Erectile dysfunction (ED) means that you cannot get a proper
erection. Most cases are due to narrowing of the arteries that take
blood to the penis. This is due to a build-up of fatty deposits
(atheroma) in these arteries in the same way that heart arteries are
affected in people with heart disease. ED is usually treatable, most
commonly by a tablet taken before sex. You may also receive lifestyle
advice and treatments to minimise your risk of heart disease.

What is erectile dysfunction?

Erectile dysfunction (ED) means that you cannot get and/or maintain
an erection. In some cases the penis becomes partly erect but not hard
enough to have sex properly. In some cases, there is no swelling or
fullness of the penis at all. ED is sometimes called impotence.

How common is erectile dysfunction?

Most men have occasional times when they cannot get an erection. For
example, you may not get an erection so easily if you are tired,
stressed, distracted, or have drunk too much alcohol. For most men it is
only temporary, and an erection occurs most times when you are sexually
aroused.
However, some men have persistent, or recurring, ED. It can occur at
any age, but becomes more common with increasing age. About half of men
between the ages of 40 and 70 have ED. About 7 in 10 men aged 70 and
above have ED.

How does an erection normally occur?

When you are sexually aroused, messages from your brain travel down
nerves to your penis. Chemicals called neurotransmitters are then
released from the ends of the nerves in the penis. Stimulation of the
penis can also cause local nerve endings to release neurotransmitter
chemicals.
The neurotransmitters which are released in the penis cause another
chemical to be made, called cyclic guanosine monophosphate (cGMP). This
chemical causes the arteries in the penis to widen (dilate). This allows
extra blood to flood into the penis. The rapid inflow of blood causes
the penis to swell into an erection. The swollen inner part of the penis
also presses on the veins nearer to the skin surface of the penis.
These veins normally drain the penis of blood. So, the flow of blood out
of the penis is also restricted, which enhances the erection.
Once you stop having sex, the level of cGMP falls, the blood flow to
the penis returns to normal, and the penis gradually returns to the
non-erect state.

What causes erectile dysfunction?

There are several causes which tend to be grouped into those that are
mainly physical and those that are mainly due to mental health
(psychological).

Physical causes

About 8 in 10 cases of ED are due to a physical cause. Causes include:

Reduced blood flow to the penis. This is, by far,
the most common cause of ED in men over the age of 40. Like in other
parts of the body, the arteries which take blood to the penis can become
narrowed. The blood flow may then not be enough to cause an erection.
Risk factors can increase your chance of narrowing of the arteries.
These include getting older; high blood pressure; high cholesterol; smoking.

Diseases which affect the nerves going to the penis. For example, multiple sclerosis, a stroke, Parkinson's disease, etc.

Diabetes. This is one of the most common causes of ED. Diabetes can affect blood vessels and nerves.

Hormonal causes. For example, a lack of a hormone
called testosterone which is made in the testes. This is uncommon.
However, one cause of a lack of testosterone that is worth highlighting
is a previous head injury. A head injury can sometimes affect the
function of the pituitary gland in the brain. The pituitary gland makes a
hormone that stimulates the testis to make testosterone. So, although
it may not at first seem connected, a previous head injury can in fact
lead to ED. Other symptoms of a low testosterone level include a reduced
sex drive (libido) and changes in mood.

Injury to the nerves going to the penis. For example, spinal injury, following surgery to nearby structures, fractured pelvis, radiotherapy to the genital area, etc.

Side-effect of certain medicines. The most common
are some antidepressants; beta-blockers such as propranolol, atenolol,
etc; some 'water tablets' (diuretics); cimetidine. Many other less
commonly used tablets sometimes cause ED.

Alcohol and drug abuse.

Cycling. ED after long-distance cycling is thought
to be common. It is probably due to pressure on the nerves going to the
penis, from sitting on the saddle for long periods. This may affect the
function of the nerve after the ride.

Excessive outflow of blood from the penis through the veins (venous leak). This is rare but can be caused by various conditions of the penis.

In most cases due to physical causes (apart from injury or after
surgery), the ED tends to develop slowly. So, you may have intermittent
or partial ED for a while, which may gradually get worse. If the ED is
due to a physical cause, you are likely still to have a normal sex drive
apart from if the cause is due to a hormone problem. In some cases, ED
causes poor self-esteem, anxiety, and even depression. These reactions
to ED can make the problem worse.

Mental heath (psychological) causes

Various mental health conditions may cause you to develop ED. They include:

Stress - for example, due to a difficult work or home situation.

Anxiety.

Relationship difficulties.

Depression.

Typically, the ED develops quite suddenly if it is a symptom of a
mental health problem. The ED may resolve when your mental state
improves - for example, if your anxiety or depression eases. However,
some people become even more anxious or depressed when they develop ED.
They do not realise it is a reaction to their mental health problem.
This can make matters worse and lead to a vicious circle of worsening
anxiety and persisting ED.
As a rule, a psychological cause for the ED is more likely than a
physical cause if there are times when you can get a good erection, even
though most of the time you cannot. (For example, if you can get an
erection by masturbating, or wake up in the morning with an erection.)

Erectile dysfunction, heart disease and other vascular diseases

Although ED can be caused by various conditions, about 7 in 10 cases
are due to narrowing of the small arteries in the penis. This causes
reduced blood flow to the penis. This is the same problem that can occur
in other blood vessels. For example, narrowing of the blood vessels in
the heart (the coronary arteries) is a cause of angina
and other heart problems. Narrowing of blood vessels to the brain is a
risk factor for having a stroke. Narrowing of blood vessels in the legs
can cause peripheral arterial disease.
The root cause of the narrowing of the arteries is caused by atheroma.
Atheroma is like fatty patches or plaques that develop within the
inside lining of arteries. (This is similar to water pipes that get
furred up with scale.) Plaques of atheroma may gradually form over a
number of years in one or more places in the body, commonly in arteries
going to the heart, brain, legs and penis. In time, these can become
bigger and cause enough narrowing of one or more of the arteries to
cause symptoms and problems.
Certain risk factors increase the risk of more atheroma forming which
can make atheroma-related conditions worse. These are discussed in more
detail in a separate leaflet called Preventing Cardiovascular Diseases. Briefly, risk factors that can be modified and may help to prevent atheroma-related conditions from getting worse are:

Smoking. Smoking is one of the biggest risk factors
for developing an atheroma-related condition. Smoking roughly doubles
your chance of developing ED. Young smokers may not be aware that they
have a much greater risk of developing ED by middle age compared with
non-smokers. If you smoke, make every effort to stop.

High blood pressure. Make sure your blood pressure is checked at least once a year. If it is high, it can be treated.

If you are overweight, losing some weight is advised.

A high cholesterol. This can be treated if it is high.

Inactivity. We should all aim to do some moderate
physical activity on most days of the week for at least 30 minutes. For
example, brisk walking, swimming, cycling, dancing, gardening, etc.

Diet. Aim to eat a healthy diet. See separate leaflet called Healthy Eating.

Alcohol excess.

