The Development of the Cosmeticians' and Cosmetologists' Professions

Insurance companies do not tend to differentiate between cosmeticians and cosmetologists when it comes to providing professional liability insurance. The difference is actually important, and should be reflected in the insurance policies provided to cover professional liability insurance.

The concept of professional liability insurance is that it is provided to professionals only. The basics of professional liability insurance is to cover malpractice or the unfortunate results of the excellent practice of well trained and certified professionals, who acquired their certification according to high standards and well-known institutions. This should have led the insurance industry to prefer the higher trained professionals, and in the beauty field these are the cosmetologists.

What differentiates between cosmeticians and cosmetologists is that a cosmetician is someone who sells or applies cosmetics or works in a beauty parlor, while cosmetologists are experts in the use of cosmetics, or beauty specialists.
Not everyone will agree with the different definitions, and we received many responses claiming that the difference is the need of the cosmeticians to improve their public image and, not necessarily the result of better education or expertise.

The first use of cosmetics was in ancient Egypt. The Egyptians created a number of cosmetics: oils, herbs, fragrances for both beauty and religious purposes. Both women and men would use kohl (a black powder kept in a small jar) to outline their eyes and eyebrows. Throughout history, there has been a constant, consistent increase in the use of cosmetics, and the cosmetician's profession was increasingly in demand, with ever-growing status for not just a professional, but for an artist.

The increase in the cost of cosmetics along with the increase in public awareness to consumerism has created the need for protection of the cosmetician from public lawsuit in more complex ways than ever.

Professional liability insurance used to be the main answer to the needs of the cosmetician, but nowadays, this excellent coverage provides only a fraction of the protection required by the cosmetician of the 21st century: The use of applied products and the development of consumer protection laws has elevated the need to provide protection from lawsuits involving the products used (and many cosmeticians still use self-made products, and cannot rely on product liability of a manufacturer). The emergence of hair and beauty salons has created the need to include public or general liability as well, and in the age of gender awareness, a certain protection is required against discrimination and gender harassment.

It is an opportunity, both for the insurance world and for the professionals in the beauty industry to recognize the potential of cosmetologists and to elevate the level of professionals in the beauty industry. Better training, regulated education and instruction, continuous updating - all these will create a better environment for cosmeticians and their customers, and will forge a broader way for protection solutions by the insurance industry.

Today, people are much more aware of their looks. They purchase expensive products, pay a lot for beauty treatments and expect their cosmetician or cosmetologist to be a highly trained and excellent practitioner. The insurance world will either go with this trend and support the enhancement of the beauty practitioners' professionalism or will pay the price of old fashioned protection products. The one who will pay the price in this case may very well be the cosmetician who did not continue to develop professionally.

Tips for patient data security: Policies, education, funding

Many hospital employees today, believe it or not, are not well versed in distinguishing confidential patient information from non-confidential information, according to Andrew Lenardon (pictured), a director of indirect and solution sales for Shred-it International, an information security and document destruction company. What's more, Lenardon says, such employees likely wouldn't know how to react in the event of a privacy breach. And with organizations like the World Privacy Forum estimating upward of 500,000 cases of medical identity theft annually, such a lack of preparedness is hardly what your hospital needs.

Lenardon recently chatted with FierceHealthcare about some of the various privacy issues facing healthcare organizations today, and what he's learned in dealing with those issues.

FierceHealthcare: What are the patient data security issues at hospitals?

Lenardon: There's a lot, to be blunt. Some of the security issues are employee awareness and management support. If your employees aren't aware of the security issues or the security protection requirements, that's a big problem. If the management isn't supporting those policies or creating those policies in the first place, that's a huge issue, as well.

Some other areas that hospital executives need to think about is how they're handling the hard drives in copiers, laptops, and external storage devices, as well as confidential paper and disposing of that information in a secure manner. You have to think about things like patient records being sent to the wrong places. You also have to think about non-paper issues, such as patient health information that is on plastic--like patient cards--that gets misplaced. You also need to think about medical files that are left unattended. Having a complete lack of security on some hospital floors is a big issue, where you've got people coming and going within a hospital setting that are not background checked and not employees of the organization, and they can walk right into those environments. That's a very large physical access issue that leads to breaches.

