The future of IBS depends on our increasing knowledge of the processes (mechanisms) that cause IBS. Acquiring this knowledge, in turn, depends on research funding. Because of the difficulties in conducting research in IBS, this knowledge will not come quickly. Until we have an understanding of the mechanisms of IBS, newer treatments will be based on our developing understanding of the normal control of gastrointestinal function, which is proceeding more rapidly. Specifically, there is intense interest in intestinal neurotransmitters, which are chemicals that the nerves of the intestine use to communicate with each other. The interactions of these neurotransmitters are responsible for adjusting (modulating) the functions of the intestines, such as contraction of muscles and secretion of fluid and mucus.&lt;br /&gt;&lt;br /&gt;5-hydroxytriptamine (5-HT or serotonin) is a neurotransmitter that stimulates several different receptors on nerves in the intestine, including one called the 5-HT4 receptor. Stimulation of these receptors by 5-HT increases muscle contractions in the colon. An example of an experimental drug that affects intestinal neurotransmission is tegaserod. This drug mimics the effect of 5-HT on the 5-HT4 receptor. Thus, because it increases intestinal muscle contractions, tegaserod is being tested for effectiveness in treating constipation-predominant IBS as well as constipation in general.&lt;br /&gt;&lt;br /&gt;Another drug that affects neurotransmission is ondansetron. This drug blocks a different receptor, the 5-HT3 receptor, and thereby reduces colonic contractions. Thus, ondansetron (Zofran) has been effective in treating diarrhea-predominant IBS in initial studies. There are drugs that stimulate another receptor, the 5-HT1 receptor. Examples of this type of drug are sumatriptan (Imitrex) and buspirone. These drugs are believed to reduce the responsiveness (sensitivity) of the sensory nerves to what's happening in the intestine. The 5-HT1 receptor stimulators, however, have not yet been tested for effectiveness in IBS. In preliminary studies, fedotozine has been shown to improve functional gastrointestinal symptoms. The mechanism of action of fedotozine is not known, but it also may act by reducing the sensitivity of the sensory nerves.&lt;br /&gt;&lt;br /&gt;Finally, there is the issue of a relationship between IBS and intestinal bacteria. The initial approach to IBS-treatment or testing--depends on the patient's symptoms and their duration. If the symptoms clearly fit the definition for IBS and have been present for several years without change, then there is less need for extensive testing to exclude other intestinal and non-intestinal diseases. Rather, treatment that is directed at specific symptoms, as discussed previously, can begin. The role of antibiotics and/or probiotics is currently being studied.&lt;br /&gt;&lt;br /&gt;On the other hand, if the symptoms are of recent onset (such as weeks or months), progressively worsening, severe, or associated with "warning" signs, then early testing is appropriate. Warning signs include loss of weight, nighttime awakening, rectal bleeding, and signs of inflammation, such as fever or abdominal tenderness. Testing also is appropriate if, in addition to the symptoms of IBS, there are other prominent symptoms that are not part of IBS (for example, abdominal distention, increased flatus, or vomiting). Finally, testing is warranted if attempts at treating the symptoms of IBS are unsuccessful.&lt;br /&gt;&lt;br /&gt;If there are symptoms that suggest non-IBS diseases, tests that are specific for these conditions should be done first. The reason is that if these other tests disclose disease other than IBS, it may not be necessary to do additional testing. Examples of symptoms and possible testing include:&lt;br /&gt;&lt;br /&gt;
&lt;ul style="text-align: left;"&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Vomiting: upper gastrointestinal endoscopy to diagnose inflammatory or obstructing diseases; and gastric emptying studies and/or electrogastrography to diagnose impaired emptying of the stomach.&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Abdominal distention with or without increased flatulence: upper gastrointestinal and small intestinal X-rays to diagnose obstructing diseases; and hydrogen breath testing to diagnose SIBO.&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Constipation without pain: colonoscopy or barium enema to exclude colonic cancer; marker studies to diagnose slow colonic transit; and ano-rectal motility studies to diagnose rectal muscle disorders&lt;/li&gt;

&lt;br /&gt;For a patient with typical symptoms of IBS who requires testing to exclude other diseases, the testing might reasonably include a standard screening panel of blood tests and stool specimens for examination for parasites, pus, and blood. A plain X-ray of the abdomen may be done during an episode of abdominal pain (to look for intestinal blockage or obstruction). Testing for lactose intolerance or a trial of a strict lactose-free diet should be done. Colonoscopy (and, possibly, esophago-gastro-duodenoscopy, or EGD) would be the next test, probably with multiple biopsies of the colon (and stomach and duodenum if EGD is done). Finally, small intestinal X-rays might be done.&lt;br /&gt;&lt;br /&gt;If all of the above appropriate testing reveals no disease that could be causing the symptoms, other tests should be considered. These tests include hydrogen breath testing to diagnose SIBO and antro-duodenal and colonic motility studies to diagnose intestinal muscle or nerve disorders. These studies probably should be done at centers that have experience and expertise in diagnosing and treating these diseases.&lt;/div&gt;

