I say "mere" because Seroquel alone brings in this much revenue to AZN in a few weeks. They made almost $5 billion on the drug last year. The settlement is barely dissuasive in the financial sense. We all know if you can get your product used in off-label areas where you can be the first atypical used, then your foothold will be strong and marketing costs will stay low. If you happen to get the indication later, then that's just icing on the cake (See Seroquel in Bipolar and Depression and Austism).&lt;br /&gt;&lt;br /&gt;Now, &lt;a href="http://online.wsj.com/article/BT-CO-20100809-702486.html"&gt;Astrazeneca is settling its lawsuits with individuals&lt;/a&gt;. Yesterday they announced that 17,500 of the suits had been settled for only $198 million. I don't know which firm AZN retains, but those suits earned every penny. $198 million barely registers. Probably 2 weeks of Seroquel sales. This is all great news for AZN as they work to put the bad press behind them. There are many lawsuits left, which is good because there isn't much time left for Seroquel which goes off-patent in March 2012 if AZN can fend off Teva for that long. As was rightly noted in the article, it's highly debatable that the study truly remained blinded as the effects of ketamine are well-known and were likely evident to the patients within an hour of infusion.  However, the study is interesting in terms of understanding how yet another product used off-label can provide assistance to this long-suffering population. One change that I believe is a good one for the reasons discussed &lt;a href="http://www.child-psych.org/2010/02/childhood-bipolar-disorder-is-not-bipolar-dsm-v-and-the-new-temper-dysregulation-disorder-with-dysphoria.html"&gt;here &lt;/a&gt;is the creation of the disorder known as Temper Dysregulation Disorder (TDD).  This disorder should significantly decrease the diagnosis of bipolar disorder in children.  With bipolar seen as a stigmatizing and lifelong disorder, the prospects for children diagnosed with the condition were not good, and the label itself could be self-defeating and self-esteem destroying.  To tag a child with such a severe disorder is truly dangerous from a psychological perspective.  Nonetheless, psychiatrists must provide diagnoses in order to insure insurance reimbursement for their clients at the least.  This new disorder will allow psychiatrists who had been reluctant to label a child as bipolar to have a new more attractive alternative tag to attach that is time-limited to childhood and carries none of the stigma of the bipolar diagnosis.&lt;br /&gt;&lt;br /&gt;So, we'll see a decrease in the diagnosis of bipolar disorder, but this will be more than offset by the diagnosis of TDD.  I can see the atypical psychotic marketing teams sitting in their conference rooms now correlating the symptoms of TDD and bipolar to see which TDD symptoms they can lawfully market for their product because they also happen to be symptoms of bipolar.  It may not even be necessary since atypicals are the clear choice among psychiatric drugs to treat the symptoms of TDD.  You might try an anti-depressant, but as soon as the parents complain, the next step will be the atypical. &lt;br /&gt;&lt;br /&gt;How long before the first company steps up to bat in a clinical trial to get the first antipsychotic label for TDD? Given that the ad is so depression-focused I wonder if the marketing team at AZ had been making this ad for the depression indication for which they're still waiting on FDA approval.  If you mute the ad and just look at it, it could easily be an ad for any of the products currently advertising for major depression.  I wonder if they just got tired of waiting on the approval and had the ad agency change the voiceover to accomodate the comparable indication that they do have in bipolar depression.  Just an outsider's guess.&lt;br /&gt;&lt;br /&gt;The ad itself is pretty depressing.  I know the Abilify ads were criticized for being too "sunny" in their conclusions, but geez this ad could use a little.  If I wasn't depressed before viewing it, I sure am now.  The fair balance is rough too.  All 53 seconds of it.  It's always hard to hear the drug you're taking has an increased risk of DEATH. &lt;br /&gt;&lt;br /&gt;The one thing this ad will accomplish if it doesn't get lost in the other 85 seconds is that some patients can take just 1 pill.  It's the last thing we hear before the fair balance kicks in.  This of course is basically the only advantage of Seroquel XR over it's soon-to-be-generic parent Seroquel.  However, the suggestion is erroneous.  Patients suffering from Bipolar disorder rarely can take a single medication to control their systems, esp. given the associated comorbidities to manage as well as the drugs to counter act the side effects of Seroquel.  Still, the 1 pill promise could drive patients to talk to their doctor about Seroquel which is the first step in this ad paying off for AZ. Now a product of Schering-Plough, Saphris is &lt;a href="http://atypicalanti-psychotic.blogspot.com/2007/11/pipeline-products.html"&gt;approved &lt;/a&gt;for use in bipolar and schizophrenia.  However, it enters a saturated market with competitors that have well-entrenched positions.  Additionally, it's the first to enter since generic risperidone became available.Further, Saphris fails to differentiate itself from the side effects ubiquitous in this class.  Given the hurldes, I would anticipate slow trial and uptake of Saphris without a heavy DTC focus or very aggressive pricing.  