"Make a difference in the world of cancers" - specifically "head and neck cancers"

April was Oral Cancer Awareness month and awareness among the public is low. I hope to raise awareness in order to save lives in my blog. This was further strengthened by three patients who were misdiagnosed and luckily came to me at the right time. Patient1 was diagnosed as simple ulcer!!!! When in realty he had cancer, patient two came to me stating that he has a big ulcer in the tongue when in realty again it was cancer. The ultimate was patient 3 who was diagnosed as having a fungal disease and was being treated for the same when in realty he had cancer of palate!!! He is currently undergoing treatment.

Together, we have the opportunity to make a difference in the world of cancers specifically head and neck cancers.

Cancers of the head and neck are the most common malignancies seen in the country. Every year about 20% of all cancer deaths in India are due to head and neck cancer which are primarily caused by tobacco consumption. While smoking is still a major risk factor, the fastest growing segments of oral cancer patients are young, healthy, non-smoking individuals due to the usage of tobacco in its various forms.

In India it is estimated that there are 7 lakh new cases coming every year and most present in advanced and incurable stage at the time of diagnosis. More of the patients are between the ages of 35 and 65 years.

"Despite improvements in diagnosis and local management the death rate associated with this cancer is particularly high not because it is hard to discover or diagnose, but due to the cancer being routinely discovered late in its development. It is much higher than that of cancers which we hear about routinely such as cervical cancer, breast cancer or skin cancer.

One of the real dangers of this cancer is that in its early stages, it can go unnoticed. It can be painless and by the time the cancer is discovered it would have spread to other areas namely the lymph nodes in the neck and invade deep into local structures.

Fortunately, most head and neck cancers produce early symptoms and ENT surgeons and Dentists can, in many cases see or feel the precursor tissue changes or the actual cancer while it is still very small, or in its earliest stages.

What you should do: You should know the potential warning signs. All of the symptoms and signs described here can occur with no cancer present. In fact, many times complaints of this type are due to some other condition. But you can’t tell without an examination. So if they do occur, see your doctor to be sure.

Remember—when found early, most cancers in the head and neck can be cured with few side effects. Cure rates for these cancers could be greatly improved if people would seek medical advice as soon as possible and practice health habits which help prevent these diseases. Remember—successful treatment of head and neck cancer depends on early detection. Knowing and recognizing its signs can save your life.

Symptoms of head and neck cancer

A lump in the neck. Cancers that begin in the head or neck usually spread to lymph nodes in the neck before they spread elsewhere. Not all lumps are cancer. But a lump (or lumps) in the neck that lasts more than two weeks should be seen by a specialist as soon as possible.
Change in the voice. Most cancers in the region of the voice box cause some change in voice. If you are hoarse or notice voice changes for more than two weeks, see your doctor.

A growth in the mouth. Most cancers of the mouth or tongue cause a sore or swelling that may be painless. If an ulcer or swelling is accompanied by lumps in the neck, you should be concerned. It may also appear as a white or red patch of tissue in the mouth, or a small indurated ulcer which looks like a common canker sore. Because there are so many benign tissue changes that occur normally in your mouth, and some things as simple as a bite on the inside of your cheek may mimic the look of a dangerous tissue change, it is important to have any sore or discolored area of your mouth, which does not heal within 14 days, looked at
Bringing up blood. If blood appears in your saliva or phlegm for more than a few days, you should see your physician.

Swallowing problems. Cancer of the throat or esophagus (swallowing tube) may make swallowing solid foods—and sometimes liquids—difficult. The food may “stick” at a certain point or come back up. If you have trouble almost every time you try to swallow something, you should be examined by a physician. Usually a barium swallow x-ray or a CT scan or an endoscopy will be performed to find the cause.

Changes in the skin. The most common head and neck cancer is basal cell cancer of the skin. Basal cell cancers appear most often on sun-exposed areas like the forehead, face, and ears, but can occur almost anywhere on the skin. Parts of the ulcer may heal, but the major portion remains ulcerated. Some basal cell cancers show color changes. Most squamous cell cancers occur on the lower lip and ear. They may look like basal cell cancers, and if caught early and properly treated, usually are not dangerous. However, any mole that changes size, color, or begins to bleed may mean trouble.

Persistent earache. Constant single sided ear pain lasting for more than 2 weeks can be a sign growth in the throat.

If your dentist or doctor decides that an area is suspicious, the only way to know for sure if it is something dangerous, is to do a biopsy of the area. This is not painful, is inexpensive, and takes little time. It is important to have a firm diagnosis as early as possible.