It is unclear if diet has much effect on the symptoms of IBS. Nevertheless, patients often associate their symptoms with specific foods (such as salads, fats, etc.). Although specific foods might worsen IBS, it is clear that they are not the cause of IBS. The common placebo response in IBS also may explain the improvement of symptoms with the elimination of specific foods in some people.&lt;br /&gt;&lt;br /&gt;Dietary fiber often is recommended for patients with IBS. Fiber probably is of benefit in IBS patients with constipation, but it does not reduce abdominal pain. Lactose (milk sugar) intolerance often is blamed for diarrhea-predominant IBS, but it does not cause IBS. Because they both are common conditions, lactose intolerance and IBS may coexist. In this situation, restricting lactose will improve, but not eliminate the symptoms. &lt;br /&gt;&lt;br /&gt;Lactose intolerance is easily diagnosed by testing the effect of lactose (hydrogen breath testing) or trying a strict lactose elimination diet. Intolerance to sugars other than lactose, specifically, fructose, sucrose, and sorbitol, may cause symptoms that are similar to IBS or make IBS worse. Patients with IBS are frequently found to be suffering from depression, but it is unclear if depression is the cause of IBS, the result of the IBS, or unrelated to the IBS. Several trials have shown that antidepressants are effective in IBS in relieving abdominal pain and, perhaps, diarrhea. &lt;br /&gt;&lt;br /&gt;The antidepressants work in IBS, however, at relatively low doses that have little or no effect on depression. It is believed therefore, that they are working not by combating depression, but in different ways (through different mechanisms). For example, these drugs have been shown to adjust (modulate) the activity of nerves and to have analgesic (pain-relieving) effects as well. &lt;br /&gt;&lt;br /&gt;Commonly used psychotropic drugs include the tricyclic antidepressants, amitriptyline (Elavil, Endep), desipramine (Norpramine), and trimipramine (Surmontil). The most widely studied drugs for the treatment of abdominal pain are a group of drugs called smooth-muscle relaxants.&lt;br /&gt;&lt;br /&gt;The gastrointestinal tract muscle is composed of a type of muscle called smooth muscle. (By contrast, skeletal muscles, such as the biceps, are composed of a type of muscle called striated muscle.) Smooth muscle relaxing drugs reduce the strength of contraction of the smooth muscles but do not affect the contraction of other types of muscles. They are used in IBS with the assumption (not proved) that strong or prolonged contractions of smooth muscles in the intestine-spasms-are the cause of pain in IBS. There are smooth muscle relaxants that are placed under the tongue, like nitroglycerin for treatment of angina, so that they may be absorbed rapidly. Smooth muscle relaxants are approximately 20% more effective than a placebo in reducing abdominal pain. It is not clear if smooth muscle relaxants have a beneficial effect on constipation or diarrhea.&lt;br /&gt;&lt;br /&gt;Some commonly used smooth muscle relaxants are hyoscyamine (for example, Levsin) and methscopolamine (for example, Pamine). The most widely studied drug for the treatment of diarrhea in IBS is loperamide (Imodium). Loperamide appears to work by inhibiting (slowing down) the contractions of the muscles of the small intestine and colon. Loperamide is approximately 30% more effective than a placebo in improving symptoms in patients who have diarrhea as the predominant manifestation of their IBS. It is not clear if loperamide reduces abdominal pain. Loperamide can cause constipation. Therefore, the dose must be carefully adjusted and individualized for each patient. Alosetron (Lotronex) is used to treat diarrhea and abdominal discomfort that occurs in women with severe IBS that does not respond to other simpler treatments.&lt;br /&gt;&lt;br /&gt;Alosetron, like tegaserod, affects the serotonin receptors. (See the discussion above of tegaserod.) Alosetron blocks the 5-HT3 receptor, a receptor that causes contractions when serotonin binds to it. Alosetron, by blocking 5-HT3 receptors, prevents serotonin from binding and thereby prevents contractions.&lt;br /&gt;&lt;br /&gt;Alosetron was approved by the FDA in February 2000, but was withdrawn from the market in November, 2000, because of serious, life-threatening, gastrointestinal side effects. In June 2002, it was approved again by the FDA for marketing but in a restricted manner as part of a drug company-sponsored program for managing the risks associated with treatment. The use of alosetron is allowed only in women with severe, diarrhea-predominant, IBS who have failed to respond to conventional treatment for IBS.&lt;br /&gt;&lt;br /&gt;The most common side effect with alosetron is constipation. One-quarter to one-third of patients may develop this side effect, but in only 10% (10 out of every 100 patients) will the drug need to be stopped temporarily or permanently.&lt;br /&gt;&lt;br /&gt;A rare side effect with alosetron is severe intestinal inflammation caused by poor circulation of blood (ischemic colitis). This complication is life-threatening, may require surgery, and has even caused death in a small number of patients. Constipation is due to the slow transport (transit) of intestinal contents through the intestines, primarily the colon. This slow transit may be due to either abnormal function of the muscles of the entire colon or just the muscles of the anus and rectum.