Diabetes. If you have diabetes, good control of the
blood sugar level and blood pressure can help to minimise the impact of
diabetes on the blood vessels.

Note: ED caused by narrowed arteries commonly
develops several years before any symptoms or problems develop due to
narrowing of the heart (coronary) arteries (or other arteries).
Therefore, ED is thought of as an 'early warning signal' that heart
disease (or other cardiovascular diseases) may develop. A large
international study found that men with ED are twice as likely to have a
heart attack, 10% more likely to have a stroke and 20% more likely to
be admitted to hospital with heart failure. This is why some of the
tests listed below may be advised if you develop ED. Also, you are
likely to be advised by your doctor on how to modify any risk factors
listed above. The aim is to prevent heart disease and other
cardiovascular disease from developing.

What should I do if I develop persistent erectile dysfunction?

It is best to see your GP. He or she is likely to discuss the
problem, go over any medication you may be taking, and do a physical
examination. This can help to identify or rule out possible underlying
causes. Before treatment, your GP may suggest some tests.

What tests may be done?

Depending on your symptoms, likely cause of the ED, age, etc, your
doctor may suggest that you have some tests. These are mainly to check
up on any risk factors listed above which increase the risk of
developing narrowing of the arteries. Tests may include:

A blood test to check the level of cholesterol and other fats (lipids).

Blood sugar level.

Blood tests to rule out kidney and liver disease.

A urine test.

A check of your blood pressure.

A heart tracing (ECG).

Other heart tests which are sometimes done, where appropriate, if heart disease is suspected.

If you have a low sex drive, or if you have had a previous head
injury, ED may be due to a hormonal problem. In this situation a blood
test to check the level of the hormone testosterone (and sometimes
prolactin) may be advised.
In a small number of cases, tests to check the way the blood
circulates through the penis, using scans or drug injections, may be
done in hospital clinics.

What are the treatment options?

A referral to a specialist is sometimes needed for assessment and
treatment. However, GPs are now treating more cases of ED than
previously, as the treatment options have improved in recent years. Your
GP is more likely to suggest referral if you have been found to have an
hormonal problem, if the cause is related to another condition like
circulatory problems, or if you are young and the condition started
after an injury.
The following gives a brief summary of treatment options. There is a
good chance of success with treatment. There are pros and cons of each
treatment, and your doctor will advise further.

Have you considered your other medication?

As mentioned, some medicines can cause ED. Check the leaflet that
comes with any medication that you take to see if ED is a possible
side-effect. Do not stop any prescribed medication, but see your doctor
if you suspect this as the cause. A switch to a different medicine may
be possible, depending on what the medicine is for.

Medication (tablets taken by mouth)

In 1998, the first tablet to treat ED was launched. This made a huge
impact on the treatment of ED. For treating ED, tere are now four
different tablets licensed in the UK:

Three of the tablets work by increasing the blood flow to your
penis. They do this by affecting cGMP, the chemical involved in widening
(dilating) the blood vessels when you are sexually aroused (described
above). They are sildenafil (Viagra®), tadalafil (Cialis®), and vardenafil (Levitra®).
You take a dose before you plan to have sex. Because of the way they
work, these medicines are called phosphodiesterase type 5 (PDE5)
inhibitors.

So, even if the nerves or blood vessels going to your penis are not
working so well, a tablet can cause the blood flow to increase in your
penis, and cause an erection. Tablets can treat ED caused by various
underlying conditions.
Note: none of these tablets will cause an erection unless you are sexually aroused.
There is a good chance that a medicine will work (about an 8 in 10
chance of it working well). However, they do not work in every case.
There are pros and cons for each of the above, and your doctor will
advise. For example, you may not be able to take certain tablets for ED
if you have certain other medical conditions or take certain other
medicines. For example, you should not take a PDE5 inhibitor if you take
nitrate medicines (including glyceryl trinitrate (GTN)) which are often
used to treat angina. Also, if you take a certain type of medication,
called alpha-blockers,
for high blood pressure (doxazosin, indoramin, terazosin or prazosin)
you need to wait until you are on a stable dose, as the combination with
a PDE5 inhibitor can cause a sudden pressure drop.

Injection treatment

This was the most common treatment before tablets became available.
It usually works very well. You are taught how to inject a medicine into
the base of the penis. This causes increased blood flow, and an
erection usually develops within 15 minutes. (Unlike with tablets, the
erection occurs whether of not you are sexually aroused.)

Urethral medication

You can place a small pellet into the end of the urethra (the tube
which passes urine and opens at the end of the penis). The pellet
contains a similar medicine to that used for the injection treatment.
The medicine is quickly absorbed into the penis to cause an erection,
usually within 10-15 minutes.

Vacuum devices

There are several different devices. Basically, you put your penis
into a plastic container. A pump then sucks out the air from the
container to create a vacuum. This causes blood to be drawn into the
penis and cause an erection. When erect, a rubber band is placed at the
base of the penis to maintain the erection. The plastic container is
then taken off the penis and the penis remains erect until the rubber
band is removed (which must be removed within 30 minutes).

Penile prosthesis

A surgeon can insert a rod permanently into the penis. The most
sophisticated (expensive) type can be inflated with an inbuilt pump to
cause an erection. The more basic type keeps the penis rigid all the
time.

Other treatments

Treating an underlying cause

For example, treating depression, anxiety, changing medication,
cutting back on drinking lots of alcohol, or treating certain hormonal
conditions may cure the associated ED.

Lifestyle and other advice

As mentioned above, ED is often a marker that heart disease or other
cardiovascular diseases may soon develop. Therefore, you should review
your lifestyle to see if any changes can be made to minimise the risk of
developing these problems. For example, stop smoking if you are a smoker, take regular exercise, eat a healthy diet, etc.
Also, your doctor may prescribe a statin medicine to lower your blood cholesterol level if your risk of developing cardiovascular disease is high.

Counselling

Sometimes couple counselling, or sex therapy is useful. These are
most useful if certain mental health (psychological) problems are the
cause of, or the result of, ED.
In some cases, sex therapy is used in addition to another treatment option.

Treatment for erectile dysfunction on the NHS

The Department of Health states: "From 1 July 1999 only those
patients suffering from one of the specified medical conditions are
eligible to receive drug treatments for impotence on the NHS. Other men
can receive a private prescription from their own GP."
The specified medical conditions are diabetes, multiple sclerosis,
Parkinson's disease, poliomyelitis, prostate cancer, an operation to
remove the prostate gland (prostatectomy), radical pelvic surgery, renal
failure treated by dialysis or transplant, severe pelvic injury, single
gene neurological disease, spinal cord injury and spina bifida.
A specialist can also prescribe drug treatments on the NHS if your
condition causes severe distress in your life. Examples would be
relationship breakdown, social problems or effects on mood or behaviour.
Therefore, unless you have one of the above conditions or get
endorsement from a consultant, you have to pay the full cost of any
tablets prescribed to treat ED. This policy may be reviewed by the
Department of Health in the future.

How and why does it work you may ask?