QUIT SMOKING - HOW TO DO IT

Today a patient of mine requested me to blog about how one can try quitting to smoke, he also had doubts about Nicotine replacement therapy. I am skipping that part since I believe that the mind is the biggest challenger, so here it goes....

There are over 4,000 chemicals in cigarette smoke including formaldehyde (used to preserve dead bodies); ammonia (used in strong cleaning liquids) and cadmium (a highly poisonous metal used in batteries). Quitting smoking is not easy, but it can be done.

Why do people start to smoke?

Common reasons for starting include peer pressure, the desire to be ‘grown-up’, natural curiosity and a sense of rebellion or freedom. Youngsters with parents who smoke are particularly susceptible. Children see adults smoking in an attempt to relieve stress, tension and boredom. Adult smokers may appear more confident and better able to cope and children want to mimic this ‘grown-up’ behavior. Many actors, film stars and singers smoke. Indeed, it could be argued that smoking is sold as a lifestyle rather than a product and the illusion of style is that smoking is a ‘cool thing’ to do.

Why do people continue to smoke?

The main factors that contribute to people continuing to smoke are the physical addiction to nicotine, the daily rituals around the habit and the emotional and psychological dependence.

Physical addiction

Nicotine is a drug found naturally in tobacco and is highly addictive. Over time, the body becomes both physically and psychologically dependent on nicotine. When smoke is inhaled, nicotine is carried deep into the lungs, where it is absorbed quickly into the bloodstream and carried throughout the body. Nicotine can be found in breast milk of smokers. During pregnancy, nicotine freely crosses the placenta and has been found in the umbilical cord blood of newborn infants. Nicotine produces pleasant feelings that make the smoker want to smoke more. After a while, the smoker develops a tolerance and then smokes to maintain this level of nicotine. In fact, nicotine, when inhaled in cigarette smoke, reaches the brain faster than drugs that enter the body by way of injections!

Psychological and emotional dependence

Smoking means different things to different people. For many, cigarettes are a friend, a relief from boredom, and are also seen as a form of stress relief. For those on a low income, smoking is often identified as their ‘one luxury ’.

Stress

Many smokers believe that smoking relieves stress and there is no doubt that nicotine withdrawal may be followed by unpleasant mood changes. Stress levels can worsen withdrawal, strongly linking tobacco use with poor emotional and mental health. Talking to a friend or family member about what is causing the stress could be a good way to clearly identify just how smoking is a contributory factor.

Benefits of quitting

Half of all smokers die early from a smoking related disease and one in four smokers die in middle age (35-64) as a result of their habit. Diseases caused by smoking can cause a great deal of pain and suffering for smokers and their loved ones. Additionally, the sudden loss of an only parent can be particularly hard for the surviving children. There are many serious and fatal diseases directly caused by smoking.

The following are the most common causes of smoking-related death:

• Coronary heart disease, which may result in heart attack, or other vascular disease, perhaps leading to stroke.

• Chronic Obstructive Pulmonary Disease (COPD), which may include chronic bronchitis and emphysema. Infections such as pneumonia are more likely to be fatal

• Lung cancer, as well as most other forms of cancer.

• In addition, impotence, peptic and duodenal ulcers and fertility problems may be associated with smoking.

• Even everyday complaints such as coughing, sneezing and shortness of breath on exertion can be attributed to smoking.

Smoking also causes premature wrinkling of the skin, bad breath, bad smelling clothes and hair.

For women, there are unique risks. Women over 35 who smoke and use birth control pills are in a high-risk group for heart attack, stroke, and blood clots of the legs. Women who smoke are more likely to have a miscarriage or a lower birth-weight baby. People who stop smoking before age 50 cut their risk of dying in the next 15 years in half compared with those who continue to smoke. Ex-smokers also enjoy a higher quality of life with fewer illnesses

Improved health benefits

Smokers are always being told about the harmful effects of their habit; however, people are far less aware of the dramatic health benefits of quitting and just how quickly they begin. It’s always worth emphasizing that the health benefits from stopping begin almost immediately and continue to increase for many years:

20 minutes - Blood pressure and pulse return to normal

8 hours - The oxygen level in your blood increases to normal level. Chances of a heart attack start to fall

24 hours - Carbon monoxide leaves the body. The lungs start to clear out mucus and debris

48 hours - Nicotine is no longer found in the body. Sense of taste and smell improve

72 hours - Breathing becomes easier. Energy levels increase

2-12 weeks - Circulation improves throughout the body

3-9 months - Coughing, shortness of breath and wheezing improve.

5 years - Risk of having a heart attack falls to about half that of a smoker.

10 years - Risk of lung cancer falls to around half that of a smoker.

During exercise, chemicals called ‘endorphins’ are released in the brain, which have a tranquillizing effect and make people feel good.