&lt;br /&gt;&lt;br /&gt;The treatment of constipation in IBS usually begins with a trial of the supplements and medications that are used to treat constipation of any cause. In 2002, the FDA approved tegaserod (Zelnorm), the first drug designed specifically for the treatment of abdominal pain and constipation in women with IBS. However, in March of 2007, the FDA asked Novartis to suspend sales of tegaserod (Zelnorm) in the United States because a retrospective analysis of data by Novartis from more than 18,000 patients showed a slight increase in the incidence of cardiovascular events (heart attacks, strokes and angina) among patients on Zelnorm compared to placebo. The data showed that cardiovascular events occurred in 13 out of 11,614 patients treated with Zelnorm (0.11%), compared to one cardiovascular event in 7,031 (0.01%) placebo-treated patients. However, it is unclear whether Zelnorm actually causes heart attacks and strokes. Doctors and scientists will be scrutinizing the data further to determine the long-term safety of Zelnorm.&lt;br /&gt;&lt;br /&gt;The mechanism whereby tegaserod reduces constipation is interesting. It is the contractions of the intestinal muscles that controls transit of food through the intestine. More contractions speed transit, fewer contractions slow transit. In constipated patients, contractions are fewer. One important chemical in the control of the contractions is serotonin. Serotonin is manufactured by the nerves in the intestine. It is released by the nerves and then travels to other nerves where it binds to receptors on the nerves. It is, in scientific terms, a "neurotransmitter" that allows nerves to communicate with each other. When it binds to receptors on nerves that control the contractions of intestinal muscles, serotonin can either promote or prevent contractions depending on the type of receptor it binds to. Binding to some types of receptors causes contractions, and binding to other types of receptors prevents contractions. The serotonin 5-HT4 receptor prevents contractions when serotonin binds to it. Tegaserod blocks the 5-HT4 receptor, prevents serotonin from binding to it, and thereby increases contractions of the intestinal muscles. The increased contractions speed the transit of intestinal contents. In addition, tegaserod reduces the sensitivity of the intestinal pain-sensing nerves and can thereby reduce the perception of pain.&lt;br /&gt;&lt;br /&gt;In a randomized, double blind, placebo-controlled study involving more than 1000 patients (80% women) with constipation-predominant IBS, tegaserod was found to be more effective than placebo in increasing the frequency of stools, relieving abdominal pain and discomfort, and decreasing the sensation of bloating in women. (There were an insufficient number of men in the study to draw conclusions about the effectiveness of treatment in men.) The beneficial effects of treatment started during the first week of treatment and were sustained throughout the 12-week period of study.&lt;br /&gt;&lt;br /&gt;Diarrhea was the only side effect in the tegaserod study. Diarrhea usually occurred early during treatment and resolved quickly even if the treatment was continued. There was no effect of tegaserod on blood counts, liver and kidney tests, electrocardiograms, blood pressure, pulse, and body weight. (A medication similar to tegaserod, called cisapride [Propulsid], which also promoted intestinal muscle contractions, was withdrawn from the market due to rare but potentially fatal effects on the electrical rhythm of the heart. So far, there have been no reports of rhythm disturbances related to tegaserod.) Patients with major liver or kidney disease should not take tegaserod. The safety of tegaserod in a fetus or nursing infants has not been studied and is unknown. These studies are done with either radioactive compounds or markers that can be seen on X-rays of the abdomen. &lt;br /&gt;&lt;br /&gt;It also is possible to pass catheters into the stomach and small intestine or the colon to determine if the muscles of these organs are working normally (antro-duodenal and colonic motility studies, respectively). Patients with suspected IBS often undergo abdominal ultrasonography (US), computerized tomography (CT or CAT scans), or magnetic resonance imaging (MRI). These tests are used primarily to diagnose non-intestinal diseases. (Although these tests also may diagnose intestinal diseases, their value for this purpose is limited. As described above, X-ray and endoscopy are better tests.) &lt;br /&gt;&lt;br /&gt;It also is important to realize that US, CT, and MRI are powerful tests and will uncover abnormalities that are unrelated to IBS. The most common example is the finding of gallstones that, in fact, often cause no symptoms. The exclusion of non-functional disease in patients with suspected IBS is an important concern. There are many tests designed to exclude non-functional diseases. The primary issue, however, is to decide which tests are reasonable to perform. Tests are selected individually since each case different. Nevertheless, there are some basic tests that are often performed to exclude non-functional gastrointestinal disease. These tests identify anatomic (structural) and histological (microscopic) diseases of the intestines.&lt;br /&gt;&lt;br /&gt;
&lt;ul style="text-align: left;"&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; As always, a detailed history from the patient and a physical examination frequently will suggest the cause of the symptoms.Routine screening blood tests are performed looking for clues to unsuspected diseases.&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Examination of stool also is a part of the evaluation since it may reveal infection, signs of inflammation, or blood and direct further diagnostic testing. Sensitive stool testing (antigen/antibody) for Giardia lamblia would be reasonable because this parasitic infection is common and can be acute or chronic.&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Some physicians do blood testing for celiac disease (sprue), but the value of doing this is unclear. Moreover, if an EGD is planned, biopsies (taking samples of tissue) of the duodenum usually will make the diagnosis of celiac disease.&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Both X-rays and endoscopies can identify anatomic diseases. Only endoscopies, however, can diagnose histological diseases because biopsies can be taken during the procedure. The X-ray tests include:&lt;/li&gt;