Marijuana is known for its medicinal effects of calming an individual. This will help prevent an anxious premature ejaculator from ejaculating too quickly. There may be some side effects though. Some users of marijuana have reported that at times they became impotent (unable to get an erection) after smoking marijuana.

Some people are against marijuana as a premature ejaculation aid. However, if you do not mind the drug and you have legal access to marijuana, you may want to give it a shot.

What are premature ejaculation condoms?

They are basically condoms which are especially manufactured for men who ejaculate prematurely.

How do they help to prevent PE?

There are different ways how these condoms help to delay ejaculation.
Some condoms are simply just thicker, thus reducing the friction and
other sensations you have when you have intercourse. Other condoms
actually contain a chemical which temporarily makes the skin of your
penis a bit numb.

What chemicals are used to numb the penis?

They often contain benzocaine
on the inside of the condom. This is a local anesthetic that is often
used to relieve pain. There are possible side effects when using
benzocaine on your skin, among them allergic reactions (itching, hives,
swelling, tingling sensations, headache, etc.). In rare cases benzocaine
can also cause methemoglobinemia, which is a serious blood disease.

Is the numbing effect immediate?

Now when the numbing of sensations is simply caused by a kind of
thicker rubber, then yes, of course. But if benzocaine is used to reduce
sensations, then it usually takes 1-2 minutes until the numbing effect
really sets in after putting on the condom.

Can I just use two condoms on top of each other?

You surely can do that, but I advise you against doing that. One
issue is that when you wear two condoms on top of each other, it creates
additional friction (rubber with rubber) which can cause your condoms
to tear or break.
There’s a famous sex study by Dr. Alexa Albert about condom use of
commercial sex workers, which kind of makes a different point, however, I
do advice against this practice.

Which PE condoms shouldn’t you use?

This is just my personal opinion, but I do not recommend the Trojan Extended Pleasure
Condoms because the chemical that is used to numb the penis can also
numb the vagina of your sex partner (causing her to feel less excited),
because the chemical is not just on the inside of the condom, but
everywhere.

What’s the best PE condom?

Among the best delay condoms are Durex Performax Condoms, which are made from natural rubber latex and are electronically tested.

Topical anesthetic agents seem to be effective and are generally well tolerated for patients with premature ejaculation (PE), according to a review published in Urology (2013;81:799-804).
Chunxiao Pu, MD, from Sichuan University in China, and colleagues
conducted a systematic literature review and meta-analysis including
data from eight eligible randomized controlled trials to assess the
efficacy of topical anesthetic agents on intravaginal ejaculatory
latency time (IELT).

In the pooled analysis, the researchers found that IELT was
significantly improved in the topical anesthetic agent group versus the
placebo group. In subgroup analyses, significant improvements were
observed in the domains of ejaculatory control, sexual satisfaction, and
distress in the Index of Premature Ejaculation questionnaire. Compared
with the placebo group, the topical anesthesia group experienced a
significant 4.2 times greater overall incidence of adverse events,
although almost all were mild and transient.
"Considering these results, on-demand topical anesthetic agents could
be considered a treatment option for patients with lifelong PE," the
investigator stated. "Large, multicenter, prospective randomized
controlled trials should be anticipated in the future to confirm the
safety and efficacy of topical anesthetic agents."

The foreskin (prepuce) has been described as a simple fold of skin, with
its structures and functions ignored. The foreskin actually is a
complex organ, an integral part of the penis, with many important
functions.

The foreskin protects the glans and the urinary meatus (opening) and performs many other integrated functions.

The
immunological functions of the foreskin help protect the body from
pathogens. The sphincter action of the preputial orifice prevents entry
of infectious contaminants. Glands secrete lysozyme, an enzyme that
breaks down cell walls of pathogens. The sub-preputial moisture
lubricates and protects the mucosal lining of the glans and inner
foreskin. The high vascularization of the foreskin brings cells to fight
infection, while Langerhans cells secrete cytokines, proteins that
regulate the intensity and duration of immune responses, and langerin, a
substance that provides a barrier to HIV infection.

The
foreskin provides the skin necessary to accommodate a full erection.
Even when erect, the foreskin has a gliding action that provides
additional sexual pleasure to the man and his partner. The gliding
mechanism of the foreskin facilitates insertion and reduces friction and
chafing during sex. The foreskin also provides a seal, keeping the
secretions necessary for comfortable intercourse contained within the
vagina.

The foreskin contains 20,000–70,000 erogenous nerve
endings, the majority of which are concentrated in the ridged band,
which encircles the inner opening of the foreskin. When the penis is
flaccid, the nerve endings are protected but, when erect, they are
exposed. Circumcision removes about three-fourths of the nerve endings
in the penis, leaving the circumcised penis severely disabled by
comparison.

These nerve endings allow a man to modulate his
sexual experience. Without them, a man may feel pleasure, but, without
warning, reaches ejaculation quickly. The greatest complaint of
circumcised males is premature ejaculation.

Without the
foreskin’s protection, the glans becomes keratinized (calloused). The
free nerve endings in the glans become buried in layers of callused
skin. Many circumcised males complain about sexual dysfunction and
impotence because they lack the feelings and feedback meant to be
provided by nerve endings that were amputated with circumcision.
Circumcised men are 4.5 times more likely to be diagnosed with erectile
dysfunction than intact men.

The foreskin plays an important role in protection, sexual function, and sexual pleasure.

Research Source: The Cochrane Library

KEY POINTS

Little evidence supports behavioral therapy as an effective intervention
for premature ejaculation, according to a new Cochrane review.

Behavioral therapy for premature ejaculation can include different
positions, better control of movements, more mental focus, a start-stop
approach and some medications.

Therapy for premature
ejaculation can cost $100 or more a session and eight sessions may be
necessary over the course of several months.

A
new review finds little reliable research to support treating premature
ejaculation by teaching men how to control their bodies with their
minds. Still, two of the review authors say it is too early to discount
so-called behavioral therapy.

“We need to do more to prove it
works,” said review co-author Stanley Althof, executive director of the
Center for Marital and Sexual Health of South Florida.

Physicians
and mental health specialists are not certain of how many men suffer
from premature ejaculation, although they have managed to come up with a
definition of it as ejaculation that occurs within a minute or less of
the penis entering the vagina.

By this strict definition, around 2
percent to 5 percent of men suffer from premature ejaculation, Althof
said. In addition, many more who fail to meet the definition still think
they do not spend enough time inside the vagina, he said.

“If
you ask men themselves if they have premature ejaculation, you’re likely
to get 20 to 30 percent who say they have it,” Althof said.

The
cause of premature ejaculation is unknown, but researchers have linked
it to a variety of conditions, which include anxiety, a malfunctioning
ejaculation reflex, prostate disease and chemical problems in the brain.
Genetics could also have something to do with it.

In some cases,
physicians prescribe antidepressants like Paxil, Zoloft and others.
“These medications are a simple treatment and cost effective if not used
for a lifetime,” said lead review author Tamara Melnik, professor of
internal medicine and evidence-based medicine at the Universidade
Federal de São Paulo in Brazil.