Stopping smoking can bring other opportunities. Many ex-smokers have found that the effort they invested in stopping smoking has helped them to have more belief in themselves and their capabilities. As a result people who have quit smoking have also gone on to make other positive life changes, such as taking advantage of new opportunities at work.

Cost

Smoking is expensive. It isn't hard to figure out how much you spend on smoking: multiply how much money you spend on tobacco every day by 365 (days per year). The amount may surprise you. Now multiply that by the number of years you have been smoking and that amount will probably astound you. Multiply the cost per year by 10 (for the upcoming 10 years) and ask yourself what you would rather do with that much money. And this doesn’t include other possible expenses, such as the health care costs due to tobacco-related conditions.

Social Acceptance

Smoking is less socially acceptable now than it was in the past. Most workplaces have some type of smoking restrictions. Friends may ask you not to smoke in their houses or cars. Public buildings, aircrafts, music halls and even cinema halls are now largely smoke-free. Like it or not, finding a place to smoke is going to be a hassle.

Health of Others

Smoking not only harms your health but the health of those around you. Exposure to secondhand smoke (passive smoking) includes exhaled smoke as well as smoke from burning cigarettes. Studies have shown that secondhand smoke causes thousands of deaths each year from lung cancer and heart disease in healthy nonsmokers.

Smoking by mothers is linked with sudden infant death syndrome (SIDS) and low-birth weight infants. Babies and children raised in a household where there is smoking have more ear infections, colds, bronchitis, and other respiratory problems than children from nonsmoking families. Secondhand smoke can also cause eye irritation, headaches, nausea and dizziness.

Setting an Example

If you have children, you probably want to set a good example for them. When asked, nearly all smokers say they don't want their children to smoke, but children whose parents smoke are more likely to start smoking themselves. You can become a good role model for them by quitting now.

How can I stop smoking?

Quitting is hard. Usually people make 2 or 3 tries, or more, before finally being able to quit. Each time you try to quit, you can learn about what helps and what hurts. Write down your personal reasons for stopping. Be specific. Keep your list with you so you can look at it when you feel the urge to smoke. To help you understand your smoking habit, keep a diary of when and why you smoke. Using information from this diary, you can make a plan to deal with the things that make you want to smoke.

Smokers often say, "Don't tell me why to quit, tell me how." There is no one right way to quit, but there are some key elements in quitting smoking successfully:

1. Making the Decision to Quit

The decision to quit tobacco use is one that only you can make. Others may want you to quit, but the real commitment must come from you.

You will be more likely to stop tobacco use if you:

believe that you could get a tobacco-related disease and this worries you
believe that the benefits of quitting outweigh the benefits of continuing tobacco use
know of someone who has had health problems as a result of their tobacco use

2. Get ready: Once you've made a decision to quit, you're ready to pick a quit date. This is a very important step. Pick a specific day within the next month as your "Quit Day." Set a quit date 2 to 4 weeks from now so you'll have time to get ready. Change your environment. Get rid of ALL cigarettes and ashtrays in your home, car and place of work. Don't let people smoke in your home.

3. Get support and encouragement: You have a better chance of being successful if you have help. Tell your family, friends and coworkers that you are going to quit. Ask them not to smoke around you or leave cigarettes out where you can see them.

4. Keep trying: Be prepared for relapse. What if you do smoke? Don't be discouraged if you start smoking again. Remember, most people try several times before they finally quit. Here are some difficult situations to watch for:

Alcohol. Avoid drinking alcohol. Drinking lowers your chances of success.
Other smokers. Being around smoking can make you want to smoke.
Bad mood or depression. There are a lot of ways to improve your mood other than smoking.

How should I get ready to stop smoking?

Just before your stop date, get rid of all of your cigarettes, matches, lighters and ashtrays.

Quitters can approach their attempt in different ways.

Cold turkey: The phrase ‘going cold turkey’ means stopping smoking immediately. In other words if someone smoked a pack today, they would be going ‘cold turkey’ if from tomorrow they smoked none at all. Stopping outright is most likely to be successful.

Cutting down: Cutting down over a length of time can be particularly difficult, as consumption often goes back to what it was before. Smokers may inhale longer and harder to get the nicotine they want.

What will happen when I stop smoking?

How you feel when you stop depends on how much you smoked, how addicted your body is to nicotine and how well you get ready to stop smoking. They are called nicotine withdrawal symptoms. The symptoms are strongest during the first few days after you stop smoking, but most go away within a few weeks.

Craving

An intense desire to smoke which typically lasts 2 to 3 minutes before subsiding. This becomes less frequent and less intense during the first 3 weeks.