1. Physicians, patients find Meaningful Use successful, practices say

It's a daunting task, but the small percentage of office-based physicians who have implemented EHRs and participated in the government's meaningful use incentive program indeed have lived to tell about it.

Ft. Wayne (Ind.) Cardiology, a 24-physician cardiology group that has been using health IT since 1996, is one example of a practice that already has attested to the government that it was a meaningful user and has received the maximum $18,000 per physician incentive for 2011.

In addition to the EHR, Ft. Wayne physicians have access to e-prescribing and decision-support tools, as well as the ability to report quality measures directly to the American College of Cardiology's PINNACLE registry, Dr. Michael Mirro told Internal Medicine News. The CCHIT-certified system also includes a meaningful use "meter" that tells physicians how well they are meeting the goals.

The cost of the web-based system is about $2,000 per physician per year, less than average because the practice is owned by a hospital. In addition, the practice already has achieved the stage 2 goal of incorporating a patient web portal.

Like most practices, Ft. Wayne experienced its share of growing pains in adopting the technology. One way the practice helped smooth the transition, however, is by offering point-and-click templates for physicians who don't know how to type.

Meanwhile in rural Kansas (with not enough broadband access for web-based systems), family physician Dr. Jennifer Brull has incurred far greater expenses--about $30,000 per physician so far--on her road to meaningful use. She also told IMN that the first six months following implementation were particularly challenging both mentally and physically.

Despite the struggles, Brull hasn't looked back. "I've found it easier to be more collaborative [with my patients]," she said. For example, she recently used an EHR-generated graph to show a patient that she had gained 40 pounds over three years. The graph made much more of an impression on the patient than Dr. Brull could have done by reading numbers off a paper chart, she said.

In April, Brull collected the full Medicare incentive payment for 2011.

Another recent EMR adopter, North Carolina pediatrician Dr. Bill Adams, recently shared an insightful summary of his practice's six-week implementation period on the blog Pediatric Inc. The post includes a concise list of items the doctor appreciates, regrets, and looks forward to regarding his EMR. His closing: The practice has 40 linear feet of chart shelving for sale.

2. Partners' mEHR app improves workflow, communication with patients

Boston-based Partners HealthCare kicked its mobile strategy into high gear this week, launching an app for its electronic health record, and setting its sights on mobile radiology next.

The healthcare system announced Tuesday that it had completed a light-speed pilot/rollout of only 90 days. Already more than 2,000 clinicians are using the new app, and it's having a "positive impact on workflow" for most, John Pappas, Partners' associate director of clinical systems integration, said in an announcement.

"Instead of finding a workstation, logging in, accessing the patient record and then going to see a patient, they can just pull the iPhone from a pocket and review results at the bedside while they're talking to the patient," he said. "And, they can check on the latest information-test results, for example-at any time from any location."

But it's not just physicians who want that bedside review, Robert Havasy, project specialist for Partners' Center for Connected Health told FierceMobileHealthcare. Patients, too, are eager for hospitals to get their mobile initiatives underway.

"Focus group after focus group of patients tell us that this is the way they expect to interact with their physicians," Havasy said. "They bank online, they text their family and friends, and they expect the same systems to work in medicine. It's the way everything is moving."

On the back-end of the project, it was a "presentation layer" that helped get the EHR onto mobile devices, Pappas said. It solved the biggest difficulty with mobile-enabling the EHR, putting a PC's worth of patient data onto the far smaller smartphone or tablet screen.

"We initially developed the presentation layer for the iPhone. The fact that they're encrypted devices and were being used by many physicians made them a logical choice for the initial rollout," Pappas said. They've since extended the app to Blackberry and iPad platforms.

One interesting note: Partners chose not to provide mobile devices to its staff, instead allowing them to use their personal devices to access the EHR. It means the IT department has less control, but it seems the project's speed simply didn't allow time for researching, vetting, buying and enabling a host of company-owned devices.