It turns out that one of the side
effects of some antidepressants - delayed orgasm - might actually help
men who suffer from premature ejaculation. Unfortunately, as Melnik
notes, the medications often come with other side effects that are not
so benign.

Topical anesthetics, like lidocaine, that numb the
penis are another option, said Dr. Ege Serefoglu, a urologist at
Kiziltepe State Hospital, in Turkey. However, some men do not like the
treatment because they must apply the cream 15 minutes before
intercourse and use a condom, said Serefoglu, who has no affiliation
with the study.

Enter behavioral therapy, which aims to treat
premature ejaculation either with or without the assistance of
medication. The therapy discourages men from trying to distract
themselves during sex by thinking about things like baseball scores or
stock prices, said Althof, an emeritus professor at Case Western Reserve
University School of Medicine.

Instead, he said, “It’s like
looking at a speedometer. We teach men to hover in the midrange of
excitement and learn to slow down or speed up when they notice where
they are.”

But does it work? The review authors searched for
evidence and found four studies that met their criteria with a total 253
participants.

The review was published by The Cochrane
Collaboration, an international organization that evaluates medical
research. Systematic reviews like this one draw evidence-based
conclusions about medical practice after considering both the content
and quality of existing medical trials on a topic.

In one study
in this review, some participants learned how to use several techniques
- including different positions, better control of their movements, more
mental focus and a start-stop approach - to delay ejaculation. Compared
to other men who did not learn about the techniques, these men took
longer to reach orgasm and both they and their female partners reported
greater sexual satisfaction.

Another study found that men did
better with the antidepressant Celexa compared to therapy only, and when
they got medication and therapy instead of just the medication. A third
study found that men did better when they took Thorazine, a drug used
to treat schizophrenia, and underwent therapy than those who just took
the medication. Full statistics from the fourth study were unavailable.

The
review authors say that the studies do not confirm that the therapy
works. The number of studies is small, their techniques are similar and
they do not look at other therapy-based approaches, the review notes.

Still,
co-author Althof said therapy has value even if the research is not yet
convincing. Treatments are not cheap, however. He said they can cost
$100 to $125 a session or more and that eight sessions could be
necessary over two to three months.

“It’s an investment,” he
said. “You’re investing in significantly improving your sexual life.
There’s not a guarantee that you will, but there’s a likelihood that you
will.”

Serefoglu, the Turkish urologist, said men who feel
distress from premature ejaculation - some men do not - should undergo a
medical examination.

In some cases, a patient might wish to try behavioral therapy, Serefoglu said, but for now it’s not proven to work.
An experimental approach in which penile nerve tissue is frozen to knock out overactive nerves helped men with premature ejaculation last three times longer.

Researchers tested the
technique on 24 men who hadn't been helped by standard treatments. They
lasted an average of 110 seconds before ejaculation, or nearly two
minutes, over the three months they were followed, compared with 36
seconds before treatment.

"That's about what you see with standard drug therapy," says
researcher J. David Prologo, MD, assistant professor of interventional
radiology at Case Western Reserve University’s School of Medicine in
Cleveland. A time to ejaculation of more than two minutes is normal, he
tells WebMD.

Its supporters say the technique, or a similar one involving heat
therapy, could someday become a standard treatment for the condition.

But other experts tell WebMD that questions about the long-term
consequences remain. Also unknown: whether men would opt for the
treatment.

The findings were presented here at the annual meeting of the
Radiological Society of North America. Prologo consults for Galil
Medical, which funded the study.

You’re not alone. Did
you know that approximately 1 in every 3 men suffer from Premature
Ejaculation? If you do the math, our world’s population 6,602,224,175
(July 2007 est.) divided by 3 equates to approximately 2,200,000,000
(rounding down of course). Then you get that number and divide it by
half (estimating that half of our world’s population are males) and you
get 1,100,000,000. It is likely that one of your buddies also suffer
from PE as well. However, since premature ejaculation is a sensitive
subject subject for males, it is unlikely for men to discuss such
matters with their circle of friends.

There’s actually muscles down there. For
those of you who work out on a continual basis, you may know that
without consistency, your muscles will be underdeveloped. Well, that
obviously means the opposite is valid–working out on a continual basis
will yield stronger muscles. So, the concept for curing Premature
Ejaculation is quite logical–strengthen the muscle (Pubococcygeus Muscle
many time referred to as PC Muscle) down there and wah-lah–bye
bye premature ejaculation. However, the problem is not about
understanding the concept of the cure, but rather performing the
necessary actions to apply the concept.

Patience is a virtue.
Remember when you were a kid and your parents used to tell you about the
race between the hare and the tortoise? Well, the important thing is to
remember the moral of that classic fable–no matter how severely
disadvantaged one is, with perseverance one still succeeds. So, when you
guys are on your journey down that long dirt road, remember your
diligence will pay off!

Sleep is crucial. Lack
of sleep leads to low serotonin levels in the brain. When serotonin
levels are low in the brain, the lower levels trigger your body to
ejaculate more rapidly. Get a good night’s rest and make it a routine so
you can get rid of your premature ejaculation once and for all!

One of the scientists
responsible for the invention of Viagra has done it once again and has
created a revolutionary treatment for those who suffer from premature
ejaculation (PE). The treatment, named Tempe, is simply sprayed on in
order to prolong the time to ejaculation and results have been extremely
positive.

The inventor himself thinks this will become the next big
thing. There are only two treatments available in the UK for the
treatment of PE, Priligy and Emla. One is expensive and the other one
can produce a numbing sensation so can defeat the purpose.

Tempe is said to help men last
three times longer than they normally would and is inexpensive compared
with the existing effective treatments. The treatment contains low dose
anaesthetics and starts working within 5 minutes.

The pharmaceutical company responsible for the drug, Plethora Solutions, is feeling positive about gaining European approval and estimate that the treatment will be available in 2013.

While premature ejaculation
can cause many problems within a couple’s sex life, it’s nothing a
mature, loving relationship can’t handle. If you are close enough to
discuss the issue openly and honestly, you may have already explored
various ways to make sex more enjoyable. On the other hand, if you are
new to premature ejaculation, you may be wondering if a better sex life
is even possible.

Many
experts suggest concentrating on the moment. The man must ensure that
he is paying attention to the situation before him, and not trying to
distract himself. It is common for men to force their minds to wander,
in the hopes that it will cause them to last longer, but the opposite
appears to be true. It now appears as though outside thoughts will also
cause the man to lose even more control of the moment.

Men
who experience premature ejaculation are often encouraged to pay close
attention to “that” moment, where their body is suddenly nearing the
finish line. It is believed that by learning to recognize this moment
as soon as it appears, a man can learn to better control himself over
time. If the process is successful, he will be able to endure
increasingly longer stints of intercourse.

Premature
ejaculation also responds to relaxation in some instances. To try this
technique, men are encouraged to enjoy a variety of sexual
stimulation, instead of concentrating solely on the penis. He should
try to enjoy each moment, breathing slowly and deeply to prevent his
body from assuming control, and attempt to find a state of relaxation
similar to that of a soothing massage. This way, he teaches his body to
better handle the stimulation without charging through to the end.