Increased appetite

Nicotine is known to suppress a person’s appetite, which leaves many smokers able to skip meals. When people give up, the resulting lack of nicotine can cause cravings, which may also be interpreted as hunger and an increased appetite cause weight gain.

Light-headed / dizzy feelings

This may occur as the level of carbon monoxide in the blood starts to fall and oxygen supply to the brain increases.

Tingling sensations in the body

This could be a sign of better circulation to the hands and feet.

Constipation

Tobacco has a laxative effect on which the bowels learn to rely.

The above are signs of recovery and all the symptoms are temporary and none of them are life threatening, unlike smoking!

How do I deal with urges to smoke?

If you have tried to quit before, you will probably recognize many of these common rationalizations.

I’ll just use it to get through this rough spot.
Today is not a good day; I’ll quit tomorrow.
It's my only vice.
How bad is tobacco, really? My uncle chewed all his life and he lived to be 90.
You've got to die of something.
Life is no fun without smoking.

You probably can add more to the list. As you go through the first few days without tobacco, write down any rationalizations as they come up and recognize them for what they are: messages that can trap you into going back to using tobacco.

Use the ideas below to help you keep your commitment to quitting:

Avoid: people and places where you are tempted to smoke. Later on you will be able to handle these with more confidence.

Alter: your habits. Switch to juices or water instead of alcohol or coffee. Take a different route to work. Take a brisk walk instead of a coffee break.

Alternatives: Use oral substitutes such as sugarless gum or hard candy, raw vegetables such as carrot sticks.

Activities: Learn how to handle stress and the urge to smoke Try and distract yourself from urges to smoke. Talk to someone, go for a walk, or get busy with a task. Take a hot bath, exercise, read a book.

Deep breathing: When you were smoking, you breathed deeply as you inhaled the smoke. When the urge strikes now, breathe deeply and picture your lungs filling with fresh, clean air. Remind yourself of your reasons for quitting and the benefits you'll gain as an ex-smoker.

Delay: If you feel that you are about to light up, delay. Tell yourself you must wait at least 10 minutes. Often this simple trick will allow you to move beyond the strong urge to smoke.

What you're doing is not easy, so you deserve a reward. Put the money you would have spent on tobacco in a jar every day and then buy yourself a weekly treat. Buy a book, go out to eat, or save the money for a major purchase.

What if I smoke again?

Staying stopped is the key issue for most smokers. And most relapses occur within the first 3 months of quitting. So don't be discouraged if you start smoking again. Don't feel like a failure. Think about why you smoked and what you can do to keep from smoking again. Set a new stop date. Many ex-smokers did not succeed at first, but they kept trying. In fact, smokers usually need several attempts before they are able to quit for good.

Just remember that even one puff on a cigarette can cause a relapse, so don't risk it.

International Workshop on Skull Base and Cochlear Implantation conducted by Dr. Kumaresh

Neurotology is a specialty which manages diseases and disorders of hearing, balance, cranial nerves, implantation otology and the skull base. The skull base is an extremely complex region which is a very tight space with limited visibility, in which enormous critical structures are encountered. In view of this surgery of the skull base is technically challenging since many lesions arise in close proximity to many vital neural and vascular structures.

The surgical approaches employed to obtain adequate exposure of the skull base may be extra cranial, intracranial, or, in many instances, a combination of these.

The four main routes by which lateral skull base lesions are typically approached are:

transtemporal
infratemporal
transfacial
intracranial

Extensive lesions may require a combination of these approaches to gain adequate exposure, and each approach has its own advantages and limitations.

The course objective was to improve understanding of the complex anatomy of the lateral skull base and to enhance the surgical skill in a variety of lateral skull base approaches and also teach Cochlear Implantation technique.

Dr.Kumaresh was the organizing Chairman and the invited National Faculty was Dr. Deepak Haldipur, the international faculty was my mentor and Director of the Skull Base Fellowship and of Otology & Neurotology Fellowship and Adult Cochlear Implant Program, Dr. Ravi N Samy.

The workshop was mainly on Transtemporal which includes transcochlear, translabyrinthine, and combined routes and the Middle Fossa Approaches, participants were provided a sound conceptual framework of these approaches and also on the anatomy of the middle cranial base. It was a three day program that included cochlear implantation surgery on the skull bones of cadavers thereby imparting necessary detailed instruction on the trantemporal approaches and initiating surgeons into cochlear implant surgeries.

The workshop was conducted at Ramiah Advanced Skill lab which is a state-of-the-art facility tailored specifically for skills development. The registrations were closed within 30 minutes of it being announced!!!!! This shows the success of the workshop. It was also because of the tireless efforts of the HOD and Professors at Ramiah, Dept. of ENT namely, Dr. Reddy, Dr. Chandrakiran and Dr. Srinivas with whom I was coordinating for the workshop.

