Using de-epithelialized adipocutaneous flaps from the lumbar region in an area that is typically excised during belt-lipectomy body-lift surgery, surgeons can achieve improved gluteal projection and contour. This procedure is particularly well suited for the redundant lower-back skin commonly seen in the postbariatric surgical patient (Figure 1).1&lt;br /&gt;The technique described here exploits the excess tissue in the lower back for gluteal augmentation with bilateral, de-epithelialized, paddle-shaped dermal-fat flaps based on a medial vascular pedicle. No new scars result in patients who have already undergone a belt lipectomy, and well-shaped buttocks can be reliably achieved with low complication rates.&lt;br /&gt;&lt;br /&gt;&lt;/div&gt;&lt;div align="justify"&gt;&lt;strong&gt;Surgical Plan and MarkingsBilateral,&lt;/strong&gt;&lt;/div&gt;&lt;br /&gt;&lt;br /&gt;&lt;div align="justify"&gt;inferiorly rotated, de-epithelialized, adipocutaneous flaps based on perforators of the superior gluteal artery are used for the augmentation. A mark is made in the posterior midline 4 cm superior to the coccyx and is extended laterally in a line toward each anterosuperior iliac spine, finishing anteriorly in line with the location an abdominoplasty scar would occupy.&lt;br /&gt;The second line—the superior limit of skin and subcutaneous adipose tissue resection—depends upon the degree of laxity of the redundant skin and the subcutaneous adipose tissue to be excised. Analogous to an anterior abdominoplasty resection, the markings are parallel lines ending at the posterior midline (Figure 2).&lt;br /&gt;&lt;/div&gt;&lt;br /&gt;&lt;br /&gt;&lt;div align="justify"&gt;&lt;strong&gt;(Description of the Procedure )&lt;/strong&gt;&lt;/div&gt;&lt;br /&gt;&lt;br /&gt;&lt;div align="justify"&gt;The operation begins with the patient in the supine position. The marked anterior area is carefully de-epithelialized, and the anterior portions of the flaps are released bilaterally. The patient is then turned to the prone position, and the de-epithelialization of the posterior aspect of the flaps is completed.&lt;br /&gt;Next, bilateral gluteal pockets are created above the gluteal muscle fascia plane, so that they can receive the dermal-fat rotation flap. An incision is made down to the gluteal fascia, and the thick dermal-fat flaps are undermined toward the gluteal fold, just above the gluteal fascia (Figure 3). The flaps’ vascular pedicles consist of the cutaneous perforators of the superior gluteal artery (Figure 4). After careful hemostasis is obtained, the viability of the dermal-fat flaps is tested. The two edges of the flaps are rotated and sutured together in the muscle-fascia plane. The thick dermal-fat flap covering the rotation flap is advanced in the cephalic direction and closed in layers, using the superficial fascial system for strength.2&lt;br /&gt;Satisfactory Results The 30 patients in this series were quite satisfied with the postoperative contour improvements in the gluteal region following gluteal autologous fat augmentation in combination with a belt lipectomy. A widened scar occurred in a few areas in the posterior incision that was likely due to excess tension on the closure line. In addition, we observed a seroma that was successfully treated with a long-term drain (10 days). Some live day to day with the morbid obesity, a condition that increases the risk of heart disease, diabetes and cancer. One of the most efficient ways to combat these risks is through bariatric surgery or bariatric bypass surgery.&lt;br /&gt;&lt;br /&gt;Bariatric bypass surgery is also commonly called gastric bypass surgery. In lay terms, it has been come to be called stomach stapling, but the term is often misleading.&lt;br /&gt;&lt;br /&gt;There are basically two kinds of bariatric bypass surgery: restrictive and malabsorption. With restrictive bariatric surgery, the stomach is made smaller, reducing the amount of food a patient is able to intake which results in weight loss. The malabsorbtion method is different, in that, the small intestine is shortened and a small pouch is inserted, carrying food directly from the esophagus to the small intestine for excretion. More specifically, a small pouch is created at the top of the stomach and the rest is permanently separated. The small intestine is then attached to the new pouch creating a new digestive tract.&lt;br /&gt;&lt;br /&gt;Restrictive operations for obesity include adjustable gastric banding (AGB) and vertical banded gastroplasty (VBG). Restrictive operations serve only to restrict food intake and do not interfere with the normal digestive process. To perform the surgery, doctors create a small pouch at the top of the stomach where food enters from the esophagus. Initially, the pouch holds about 1 ounce of food and later expands to 2-3 ounces. The lower outlet of the pouch usually has a diameter of only about ¾ inch. This small outlet delays the emptying of food from the pouch and causes a feeling of fullness.&lt;br /&gt;&lt;br /&gt;Malabsorptive operations are the most common gastrointestinal surgeries for weight loss. They restrict both food intake and the amount of calories and nutrients the body absorbs.&lt;br /&gt;&lt;br /&gt;Malabsorptive surgery produces more weight loss than restrictive operations, and are more effective in reversing the health problems associated with severe obesity. Patients who have malabsorptive operations generally lose two-thirds of their excess weight within 2 years.&lt;br /&gt;&lt;br /&gt;One of the most widely used bariatric bypass procedures is the Roux-en-y approach and more often than not, physicians use laparoscopic techniques to aid in the procedure. Laparoscopic bariatric surgery uses a camera to help surgeons perform the operation with added accuracy and less intrusion for the patient. The result is a speedier recovery with an overall better cosmetic outcome.&lt;br /&gt;&lt;br /&gt;When it comes to laparoscopic bariatric bypass surgery, it is important to remember that not all patients qualify for the procedure. Qualification for this kind of surgery is dependant on the patient’s medical history and current medical condition. If the laparoscopic approach is not an option, physicians will refer to the ”open” technique, which involves a large incision through the chest and/or stomach while surgeons work with traditional bariatric methods and equipment.
