Obesity up despite more exercise

Young adults in the World are exercising more but still battling the bulge, according to a State Government health survey.
The Summery Report on Adult Health, released today, shows the number of over-weight or obese young adults continues to rise. The survery said just over half of all people were talking part in "adequate" levels of physical activity each week, up from 38 per cent in 1997.
But 47 per cent of 25 to 34 year-old were overweight in 2008, an increase fro 39 per cent in 1997.
A third of 16 to 24 year-olds were overweight compared with 22.8 per cent a decade ago, according to the report. Other results from the study, which surveyed 12,485 people, showed the number of young adults drinking risky amounts of alcohol had decreased significantly in the past six years.
There was also a reduction in smokers across all ages, apart from those aged 55-64, to about one in five people. More then half of people aged 16-34 now eat two or more servings of fruit a day - but only 8 per cent of people meet the recommended five serves of vegetables.

Why do you need Breast Lift/ Mastopexy?

Death Risks High After Weight Loss Surgery -Study

Patients who undergo weight-loss stomach surgery have a higher death rate than is true for the general population, including more suicides, perhaps linked to depression, researchers said on Monday.
The higher risk of death generally is due not to the surgery itself but to the health problems that accompany obesity, and the damage that the condition does to the body before and after surgery, the researchers said.

Dr. Bennet Omalu and colleagues at the University of Pittsburgh said a review of more than 16,000 bariatric operations done in Pennsylvania over a nine-year period found a "substantial excess of deaths owing to suicide and coronary artery disease" compared to normal death rates found in the population at large.
"It is very likely that the suicide deaths were ... underestimated because some of the deaths were listed as drug overdoses rather than suicide on the death certificate," Omalu's team wrote in their report, published in the Archives of Surgery.
"The large number of deaths due to suicide and drug overdose, in excess of what we expected, is also a cause for concern. Most of them occurred at least one year after surgery, suggesting that careful follow-up, especially the need to recognize and treat depression, should be provided," they added.
There were 440 deaths among the patients, who had an average age of 48 when the operations were performed. About 1 percent of the patients in the study died within a year of the procedures and 6 percent died within five years.
Heart disease was listed as the cause of death in 76 patients -- about 20 percent of the group -- a rate higher than would be common in the general population, the researchers found.
There were 14 suicides, compared to two that would be likely to occur in the population at large in a group of people that size, said the study
Omalu's team said surgery is an effective treatment for severe obesity, with heavily overweight patients often losing 80 percent of their excess body weight within one or two years.
The higher death rates found in the study were likely due to complications caused by obesity itself, from both before and after the surgery, they said.
In a second study, Dr. Christopher Still and colleagues at the Geisinger Health System in Danville, Pennsylvania, found that bariatric surgery patients who lost some weight beforehand get out of the hospital quicker.
The study of more than 800 patients who underwent open or laparoscopic gastric bypass surgery between 2002 and 2006 found that those who lost more than 5 percent of their excess body weight before the operation were less likely to stay in the hospital longer than four days compared to those who did not lose weight beforehand.
It also found that those who shed 10 percent of the excess weight ahead of time were more than twice as likely to have lost 70 percent of their excess weight a year later, compared to those who lost none at all.
The researchers said the effect has to do with the beneficial effects of weight loss on high blood pressure, diabetes and other problems.

And if so, why?It's hard to tell from these photos if her umbilicus (belly button) was removed during her tummy tuck. If it was, this is actually not that uncommon, as many patients who have a tummy tuck have a hernia of their belly button as well. Some surgeons will repair the hernia at the time of the tummy tuck surgery. When this occurs, there is a risk that the belly button will lose its blood supply and die. Some surgeons will pre-empt this process by actually removing the belly button at the time of the surgery, with plans to remake a new one later. I tend to repair it at the time of surgery or have it done after the tummy tuck has completely healed.

Rhinoplasty: Why I really wanted it ?

Some People have different reasons to get about the rhinoplasty done. As per my experience i went to my doctor with the complaint of my having a “ball-peen hammer nose”; clinically referred to as a ‘Hanging Columella’. Now, I never really thought of my nose as unsightly until a very rude Model said I had a “Ball-peen Hammer” nose. What the heck does that mean?!? So, ever since then I have been insecure about it (9 whole years until the day of my rhinoplasty surgery), I had considered having a nose job or rhinoplasty. As well as i wanted the tiny hump on my nose removed.