As the capsule travels through the intestines, it sends pictures of the inside of the intestines to an external recorder for later review. However, the capsule is not widely available and its value in IBS has not yet been proven.&lt;br /&gt;&lt;br /&gt;X-rays are easier to perform and are less costly than endoscopies. The skills necessary to perform X-rays, however, are becoming rarer among radiologists because they are doing them less often. Therefore, the quality of the X-rays often is not as high as it used to be. As noted above, endoscopies have an advantage over X-rays because at the time of endoscopies, biopsies can be taken to diagnose or exclude histological diseases, something that X-rays cannot do. &lt;/div&gt;

The symptoms of IBS are varied and inconsistent among patients. Moreover, there are no characteristically abnormal tests that can be used to diagnose IBS. All of this has made it difficult to define IBS and identify patients, especially for research studies. In 1992, a group of international investigators of gastrointestinal diseases met in Rome and developed a set of criteria to be used for diagnosing IBS. The criteria were modified in 1999 and again in 2006. These three sets of criteria are known as the Rome, I, II, and III criteria.&lt;br /&gt;&lt;br /&gt;The the most recent criteria, the Rome III criteria state that in order to be diagnosed with IBS, a patient should have abdominal discomfort or discomfort (not described as pain) at least once weekly for at least two months. The discomfort should be associated with two out of three of the following features:&lt;br /&gt;&lt;br /&gt;
&lt;ul style="text-align: left;"&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Relief with a bowel movement&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Onset associated with a change in the frequency of bowel movement&lt;/li&gt;
&lt;li&gt;&amp;nbsp;&amp;nbsp;&amp;nbsp; Onset associated with a change in the form (appearance) of stool&lt;/li&gt;

&lt;br /&gt;There should be no evidence of an inflammatory, anatomic (obstructive), metabolic, or neoplastic (tumorous) cause of the symptoms.&lt;br /&gt;&lt;br /&gt;Symptoms of dyspepsia (defined by Rome III criteria as abdominal discomfort or pain in the upper abdomen), abdominal distention, and increased flatus (passing gas, or flatulence) do not fall within this definition. The complications of functional diseases of the gastrointestinal tract are relatively limited. Since the symptoms are often provoked by eating, patients who alter their diets and reduce their intake of calories may lose weight. Fortunately, loss of weight is unusual in functional diseases, and it should suggest the presence of a non-functional disease. Symptoms that awaken patients from sleep also are more likely to be due to non-functional than functional diseases.&lt;br /&gt;&lt;br /&gt;Most commonly, functional diseases interfere with the patients' comfort and daily activities. For example, patients who suffer from morning diarrhea may not leave their home until the diarrhea stops. If the diarrhea is constant, they may go only to places where they know that a toilet is readily available. Patients who develop pain after eating may skip lunch. Very commonly, patients associate symptoms with specific foods, such as milk, fat, vegetables, etc. Whether or not such associations are real, these patients will restrict their diets accordingly. Milk is the food that is most commonly eliminated, often unnecessarily and to the detriment of adequate calcium intake. The interference with daily activities also can lead to problems with interpersonal relationships, especially with spouses. Antidepressants are used to treat depression, anxiety, or both by correcting
imbalances in brain chemistry. For people who have &lt;span class="hwlinktext"&gt;&lt;b style="mso-bidi-font-weight: normal;"&gt;irritable bowel syndrome&lt;/b&gt;&lt;/span&gt; (IBS),
doses much lower than those usually used to treat depression can help relieve
symptoms of IBS such as pain, bloating, and feeling like you are unable to pass
a stool.&lt;br /&gt;
&lt;br /&gt;


They may be used to treat chronic, unremitting abdominal pain that
interferes with your daily activities. Here are some examples of
antidepressants used to treat IBS. Your doctor may give you one that is not in
this list.&lt;br /&gt;

For people who have IBS along with depression and anxiety, these medicines
may be used in doses that are usually used to treat depression or anxiety. Some
antidepressants may worsen constipation. Others may worsen diarrhea. You may
start to feel better within 1 to 3 weeks of taking antidepressant medicine. But
it can take as many as 6 to 8 weeks to see more improvement. If you have

