Study: Once-Daily Tablet as Effective as Multiple Pill Dosings for Treating Oral Yeast Infection in HIV/AIDS Patients

DETROIT - A once-daily medication option for treating the most common mouth infection in HIV/AIDS patients has shown to be just as effective and safe as taking an anti-fungal pill five times a day, according to a Henry Ford Hospital study.

Researchers found that a small tablet applied daily that sticks to the gum and dissolves inside the mouth with few or no side effects is a novel, convenient option for treating an infection called oral candidiasis (OC), which occurs in about one third to one half of HIV patients and up to 90 percent of AIDS patients.

The study is believed to be the largest to date involving HIV/AIDS patients with OC. The infection is also common in patients suffering from many forms of cancer, especially those with head and neck cancer, in which the infection rate is as high 77 percent.

The study is being presented at the 49th annual Interscience Conference on Antimicrobial Agents and Chemotherapy in San Francisco.

"This is an exciting new, convenient way for treating this infection," says Jose A. Vazquez, M.D., the study's lead author and a Henry Ford Infectious Disease physician.

"It's a tablet that you just stick on the gum and it releases an anti-fungal agent over the course of six to eight hours. Because the anti-fungal agent stays in the mouth, it provides the same relief as the oral medication but with few or no side effects."

Dr. Vazquez says medication taken orally or by injection often is absorbed into other parts of the body, thus causing side effects for patients such as nausea, vomiting, diarrhea and liver dysfunction.

The study compared the effectiveness and safety of taking 50 milligrams of a miconazole mucoadhesive buccal tablet once-daily to 10 milligrams of clotrimazole, a common anti-fungal pill five times a day. Of the 578 patients enrolled in the randomized study, 291 received the tablet and 287 received the clotrimazole pill.

Facts about Influenza(H1N1 Virus)

According to the Center for Disease Control and Prevention (CDC) Millions of people in the United States - about 5% to 20% of U.S. residents - will get influenza each year.

Also according to the CDC, an average of about 36,000 people per year in the United States die from influenza-related causes, and more than 200,000 have to be admitted to the hospital as a result of influenza-related causes.

People age 65 years and older, people of any age with chronic medical conditions (such as asthma, diabetes, or heart disease), pregnant women, and young children are at higher risk for complications from seasonal influenza. However, for reasons we don't know yet, H1N1 has affected younger people (under 25) more so than older people. So far, people older than 64 do not appear to be at increased risk of H1N1.

Pneumonia, bronchitis, and sinus and ear infections are three examples of complications from flu.

The symptoms of H1N1 in people are similar to the symptoms of seasonal flu and include fever, cough, sore throat, body aches, headache, chills and fatigue. Some people have reported diarrhea and vomiting associated with swine flu. Severe illnesses and death has occurred as a result of illness associated with this virus.

H1N1 and seasonal flu are similar in many ways in terms of how they spread, their signs, symptoms and severity.

The CDC is testing a vaccine specific to H1N1 and is expected to be ready for public use sometime this fall. This vaccine is not intended to replace the annual seasonal flu vaccine, however. And unlike the seasonal flu vaccine, the H1N1 vaccine will be given in two doses about 30 days apart.

The CDC recommends the following groups receive the H1N1 vaccine as well as those who were previously diagnosed with H1N1:

-- Pregnant women.
-- Caregivers of children younger than 6 months of age.
-- Health care workers and Emergency Medical Service personnel.
-- People 6 months to 24 years of age.
-- People aged 25 to 64 who have a health condition associated with a higher risk of medical complications from the flu.

Nerve-Sparing Technique Proves To Be Better

Robotic Radical Cystectomy

Robotic surgery to remove the bladder for large tumors or disease. Radical cystectomy remains the standard treatment for muscle invasive bladder cancer. However laparoscopic radical cystectomy remains extremely technically challenging even for experienced laparoscopic surgeons. Following the successful application of the da Vinci robotic surgical system for radical prostatectomy, attention has now turned to the use of robot assistance for laparoscopic cystectomy. The surgery includes removal of the bladder and the removal of the prostate in men and the uterus, ovaries, cervix, and part of the vagina in women. After the removal of the bladder, a piece of the intestine is reconstructed to enable the diversion, storage, and passage of urine. Robotic Kidney Removal

Minimally-invasive surgery for kidney removal has become a new standard of care for the removal of kidney cancer. The procedure can also be performed for kidneys which no longer function. The surgery itself, known as a Robotic Radical Nephrectomy, is performed with tiny keyhole incisions.