The laboratory contained 6 stations with portable leica microscopes, Medtronic drills, skitter drills and implant dummies from Digisonic and Nurotron. Suction/irrigation and surgical trays were also available in each station.

My 5 year old son had hearing problem since over 2 years. We had visited at least 5 ENT doctors in well known hospitals, got tons of tests done and not much help.

Last doctor identified that it was because of Eustachian Tube Dysfunction but his treatment with nasal sprays gave only temporary relief and hearing problems reoccured within a month. Hearing problem was severely affecting his learning abilities and speech.

He is the most tech savvy doctor we had met, he shared a lots of links/videos about my son's condition/surgery. X-ray confirmed that he had Adenoid hypertrophy and that was the reason for his ETD. He did the surgery and son was back to normal next day. Since then son's hearing is completely normal and we think he will improve his speech very soon.

Avoid using "Ear Buds" for cleaning wax or for itching in the ears

Understand that ear wax is a sticky stuff that traps anything foreign that flies, crawls, or is blown into the ear canals. Dirt, tiny bits of plant material, small insects, bacteria, and so forth are immobilized by wax. The primary purpose of ear wax is to protect your ear canal and ear drum from such foreign materials.
Having said this how do you clean your ears? For one do not put anything inside the ears, ear wax migrates out of the canal all by itself and does not need your help.
Secondly use either a baby oil to the ears of kids once every fortnight and for adults use either baby oil, clove oil or commercially available wax solvents and your ears should be clean.
Warning: People who have a history of middle ear infections or a hole in the ear drum should NOT try these remedies, nor should they allow a physician to irrigate their ears. Do not self-medicate with persistent symptoms; the problem may be unrelated to wax and may be more serious.
Many patients who present with wax problems are regular users of ear-buds. Ear-buds, safety-pins and hair-pins, when used carelessly can push wax into the deeper parts of the ear-canal where wax is not formed. In such a situation, there would be further impaction of the wax. Hearing loss occurs when wax completely blocks the ear canal. Even a small amount of wax, if wedged between the ear drum and the ear canal wall, reduces the ability of the ear drum to conduct sound. Also, the skin of the ear canal and the eardrum is very thin and fragile and is easily injured. Ear wax can cause pain, infection, deafness and cough.
If removal is needed the best way is removal under direct vision by a specialist.
Irrigation or "syringing" is a standard method of wax removal. While this is a conventional and accepted method of ear wax removal, it has many disadvantages compared to removal under direct vision.
I recommend against using ear buds as well as putting other things into your ear such as hair pins. This can be dangerous because you run the risk of perforating the ear drum, as well as jamming wax deeper inside. It does not matter whether you are using the local or the International brand, the result is the same. In fact using the local ones being sold on the road side will make you dearer by few more rupees because the bud is made from discarded patient used cotton from the Govt. hospitals.
Unfortunately, I still get lots of patients who continue using ear buds, I had a 3 year child whose parents had used ear buds and had made a hole in the ear drum!!!. I also had a patient who had ear bud track in his ear and brain fluid was coming out. Another disadvantage is prolonged ear bud use can narrow your ear canal. My recommendation after Cancer Surgery

Dear Doctor
I am enclosing my recommendation:
" I was suffering from recurrent Mouth Ulcers and contacted Dr. Kumaresh from Phnom Penh, Cambodia (where I was working) by Phone and eMail and set up appointment to meet him as soon as I reached India. The moment I came to India and met him, he immediately identified my problem and on the same day Biopsy and CT Scan were done. On receipt of results, PET Scan was done to confirm Cancer in floor of mouth.
He decided it was best to
operate and remove the Jaw bone, Floor of mouth, part of tongue and some Lymph nodes in the left side of my face and replace the same with my Fibula (from Leg) and connected tissues and skin. He immediately identified one of the best Plastic Surgeons and the surgery was done with in a week in xxxxxxx Hospital, Bangalore. He recommended Post operative Radiotherapy and Chemotheapy to be done in xxxxxx Hospital.

Now I am substantially recovered from my surgery and am cured of Mouth Cancer. I am slowly recovering from the side effects of the Raidiation and Chemo Therapies.

All through my above ordeal, Dr. Kumaresh stood behind me like a rock and supported me in all my turbulations. Being a Cancer Survivor himself, he was quiet understanding and supportive in all stages of my pre and post operative stages of my treatment.

He is a good surgeon, a good care giver, very good in planning dfferent stages of treatment, good in identifying the supportive care givers and institutions but above all a very good human being, so much so that, he is now trying to help me find employment in Bangalore as I am not desirous of going back to Cambodia as Medical facilities there are negligible.