"There is definitely a growing demand for new mobile apps, as well as additions to the mEHR," Pappas said. "We're getting requests from business units and clinical groups throughout the organization."

One of the squeaky-wheel business units must be radiology because that's next on Partners' mobile priority list, hospital officials note. The radiology imaging initially will be available only on the iPad, but eventually will be enabled for Android devices, as well.

3. drchrono certification a mainstream win-win for EHRs, mobile health

The government certification of drchrono's iPad app for Meaningful Use, announced last week, represents more than a giant leap for the company, which only a few weeks ago secured $675,000 in seed money. It also has the potential to push both electronic health records and mobile health into the mainstream, simultaneously.

While a Fierce colleague rarely praises the general media's reporting on matters regarding healthcare technology (with this example being one of the only cases), we must say that outlets such as Reuters and the Washington Post seem to have hit the nail on the head with their respective reports about the news. Drchrono's app, after all, is free (even if only to the first user in a practice; additional users must pay between $99 and $199 each). And the company's user base already has climbed beyond 100,000 practitioners, up from 55,000 at the beginning of 2011.

What's more, the EHR platform is mobile via the iPad, as the first of its kind to be officially certified, according to the company, beating Epocrates to the punch). With the mobile platform, drchrono will be very convenient for physicians, who already have been transitioning in droves to more mobile pastures.

"This certification transforms our EHR app and the iPad into a potentially affordable platform that could finally drive global usage and adoption of electronic medical records," drchrono co-founder and CEO Michael Nusimow said in a statement. "The government subsidy offered to physicians who adopt our free EHR solution could be transformational in bringing electronic record keeping into every medical practice."

4. Patient records system failure looms over CSC acquisition

It will be interesting to see what kind of fruit Computer Sciences Corporation's (CSC) recent acquisition of international healthcare software supplier iSOFT will bear in the U.S., after reports that the latter's electronic patient record system in the U.K. has flopped.

Falls Church, Va.-based CSC's $189 million purchase of the Australian company, which provides clinical and business information systems to hospitals in more than 40 countries, was finalized this week, according to a CSC announcement. While iSOFT--which conducts most of its business outside of the U.S., particularly in Australia, India, Spain and the U.K.--is supposed to help CSC expand its global presence in the healthcare IT market, CSC is expected to help iSOFT make a move into the U.S. market.

But a committee of the U.K. Parliament recently found that the National Health Service's investment in electronic health records has produced no benefit for taxpayers, according to The Guardian. The project is behind schedule and over budget, the committee report said, with iSOFT described as responsible for installing a large base of the systems in the failed venture.

In response, CSC said it continues to believe that the EHR initiative "will deliver value for money to the U.K. government." CSC noted that iSOFT already has implemented similar systems successfully in 1,800 sites across the north, midlands, and east regions of England. "These systems are already delivering substantial benefits through the introduction of electronic patient record systems into primary and secondary care," the company said in a statement.

iSOFT's plans for the U.S. include introducing an enterprise scheduling solution that will become part of CSC's accountable care organization initiative. It might also bring solutions related to interoperability and clinical analytics to the U.S. market, Bob Reese, managing director of CSC's North American health delivery practice, told FierceHealthIT.

Conversely, Reese said, iSOFT "will take advantage of CSC's broad service capabilities.

"Where there are opportunities to support iSoft in providing services to existing base of business, we'll be putting those solutions there," he said. "iSOFT is uniquely positioned with CSC to strengthen their position on a global basis."

iSOFT provides products and services to more than 13,000 provider organizations around the world and employs about 3,000 people.

5. New Memorial Hermann HIE pushes health data to docs

MHMD, the physician network of Memorial Hermann Healthcare System in Houston, is implementing a new health information exchange and point-of-care reminder system to increase clinical integration and improve the quality of care.