Experimentation
through masturbation is another way to approach the issue. Sometimes,
when a man takes the time to become familiar with his limits, it can
help him fight the occurrence of premature ejaculation. While you may
be comfortable masturbating in front of your partner, this is an
experience best performed alone, since the solitude will allow you to
place your entire focus on the task at hand.

Men who experience premature ejaculation should also refrain from using drugs
and alcohol, which can aggravate the condition. In fact, many men who
have no endurance problems struggle to stay erect when they have been

ability to keep control, making it near-impossible to overcome an

We’ve long heard of premature ejaculation in men as a significant
sexual dysfunction that many men experience. Premature ejaculation is
when the man orgasms before he intends or wants to (for instance, long
before the woman has had a chance to get close to her own orgasm).

Researchers from Portugal wondered if some women experience something
similar, since nobody seems to much talk about this issue in female
sexuality. Could there be such a thing as a premature female orgasm (or
in scientific terms, “female premature orgasm”)? If so, how prevalent is
the problem?

Here’s what they discovered.

The researchers were intrigued by the fact that very little has been
written or noted about women who experience a premature female orgasm.
Yet in a 2005 University of Chicago study (Sadock, 2005), nearly 10
percent of the women felt they had reached orgasm too quickly (e.g.,
before they had intended).
The study’s sample consisted of 510 Portuguese women between the ages
of 18 and 45 with medium to high education levels. Subjects filled out a
customized questionnaire designed for this study, focusing on questions
of premature orgasm, feelings of loss of control with the orgasm,
relationship difficulties and distress surrounding their orgasm. Fifty
percent of the sample were single at the time of the study, while 40
percent were married.

Nearly 17 percent of the sample said that premature orgasm occurred
often or always, and nearly 14 percent complained about a feeling of a
lack of control over the timing of their orgasm. However, a significant
minority — 41 percent — said that premature orgasm only occurred
occasionally or rarely, and 44 percent felt no lack of control over the
timing of their orgasm.

According to the researcher’s criteria, only 3.3 percent of the
subjects met the full proposed criteria for premature female orgasm. But
41 percent of women have occasional or episodic premature orgasms —
once in awhile. And 14 percent of women fall somewhere in-between these
two groups.
Premature female orgasm isn’t just a theory. It is a concern that
affects a majority of women at one point or another in their lives,
although only a small percentage of women experience it in the extreme,
full-blown disordered manner. For most women, it’s not a serious
problem:

Some of these women report that this is due to the fact that they are in
a good relationship with their partner, are in a very excited [state],
or feel too excited with the sexual act by itself, with very intense
desire or [it's just been a] long time without having sex.

In other words, for most women, it’s not something that is too
bothersome, unlike in men (where it can lead to future performance
issues, or anxiety regarding the sexual act itself).

But for a small portion of women, it is more than bothersome — it’s
as serious a concern as it is in men. To date, however, there is no
known treatment for premature female orgasm.

Premature Ejaculation Treatment Questioned

Researchers have called into question the effectiveness of a psychological approach to treating premature ejaculation, a sexual disorder that affects up to 30 percent of men worldwide.
A Cochrane Review of
four previously published studies of 253 men found insufficient
evidence to support behavioral therapy for premature ejaculation,
despite small studies and anecdotes proclaiming its success. But some
experts maintain that the therapy, which aims to ease the anxiety
surrounding the condition for both men and their partners, can be useful
together with medication.

"The assumption behind psychosocial interventions is that when patients
learn about [premature ejaculation] they begin to develop new sexual
management strategies to deal with restrictive sexual patterns, such as
avoidance of sexual activity and an unwillingness to discuss sex with a
partner," said the review's lead author Tamara Melnik, a professor of
internal medicine at the Federal University of Sao Paulo in Brazil.
"There is a consensus that psychological aspects are very important to
ensure success and maintenance of the results after discontinuation of
medication."

Medications like Paxil, Zoloft and others in the selective serotonin reuptake inhibitor class of antidepressants can inhibit the ejaculatory reflex at low doses. And topical anesthetics
like lidocaine cream can numb the penis, drawing out intercourse. But
because none are FDA-approved for premature ejaculation, doctors are
forced to prescribe drugs off-label and monitor patients closely for
possible side effects.

"One of the greatest unmet needs in the field is the lack of
FDA-approved treatments right now," said Dr. Wayne Hellstrom, a
urologist at Tulane University Health Sciences Center in New Orleans,
La. "But behavioral therapy does work in some people. And it does work
in combination with other treatments, either oral or topical
medications."
Premature ejaculation can crumble men's sexual self-confidence and make
it difficult for them to establish comfortable sexual relationships. In
up to 90 percent of cases, psychological factors are implicated as cause
or a consequence.

"We often hear men only think with one brain, but there's still
considerable thought and emotion that goes into sex," said Dr. Jeremy
Close, a primary care physician at Jefferson University Hospitals in
Philadelphia. "Behavioral therapy is to address the stress they may be
having with not satisfying their partner because of premature
ejaculation and feelings of inadequacy because of that."
The cause of premature ejaculation remains unknown, complicating the
search for effective treatments. And outside of the four studies
included in the Cochrane review, 13 others were too small or failed to
sufficiently control biases or rule out the placebo effect resulting in
their exclusion.

"We've got a lot of studies out there and, by and large, the majority
show that most people are helped by psychotherapy. But the studies
didn't meet the bar for evidence-based research that's set very high
these days," said Dr. Stanley Althof, executive director of the Center for Marital and Sexual Health of South Florida and co-author of the Cochrane Review. "We really need to move into the 21st century with the kind of research we do."

Another barrier to behavioral therapy use is the cost. At up to $125
out-of-pocket per session for an eight-session treatment course, the
peace of mind comes at a hefty price. And unlike medications, the
psychological approach requires that both the man and his partner are on
board.
Althof, a clinical psychologist who studies the effects of diseases and
medications on sexuality, hopes ongoing research opens new doors for the
treatment of sexual disorders in both men and women.
"This isn't about just enhancing your sex life or making you a better
lover. This is about helping men or women with real problems," Althof
said. "And we have ways to do that. We just have to be more rigorous in
how we go about conducting research."

60% Men > 40 Have Sexual Dysfunctions

Recent survey which was polled in May 2012 has indicated that 60% of
men over the age of 40 suffer from some type of sexual dysfunction. That
is an extremely high and frightening numerical statistic. Approximately
44 percent suffered from impotence, approximately 40 percent suffered
from premature ejaculation, and the remaining percentage suffered from
menopausal disorders. Our main concern is the statistical data provided
for premature ejaculation.

The percentage for premature ejaculation has increased approximately
10 percent from the last decade. In the past, every 3 males suffered
from premature ejaculation. This equates to approximately 33 percent.
However, this statistical data included adults and minors under the age
of 40 as well. In conclusion, though the numbers may claim to show an
increase of 10 percent, the amount of premature ejaculators still remain
the same from the past decade.

How many men suffer from premature ejaculation?

