What is ionizing radiation?
Ionizing radiation consists of subatomic particles (that is, particles that are smaller than an atom, such as protons, neutrons, and electrons) and electromagnetic waves. These particles and waves have enough energy to strip electrons from, or ionize, atoms in molecules that they strike. Ionizing radiation can arise in a number of ways, including the following:
- From the spontaneous decay (breakdown) of unstable isotopes. Unstable isotopes, which are also called radioactive isotopes, give off, or emit, ionizing radiation as part of the decay process. Radioactive isotopes occur naturally in the Earth’s crust, soil, atmosphere, and oceans. These isotopes are also produced in nuclear reactors and nuclear weapons explosions.
- From cosmic rays originating in the sun and other extraterrestrial sources and from technological devices ranging from dental and medical x-ray machines to the picture tubes of old-style televisions.
Everyone on Earth is exposed to low levels of ionizing radiation from natural and technological sources in varying proportions, depending on their geographic location, diet, occupation, and lifestyle.
At high doses, ionizing radiation can cause immediate damage to a person’s body, including radiation sickness and death. Ionizing radiation is also a carcinogen, even at low doses; it causes cancer primarily because it damages DNA. However, the lower the dose of ionizing radiation, the lower the chances of harm.
Children and adolescents are more sensitive to the cancer-causing effects of ionizing radiation than adults because their bodies are still growing and developing. In addition, children and adolescents usually have more years of life following radiation exposure during which cancer may develop.
What cancer risks are associated with nuclear power plant accidents?
Nuclear power plants use energy released by the decay of certain radioactive isotopes to produce electricity. Additional radioactive isotopes are produced during this process. In nuclear power plants, specially designed fuel rods and containment structures enclose the radioactive materials to prevent them, and the ionizing radiation they produce, from contaminating the environment. If the fuel and surrounding containment structures are severely damaged, radioactive materials and ionizing radiation may be released, potentially posing a health risk for people. The actual risk depends on several factors:
- The specific radioactive materials, or isotopes, released, and the quantities released.
- How a person comes into contact with the released radioactive materials (such as through contaminated food, water, air, or on the skin).
- The person’s age (those exposed at younger ages are generally at higher risk).
- The duration and amount of the exposure.

The radioactive isotopes released in nuclear power plant accidents include I-131 and Cs-137. In the most severe kinds of accidents, such as the Chernobyl accident in 1986, other dangerous radioactive isotopes, such as strontium-90 (Sr-90) and plutonium-239, may also be released.
Human exposure to I-131 released from nuclear power plant accidents comes mainly from consuming contaminated water, milk, or foods. People may also be exposed by breathing dust particles in the air that are contaminated with I-131.
Inside the body, I-131 accumulates in the thyroid gland, which is an organ in the neck. The thyroid gland uses iodine to produce hormones that control how quickly the body uses energy. Because the thyroid does not distinguish between I-131 and nonradioactive iodine, the thyroid gland will accumulate either form. Exposure to radioactive iodine may increase the risk of thyroid cancer many years later, especially for children and adolescents.
Exposure to Cs-137 can be external to the body or internal. External exposure comes from walking on contaminated soil or coming into contact with contaminated materials at nuclear accident sites. Internal exposure can come from breathing particles in the air that contain Cs-137, such as dust originating from contaminated soil, or ingesting contaminated water or foods. Because Cs-137 is not concentrated in a particular tissue, the ionizing radiation that it releases can expose all tissues and organs of the body.
How have researchers learned about cancer risks from nuclear power plant accidents?
Much of what is known about cancer caused by radiation exposures from nuclear power plant accidents comes from research on the April 1986 nuclear power plant disaster at Chernobyl, in what is now Ukraine. The radioactive isotopes released during the Chernobyl accident included I-131, Cs-137, and Sr-90.
Approximately 600 workers at the power plant during the emergency received very high doses of radiation and suffered from radiation sickness. How long after exposure to I-131 is the risk of thyroid cancer increased?
Although the time it takes for the radiation to decrease by half (the half-life) of I-131 is only 8 days, the damage it causes can increase the risk of thyroid cancer for many years after the initial exposure.
A study led by National Cancer Institute (NCI) researchers followed more than 12,500 people who were younger than age 18 at the time they were exposed to high doses of I-131 (0.65 Gy on average) from the Chernobyl accident. A total of 65 new cases of thyroid cancer were found in this population between 1998 and 2007. Roughly half of these new cases were attributed to I-131 exposure. The researchers found that the higher a person’s dose of I-131, the more likely they were to get thyroid cancer (with each Gy of exposure associated with a doubling of risk). They also found that this risk remained high for at least 20 years.
What can people do to protect themselves from health risks associated with exposure to contamination from a nuclear power plant accident?