Eventually my doctor told me if I had just had the tip nipped and tucked and narrowed that the bone structure towards the top would look wide. Well, I never even thought I had a thick nose until he pointed it out. Alas! $3,000. more than just having the tip ‘done’. But, then of course after he pointed it out I obsessed about it. I was ready to have a full nose surgery (RhinoPlasty).

Labioplasty is also known as labia reduction, inner lip reduction, labia minora reduction or vaginal rejuvenation. Cosmetic labioplasty involves surgically reducing and/or reshaping the female external genital structures. Large or asymmetric labia may cause different problems for women. They may cause physical discomfort, may chaff against underwear or be visible through swimming costumes.

More commonly, large labia tend to make women self-conscious about their appearance, which may impact on their sexual and emotional relationships. Their self-confidence may also be affected. Here is how one patient explains it:

"My labia would actually protrude. I would stand up and it would pop out and I would want to tuck it back up, but obviously I couldn't if I was in public. When it popped out, it rubbed on my underpants, which felt awful and was so unpleasant. I would complain to my mother about it, but she thought I would "grow out of it" and that it would shrink in time. But it didn't. "I had no self-confidence and when I was naked it dangled. My partner would joke about it but luckily, he wasn't really bothered by it at all — it was me that was bothered. I always made sure there was a lot of "mood lighting" in the bedroom."

Labioplasty is requested by women of all ages and walks of life. Often just feeling good about how you look can build self-confidence and self-esteem. Some patients report improvement in their sex lives as a result of this procedure and the effect it has on their confidence.

"I am so happy now: it's really changed my quality of life. Little things, like wearing a swimming costume, are different for me now. Commonly asked questions How is the procedure performed?

Labioplasty can be performed in a day surgery, usually under general anaesthetic or sedation depending on your preference. The procedure takes 60-90 minutes. The excess tissue is marked out and removed. Once the surgery is completed, you will be transferred to the recovery ward for a couple of hours until you are ready to go home.

How long does recovery take?

Recovery is a gradual process but it usually takes about a week before returning to work. You can get back to exercise about two weeks after the operation and playing sport after about four to six weeks and sexual activity can resume after four to six weeks depending on your comfort levels.

What are the risks?

Labioplasty is a safe procedure but like all operations, has some risks. Bruising will persist for about a week and swelling will decrease over about six weeks. Asymmetry is possible and scars may also detract from the result. Nevertheless, labioplasty is a procedure with very high satisfaction.

that accrue from massive weight loss after bariatric surgery are well established in the surgical literature. As a result, the procedure is being performed more frequently than ever. However, a massive pannus and redundant skin and soft tissue in many areas of the body—including the hips, thighs, arms, back, and gluteal region—often remain following massive weight loss. These areas of excess tissue can cause considerable morbidity for the bariatric-surgery patient

In addition to the obvious aesthetic benefits that result after this tissue is resected, relief from chronic intertriginous infections and improvements in mobility and ability to perform daily functions can be expected when these real deformities are addressed by the plastic surgeon.
One body area that has received little attention following massive weight loss is the gluteal region. Using de-epithelialized adipocutaneous flaps from the lumbar region in an area that is typically excised during belt-lipectomy body-lift surgery, surgeons can achieve improved gluteal projection and contour. This procedure is particularly well suited for the redundant lower-back skin commonly seen in the postbariatric surgical patient (Figure 1).1
The technique described here exploits the excess tissue in the lower back for gluteal augmentation with bilateral, de-epithelialized, paddle-shaped dermal-fat flaps based on a medial vascular pedicle. No new scars result in patients who have already undergone a belt lipectomy, and well-shaped buttocks can be reliably achieved with low complication rates.

Surgical Plan and MarkingsBilateral,

inferiorly rotated, de-epithelialized, adipocutaneous flaps based on perforators of the superior gluteal artery are used for the augmentation. Preoperative markings are performed with the patient in a standing position and are based upon two anatomic landmarks: the coccyx and the anterosuperior iliac spine. A mark is made in the posterior midline 4 cm superior to the coccyx and is extended laterally in a line toward each anterosuperior iliac spine, finishing anteriorly in line with the location an abdominoplasty scar would occupy.
The second line—the superior limit of skin and subcutaneous adipose tissue resection—depends upon the degree of laxity of the redundant skin and the subcutaneous adipose tissue to be excised. Analogous to an anterior abdominoplasty resection, the markings are parallel lines ending at the posterior midline (Figure 2).