This study was published in the Journal of the American Medical
Association. It was based on data from the 1992 National Health and
Social Life Survey – which was a huge collection of interviews with both
men and women.
In total, 1,749 women and 1,410 men aged 18 to 59 participated. And the conclusion is the following.

30% of men said they regularly come too quickly

14% said they had no interest in sex

8% said they regularly experienced no pleasure from sex

No one can tell the exact number of men who suffer from premature
ejaculation, but everybody agrees that PE is the most common sexual
problem men around the world experience.
Urologists Andrew McCullough, M.D., of the New York University school
of medicine, and his colleague James Barada, M.D., of the Albany
College of Medicine, say PE is three times more common than erectile
dysfunction (ED), that is impotence.
While estimates vary from 5% to even 75%, most studies report that 20% to 30% of men ejaculate too quickly on a regular basis.
The reason why different studies report different numbers is the lack
of universally accepted definition of what PE actually is. On top of
that different, studies are conducted in a different way.
For example some are done with stopwatch, measuring exactly how long
man can go. Some are self-reported and based on man’s own feeling of the
level of control he has over his ejaculation. And some simply determine
if he is happy with his sexual stamina, or if he wants to last longer.
Anyways, it’s safe to say that 30 million American men, and about 600
million men worldwide suffer from PE. In other words, one of every five
men you know has the same problem as you.
As you can see, you’re far from alone.

PE also strikes men of all ages.
Previously we believed it was more common among younger men, when sex is still relatively new to them. Now we now better.
Recent studies report that middle-aged men still have this problem. And, in fact men of all age groups are equally at risk.
Another interesting thing is that some studies reported racial differences when it comes to PE.
Here are some of them:
One recent telephone survey, which included 1,320 American men, revealed that premature ejaculation is affecting:

21% of non-Hispanic African Americans

29% of Hispanics

16% of non-Hispanic Whites

Also a study done by National Health and Social Life Survey reported that premature ejaculation affected:

34% African American men

29% White men

27% Hispanic men

Some other studies reported that PE is more common in East Asia
(countries like China, Indonesia, Japan, Korea, and Malaysia), than in
Middle East and Africa (Algeria, Egypt, Morocco, and Turkey).
The presence of premature ejaculation in Europe is said to be somewhere between that of East Asia and Middle East / Africa.

Anyway, because of the small number of such studies, and different
ways in which they were conducted, it is impossible to draw any final
conclusions.

5 tips to help prevent Premature Ejaculation

Learn how to increase your stamina and become a better lover with these
5 tips to control premature ejaculation. Premature ejaculation is a
very common problem in men, especially younger men. While it can be an
embarrassing problem, it's important to understand that it is not only
happening to you, and there are things you can do to prevent it. In this
article, we will look at five tips to control premature ejaculation.

Things needed to control premature ejaculation:

  A willing partner

  Condom

  Cock ring

  Time to yourself

The ability to concentrate under pressure

Masturbate before sex. One to two hours before you
think you may have sex, you will need some time to yourself. During this
time, you will want to masturbate. What happens when you masturbate
before sex is that it helps relax the body and release built-up sexual
tension. This is an effective way to control premature ejaculation in
men of all ages.

Get a little help from your partner. This next tip
involves assistance from your partner, which is a nice way to keep her
involved in the sexual activity. What you'll do is, at the moment you
feel you are about to climax during sex, pull out and grasp the base of
your penis tightly. Your partner can do this for you. Hold the penis,
maintaining a firm grip for a count of 30 and then begin sex again.
Repeat this process two to three times before allowing yourself to
climax. This technique has proven to be a very effective way to control
premature ejaculation.

Distract yourself. If you have the ability to
concentrate under pressure, you can try to learn to distract yourself
enough to control premature ejaculation. Instead of focusing on how good
sex feels, try to distract yourself with a complicated math problem, or
by reciting the alphabet backwards. Try to think of anything that
requires a lot of concentration, which will work to, hopefully, prolong
your sexual performance.

Desensitize. Wear a condom during sex to decrease
sensitivity. This will, of course, make sex slightly less enjoyable, but
it will also help to control premature ejaculation. Why don't men like
to wear condoms? Because sex doesn't feel as good! That is precisely the
point of wearing a condom when you are trying to learn how to last
longer in bed.

Wear a cock ring. Like a condom, a cock ring will
help to desensitize the sensation during sex and help you control
premature ejaculation. But there is an added bonus to wearing a cock
ring; it will give you a rock hard erection. A cock ring is a soft,
flexible ring that fits over the base of the penis and behind the
testicles. It can only be worn for twenty to thirty minutes at a time,
which is all the time you'll need to pleasure your partner and ejaculate
in an adequate amount of time.

Prostate Massage

You can massage your prostate by simply inserting your finger into the
anus and reaching back and up towards the navel until you feel the
prostate gland. Use natural-content personal lubricant of the type
specific to this purpose (available in drug stores) to facilitate
insertion of the finger, and gently rub back and forth on the prostate
using as much pressure as you can without causing pain.

This massage can be made even more pleasurable if your partner is
willing to do the massage for you. You need not concern yourself about
about bacterial transmission if the hands are clean.

Traditionally, doctors would massage their patients' prostates regularly
to cure or prevent disease. Now, however, most doctors only check
the prostate, and if they find a problem prefer to cut it out or
prescribe medication. But a doctor is not necessarily required for
prostate massage. You can do it yourself or have your partner do it for
you. Try this practice and discover its health benefits. When you do it
yourself, you can be very gentle and make the treatment pleasurable as
well as efficacious.

Some people have a psychological block against examining or massaging
their prostates. They think the anus is dirty. But there is nothing
dirty about the body, which must be cared for. The only thing that might
be considered dirty is disease; and these practices are intended to
prevent and cure disease through regular care for the entire body,
including the anus and prostate.

Many men are troubled by these massages because they think there is
something "homosexual" about it, especially if it is pleasurable. They
might feel differently if they knew the extent to which neglect
ultimately results in prostate dysfunction, malfunction, and disease.
Inserting a finger into the anus never turned anyone into a homosexual.

It is important that men examine themselves often by checking the
condition of their prostates. You can determine the condition of the
prostate by feeling for changes in size and resilience. Check this as
often as possible, at least weekly, and you will be able to establish a
standard for yourself. By doing so, you will be able to detect any
changes in the size and flexibility of your prostate.

A very hard, enlarged, or painful prostate is an indication of a
disorder that could lead to cancer or some other major problem. However,
prostate disorders do not develop overnight, and regular massage can
prevent and reverse problems.

Medical science accepts that it is normal for men over the age of fifty
to experience problems with their prostates. Although prostate problems
may be common, they are not normal. They are abnormal.

There is no need for any man to have prostate problems. With regular
examination and massage prostate
problems may never occur at all.

Are you wondering how to overcome premature ejaculation? In this
article we are going to talk about the statement It's all in the thrust.
There
are thousands upon thousands of men out there that are wishing that
they knew what they were doing in bed, they desperately want to know how
to overcome premature ejaculation. Who can blame them? Who wants to
know that they are a terrible failure, horrible lover etc?