What should cancer patients do if they live in an area that may be contaminated due to a nuclear power plant accident?
Cancer patients who are being treated with systemic chemotherapy or radiation therapy should be evacuated from the area where a nuclear power plant accident has occurred so their medical treatment can continue without interruption. Patients should always keep a record of the treatments they have had in the past and that they may be currently receiving, including the names of any drugs and their doses. These records may be important in the aftermath not only of a nuclear power plant accident but also of other large-scale events that may disrupt medical services, when medical records may be lost.
Local or national authorities may also advise certain people (newborns, infants, children, adolescents, and women who are pregnant) in areas with high I-131 contamination to take potassium iodide (KI) to prevent the accumulation of I-131 in their thyroid. KI should not pose a danger to someone who previously received radiation therapy or chemotherapy. Patients who are actively being treated for cancer and who are advised to take KI should consult with their doctor before taking the medication, so their doctor can evaluate their treatment plan and their health status, including their nutritional status, to determine the safety of KI treatment for them.
What research is NCI currently supporting on ionizing radiation and cancer risk?
Researchers at NCI and elsewhere continue to learn about the cancer risks from ionizing radiation by studying various groups of people, including those who were exposed as a result of the Chernobyl accident, survivors of the atomic bomb explosions in Japan during World War II, and those who were exposed to medical forms of radiation.

Pericardial mesothelioma is a very rare form of mesothelioma. It affects the pericardium, which is a very thin lining of tissue that surrounds the heart.

Novel Anti-Mesothelioma Strategy

A drug commonly used to treat kidney cancer may increase the effectiveness of chemotherapy for mesothelioma, according to a study published in the May issue of the Journal of Thoracic Oncology.

Temsirolimus, a kinase inhibitor, blocks the action of mammalian target of rapamycin (mTOR), a protein that regulates cell growth, which can slow tumor growth. It's used to treat advanced renal cell carcinoma.

But researchers in Austria have found that temsirolimus also may slow the growth of malignant pleural mesothelioma cells. Mesothelioma, a cancer that is usually caused by exposure to asbestos and may not appear until 30 to 50 years after exposure, frequently resists chemotherapy and radiation treatment.

The researchers found that temsirolimus strongly blocked mTOR-mediated signals and had a cytostatic, or growth-stopping, effect on all mesothelioma cells. However, mesothelioma cells that were resistant to cisplatin, a widely used chemotherapy drug, showed hypersensitivity against temsirolimus.

That suggests that mTOR inhibitors like temsirolimus might be a promising treatment strategy either in combination with chemotherapy or as second-line treatment after chemotherapy failure.

"Malignant mesothelioma is a severe human malignancy characterized by a very bad prognosis, with a mean patient survival time of less than one year," said Professor Walter Berger, PhD, of the Institute of Cancer Research at the Medical University of Vienna. "This unacceptable situation is mainly caused by late diagnosis combined with a distinct resistance to all forms of systemic therapy available so far. Mesothelioma is frequently caused by asbestos exposure and unfortunately - based on the long latency period - the incidence peak lies, despite the ban on asbestos, still ahead. Consequently, novel therapeutic options for this devastative disease are urgently needed.

