ares.

To help bring surgical care to every region of the continent, Oyesola co-developed CompactOR, or the "Hospital in a Box": a portable medical system that contains anesthetic and surgical equipment. It consists of a pop-up, portable, solar-powered OR for off-grid medicine in Africa and elsewhere.It contains anaesthetic equipment, a defibrillator, a burns unit, plaster-making facilities, surgical equipment and a built-in operating table. It even comes with its own tent to create an ad hoc field hospital..The operating suite is light enough to be dropped into inaccessible zones by helicopter, and can be powered by solar panels.The system is powered by a truck battery, and is made to be readily recharged via solar panel. The basic kit, minus battery, costs about £14,000, or roughly US$25,000; additional modules provide support for an extensive selection of drugs and more specialized medical treatments (including orthopedic surgery).

Although the Hospital-in-a-Box may save lives, Oyesola reminds us that with meager pay and inadequate facilities, there is still little incentive for medical professionals to remain in Africa. Proper education and technical training could pave the way for more, and more capable, new physicians to learn and stay in Africa -- and start the healing of the continent.

Using Computer assisted health risk assessment tools

There are a number of eHealth tools used at various institutions and a Computer assisted Health risk assessment is a good example of a simple tool offering a lot of benefits. In a computer-assisted health-risk assessment (HRA), patients complete a computer survey before seeing their clinician. The interactive program then prints an individualized risk report for the clinician and a recommendation sheet for the patient just before the medical consultation. The intention of such computer-assisted health-risk assessment is to facilitate face-to-face consultation with the provider and not to substitute for patient self-care

This is a small study to enhance understanding about computer-assisted health-risk assessments from physicians’ perspectives. Ten Physicians were interviewed on their experience with Computer assisted health-risk assessment, after completion of a trial at a Canadian, urban, multi-doctor, hospital-affiliated family practice clinic.

The key benefits identified include-

Improved time efficiency, by asking questions on health risks prior to the consultation and triggering patients’ self-reflections on the risks

However, they were unconvinced about the suitability of such risk assessment for all visits to detect new risk information. In terms of feasibility, physicians displayed general acceptance of the risk assessment tool but considered it most feasible for periodic health exams and follow-up visits.

Participants perceived computer-assisted health-risk assessment as a useful tool in family practice, particularly for identifying psychosocial issues. Physicians displayed a general acceptance of the computer tool and indicated its greater feasibility for periodic health exams and follow-up visits than all visits. Future physician training on psychosocial issues should address physicians’ concerns by emphasizing the varying forms of “clinical success” for the management of chronic psychosocial issues. Future research is needed to examine the best ways to implement this program in diverse clinical settings and patient populations.

After stringent criticism by many doctors, including Dr Wolfgang Wodarg, the World Health Organization has found support from expected quarters. Countries which head quarter the MNC pharmaceuticals have openly come out in support of the Pharma industry and the WHO.

Sample this-

"While some have questioned some of the actions taken by the international community, the outcomes speak for themselves. I believe we made the right decisions at the right times," said US Secretary for Health Kathleen Sebelius.

The French health minister Roselyne Bachelot said "I want to express, in France's name, our solidarity with the WHO, which has been taken to task in an unjust manner,". She told the assembly.that knowledge about the severity of the virus was "imperfect" in the beginning yet decisive action had to be taken. She also expressed concern about the perception of swine flu threat, especially for younger people, arguing that public thinking had been muddled and underestimated the risks. She also warned that doubts about expert advice and the role of the pharmaceutical industry could undermine future alerts.

Below is a table quantifying the Case fatality rate of Swine Flu ( approx 0.03%), which is lower than case fatality in seasonal flu ( approx .08%), but which has been handled with much more alacrity and financial resources.

Americans see Light - Health care reform passes the Senate approval

Finally, Healthcare reform is a reality. Passed by the House with a 219-212 vote, the bill now goes to President Obama for signing. And to complete the package, a bill of negotiated changes to the newly passed legislation will go to the Senate for a vote. It took a lot to explain ( a 18-day stretch in which Obama traveled to four states and lobbied more than 60 wavering lawmakers in person or by phone to secure passage of the health reform bill) to all the American lawmakers that health is a right and not a derivative of how much one can pay the Health management organizations or Insurers.

The reform package will extend insurance coverage to some 32 million more Americans, adding millions more who can now access health care.

Much of the money in the bill would be devoted to subsidies to help families at incomes of up to $88,000 a year pay their premiums. The bill will allow senior citizens get more help paying for drugs in Medicare. People with health problems that left them uninsurable could now qualify for coverage through a federal program.

Republican critics said the $940 billion legislation was a heavy-handed intrusion in the healthcare sector that will drive up costs, increase the budget deficit and reduce patients' choices.

Medical Apologies in healthcare reforms.

Recently, i read an article on Medical apologies, and its position in the healthcare reform. The idea does sound like a possibly effective self-regulation method. It can definitely bring down litigation costs and encourage openness and efforts to improve quality by healthcare institutions.