DR. KUMARESH IS A GREAT DOCTOR AND I WISH HIM WELL IN ALL HIS FUTURE ENDEAVOURS."

Inside the mind of Singapore's Youngest Millionaire

Some of you may already know that I travel around the region pretty frequently, having to visit and conduct seminars at my offices in Malaysia, Indonesia, Thailand and Suzhou (China). I am in the airport almost every other week so I get to bump into many people who have attended my seminars or have read my books.

Recently, someone came up to me on a plane to KL and looked rather shocked.. He asked, 'How come a millionaire like you is travelling economy?' My reply was, 'That's why I am a millionaire. ‘He still looked pretty confused. This again confirms that greatest lie ever told about wealth (which I wrote about in my latest book 'Secrets of Self Made Millionaires’). Many people have been brainwashed to think that millionaires have to wear Gucci, Hugo Boss, Rolex, and sit in first class in air travel. This is why so many people never become rich because the moment that earn more money, they think that it is only natural that they spend more, putting them back to square one.

The truth is that most self-made millionaires are frugal and only spend on what is necessary and of value. That is why they are able to accumulate and multiply their wealth so much faster. Over the last 7 years, I have saved about 80% of my income while today I save only about 60% (because I have my wife, mother in law, 2 maids, 2 kids, etc. to support). Still, it is way above most people who save 10% of their income (if they are lucky). I refuse to buy a first class ticket or to buy a $300 shirt because I think that it is a complete waste of money. However, I happily pay $1,300 to send my 2-year old daughter to Julia Gabriel Speech and Drama without thinking twice.

When I joined the YEO (Young Entrepreneur' s Organization) a few years back (YEO is an exclusive club open to those who are under 40 and make over $1m a year in their own business) I discovered that those who were self-made thought like me. Many of them with net worth's well over $5m, traveled economy class and some even drove Toyotas and Nissans (not Audis, Mercs, BMWs).

I noticed that it was only those who never had to work hard to build their own wealth (there were also a few ministers' and tycoons' sons in the club) who spent like there was no tomorrow. Somehow, when you did not have to build everything from scratch, you do not really value money. This is precisely the reason why a family's wealth

(no matter how much) rarely lasts past the third generation. Thank God my rich dad (oh no! I sound like Kiyosaki) foresaw this terrible possibility and refused to give me a cent to start my business.

Then some people ask me, 'What is the point in making so much money if you don't enjoy it?' The thing is that I don't really find happiness in buying branded clothes, Jewellery or sitting first class. Even if buying something makes me happy it is only for a while, it does not last. Material happiness never lasts, it just gives you a quick fix. After a while you feel lousy again and have to buy the next thing which you think will make you happy. I always think that if you need material things to make you happy, then you live a pretty sad and unfulfilled life.

Instead, what makes ME happy is when I see my children laughing and playing and learning so fast. What makes me happy is when I see my companies and trainers reaching more and more people every year in so many more countries. What makes me really happy is when I read all the emails about how my books and seminars have touched and inspired someone's life. This happiness makes me feel really good for a long time, much much more than what a Rolex would do for me.

I think the point I want to put across is that happiness must come from doing your life's work (be in teaching, building homes, designing, trading, winning tournaments etc.) and the money that comes is only a by-product. If you hate what you are doing and rely on the money you earn to make you happy by buying stuff, then I think that you are living a meaningless life.

Man of the Millennium.....

Mr. Kalayanasundaram worked as a Librarian for 30 years. Every month in his 30 year experience(service), he donated his entire salary to help the needy. He worked as a server in a hotel to meet his needs. He donated even his pension amount of about ten lakh rupees to the needy.

He is the first person in the world to spend the entire earnings for a social cause. In recognition to his service, the American government honoured him with the 'Man of the Millennium' award. He received a sum of Rs 30 crores as part of this award which he distributed entirely for the needy as usual.

Logical Thinking with a Lateral approach!

Many years ago in a small Indian village, A farmer had the misfortune of owing a large sum of money to a village moneylender. The Moneylender, who was old and ugly, fancied the farmer's beautiful Daughter. So he proposed a bargain.

He said he would forgo the farmer's debt if he could marry his Daughter. Both the farmer and his daughter were horrified by the Proposal. So the cunning money-lender suggested that they let Providence decide the matter. He told them that he would put a black Pebble and a white pebble into an empty money bag. Then the girl would Have to pick one pebble from the bag.

1) If she picked the black pebble, she would become his wife and her father's debt would be forgiven.
2) If she picked the white pebble she need not marry him and her father's debt would still be forgiven.
3) But if she refused to pick a pebble, her father would be thrown into Jail.