"In our preclinical study, published in the JTO, we were able to demonstrate that inhibition of the major oncogene mTOR is active against human mesothelioma especially after development of chemotherapy resistance both in vitro and in vivo," Berger said. "These results suggest the initiation of clinical trials involving mTOR inhibitors as a novel anti-mesothelioma strategy."

The study was supported by the Medical Scientific Fund of the Mayor of the City of Vienna, Initiative Krebsforschung of the Medical University Vienna, and a Wyeth/Pfizer research grant.

New accurate mesothelioma test developed

Rosetta Genomics, a molecular diagnostics company, has developed a new test for mesothelioma which takes advantage of microRNA biomarkers. Biomarkers are pieces of genetic information expressed in the bloodstream or in tissue samples of patients which can help doctors and researchers to screen for specific responses in the body that correlate to certain illnesses, conditions or other phenomena that incite biological responses.
The test, which Rosetta Genomics is marketing as miRview meso, is performed on a tissue sample removed during a biopsy and takes about ten days to produce results. While the test is reported as incredibly accurate, it still relies on a biopsy – much like diagnostic tests for mesothelioma have in the past. This means that patients must first be identified as candidates for malignant mesothelioma before the test can take place – a process which relies heavily on a well informed, proactive patient.
Individuals who are suffering from mesothelioma symptoms should discuss any history of asbestos related work with their doctor, and should advocate accurate testing as soon as possible in order to avoid a late diagnosis. A late diagnosis can lead to reduced mesothelioma treatment options associated with short-term survival of the disease.
The test developed by Rosetta Genomics differentiates malignant pleural mesothelioma (MPM) from other carcinomas using microRNA, which is believed to be considerably more stable than other biomarkers. The tests, according to researchers, are an important part of a proper diagnosis which accompanies more effective treatment.
“Differentiating MPM from primary and metastatic carcinoma in the lung and pleura can be challenging for pathologists, even with the use of immunohistochemistry,” said the Medical Director and Head of Clinical Laboratory Research at Rosetta Genomics, Tina Edmonston, M.D. “There is no single immunohistochemical marker that distinguishes between MPM and various carcinomas that may be in the differential diagnosis. Moreover, the choice of markers, as well as interpretation of the results in equivocal cases can be subjective.”
“Malignant pleural mesothelioma is a very aggressive solid malignant tumor of the pleura that leads to a severe, clinically-symptomatic disease with very poor prognosis. Accurately distinguishing MPM from other carcinomas affecting the lung and pleura is important, as it guides treatment decisions, particularly for the newer immunomodulating and targeted therapeutics on the market and in development,” Dr. Edmonston continued.
Kenneth A. Early diagnosis prevails as the most recognized factor contributing to increased survival in mesothelioma patients. While the same can be said of many diseases, and specifically many cancers, it’s particularly true in the case mesothelioma owing to a variety of variables.
Mesothelioma, a cancer which affects primarily the area around the lungs, abdomen or heart, is caused by exposure to asbestos fibers. The disease takes varying amounts of time to develop from patient to patient, and has been known to occur in cases with widely differing levels of exposure to the dangerous mineral. One of the defining characteristics of the disease is its long latency period, or the unusual amount of time that it takes for malignant tumors to develop after exposure to asbestos fibers occurs. In some patients, it can take as long as fifty years for the disease to present from the time of initial exposure.
An early diagnosis occurs most often in patients who are younger, healthier, and more fit to recover from the harsh mesothelioma treatments prescribed by doctors who intend to attack the disease. The vast majority of mesothelioma patients are beyond retirement age and already experiencing failing health; as such their suggested treatments are often confined to palliative measures aimed at decreasing pain and discomfort rather than attacking the cancer itself. In the rare cases when the disease is diagnosed early, doctors may prescribe radical treatment regimens such as curative surgery combined with intra-pleural chemotherapy and radiotherapy – a harsh treatment which is difficult to recover from, but one intended to eradicate malignant tissues.
Limited spreading of the cancer is also a benefit of early diagnosis, in addition to relative youth and ability to recover. Mesothelioma which can be contained and treated in one, localized site is more likely to be effectively destroyed without causing irreparable collateral damage to the surrounding tissues.
Unfortunately, effecting an early diagnosis continues to be difficult. Last year the Oxford Centre for Respiratory Medicine contributed a study on improved diagnosis methods to the American Journal of Respiratory and Critical Care Medicine. The study involved 167 volunteers and investigated the efficacy of measuring mesothelin in pleural fluids as a new method for mesothelioma diagnosis.
The study, like others of its kind, was promising but not practically applicable. The best method of early diagnosis still lies in patients discussing any history they have in the asbestos industry with their doctors, and completing a biopsy as soon as possible.