&lt;b&gt;FDA Advisories.&lt;/b&gt; The U.S. Food and Drug Administration (FDA) has
issued: &lt;br /&gt;

     on the antidepressant Paxil (paroxetine) and birth defects. One new study
     showed that women who took Paxil during their first 12 weeks of pregnancy
     had a slightly higher chance of having a baby with birth defects. &lt;/li&gt;
&lt;li class="MsoNormal" style="line-height: normal; mso-list: l1 level1 lfo2; mso-margin-bottom-alt: auto; mso-margin-top-alt: auto; tab-stops: list .5in;"&gt;An advisory
     on antidepressant medicines and the risk of suicide. The FDA does not
     recommend that people stop using these medicines. Instead, a person taking
     antidepressants should be watched for warning signs of suicide. This is
     especially important at the beginning of treatment or when the doses are
     changed.&lt;/li&gt;

When every muscle in your body seem to be shouting out in pain, with some tender areas that are unusually painful when touched, and you somehow feel tired as if devoid of all energy, and don't get enough sleep, you may be experiencing symptoms of Fibromyalgia.&lt;br /&gt;

Fibromyalgia is a medical condition characterized by widespread chronic pain in your muscles, ligaments and tendons, as well as fatigue and multiple tender points. Tender points are parts on your body where even a slight pressure may cause pain. They may be found on the neck, shoulders, back, hips, arms, and legs which may hurt when pressure is put on them. More women than men are afflicted with fibromyalgia, and it shows up in people of all ages. It used to be known by other names such as fibrositis, chronic muscle pain syndrome, psychogenic rheumatism and tension myalgias.&lt;br /&gt;
&lt;br /&gt;
Signs and symptoms may vary depending on the the severity of the condition. In some cases, symptoms will probably never disappear completely. At any rate, it may be reassuring to know that fibromyalgia is not progressive or life-threatening. There are available treatments and self-care procedures which can improve this condition and one's general health.&lt;br /&gt;

Signs and Symptoms&lt;br /&gt;

Many factors can affect symptoms of fibromyalgia such as the weather, stress, physical activity or even the time of day. Common signs and symptoms include:&lt;br /&gt;

· Widespread pain - When pressure is applied in specific areas of your body, including the back of your head, upper back and neck, upper chest, elbows, hips and knees, pain accompanied by stiffness is often felt and may persists for several months.&lt;br /&gt;

· Fatigue and sleep disturbances - People who suffer from this condition often wake up tired and unrefreshed in spite of getting enough sleep. According to some studies, this sleep problem is the result of a sleep disorder in which deep sleep is frequently interrupted by bursts of brain activity similar to wakefulness, thus, missing the deep restorative stage of sleep. Nighttime muscle spasms in your legs and restless legs syndrome also may be associated with fibromyalgia.&lt;br /&gt;
&lt;br /&gt;
· Irritable bowel syndrome (IBS) - Constipation, diarrhea, abdominal pain and bloating associated with IBS are common in people with fibromyalgia.&lt;br /&gt;
&lt;br /&gt;
· Headaches and facial pain – Symptoms of headaches and facial pain may be related to the tenderness or stiffness in the neck and shoulders.&lt;br /&gt;
&lt;br /&gt;
· Heightened sensitivity – People with fibromyalgia are usually sensitive to odors, noises, bright lights and touch.&lt;br /&gt;
&lt;br /&gt;
Other common signs and symptoms include:&lt;br /&gt;

· Numbness or tingling sensations in the hands and feet (paresthesia)&lt;br /&gt;

Possible Causes&lt;br /&gt;

There is no known cause of fibromyalgia. Recent findings focus around a theory called "central sensitization" which states that people with fibromyalgia have a lower threshold for pain due to increased sensitivity of the brain to pain signals. This is why people with fibromyalgia seem to overreact when a slight pressure is applied on a certain part of the body which would not hurt if done to a person in normal condition. However, what initiates this process of central sensitization still baffle most research experts. Other theories cited as to the cause of fibromyalgia include:&lt;br /&gt;
&lt;br /&gt;
· Sleep disturbances – Some researchers suggest that disturbed sleep patterns may be a cause rather than just a symptom of fibromyalgia.&lt;br /&gt;
&lt;br /&gt;
· Injury – Accidents that cause injuries or trauma, particularly in the upper spinal region, which affect the central nervous system may trigger the development of fibromyalgia.&lt;br /&gt;
&lt;br /&gt;
· Infection - Some researchers believe that a viral or bacterial infection may trigger fibromyalgia.&lt;br /&gt;
&lt;br /&gt;
· Abnormalities of the autonomic (sympathetic) nervous system – When the sympathetic nervous system fails to function normally during nighttime, it leads to fatigue, stiffness, dizziness and other signs and symptoms associated with the condition. The sympathetic nervous system is part of the autonomic nervous system which controls involuntary bodily functions such as heart rate, blood vessel contraction, sweating, salivary flow and intestinal movements.&lt;br /&gt;
&lt;br /&gt;
· Changes in muscle metabolism – A decreased blood flow to muscles may contribute to decreased strength and fatigue. Such differences in metabolism and abnormalities in the hormonal substance which influences the activity of nerves may play a role.&lt;br /&gt;
Psychological stress and hormonal changes also may be possible causes of fibromyalgia.&lt;br /&gt;