Memorial Hermann, a national trend setter among integrated delivery systems, has about 3,500 employed and independent physicians in its affiliated network. But only the physicians in what MHMD calls its "advanced primary care practices" will participate in the rollout of the new patient-centered medical program, according to an article in Healthcare IT News.

The central health IT feature in this program is an innovative point-of-care tool that will provide physicians and nurses with up-to-date information on the care of each patient when they come for a visit or call a practice on the phone. Apparently, the new HIE will supply the data that will be automatically forwarded to clinicians at every point of contact with the patient. For example, physicians can be prompted to order needed tests, or can be informed about medication changes made by other doctors. This will fill a gap in the dissemination of information to providers because information will be pushed to them, instead of them having to search for the data.

Physicians in the advanced primary care practices program are seeking to build patient-centered medical homes. Besides using the new HIE and point-of-care service, they must follow evidence-based guidelines, report quality data, promote patient self-management, use registries and prescribe electronically.

Over the past few years, MHMD has helped hundreds of physicians adopt electronic health records. It claims to be the largest clinically integrated physician network in Texas; however, it has been less successful in contracting with payers than it had hoped, according to a recent network newsletter.

The latest issue of the Journal of Oncology Practice offers a treasure-trove of electronic medical record goodies, with articles ranging from the relevance of Meaningful Use to CPOE implementation tips and traps.

Writing about the vital role that EMRs will play in healthcare delivery, Dr. Peter Paul Yu, of the Palo Alto (Calif.) Medical Foundation, points out the many problems plaguing our healthcare system--a fragmented provider base; poor communication among providers and patients; misaligned stakeholder incentives; disparate governance structures and insufficient financial resources.

Nothing short of an entire societal culture shift will save us, he argues. "Although widespread adoption of electronic health records is frequently and correctly cited as necessary to meet the challenge of doing better with less, technology alone will not accomplish this," he writes.

Meanwhile, Johns Hopkins researchers Allen R. Chen and Christoph U. Lehmann share the key learnings of their CPOE implementation at Johns Hopkins Children's Medical and Surgical Center in Baltimore. While the hospital used a "standard, commercially available CPOE system," Chen and Lehmann describe the painstaking customization that was necessary to address the complexities of pediatric oncology, including the development of 30 medical logic modules and 110 order sets.

Their efforts have paid off. Docs more often submit orders that adhere to specific research protocols and standard order sets. And medication-related patient safety events have decreased by 39 percent. "Acceptance of the system is high in all clinical disciplines," they write. --Read the full Journal issue

1. Prime Healthcare accused of Medicare fraud, seeking legal action against press

In a heated battle between Prime Healthcare and California Watch, the Southern California hospital system is accused of admitting an unusually high number of emergency Medicare patients to reap the financial benefits.

California Watch on Friday reported that past patients and former employees said Prime Healthcare intentionally is keeping insured emergency patients, mostly elders, in the hospital as part of business strategy for increased Medicare payments. In addition, the publication reports Prime is currently under investigation for its billing practices by the U.S. Department of Justice, having been accused of upcoding for extra Medicare revenue; however, Prime said it is unaware of any such federal investigation.

According to the California Watch article, Prime Healthcare's percentage of Medicare patients admitted from the emergency room to hospital beds jumped from 45 percent to 63 percent after 2005 when Prime added 11 hospitals to its network.

Prime, disputing the claims, said "Taking the state average is not a fair way of reviewing admission rates as admissions patterns vary widely based on a lot of factors such as geography, average age of the seniors, how sick the seniors are, whether the patients come from nursing homes and board and care, belong to Medicare HMOs or not, and how busy the ER is and how often it closes," in a Saturday press release.

Futhermore, the CEO of Shasta Regional Medical Center, under parent company Prime Health Services, refuted any wrongdoing regarding the high rate of admitted Medicare patients. "Most of our business comes through our emergency room," said CEO Randall Hempling in a Record Searchlight article. "We focus on our emergency department, and we are proud of that."

Prime Healthcare denies the accusations, calling California Watch "nothing more than a mouthpiece for SEIU," and the "propaganda" report as "flawed," according to press releases.