They were standing on a pebble strewn path in the farmer's field. As they talked, the moneylender bent over to pick up two pebbles. As he picked them up, the sharp-eyed girl noticed that he had picked up two black pebbles and put them into the bag.
He then asked the girl to pick a pebble from the bag.

Now, imagine that you were standing in the field. What would you have done if you were the girl? If you had to advise her, what would you have told her?

Careful analysis would produce three possibilities:
1. The girl should refuse to take a pebble.
2. The girl should show that there were two black pebbles in the bag and expose the money-lender as a cheat.
3. The girl should pick a black pebble and sacrifice herself in order To save her father from his debt and imprisonment.

Take a moment to ponder over the story. The above story is used with the hope that it will make us appreciate the difference between lateral and logical thinking.

The girl's dilemma cannot be solved with Traditional logical thinking. Think of the consequences if she chooses the above logical answers.
What would you recommend to the Girl to do? Well, here is what she did ....
The girl put her hand into the moneybag and drew out a pebble. Without looking at it, she fumbled and let it fall onto the pebble-strewn path where it immediately became lost among all the other pebbles.

"Oh, how clumsy of me," she said. "But never mind, if you look into the bag for the one that is left, you will be able to tell which pebble I picked."
Since the remaining pebble is black, it must be assumed that she had picked the white one. And since the money-lender dare not admit his dishonesty, the girl changed what seemed an impossible situation into an extremely advantageous one.

Lessons to Learn from This Story:

Ants Problem:
Ants hate cucumbers.
Keep the skin of cucumbers near the place or ant hole.

To get pure and clean ice:

Boil water first before freezing.

To make the mirror shine:
Clean with Sprite.

To remove chewing gum from clothes:
Keep the cloth in the freezer for an hour.

To whiten white clothes:
Soak white clothes in hot water with
a slice of lemon for 10 minutes.

To give a shine to hair:
Use one teaspoon of vinegar on hair, then wash hair.

To get maximum juice out of lemons:
Soak lemons in hot water for one hour,
and then juice them.

To avoid smell of cabbage while cooking:
Keep a piece of bread on the cabbage
in the vessel while cooking.

To avoid tears while cutting onions:
Chew gum.

To boil potatoes quickly:
Skin one potato from one side only before boiling.

To remove ink from clothes :
Put regular white toothpaste on the ink spots generously
and let it dry completely, then wash.

To skin sweet potatoes quickly:
Soak in cold water immediately after boiling.

To get rid of mice or rats:
sprinkle black pepper in places where you find mice or rats.
They will leave.

Four friends, who hadn't seen each other in 30
years, reunited at a party.

After several drinks, one of the men had to use the rest room. Those who remained talked about their kids. The first guy said, "My son is my pride and joy. He started working at a successful company at the bottom of the barrel. He studied Economics and Business Administration and soon began to climb the corporate ladder and now he's the president of the company. He became so rich that he gave his best friend a top of the line Mercedes for his birthday!"

The second guy said, "Darn, that's terrific! My son is also my pride and joy. He started working for a big airline, and then went to flight school to become a pilot. Eventually he became a partner in the company, where he owns the majority of its assets. He's so rich that he gave his best friend abrand new jet for his birthday!"

The third man said: "Well, that really is terrific! My son studied in the best universities and became an engineer. Then he started his own construction company and is now a multi-millionaire. He also gave away something very nice and expensive to his best friend for his birthday: A 30,000 square foot mansion!"

The three friends were congratulating each other just as the fourth friend returned from the restroom and asked: "What are all the congratulations for?" One of the three said: "We were talking about the pride we feel for our sons' success. ...What about your son?"

The fourth friend replied: "My son is gay and makes a living dancing as a stripper at a nightclub."

The three friends said: "What a shame...what a disappointment!"

Then fourth friend replied: "No, I'm not ashamed. He's my son and I love him. And he hasn't done too bad either. His birthday was two weeks ago, and he received a beautiful 30,000 square foot mansion, a brand new jet and a top of the line Mercedes from his three boy friends! Starting my Practise Nova Specialty Surgeries

It was a very memorable 5 years at Apollo in which I have seen ups and downs in both my personal and professional life.
Before I write about the new center I need to write a few lines about the top management of Apollo Bangalore.
It is not often that you see the management folks treat its doctors with humane touch or go out of the way to make sure the specialist is taken care of at their times of need.