I am
sure that you do not like feeling this way. There are simple techniques
that you can use to keep yourself from feeling this way and satisfy your
partner once and for all. Let's go ahead and talk about the statement
It's all in the thrust.
Does It Matter How I Penetrate?
You
may be a little confused about what I mean by this statement but it's
extremely important that you listen. When you penetrate a woman's vagina
it of course gives her great sensations and there are certain ways that
feel better than others to a woman. The same is true with a man.
The Wrong Way
If
you are going into her vagina with long fast thrusts this may feel good
but this is not going to help you achieve the goal of lasting longer.
This also may make her less sensitive if you do this right away at the
beginning. Save the heavy stuff until the end.
The Right Way
Until
you start feeling that you are ready to release it's ok to do strokes
that are longer and faster but when you start to feel that you are
getting ready to cross that plateau buddy you better slow down or you
are going to blow. Slow the strokes down but also go as deep into the
vagina as you can and as much as is comfortable with your partner. Also
as you do this begin to rock your hips and stimulate her clitoris with
your pelvic bone. You will be able to give her a pleasurable time but
you will also be able to regain your control over the sensations that
you are feeling in your penis.

Acupuncture may be another treatment option for premature ejaculation (PE), new findings suggest.
In a study of 90 PE patients, the therapy proved superior to a sham
treatment but less effective than paroxetine, a selective
serotonin-reuptake inhibitor known to be a known therapeutic effect on
PE, according to an online report in European Urology.

Didem Sunay, MD, and colleagues at the Ministry of Health Ankara
Training and Research Hospital in Ankara, Turkey, randomly assigned
patients to receive paroxetine 20 mg/day, acupuncture, or sham
acupuncture (placebo acupuncture). Each group had 30 patients.
Investigators evaluated PE by measuring intravaginal ejaculation latency
times (IELTs) and using the Premature Ejaculation Diagnostic Tool
(PEDT). IELTs were determined by using a partner-held stopwatch.

Median PEDT scores of paroxetine, acupuncture, and placebo groups
were 17.0, 16.0, and 15.5, respectively, before treatment, and 10.5,
11.0, and 16.0 after treatment.
Paroxetine, acupuncture, and placebo acupuncture increased IELTs by
82.7, 65.7, and 33.1 seconds, respectively. The extent of ejaculation
delay caused by paroxetine was significant greater than that of
acupuncture, the researchers found.

“The result of the present study showed that real acupuncture was
more effective than placebo acupuncture,” the authors stated. “This
result seems to refute the hypothesis that it has a placebo effect.”
The authors concluded: “Although acupuncture is less effective than
daily paroxetine, it had a significantly stronger ejaculation-delaying
effect than placebo and seems to be an alternative to other treatment
models in the treatment of PE.”

What if I advised you right now that
each and every disease, mental and bodily dysfunction and health-related
problem identified to guy has its basis in a really particular strand
of hair of the hundreds of thousands of strands on your head? That all
we necessary to cure the condition, disorder or condition, we basically
have to uncover that specific strand and pluck it out...

You'd feel I'm nuts, appropriate? Or if you happen to be a really
scientifically tolerant personal, you would most likely give me the
advantage of doubt and lengthen me the opportunity to prove my theory
before rejecting it out of hand. Effectively, perpetuity is how
prolonged you'd have to wait for my evidence- permit by yourself a
single piece of evidence in assistance of my claims really! And you know
why? Simple: because they are fake to get started with!

When it arrives to homeopathic approach to treating illnesses, and, as
we shall see, which includes homeopathy for premature ejaculation, which
is specifically what the scientist, German doctor Samuel Hahnemann, who
created homeopathy famous did: he conjured up a bunch of theories out
of thin air, convinced himself they had been accurate, wrote a polemic
for them primarily based on specious arguments, and in some way managed
to persuade a little portion of the planet that his statements have been
true- without having a one piece of scientific proof to assist it!

Homeopathy is truly a philosophy that interprets diseases and illness as
becoming triggered by disturbances in a hypothetical "vital force" or
"life drive." It sees these disturbances as exhibiting by themselves up
as exclusive symptoms using the deal with of a condition. Granted, the
concept of existence as getting or getting electricity of some kind just
isn't a new 1, nor is it outlandish. But this is not exactly where
Hahnemann gets strange...

In an experiment conducted in the 1790s with a bark of some tree
utilised as a standard medication for malaria, Hahnemann observed that
the consequences he skilled from ingesting the bark were comparable to
the signs of malaria. From this single observation, the doctor helps
make a daring leap of creativity: he postulates that cures "work by
means of similarity," and that a treatment of a illness should be ready
to create signs in a wholesome personal equivalent to people of the
condition becoming taken care of in the ill individual.

Through further experiments with other substances, Hahnemann ultimately
arrived up with the "regulation of similars," otherwise recognized as
"permit like be cured by like" as a basic therapeutic theory of
homeopathy.

He thought that by using medicines to induce signs, the synthetic
symptoms would stimulate the "essential drive," causing it to neutralise
and expel the first condition and that this artificial disturbance
would obviously subside when the dosing stopped.

Homeopathy is, consequently, primarily based on the perception that a
substance which in massive doses will generate signs and symptoms of a
certain condition will, in extremely small and diluted doses, cure it.
And this is what homeopathy has appear to be: a type of choice
medication in which practitioners take care of individuals employing
highly diluted preparations that are considered to cause healthier men
and women to display signs that are equivalent to those shown by the
individual.

All - without having exception - of the 48 or so homeopathic medications
and preparations on the marketplace that guarantee to treatment
premature ejaculation work under these identical unfounded concepts!

Hahnemann's "regulation of similars" (and the resulting homeopathy
technique to therapy of conditions) is really absolutely nothing far
more than unproven claims created by a lone scientist it is not a
correct law of nature.

With its outlandishly wacky ideas such as drinking water getting a
memory, its use of solutions that lack energetic elements (the kind of
substances that are actively concerned in combating a disease), and the
general lack of convincing scientific proof to assistance its efficacy,
have all contributed to homeopathy being explained as a pseudoscience,
quackery, and a "cruel deception" by its critics.

In 2010, an inquiry into the evidence-base for homeopathy performed by
the United Kingdom's Home of Commons Science and Technologies Committee
concluded that "homeopathy is no much more powerful than a placebo
is..."

Hahnemann proposed homeopathy in reaction to the state of standard
Western medication at that time, which was typically brutal and a lot
more harmful than valuable (circumstance in stage: Electric Shock
Treatment, for illustration). We have appear a long way given that the
time of Hahnemann, making great breakthrough in standard medication
although. And whilst I am sympathetic to the cause of "option medicine,"
and definitely a proponent of it myself, we actually ought to quit
using our cues from quackery!