Prime Healthcare is planning on suing California Watch for "repeated fraudulent and defamatory statements," according to a Friday press release from Prime.

"It is unfortunate that California Watch continues to rely on manipulated data and unsubstantiated allegations fed by Service Employees International Union (SEIU) to publish fraudulent and defamatory statements about Prime Healthcare," said Prime in the Saturday press release.

At press time, California Watch's "Editor's Note" mentioned the publication had not been served and could not fully comment, but the publication stands by its reporting. "[T]he company has yet to present to us a single factual error that has merited correction or clarification," said the California Watch editor about Prime.

2. Cost savings in ACOs winning provider support

Although healthcare executives may be split on their support or opposition to accountable care organizations (ACOs), the touted costs savings attached to ACOs may be winning over the provider groups, reports USA Today.

Between 60 and 80 healthcare organizations currently use private accountable care models, according to Elliott Fisher, director of Dartmouth's Center for Health Policy Research, according to the article. By, next year, that number will rise to 100 ACOs.

According to Dartmouth and the Brookings Institution, by 2013, there will be 200 ACOs and soar between 500 and 1,000 within three years.

Why the surge? Some organizations believe there's more to be gained with ACOs.

"It's a risk, but the payoff is better," said Judith Rich, president of TMC Healthcare in Arizona, in the article. "It's like the stock market."

Mayo Clinic, who will be notably absent from the ACO program, said the program might not be the best move for some institutions. "If you're already a high-quality, low-cost facility, it's really hard to move up," said Patricia Simmons, medical director of government relations at Mayo. "So what's the incentive? If you have less quality, you've got a great opportunity."

3. Foreign-trained physicians tapped to end shortage

With the cloud of a physician shortage looming over the heads of healthcare executives, many organizations may find the solution to filling in the workforce gap with foreign-trained healthcare professionals.

For instance, the Welcome Back Initiative is a free-of-charge, nonprofit organization, aimed at helping foreign-born healthcare professionals gain credentials to work in the U.S. Many healthcare professionals already exist in the country but are untapped and working below their education level, according to an American Medical News article.

"I'm a doctor from my country, and I want to do it here. I love medicine. I love my patients," said Dr. Maria Montenegro in the article. "I don't care if I have to go to Alaska to practice medicine, I want to do it."

The existing pool of healthcare professionals may be one solution to the workforce crisis, although there may be language, educational, or cultural challenges with immigrant healthcare professionals.

The Welcome Back Initiative helps them market their skills and obtain further education when necessary, in addition to attaining cultural competency and language skills, according to the Welcome Back Initiative website.

How successful is the program? Out of the 4,022 physicians in the program, about 100 have entered residency training, according to American Medical News.

Aside from only numbers, the program and foreign-trained professionals might add another benefit to the healthcare industry--diversity. According to the Welcome Back Initiative website, the program helps add diversification to the medical profession.

"Having those who speak the language and have the cultural competencies is important," said Migration Policy Institute Director of Communications Michelle Mittelstadt, in the article. "The health care system is already taxed and will be taxed even more as health reforms move forward. This is about taking advantage of the pool of people already in this country."

4. Hard times for hospitals call for more donations

As many hospitals continue to hemorrhage money or worse, announce layoffs and closings, facilities are looking for ways to curb Medicare and Medicaid payment reductions and uncompensated care. One way might be to focus efforts on philanthropic donations.

For example, Shriners Hospitals will end its 89 years of free care, a move that comes from reduced donations, reports UPI. Shriners' endowments have been hard hit by the economic downturn. The hospitals, instead, will bill insurance companies and charge families that can afford the copayments.

"We're not a poor organization," said Doug Maxwell, president and chief executive officer of Shriners Hospitals for Children, based in Tampa, Fla., in the article. "But if we don't make wise decisions now, we won't last."

University of California, San Francisco, on the other hand, recently has received an outpouring amount of donations though heavy philanthropy efforts, reports the San Francisco Gate.