CEO was extremely helpful during my tenure at Apollo. During my illness I was given special rooms to make me comfortable (Room cost around Rs.10,000-25,000). I got medicines at very discounted price and also certain privileges not heard of in other Institutions even within the same group. In fact he was one of the reasons for me continuing much longer than planned at Apollo. If not for him both my recovery and financial situation would have been miserable. I am thankful to the CEO and the DMS for accepting my request of visiting.

I will continue to operate and consult at both Apollo and Nova but for all practical purpose my consulting time at Apollo will be limited.
Coming to my new Avatar: I will be initiating new procedures and also setting up a state of art ENT OPD at Nova Specialty Surgeries (I will be at the Koramangala Unit).

I will be I/C of Africa, Middle East clinics and will also be consulting and operating monthly once at Chennai (To be started soon).

The Africa visit is already over and the foundation laid for my return visit in end April/early May

I will be starting the Balloon Sinuplasty and spearheading the Cochlear Implantation of the group along with Laser surgeries and also head and neck surgeries.

Added to this is the fact I will be doing routine CME's, workshops, working on a collaboration with my Alma Mater in US and many many more things which are a huge personal and professional lift.

The beauty of the place is surgeries can be done on Sundays, everyday starting at 6 AM and there is no need for an overnight stay and yet Insurance covers for all the procedures. This fits my schedule and mode of working perfectly. Added to this is the marketing, PR, web activity and what not.

All in all it is a huge personal and professional gratifying move and I hope in the long run it turns to be a right decision...

LETTER OF THE EDITOR OF "THE TIMES OF INDIA " TO THE PRIME MINISTER OF INDIA

I am born and brought up in Mumbai for last fifty eight years. Believe
me, corruption in Maharashtra is worse than that in Bihar . Look at
all the politicians, Sharad Pawar, Chagan Bhujbal, Narayan Rane, Bal
Thackray , Gopinath Munde, Raj Thackray, Vilasrao Deshmukh all are
rolling in money. Vilasrao Deshmukh is one of the worst Chief
ministers I have seen. His only business is to increase the FSI every
other day, make money and send it to Delhi , so Congress can fight
next election. Now the clown has found new way and will increase FSI
for fishermen, so they can build concrete houses right on sea shore.
Next time terrorists can comfortably live in those houses, enjoy the
beauty of the sea and then attack our Mumbai at their will.
Recently, I had to purchase a house in Mumbai. I met about two dozen
builders. Everybody wanted about 30% in black. A common person like me
knows this and with all your intelligence agency & CBI, you and your
finance ministers are not aware of it. Where all the black money goes?
To the underworld, isn't it? Our politicians take help of these
goondas to vacate people by force. I myself was victim of it. If you
have time please come to me, I will tell you everything.
If this has been a land of fools, idiots, then I would not have ever
cared to write to you this letter. Just see the tragedy. On one side
we are reaching moon, people are so intelligent; and on the other
side, you politicians have converted nectar into deadly poison.I am
everything - Hindu, Muslim, Christian, Scheduled Caste, OBC, Muslim
OBC, Christian Scheduled Caste, and Creamy Scheduled Caste; only what
I am not is INDIAN. You politicians have raped every part of Mother
India by your policy of divide and rule.
Take example of our Former President Abdul Kalam. Such an intelligent
person; such a fine human being. But you politician didn't even spare
him and instead choose a worthless lady who had corruption charges and
insignificant local politician of Jalgaon WHO'S NAME ENTIRE COUNTRY
HAD NOT HEARD BEFORE. Its simple logic your party just wanted a rubber
stamp in the name of president. Imagine SHE IS SUPREME COMMANDER OF
INDIA 'S THREE DEFENSE FORCES. What morale you will expect from our
defense forces? Your party along with opposition joined hands, because
politicians feel they are supreme and there is no place for a good
person.
Dear Mr Prime minister, you are one of the most intelligent person, a
most learned person. Just wake up, be a real SARDAR. First and
foremost, expose all selfish politicians. Ask Swiss banks to give
names of all Indian account holders. Give reins of CBI to independent
agency. Let them find wolves among us. There will be political
upheaval, but that will be better than dance of death which we are
witnessing every day. Just give us ambience where we can work honestly
and without fear. Let there be rule of law. Everything else will be
taken care of.
Choice is yours Mr. Prime Minister. Do you want to be lead by one
person, or you want to lead the nation of 100 Crore people?

It was really a surprise when I came to know that I was nominated for the Best ENT surgeon award given by the Brand Academy
Initially I was very skeptical as I have been receiving requests for award nominations and knew they were all fad.

Then I started doing my research and was convinced that these guys are not giving awards for money and they mean business.

I need to thank my patients for suggesting my name under the Top Doctors category and it is truly satisfying and gratifying to know that I was the only ENT surgeon to get this unique honor.