Through these incisions, robotic surgical instruments and a camera are inserted to perform the surgery. After inflating the abdomen with gas, the colon is moved away from the kidney. The kidney normally has a covering of fat around it. This fat is left on the kidney so the cancer or tumor is not exposed.

After the kidney is dissected, the blood vessels going to the kidney are located. The kidney normally has one artery bringing blood to the kidney and one vein exiting the kidney and bringing blood back to the body. In some cases, there are more than one artery and vein. This is not considered abnormal, but it is less common. When the artery and vein are identified, they are clipped or ligated. This cuts off the blood flow to the kidney permanently.

Once the blood vessels are divided, the rest of the kidney is removed from its attachments to the liver or spleen, back muscles and the adrenal gland. In some cases, the adrenal gland is removed with the kidney. After the kidney is totally freed up, it is removed from the body through a small incision. Then the incisions are closed and the patient is taken to the recovery room.

New Technique of nerve-sparing Robotic Prostatectomy

Developed by surgeons at Henry Ford Hospital's Vattikuti Urology Institute, a new technique of nerve-sparing robotic prostatectomy called the Veil of Aphrodite results in significantly better erectile function outcomes than in those who receive conventional nerve-sparing surgery - without compromising cancer control.

Of patients with no pre-existing erectile dysfunction who underwent a robotic radical prostatectomy incorporating the new nerve-sparing technique, the Veil of Aphrodite, 97 percent achieved erections strong enough for intercourse and 85 percent reported that their erections were normal. In contrast, 74 percent of patients who underwent a standard nerve sparing procedure had erections leading to intercourse, and only 26 percent reported normal erections. Additionally, 17 percent of patients who underwent a conventional nerve sparing procedure and 51 percent of patients who underwent the Veil of Aphrodite nerve sparing procedure had normal erections without medications. This is currently our preferred procedure for removal of the prostate.A team of surgeons led by Dr. Mani Menon, performs the operation. We have performed over 4000 such operations over a 7-year period,the word's largest experience.The unique procedure combine the oncological principles of "open" radical prostatectomy with the minimum invasiveness of laparascopic surgery,Established by Dr. Menon.

With the state of the art “da Vinci” system,the surgeon uses a three – dimensional computer vision system to manipulate robotic arms.These robotic arms hold special surgical instruments that are inserted into the abdomen through tiny incisions. A laparascope- a long,thin,lighted telescope- is inserted through one incision and connected to the computer monitor that allows the surgeon to see inside the body.

Dr. Mani Menon is one of the foremost urologists in the united states (U.S). Dr. Menon (born July 09,1948), in Trichur, India, is an American surgeon and the director of Vattikuti Urology Institute at the Henry Ford. He graduated from JIPMER Pondicherry and did his residency in urology at the Brady Urological Institute at Johns Hopkins in Baltimore, Maryland, USA with Dr.Walsh,Dr. Menon developed a technique to measure androgen receptors in the human prostate, the first such measurement.Later he moved to Washington university in St. Louis, where he quickly established himself as a Surgeon-Scientist, performing complex re-constructive surgery, while establishing his own research laboratories .

The techniques involved in nerve-sparing, radical prostatectomy were being developed by Dr. Walsh when Dr. Menon was at Johns Hopkins .This enabled him to become one of the first surgeons in the world to perform this delicate operation.

At the age of 34, he became the youngest chairman of urology in the U.S.A. In 1997, Dr. Menon was recruited to Henry Ford Hospital to invigorate the prostate cancer programs there. At Henry Ford Hospital, Dr. Menon and his team developed techniques for robotic surgery in general and robotic prostatectomy . The operation was redesigned to take advantage of the features of the robot.

The robotic approach that Dr. Menon has developed, Vattikuti Institute Prostatectomy or VIP, has become the gold standard for the surgical treatment of prostate cancer .
Dr. Menon has published more than 250 papers, primarily in the field of robotic surgery, renal transplantation and urolithiasis Dr. Menon is profiled in the best doctors of America and the International Who's Who in biology and Medicine .He has been a visiting professor at various institutions not only inside the USA, but also all across the globe, where he has lectured and performed
robotic surgery .

In 2008, the president of India awarded him the B.C.Roy National award for Eminence in medicine recognizing his key role in developing the field of robotic surgery.

Dr. Menon's early interest were in reconstructive surgery for complex renal stones, while his search interests were in cell-crystal interactions as they relate to renal stones.
At Henry Ford Hospital, Dr. Menon and his team developed techniques for robotic surgery in general prostatectomy .The robotic that Menon has developed, Vattikuti Institute Prostatectomy or VIP, has become the gold standard for the surgical treatment of prostate cancer.