Medical Apologies: Do right and do well

An often overlooked tool in health care providers' struggle with the malpractice crisis is the medical apology. Two thirds of the states provide some form of protection for the medical apology (i.e., a simple apology is not admissible in court as an admission of culpability), and settlements reached post-apology are almost invariably lower that they would be otherwise. (In the current environment, articles on medical apologies are popping up everywhere ... even in the NY Times business section.)

It is important to note that an effective apology policy does not stop with the simple apology -- I'm sorry that this happened to you -- but must include a commitment to conduct a root cause analysis, to communicate the results to the patient and/or patient's family, to implement systems improvements based on the results of the root cause analysis, and to offer a specific apology once the analysis is complete, and an offer of monetary compensation if the provider or its systems were at fault.

Often, following a bad outcome, a patient's family member may wonder if he or she could have done something different to prevent the bad outcome, and the analysis can serve to put his or her mind at ease. In addition, patients and family members often sue because they want to be sure that 'nothing like this ever happens to anyone else.' If a provider can demonstrate that a root cause analysis has been done and systems improvements made so that the same mistakes cannot be repeated, then that impetus for filing suit is removed.

For example, the University of Michigan health system reduced malpractice payouts by about 50% following implementation of its program. Other examples of successful programs abound. (And here's another resource on medical apologies.)

I have been a strict proponent of increasing use of eHealth tools to ensure democratization of better quality health services, best quality healthcare and paperless clean environment for ALL.

Availability of a skilled workforce is definitely one very obvious obstacle in countries like India and Brazil.One of the challenges to implementing health information and communication technology is the need for a skilled workforce that understands health care, information and communication technology, and the people and organizational challenges involved. The intersection of these areas is commonly known as the discipline of biomedical and health informatics (or health informatics for short)

This paper explains the need for skilled and trained professionals for better utilization of information technology and the various peculiarities faced in Developing countries.

E-health and m-health applications hold vast promise to improve global health. As these projects develop, leaders need to be cognizant of the need for a well-trained workforce to lead their implementation. An ideal approach will include needs assessment as well as education and training opportunities for that workforce. Successful local examples can be expanded into larger networks whose scale can be leveraged to more rapidly and effectively disseminate them. Such an approach should also foster the establishment of academic partnerships and centers of excellence in education and research in developing countries for sustainable capacity building while still being responsive to local needs

Preventing hospital acquired infections

Hospital acquired Infections, opaquely called 'Iatrogenic" infections, are a major cause of morbidity and mortality. It has been estimated that such infections in hospitalized patients increase hospital stay by 4 to 5 days, resulting in extra expenditure of billions of dollars. A Center for Disease Control (CDC) report published in March-April 2007 estimated the number of U.S. deaths from healthcare associated infections in 2002 at 98,987.According to the World Health Organization, “At any given time, 1.4 million people worldwide are estimated to be suffering from an infection acquired in a health facility. The risk of acquiring healthcare-associated infections in developing countries is 2-20 times higher than in developed countries".Proper aseptic techniques and strict following of all protocols can drastically cut down this "needless" infection rate.

It is important that the latest guidelines on proper infection management are known to all medical personnel. W.H.O and Big Pharma cooked up the "Swine-Flu Scare"?

Eminent doctors are now coming out openly with comments on the alleged "Swine-Flu-pandemic conspiracy". Dr Wolfgang Wodarg, an epidemiologist, claims the WHO was persuaded to change the definition of 'pandemic' by scientists linked to pharmaceutical companies in order to trigger lucrative 'sleeping' vaccination contracts. The accusations are to be investigated by the Council of Europe after Dr Wodarg, the chairman of the body's health committee, tabled a damning resolution. Dr Wodarg claims the WHO "in cooperation with some big pharmaceutical companies and their scientists re-defined pandemics and lowered the alarm threshold" so that governments would begin to order vaccines. In fact, at one time , the WHO warned against preventive anti-virals thus leaving a free field for Vaccines.

GlaxoSmithKlein is estimated to have made $1.7 billion from sales of H1N1 vaccine sales in the fourth quarter of 2009 alone. Lower than expected uptake of the vaccine has led to a huge oversupply, to the extent that France, Britain, Germany and the Netherlands are all attempting to sell or give away surplus shots - or cancel orders, where possible.

Quite apart from the cost implications to already stretched national health budgets, if Dr Wodarg is right, then millions of healthy people have been, as he puts it, "unnecessarily" exposed to "the risk of poorly tested vaccines" for a flu strain that is "vastly less harmful" than all previous flu epidemics.

Reducing Medical errors in Clinical practice

Ten years ago, a national panel of health care experts released a landmark report on medical errors in the American health care system. Published by the Institute of Medicine, “To Err is Human: Building a Safer Health System” estimated that as many as 98,000 people died in hospitals each year as a result of preventable mistakes. Being hospitalized, it turned out, was far riskier than riding a jumbo jet.

Preventable medical mistakes and infections are responsible for about 200,000 deaths in the U.S. each year, according to an investigation by the Hearst media corporation. The report comes 10 years after the Institute of Medicine's "To Err Is Human" analysis, which found that 44,000 to 98,000 people were dying annually due to these errors and called for the medical community and government to cut that number in half by 2004.