Recent Blue Cross Broad Street run in Philadelphia included members of one family on a singular mission: to carry out last wishes of their late father, James “Chalk” Thompson. Following their run, family and friends of Thompson’s gathered for a benefit to raise funds for Mesothelioma Applied Research Foundation.
Thompson died after battling mesothelioma cancer, a rare and aggressive disease affecting about three thousand Americans each year. Thompson wanted desperately to spread the word about his disease in hopes of raising awareness about mesothelioma and its cause, asbestos. Sadly, due to his condition, he was unable to undertake such a task. His family and friends, though, are stepping up in his memory. Thompson’s daughter-in-law, Leslie Gabor, said, “He said, ‘Promise me that you will get awareness out there and . . . tell people the signs so they can get checked.’”
Mesothelioma is a harsh disease, mercilessly attacking healthy tissues protecting lungs, heart and other abdominal cavities. Characterized by a latency period spanning several decades, mesothelioma is difficult to diagnose until its final and most aggressive stage. Following diagnosis, mesothelioma life expectancy is short and grim.
Thompson believed his mesothelioma was a result of his fifty-three years as an insulator in and around Philadelphia, Pennsylvania. Thompson’s son, Jim, said of his late father, “He wanted to go to the union halls . . . and try to get the word out to other guys about getting checked, but it didn’t work out that way, so now we’re picking up the pieces and trying to run with it. Hopefully, anybody that wants to know will be able to get information. If we could save one person from going through what my father went through, it would be well worth it.”

Why Can’t Mesothelioma Victims Sue the Government?