Whenever you experience general aching or widespread pain that lasts several months and is accompanied by fatigue, it is time to see your doctor. Many of the symptoms of fibromyalgia mimic those of other diseases, such as low thyroid hormone production (hypothyroidism), polymyalgia rheumatica, neuropathies, lupus, multiple sclerosis and rheumatoid arthritis. Only your doctor can tell if one of these other conditions may be causing such symptoms.&lt;br /&gt;

Fibromyalgia isn't progressive and does not lead to other serious conditions or diseases. Nevertheless, it can cause pain, depression and lack of sleep. Irritable bowel syndrome, or IBS, is a problem that affects mainly the bowel. It is a disorder characterized most commonly by cramping, abdominal pain, bloating, constipation, and diarrhea. It can be found in children, often is first identified in adolescence and can resolve unexpectedly for periods of time throughout an individuals lifespan, reoccurring at any age. IBS causes a great deal of discomfort and distress, but it does not permanently harm the intestines and does not lead to a serious disease, such as cancer. The disorder accounts for more than one out of every 10 doctor visits. For most people, signs and symptoms of irritable bowel disease are mild. It is fairly common and makes up 20–50% of visits to gastroenterologists. Lower abdominal pain, and bloating associated with alteration of bowel habits and abdominal discomfort relieved with defecation are the most frequent symptoms. It is also called the large intestine. Women are affected more often than men. It's not the same as inflammatory bowel diseases like ulcerative colitis. Colitis, for instance, means inflammation of the large intestine (colon). IBS, however, does not cause inflammation and should not be confused with ulcerative colitis, which is a more serious disorder.&lt;br /&gt;

Some people with IBS find that increasing the amount of dietary fibre and reducing caffeine helps. The word syndrome means a group of symptoms. IBS is a syndrome because it can cause several symptoms. For example, IBS causes cramping, bloating, gas, diarrhea, and constipation. Most people can control their symptoms with diet, stress management, and prescribed medications. For some people, however, IBS can be disabling. They may be unable to work, attend social events, or even travel short distances. In many cases, you can control irritable bowel syndrome by managing your diet, lifestyle and stress. Colon motility (contraction of intestinal muscles and movement of its contents) is controlled by nerves and hormones and by electrical activity in the colon muscle. IBS should not be confused with colitis or other inflammatory diseases of the intestinal tract and IBS is not limited to the colon. In some individuals, IBS may have an acute onset and develop after an infectious illness characterised by two or more of the following: fever , vomiting , acute diarrhea , positive stool culture. This post-infective syndrome has consequently been termed "post-infectious IBS" and is acute onset Rome II criteria positive. This condition is more homogenous, being mostly IBS-D and is drawing much clinical investigation.&lt;br /&gt;

Causes of Irritable bowel syndrome&lt;br /&gt;

No one knows exactly what causes irritable bowel syndrome.&lt;br /&gt;
&lt;br /&gt;
Factor's that seem to produce sympatoms of IBS include diet, emotional stress and hormones.&lt;br /&gt;
&lt;br /&gt;
Ordinary events such as eating and distention from gas or other material in the colon can cause an overreaction in the person with IBS.&lt;br /&gt;
&lt;br /&gt;
Sometimes another illness, such as an acute episode of infectious diarrhea (gastroenteritis) can trigger IBS.&lt;br /&gt;
&lt;br /&gt;
People with IBS often report that family members also have the disorder, suggesting a possible genetic cause.&lt;br /&gt;
&lt;br /&gt;
Symptoms of Irritable bowel syndrome&lt;br /&gt;
&lt;br /&gt;

Fiber supplements or laxatives for constipation or medicines to decrease diarrhea, such as Lomotil or loperamide (Imodium).&lt;br /&gt;

An antispasmodic is commonly prescribed, which helps to control colon muscle spasms and reduce abdominal pain. Antidepressants may relieve some symptoms.&lt;br /&gt;
&lt;br /&gt;
Complementary therapies, such as acupuncture and homeopathy, are often used.&lt;br /&gt;
&lt;br /&gt;
Some people find complementary treatments such as acupuncture help, although there is little scientific proof of their effectiveness.&lt;br /&gt;
&lt;br /&gt;
Anti-diarrheal medications: Over-the-counter medications such as loperamide (Imodium) can help control diarrhea.&lt;br /&gt;
&lt;br /&gt;
Drugs which are used for diarrhoea, such as codeine, can be helpful, but are used less because they can be addictive.&lt;br /&gt;
&lt;br /&gt;
When all else fails, a small dose of a medication usually used to treat depression can be effective. This seems to work in a much smaller dose than is used for depression, and may, in part, work by mimicking the nervous system to the bowel.&lt;br /&gt;

So, if you suffer from IBS (or irritable bowel), one of the biggest questions you always face is - What can you eat? What diet will not flare up your symptoms? And what foods will actually help &amp;amp; heal your stomach?&lt;br /&gt;