After a complicated pregnancy and the birth of his daughter at UCSF, Salesforce.com founder Marc Benioff and his wife, Lynne, initially committed to donating $20 million anonymously to the hospital. However, after a meeting with UCSF Medical Center CEO Mark Laret, Marc Benioff donated $100 million toward the UCSF Benioff Children's Hospital, at the urging of Laret to use their name to encourage other donors.

The donation had a ripple effect with others donations of $90 million dedicated to UCSF, according to the article.

Donation solicitations came from innovative marketing methods.

For example, Lynne Benioff began advising the UCSF Foundation board on ways to improve the patient experience, having gone through it herself. She also raised $1 million through Facebook and Twitter.

Marc Benioff organized a celebrity benefit concert with Neil Young that brought in $3 million in donations.

"If you really want to have a company that has a philanthropic orientation, it needs to not be a bolt-on," Marc Benioff said in the article about his company philosophy. "It needs to be integrated."

What do hospitals have in common with Disney World? A lot, according to Patrick Jordan, healthcare consultant at Disney Institute.

1. Leaked Pioneer ACO draft proposal released

A leaked copy of the Pioneer Accountable Care Organization (ACO) draft proposal from the Centers for Medicare & Medicaid Services (CMS) Innovation Center has been published on The Hill today.

As many speculated what the Shared Savings Program would hold, even more questions surrounded the accelerated Pioneer ACO program.

The unofficial draft proposal indicates that patient choice will remain intact for the ACO-aligned population.

"At all times, Medicare beneficiaries will remain free to select the providers and services of their choice. The Pioneer ACO payment model does not include any restrictions on or changes to Medicare fee-for-service benefits, nor does it include provisions for beneficiaries to opt out of alignment with an ACO for purposes of expenditure calculations and quality performance measurement," according to the 43-page document. "Any future possible provisions for beneficiaries to opt out of expenditure calculations and quality measurement will mirror those in the final regulations for the [Medicare Shared Savings Program]."

In addition, providers worried that particularly chronically ill patients might affect savings. The draft indicates that ACOs will able to place caps on certain patients.

"ACOs may elect to cap the expenditures incurred by an individual beneficiary to reduce the impact of beneficiaries who incur extraordinary claims on the average performance year expenditure of the ACO-aligned population."

In addition, various reference population trends will be factored in, including disability and age, for each performance year. Baseline expenditures will also change with each performance year and will be recalculated, an indication that CMS recognizes population changes from year to year.

A major criticism of the Pioneer ACO program from doubtfuls is the timing of the program. The draft specifies that Sept. 30 (two months away) will be the deadline for participants to identify the preliminary aligned population.

2. Tenet settles Katrina lawsuit on emergency unpreparedness for $25M

In a lesson for hospitals everywhere, Tenet Healthcare Corporation's settlement for $25 million was preliminarily approved yesterday, to conclude a class-action lawsuit in which patients and families of victims trapped in Memorial Medical Center during Hurricane Katrina alleged Tenet's emergency preparedness at Memorial Medical was insufficient.

Orleans Parish Civil District Court Judge Rosemary Ledet, who approved the settlement, called the settlement "fair, reasonable and adequate," reports ProPublica.

The number of class members is unknown. There were 187 patients and about 800 visitors in the hospital during the emergency.

After the 2005 storm hit, there were 45 bodies found at the medical center. Some doctors said they injected patients with drugs to speed up their deaths, although no formal charges were brought against them, according to the article.

Tenet denies the allegations in the case. According to the article, the hospital's backup generators failed, and rescue helicopters arrived two days after the hospital flooded. Tenet and hospitals solicited the help of FEMA, the Coast Guard, the National Guard, state officials and private ambulance companies, but attorneys said the government response was chaotic.

According to U.S. Department of Health & Human Services reports, three-quarter of hospitals are now prepared for disasters, with 76 percent of hospitals that participate in the National Hospital Preparedness Program meeting 90 percent or more of measures for all-hazards preparedness in 2009.