There have been a couple of posts about individual dockworkers in the United Kingdom suing their own national government for compensation. The basis for this compensation is that business of unloading ships is, in the UK, a government enterprise. This then raises the question of why mesothelioma victims who were exposed to asbestos through some governmental enterprise (e.g., building ships for the US Navy) in this country not able to sue their government? The reason is an ancient doctrine, known as “sovereign immunity” that came to this country’s legal system from its ancestor, ironically, the UK.
The theory behind sovereign immunity is that the sovereign, or government, be it King, President, etc, cannot be sued it its own courts. The idea is that the courts come from the government and that the government therefore cannot be a defendant in a court system that owes its very existence to that same government. Early on, this proved to be unworkable; no one with any sense would make a contract to be performed in the future or loan money to an entity that could not be sued in court. The solution was to pass statutes that allowed the government to be sued in specific circumstances, like for breach of contract or failure to repay debts (governments do a lot of borrowing, which you may have noticed recently). If, however, you were injured by the negligence (in other words, a tort) of the government and its personnel, you were still out of luck. In the late 1930′s while the newly activist federal government was busy fixing the depression and preparing for WWII, they addressed the problem by passing something known as the Federal Tort Claims Act, or FTCA. The catalyst for finally passing the FTCA in 1945 was the crash of the B-25 bomber, lost in the fog, into the side of the Empire State Building.
In finally establishing tort liability against the federal government, the drafters of the FTCA had to keep in mind the reason for sovereign immunity in the first place–governments do a lot of stupid things (start wars, fail to repair levees, etc.) that hurt people, and if folks were able to sue in court for all that stuff it would be totally impractical–the government would be bankrupt and couldn’t make sound decisions. It is, after all, the ultimate responsibility of government to strike a balance between competing interests–someone usually benefits and someone else gets hurt. On the other hand, if an IRS agent, in the performance of his or her official duties, goes through a red light and hits your car, Uncle Sam should have to pick up the tab as the employer of the lousy driver.
The solution was arrived at by dividing all government actions into two categories–discretionary and ministerial. This is sometimes referred to as the “discretionary function” test. A discretionary function would be, for example, the decision by the US Air Force to haul jet fuel on public highways in tank trucks. The ministerial function would be doing the actual hauling. So if the Air Force personnel leaves the fill cap loose and the jet fuel leaks all over an exit ramp and causes accidents (this is a case I actually had) the air force is responsible for that. If however, it turns out that using the public highways to haul jet fuel in general is a bad idea, the air force cannot be liable for that, because it is based on a policy decision about how fuel should be hauled. In short, it is negligence in the execution, not the planning, that creates governmental tort liability.
The new FTCA got almost an immediate workout. On April 16, 1947, a ship loaded with ammonium nitrate fertilizer for the Marshall Plan spontaneously blew up in the port of Texas City, Texas, setting off a nearby ammunition ship, other ships, and fuel storage tanks on shore. Over 500 people were killed, and the harbor at Texas City itself was reduced to a smoldering ruin. The entire scheme about storing and shipping both the fertilizer and ammunition had been undertaken and supervised by the federal government, everybody consequently sued under the FTCA. They lost. The US Supreme Court ruled that even if the plaintiffs could prove that the federal planning had been really, really stupid (which it was) it all fell under the umbrella of “discretionary function” decision making, therefore the feds didn’t have to pay squat. The outcry was so great that congress passed special legislation to compensate the victims in Texas City. But congress left the FTCA intact.
When the asbestos litigation started in the late 1970s’ many of the workers who were getting asbestos cancer, asbestosis and lung cancer had been exposed doing work that involved the federal government. The were suing companies like Johns-Manville, Raybestos, Eagle Picher and Owens-Illinois that had supplied asbestos-containing thermal insulation products under federal contracts and to federal specifications. These companies tried to sue the feds under FTCA, and they lost for the same reason that the good people in Texas City lost–the decision to use asbestos is a discretionary one. The CEOs of GM, Ford and Chrysler, along with the President of the UAW, all came to Washington to let everybody know how incredibly broke they are. In classic Marie Antoinette fashion, each of the three CEOs flew to Washington on a separate corporate jet, which probably burned enough fuel to get a Lincoln Navigator to Jupiter and back (or something like that). Nice going, guys. The head honcho of the UAW displayed his own flair for public relations by announcing that there would be absolutely no concessions on the part of his membership to help these companies get out of the quicksand they’re in. To be fair, the UAW membership and retirees have made a lot of concessions over the last few years, but most of the public doesn’t know that, so it still probably wasn’t the brightest thing to say at the time.
The basic nut is that GM and Chrysler will tank really soon if something doesn’t go their way. Ford is in better shape (if you follow this stuff, you may recall that a year ago everyone thought Ford was in the worst shape of all), but that is kind of like saying that a guy who has been hit over the head with a pipe wrench is in better shape than someone who has been hit with a sledgehammer—it is a matter of degree that is meaningless in the long term.
A large number of the mesothelioma cases pending in the courts have GM, Ford and Chrysler (and sometimes import companies too) named as defendants. This is because many red-blooded Americans of a certain age range worked on their own cars, and those cars had asbestos brake and clutch components, as well as asbestos containing engine gaskets. In the case of a career mechanic who gets mesothelioma, the car companies will be the primary defendants.
If the big three go under, this will all, naturally, stop. Bankruptcy is not a good option because of the complexity of all the relationships these companies have with workers, creditors, retirees, suppliers, dealers et cetera, all of whom have legally enforceable interest (not to mention asbestos victims). It would take the better part of a decade or longer for these companies to come out of bankruptcy, if they ever came out at all.