There are many theories and diet programs that have been proposed for Irritable Bowel. In this article I’ll examine the main ones I’ve tried, and then share what has worked for me in my 5 years of first hand research with this condition.&lt;br /&gt;
&lt;br /&gt;
There are no Right or Wrong Foods&lt;br /&gt;
&lt;br /&gt;
The first biggest myth I have to debunk right now, is that of "right foods". The truth is that there is no right or wrong food.&lt;br /&gt;
&lt;br /&gt;
And every digestive system is unique. And every condition is unique. So, what may have worked for others may not work for you. On the other hand, what works for you may not work for others.&lt;br /&gt;
&lt;br /&gt;
In this article, and the book you may choose to buy, I’ll share some of the common foods and recipes that have been known to work. But the key thing in looking for the right diet with Irritable Bowel is to do this with a sense of experimentation. Track your diet and see what works for you, under what condition.&lt;br /&gt;
&lt;br /&gt;
The other thing is that the food is sometimes related to the problem you’re facing at the moment. For example, if you’re experiencing gassiness at the moment - it’s obviously best to avoid foods that would aggravate that.&lt;br /&gt;
&lt;br /&gt;
Do Soluble Fibers Help Digestion?&lt;br /&gt;
&lt;br /&gt;
You may have read in several books that foods rich in soluble fibers taken in small quantities over time can help IBS patients stabilize their digestive systems. Since it's one of the most popular theories out there, I gave it a good, hearty shot.&lt;br /&gt;
&lt;br /&gt;
In my case, soluble fibers didn’t help too much - although they didn’t hurt either. I've read of several case studies where long term use of soluble fibers has helped improve the strength of the digestive system. In my case, they may have helped slightly - but a huge difference.&lt;br /&gt;
&lt;br /&gt;
My advice to you is to try out soluble fibers - if it hurts, stop within the week. If it helps (or is neutral), then continue this as a lifestyle habit. Make soluble fibers part of 1 or 2 meals each day. So, it may be a safe bet to eat these foods when in doubt:&lt;br /&gt;
&lt;br /&gt;
* Oat/Oat bran&lt;br /&gt;
* Dried beans and peas&lt;br /&gt;
* Nuts&lt;br /&gt;
* Barley&lt;br /&gt;
* Flax seed&lt;br /&gt;
* Fruits such as oranges and apples&lt;br /&gt;
* Vegetables such as carrots&lt;br /&gt;
* Psyllium husk&lt;br /&gt;
The acid-alkaline balance&lt;br /&gt;
&lt;br /&gt;
Our body has a sensitive balance of acid and acidic content. This is known as the pH balance (you probably remember it from 9th grade chemistry). Usually when we lose this balance we end up getting acidity .&lt;br /&gt;
&lt;br /&gt;
The problem is that almost everything we eat is acidic! Most meat, breads, coffee, soft drinks, and even fruits are acidic … So, how can regain your alkaline balance?&lt;br /&gt;
&lt;br /&gt;
The easiest way is to drink lots of water. Yes, 2-3 liters of water each day will dilute the acidic effect and keep your pH balance normal. If you want to go one step further, drink alkaline drinks. This includes green tea, barley, wheat grass, lemon water, mango/watermelon/apple/guava juice and herb teas.&lt;br /&gt;
&lt;br /&gt;