"Our country should overcome our federalist history and bring these [efforts] together on a national mission to respond to large-scale emergencies on a very fast responsive basis. It's doable," Clif Carothers, president of EPI-Center, Inc., a company that specializes in medical evacuation, and president of care transportation company U.S. Air Ambulance, said in an interview with FierceHealthcare.

3. Execs split on ACO participation, reimbursement weighs on minds

With an almost even split between joiners and non-participants or undecided respondents, healthcare executives this week revealed their intentions to participate or forgo the accountable care organization (ACO) Shared Savings program at the 19th Annual Health Forum and the American Hospital Association Leadership Summit.

Respondents reported 45.5 percent will definitely join (18.2 percent) or probably will join (27.3 percent). On the flip side, 36.4 definitely will not join (11.4 percent) or probably will not (25 percent). The other 9.1 percent were unsure.

Another study released this week by U.S. News and World Report estimated that healthcare executives were more likely or extremely likely to join ACOs at a combined 66.4 percent of respondents. Only 3.7 percent said that joining an ACO was not at all likely.

According to the Summit poll, healthcare executives reported that hospitals will have to learn how to make money on Medicare-level reimbursement; 69.6 percent strongly agreed, and another 27.8 percent somewhat agreed.

They also reported how they ranked the following initiatives as major priorities: electronic health records (93 percent), redesigning care delivery process and procedures (89.5 percent), advanced diagnostic technology (60.7 percent), advanced therapeutic technology such as the da Vinci (34.5 percent), and social media (30.4 percent).

4. Steward acquires another hospital in Massachusetts buying spree

Continuing in a series of recent deals, Steward Health Care is at it again by announcing yesterday it will buy Saints Medical Center in Lowell, Mass., a 157-bed nonprofit hospital, reports The Boston Globe.

"Steward's mission is to keep care in the community by investing in our hospitals, growing our world-class physician network and bringing the latest medical technology to the community hospital setting," said Steward CEO Ralph de la Torre in a press release, according to the article. "Joining our system will ensure that Saints Medical Center remains a financially strong hospital that provides world-class care and acts as an economic engine for the Greater Lowell community."

The sale of Saints Medical Center is worth $98 million. Officials said the for-profit acquisition will not affect the 1,300 employee jobs at Saints, according to the article.

The Saints deal follows a rash of Massachusetts hospital deals. A decade from now, it's likely that only three major players will be left in the state's healthcare arena, according to Robert Blendon, professor of health policy and political analysis, Harvard School of Public Health, in a Kaiser article.

"There are mergers, consolidations, groups merging with larger groups, so when negotiations come, there are going to be very large players, even larger than the systems that most people envision," he said.

5. Tips for improving physician integration culture

The most sensitive aspect of physician employment, co-management, or merger is, arguably, the cultural clash that occurs between hospitals and physicians. To alleviate the challenges of hospital-physician integration, focus on leadership, teamwork, and communication, reports Healthcare Financial Management Association (HFMA) magazine.

By 2013, less than one-third of physician practices will remain independent, having elected to join larger hospital systems, according to a recent Accenture Health report.

In addition, healthcare mergers and acquisitions has hit an all- time high in terms of dollar value.

Making the most of this trend, merger and acquisition deals are 26 percent more likely to be successful if all participants pay attention to resolving cultural differences, according to a 1999 KPMG study, reports the HFMA article.

Culture can run the gamut of treatment of patients, referring physicians, and employees; decision-making process; performance rewards; risk; and quality and costs. Hospitals and physicians should identify these cultural differences even before integration takes place, said Mike Valentine, a cardiologist from Lynchburg, Va., in the article.

Just as important is identifying who the leaders are in the physician practice and the hospital. The merging organizations should use these representatives to drive the process and communicate every step to each other. The article suggests that, together, hospitals and physicians develop a measurable vision, understand the joint agreement, implement an organizational communication plan, analyze results, and measure cultural alignment.

Perhaps to the dismay of budget holders, a new study published in Health Affairs indicates that more public spending reduces preventable deaths.