(Description of the Procedure )

The operation begins with the patient in the supine position. The marked anterior area is carefully de-epithelialized, and the anterior portions of the flaps are released bilaterally. The patient is then turned to the prone position, and the de-epithelialization of the posterior aspect of the flaps is completed.
Next, bilateral gluteal pockets are created above the gluteal muscle fascia plane, so that they can receive the dermal-fat rotation flap. An incision is made down to the gluteal fascia, and the thick dermal-fat flaps are undermined toward the gluteal fold, just above the gluteal fascia (Figure 3). The same maneuver is performed on the contralateral side.
The flaps are then rotated into the pockets. The flaps’ vascular pedicles consist of the cutaneous perforators of the superior gluteal artery (Figure 4). After careful hemostasis is obtained, the viability of the dermal-fat flaps is tested. The two edges of the flaps are rotated and sutured together in the muscle-fascia plane. The thick dermal-fat flap covering the rotation flap is advanced in the cephalic direction and closed in layers, using the superficial fascial system for strength.2
Satisfactory Results The 30 patients in this series were quite satisfied with the postoperative contour improvements in the gluteal region following gluteal autologous fat augmentation in combination with a belt lipectomy. A widened scar occurred in a few areas in the posterior incision that was likely due to excess tension on the closure line. In addition, we observed a seroma that was successfully treated with a long-term drain (10 days). There were no incidences of flap loss or infection.

Knots Landing" actress Joan Van Ark showed up at a recent Hollywood benefit, using makeup to conceal what looked like some cosmetic surgeries gone wrong. The 64 year-old former TV star was seen wearing mask-like makeup which idn' exactly hide the fact that she appeared to have been under the surgeon's knife.
The actress also came with painted-on eyebrows and heavy eye makeup though that didn't conceal her blotchy skin. She seemed to wear no lipstick on her highly plumped lips.
Ironically, Van Ark filmed a guest appearance on the plastic surgery-based drama "Nip/Tuck," for which an insider claimed the actress required hours of hair and makeup preparation and "only consumed several glasses of water with honey and one banana — all day".

Bariatric Bypass Surgery

Nearly one in six Americans is either clinically obese. Some live day to day with the morbid obesity, a condition that increases the risk of heart disease, diabetes and cancer. One of the most efficient ways to combat these risks is through bariatric surgery or bariatric bypass surgery.

Bariatric bypass surgery is also commonly called gastric bypass surgery. In lay terms, it has been come to be called stomach stapling, but the term is often misleading.

There are basically two kinds of bariatric bypass surgery: restrictive and malabsorption. With restrictive bariatric surgery, the stomach is made smaller, reducing the amount of food a patient is able to intake which results in weight loss. The malabsorbtion method is different, in that, the small intestine is shortened and a small pouch is inserted, carrying food directly from the esophagus to the small intestine for excretion. More specifically, a small pouch is created at the top of the stomach and the rest is permanently separated. The small intestine is then attached to the new pouch creating a new digestive tract.

Restrictive operations for obesity include adjustable gastric banding (AGB) and vertical banded gastroplasty (VBG). Restrictive operations serve only to restrict food intake and do not interfere with the normal digestive process. To perform the surgery, doctors create a small pouch at the top of the stomach where food enters from the esophagus. Initially, the pouch holds about 1 ounce of food and later expands to 2-3 ounces. The lower outlet of the pouch usually has a diameter of only about ¾ inch. This small outlet delays the emptying of food from the pouch and causes a feeling of fullness.

Malabsorptive operations are the most common gastrointestinal surgeries for weight loss. They restrict both food intake and the amount of calories and nutrients the body absorbs.

Malabsorptive surgery produces more weight loss than restrictive operations, and are more effective in reversing the health problems associated with severe obesity. Patients who have malabsorptive operations generally lose two-thirds of their excess weight within 2 years.

One of the most widely used bariatric bypass procedures is the Roux-en-y approach and more often than not, physicians use laparoscopic techniques to aid in the procedure. Laparoscopic bariatric surgery uses a camera to help surgeons perform the operation with added accuracy and less intrusion for the patient. The result is a speedier recovery with an overall better cosmetic outcome.

When it comes to laparoscopic bariatric bypass surgery, it is important to remember that not all patients qualify for the procedure. Qualification for this kind of surgery is dependant on the patient’s medical history and current medical condition. If the laparoscopic approach is not an option, physicians will refer to the ”open” technique, which involves a large incision through the chest and/or stomach while surgeons work with traditional bariatric methods and equipment. The ”open” technique is still widely used among bariatric surgeons, although doctors tend to favour the laparoscopic approach due in large part to the recovery time and overall satisfaction of the patient.