Fresh, Vegetarian Food Is The Easiest To Digest&lt;br /&gt;
&lt;br /&gt;
Have you every left cooked meat outside, in the eat for 72 hours? You know what happens to it … it starts, attracts acterial growth and you generally throw it away, right?&lt;br /&gt;
&lt;br /&gt;
Well, did you know it takes red meat 60-72 hours to pass through your digestive system? And your stomach is hotter and more humid than any environment outside … just imagine what your digestive system has to put through to absorb and process red meat.&lt;br /&gt;
&lt;br /&gt;
White meat takes 40-48 hours to pass through. A little better, but can you still imagine leaving your grilled chicken out in the son for two days?&lt;br /&gt;
&lt;br /&gt;
Cooked vegetables take 24-30 hours to pass.&lt;br /&gt;
&lt;br /&gt;
Uncooked vegetables take 10-15 hours to pass through your intestines.&lt;br /&gt;
&lt;br /&gt;
And finally, fresh fruits take 2-3 hours to go through your system &amp;amp; get absorbed.&lt;br /&gt;
&lt;br /&gt;
What does this mean for you? Should you give up all kinds of meat? Should you become vegetarian?&lt;br /&gt;
&lt;br /&gt;
I don’t know, and I won’t tell you how to live your life. But I will tell you that if you increase your quantities of fresh foods - your stomach will have a considerably easier time working … and your body will have an easier time healing.&lt;br /&gt;
&lt;br /&gt;
I’ve shifted to having about 50% of my diet as fresh or very lightly steamed/cooked food. And I heavily choose fruits &amp;amp; vegetables over meat. If you do have to choose meat, fish is the easiest food to digest. All other meat is much harder for the stomach to digest (if you must, chicken comes first and red meat later).&lt;br /&gt;
&lt;br /&gt;
Research has found that by comparing the digestive systems of humans with herbivores and carnivores … we have almost everything identical with a herbivore. The acidic content in our stomach, the length of the digestive intestine and role of suliva.&lt;br /&gt;
&lt;br /&gt;
You don’t have to blindly shift to a vegetarian diet - but just try adding some fresh fruits &amp;amp; vegetables to your diet. Replace your breakfast with fresh fruits and notice the immediate difference it brings to your digestion and overall energy. After you’ve experienced that lightness &amp;amp; energy, you won’t need any more convincing.&lt;br /&gt;
&lt;br /&gt;
Avoid Common Trigger Foods&lt;br /&gt;
&lt;br /&gt;
If you must blindly follow one instruction from my book, let it be this - avoid these categories of food at all cost. These are proven triggers for IBS symptoms and you should stay away:&lt;br /&gt;
&lt;br /&gt;
* Coffee, tea and other caffeine drinks (For your morning jolt, try replacing coffee with some light exercise or fresh fruits)&lt;br /&gt;
* Spicy, oily and fried food (It’s a hard one, but you have to stay off KFC &amp;amp; McDonald’s for a few weeks)&lt;br /&gt;
* Dairy foods (cheese, butter, sour cream, cream cheese, milk, cream, half-and half, ice cream, whipped cream, yogurt, frozen yogurt).&lt;br /&gt;
* High-fat foods, High-protein foods &amp;amp; red meat (Your body is unable to create the enzymes required to break down fats)&lt;br /&gt;
*Gassy Foods (beans, broccoli, cabbage, onions, brussel sprouts, and garlic)&lt;br /&gt;
&lt;br /&gt;
How you eat is more important than what you eat&lt;br /&gt;
&lt;br /&gt;
There are some common guidelines that are the basis of healthy digestion. And chances are you're probably ignoring these right now. Follow these pieces of advice and you’ll see an immediate way in which food is processed inside you.&lt;br /&gt;
&lt;br /&gt;
First, remember that almost half the digestion happens in your mouth. Saliva is an extremely powerful digestive enzyme. So, when you don’t chew (and just gobble your food down), you’re doubling the work for your intestine. And your sick intestine won’t stick up for that. So, chew each bite for at least 30 seconds and eat in peace … anxiety worsens IBS.&lt;br /&gt;
&lt;br /&gt;
Second, eat only two to three meals a day. If you’re eating natural healthy food, you don’t need more than three small meals in a day. It takes a lot of energy from your body to digest food - so if you keep eating, you’ll spend all your energy in digestion and have no time to actually enjoy that energy for real work. It's okay to have snacks in between - they help keep the meals small and keep the digestive system on a low-intensity workout all day long.&lt;br /&gt;
&lt;br /&gt;
Third, small meals only. You have to understand the difference between eating for the stomach and eating for the tongue. The body doesn’t need that much to keep going and stay energetic. If you overburden it with three to five large meals in a day, of course the engine will break down. Eat two to three small, regular meals and watch the difference it makes. Planning Your Meals Makes It Easier To Stick To The Right Diet&lt;br /&gt;

Here’s something I learnt after several years of stumbling around. When I started planning my meals the night before … I found that I was able to decide what to eat early enough to actually do it.&lt;br /&gt;
&lt;br /&gt;

Irritable bowel syndrome is characterized by abdominal pain and bloating associated with diarrhea, constipation, or these two alternating.&lt;br /&gt;

After taking it three times a day for ten days, patients reported they felt better overall, and specifically, they reported less bloating.&lt;br /&gt;
&lt;br /&gt;
But some experts say this is not a perfect answer by any means.&lt;br /&gt;
&lt;br /&gt;
"The three main symptoms that define irritable bowel syndrome are abdominal pain, constipation and diarrhea, and patients who read about this study should understand that none of those three symptoms improved in this report with taking this antibiotic," says Dr. Charles Gerson, a gastroenterologist at Mt. Sinai Medical Center.&lt;br /&gt;
&lt;br /&gt;
Dr. Gerson believes you have to treat I.B.S. with perhaps medication but also by addressing the psychological component.&lt;br /&gt;
&lt;br /&gt;
"The end point of the study was only ten weeks after the beginning. And I.B.S. is a long term chronic illness and you have to see what these patients are lie in a year," states Dr. Gerson.&lt;br /&gt;
&lt;br /&gt;

Yet, this study showed the antibiotic’s beneficial effects remained for ten weeks, after only ten days of therapy--implying there is something beneficial occurring.&lt;br /&gt;

Kim has gotten relief from a different medication; in her mind, just getting better whatever it takes is the goal.&lt;br /&gt;

"I've done wonderfully I've had no side effects. I feel great now," says Kim.&lt;br /&gt;

Dr. Gerson does express concern over taking an antibiotic for this, that bacteria might develop resistance to it and then it will lose effectiveness.&lt;br /&gt;
&lt;br /&gt;
This is potentially a problem, but the good news is, this is not an antibiotic that is used for systemic illness. It is used for other conditions, such as traveler’s diarrhea, and Crohn's disease.&lt;br /&gt;