Mesothelioma is a rare form of asbestos cancer that develops in the mesothelial cells. These cells form tissue that surround or line the lungs, heart, and stomach. The mesothelial cells protect these organs by producing a lubricating fluid that allows them to move smoothly.

Tumors can be benign (non-cancerous) or malignant (cancerous). Much of the information provided in this site will address the topics and treatments around malignant mesothelioma. Symptoms are often typical of other less serious diseases and include coughing, fever, and shortness of breath.

Malignant mesothelioma, referred to as simply mesothelioma, has three main cell types:

Epithelioid

Epithelioid mesothelioma, the most common type, accounts for 50% to 70% of the cancer cells. Sarcomatoid mesothelioma, the least common type, accounts for 7% to 20% of the cells. Biphasic, or a mixture of epithelioid and sarcomatoid, account for 20% to 35% of the cells.

The types of cells do not impact the prognosis or treatment of mesothelioma. The symptoms, however, can delay an appropriate diagnosis since they are often attributed to other common health issues.

Causes of Mesothelioma

Mesothelioma is caused primarily by exposure to asbestos. Typically the asbestos exposure occurred in an profession or work environmentover a prolonged period of time. However, there have been cases where less exposure resulted in mesothelioma, including exposure to asbestos particles from washing clothes.

The Most Common Form of Mesothelioma

The majority of people who develop mesothelioma have worked in professions where they inhaled asbestos particles. This means mesothelioma that forms in the lungs, or pleural mesothelioma, is the most common form.

Other Forms of Mesothelioma

Mesothelioma in the abdomen, called peritoneal mesothelioma, is not as common. Pericardial mesothelioma, which starts in the area around the heart, is very rare, as is mesothelioma that affects the covering of the testicles.
Malignant mesothelioma should not be confused with an adenomatoid tumor. These are benign tumors that develop in certain female and male reproductive organs and begin in the mesothelium.

Asbestos is the overwhelming cause of mesothelioma in the United States. Typically the asbestos exposure occurred in an occupation or work environment over a prolonged period of time. However, it is also well-established that bystander (working near others using asbestos products) and household exposures (exposure from family members and their work clothes in the home) are known causes of mesothelioma. There is no known safe level of exposure to asbestos.

Types of Mesothelioma

There are three types of mesothelioma: Pleural Mesothelioma, Peritoneal Mesothelioma, and Pericardial Mesothelioma.

Pleural Mesothelioma accounts for 75% of all diagnoses.
Peritoneal Mesothelioma accounts for less than 20% of all mesothelioma cases.
Pericardial Mesothelioma is the most rare and accounts for very few cases.

Risk Factors

A risk factor is anything that increases your chance of getting a disease. While you may have one or many of the risk factors associated with a disease, such as mesothelioma, that does not mean that you will get the disease.

The main risk factor for mesothelioma is asbestos exposure. Prolonged exposure to asbestos through a job, or during a building demolition or remodel increases your chances. Other recognized, but very rare, known causes include exposure to zeolites (minerals primarily located in Turkey), therapeutic thoratrast radiation, and chronic inflammation of the pleura. However, there have been very few reported cases of mesothelioma from causes other than asbestos.

Symptoms of Mesothelioma

Mesothelioma symptoms include but are definitely not limited to:

shortness of breath
chronic cough,
chest pain
fatigue

Often mesothelioma is misdiagnosed because the symptoms are common to other ailments, such as colds and flu.

Stages of Mesothelioma

Stages of mesothelioma were established by the Mesothelioma Interest Group and adopted by the American Joint Committee on Cancer (AJCC), and are similar to the staging system used by most other cancers. At this time, the staging system is only applied to mesothelioma around the lung, and uses Roman numerals from I to IV (1 to 4) to describe the severity of the cancer.

Key Statistics

Each year, between 2,000 and 4,000 people are diagnosed with mesothelioma. This cancer has a very long latency stage, and most often shows up late in life and primarily in men. Unfortunately, mortality is very quick after diagnosis, typically because mesothelioma is often far advanced by the time a diagnosis is made.