The first step is to become aware of your own levels of arousal.
Experiment, take your time and give yourself a very real self-loving
exploration, not just a quick masturbatory release. Notice how your
penis moves through distinct changes before orgasm and ejaculation, he's
not just soft and then hard and spewing. There are four defined stages
of erection: lengthening and filling; swelling; full erection; rigid
erection. The fourth stage, rigid erection, characterized by a penis
that's very stiff (a boner) and very hot, signifies ejaculation is close
at hand. Through attentive self-arousal and the playful hands of your
sweetheart you can learn how to stay for longer periods of time in the
exciting, but less explosive, third stage of firm erection. When you
feel yourself moving into the hard, hot level stop stimulation, relax
and pay attention to your breathing. Breathe slowly and deeply. Tell
your partner when you are about to have an orgasm. At this point the
partner can slow down or stop to prevent a premature ejaculation.

The Power of Breath.
Conscious breathing is a key for extended lovemaking. Rapid breathing
excites and arouses you. Slow, controlled breathing, way down into your
belly, calms you and helps delay ejaculation. Focusing on your breath
takes your attention away from your genitals. Synchronizing your
breathing rhythm with your partner's strengthens your connection with
each other.

Perineum Pressure.
Pressing on the perineum, a spot midway between your scrotum and your
anus, will help to stop ejaculation because this spot reaches through to
the prostate gland. It is the prostate that contracts and expands
during orgasm and then expels the ejaculation fluid. Ask your partner to
apply this loving pressure for you.

Testes Tug. When a
man nears orgasm his scrotum rises up closer to his body. You can delay
ejaculation by gently pulling your testes down and away from your body.
Your partner can also do this for you.

Penis Tip Squeeze.
Squeezing your penis just below the head or glans can help to stop
oncoming ejaculation. It necessitates withdrawing completely from your
partner during intercourse, but is an excellent way to begin practice on
your own with masturbation.

PC (pubococcygeous) Muscle Contraction.
This is simply squeezing your pelvic floor muscles, around the scrotum,
penis and anus as you feel ejaculation approaching. An easy way to
practice this is to interrupt your urine stream when you are going to
the bathroom.

Ejaculation-free orgasm.
One important thing to understand is that the problem really is
ejaculation, not orgasm. When you feel you are just about to ejaculate,
stop moving (your partner must stop, too) and contract the PC muscle
very firmly during 10 to 20 seconds. You can then resume love-making.
For this to work, your PC muscle must be strong enough. To train it:
contract it 20 times 2s, 3 times a day. You should see progress in
ejaculation control within a few weeks.

Passion Pump. This
technique, which combines squeezing the PC muscles, rolling your eyes
upward, touching your tongue to the roof of your mouth, and visualizing
your sexual energy flowing up through your body while you practice
controlled deep breathing is one of the most effective ways of delaying
ejaculation. It's a Taoist sexual kung fu technique that has the added
benefit of helping you circulate your sexual energy through your body
rather than having it remain hot and heavy in your genital region.

Thrusting Technique.
A superb thrusting technique that brings great pleasure to a woman and
helps a man to last is a combination of deep and shallow thrusts. By
shallow thrusts we mean your penis only enters 1½ to 2 inches inside
your partner. These first couple of inches are the most sensitive part
of a woman's vagina. Deep thrusts mean your penis enters as fully as you
can. In this technique you combine a series of nine fairly quick
shallow thrusts with one slow deep one. The shallow thrusts stimulate
your partner's most sensitive vaginal tissues and at the same time
create a vacuum effect that makes her ache for the deep thrust to come.
And because most of your thrusts are shallow ones, exciting only the
first few inches of your penis, you're able to rock on for much longer!

Use foreplay to your advantage, stimulating your partner first.
If you want to have quite a bit of intercourse before ejaculation, it's
best not to allow yourself(male) to be the one getting the attention!

Let him put the condom on himself. This improves the chance of him not getting too turned on beforehand.

Tackle relationship issues.
Be sure that your head and heart are in the mood for sex as well as
your body. If you're unhappy about something with your partner then sort
it out first.

Exercise your pelvic floor. Many men say they are able to delay ejaculation by squeezing or pushing their pelvic floor muscles.

Stop and start.
This is the most common technique used in psychosexual therapy. Practice
stimulating yourself to the point just before ejaculation, then stop.
Start again when the sensations have subsided. Repeat this three times.
You should find the length of time before each stop gets gradually
longer.

Change strokes. If
you feel you've gained more control with the stop-and-start technique,
try changing your stroke to something less stimulating rather than
stopping all together.

Change positions.
During intercourse, try the stop-and-start technique when the sensations
become strong. When you're more confident, try changing positions
rather than stopping. Some men last much longer when their partner is on
top.

Remember that doing doggy style and missionary both increase pressure on the male so stay clear of these positions.

Think about football. Does the trick every time. In a really dire situation, think of your grandma's arm fat.

Relationships

Premature ejaculation can certainly take a toll on a relationship.
Whether a couple waits until after marriage or not to be sexual,
intercourse plays a huge role in the success and happiness of a
relationship. Over time, unresolved sexual problems can slowly chip away
at a relationship for both partners.

Effective communication is crucial to a healthy long-lasting
relationship, however it can often times be difficult for a man to
discuss his premature ejaculation concerns with his partner. There is a
lot of unnecessary shame that is associated with the condition and many
men think that admitting that they have a problem controlling their
arousal means that they are less of a man. Not talking about the problem
will actually make things that much worse. Misunderstandings, hurt
feelings, and anger can quickly escalate, all of which can make
premature ejaculation even worse.
On an evolutionary level, intercourse promotes what scientists call
“pair bonding” and it is crucial for a healthy and successful
relationship. During sex, and particularly during orgasm, chemicals such
as oxytocin and dopamine are released that strengthen the bond between
partners. There is an increased feeling of security and trust, as well
as an increase in emotional attachment.
These bonding emotions are crucial to the strength of a couple. When a
man isn’t able to perform as well as he should, both partners not only
miss out on these healthy bonding chemicals, but instead are left with
negative feelings of inadequacy and disappointment. This can actually
make premature ejaculation symptoms even more prominent.
Sometimes premature ejaculation will actually prevent men from pursuing
relationships in the first place. The man may actually avoid sexual
relationships altogether for fear of rejection or embarrassment. They
feel that they can’t adequately satisfy a woman so they just avoid
sexual encounters all together. Most men won’t talk about their
premature ejaculation, especially with a new partner, so when they do
attempt intercourse, the woman often misunderstands why the man can’t
perform. Over time avoiding relationships because of premature
ejaculation can lead to depression as the man is cutting himself off
from other people to avoid his issues.
Men naturally are not apt to discuss any sort of personal problem with
anyone else, especially sexual ones. So much of a man’s perception of
their masculinity rides on their ability to perform sexually that they
don’t want to let anyone know of any performance problems they may be
having. If a man is in a relationship however, he has to learn how to
talk to his partner. Even men embarking on a new sexual relationship
should open up at least a little bit.
You don’t have to pour your heart out and tell them every intimate
detail about your premature ejaculation, but open up just a little bit.
At the same time, their partners need to be patient and understanding.
Men in relationships with effective communication often times are better
able at resolving their issues than those who cannot talk to their
partner.