It probably won't be as bad as Invega unless they really blow it on the initial sales calls with over-promising efficacy.  The physicians are too savvy about this product to use it before third or fourth line in the class. There's really nothing new in the story beyond the reprehensible tactics we already knew about, but I guess what's truly shocking is how blatantly they discussed turning your problem patients into a Zyprexa zombie just to make your job easier as the nurse/physician. The representative and the mailing recommended or suggested a use for Seroquel and Seroquel XR that has not been approved by FDA, and thus created a new "intended use" for Seroquel and Seroquel XR for which the products lack adequate directions. Thus, these promotional activities and materials misbrand the drugs in violation of the Federal Food, Drug, and Cosmetic Act (Act), 21 U.S.C. 352(f)(1), and FDA implementing regulations. 21 CFR 201.1 00(c)(1) &amp;amp; 201.128.&lt;br /&gt;&lt;br /&gt;Background According to the Indications and Usage section of its FDA-approved product labeling (PI),1 Seroquel is indicated, among other things, for the following:&lt;br /&gt;&lt;br /&gt;Bipolar Disorder&lt;br /&gt;SEROQUEL is indicated for the treatment of both: • depressive episodes associated with bipolar disorder • acute manic episodes associated with bipolar I disorder as either monotherapy or adjunct therapy to lithium or divalproex. The efficacy of SEROQUEL was established in two identical8-week randomized, placebo-controlled double-blind clinical studies that included either bipolar i or II patients. Effectiveness has not been systematically evaluated in clinical trials for more than 8 weeks. The efficacy of SEROQUEL in acute bipolar mania was established in two 12-week monotherapy trials and one 3-week adjunct therapy trial of bipolar I patients initially hospitalized for up to 7 days for acute mania. The physician who elects to use SEROQUEL for extended periods in Bipolar Disorder should periodically re-evaluate the long-term  risks and benefits of the drug for the individual patient.&lt;br /&gt;&lt;br /&gt;Schizophrenia&lt;br /&gt;SEROQUEL is indicated for the treatment of schizophrenia.&lt;br /&gt;The efficacy of SEROQUEL in schizophrenia was established in short-term (6-week) controlled trials of schizophrenic inpatients. The effectiveness of SEROQUEL in long-term use, that is, for more than 6 weeks, has not been systematically evaluated in controlled trials. Therefore, the physician who elects to use SEROQUEL for extended periods should periodically re-evaluate the long-term usefulness of the drug for the individual patient.&lt;br /&gt;According to the Indications and Usage section of the FDA-approved Pi2 for Seroquel XR:&lt;br /&gt;SEROQUEL XR is indicated for the treatment of schizophrenia.&lt;br /&gt;The efficacy of SEROQUEL XR in schizophrenia was established in part, on the basis of extrapolation from the established effectiveness of SEROQUEL. In addition, the efficacy of SEROQUEL XR was demonstrated in 1 short-term (6-week) controlled trial of schizophrenic inpatients and outpatients.&lt;br /&gt;The effectiveness of SEROQUEL XR in long-term use, that is, for more than 6 weeks, has not been systematically evaluated in controlled trials. Therefore, the physician who elects to use SEROQUEL XR for extended periods should periodically re-evaluate the long-term usefulness of the drug for the individual patient.&lt;br /&gt;Promotion of Unapproved Use&lt;br /&gt;&lt;br /&gt;On Thursday, January 3,2008, at approximately 3:00 p.m., a sales representative from AZ in the - bt4)-made an unsolicited sales call to a physician at his office. The representative stated that Seroquel was approved for treatment of major depressive disorder (MDD). This representation was not made in response to a request for such information by the physician. As a result of this representation, the physician requested that AZ provide written information to support the claim that Seroquel has been approved for the treatment of MDD. In response to this request, AZ sent the physician a mailing dated January 4, 2008.  The mailing contained information about Seroquel and Seroquel XR's use for MDD, and  included summaries of eight clinical trials with referenced citations. This mailng was not the  result of an unsolicited request by the physician, but rather was prompted by the sales  representative's statements.&lt;br /&gt;According to their Pis, Seroquel and Seroquel XR are not FDA-approved for the treatment of MDD.3 Therefore, the oral statements made by the sales representative and the information provided in the January 4, 2008, mailng, misleadingly suggest a new "intended use" for Seroquel and Seroquel XR. Because the Pis for Seroquel and Seroquel XR lack adequate directions for this use, the drugs are therefore misbranded. Although the letter to the physician states that "(AZ) does not recommend the use of SEROQUEL or SEROQUEL XR in any other manner than as described in the enclosed prescribing information," this disclaimer is insufficient to mitigate the promotion of a new "intended use" for which the products lack adequate directions.&lt;br /&gt;Conclusion and Requested Action&lt;br /&gt;The oral statements made and information provided by AZ and its representative recommend or suggest a use for Seroquel and Seroquel XR that has not been approved by FDA, and  thus misbrand Seroquel and Seroquel XR in violation of the Act, 21 U.S.C. 352(f)(1), and  FDA implementing regulations. 21 CFR 201.1 00(c)(1) &amp;amp; 201.128.&lt;br /&gt;DDMAC requests that AZ immediately cease the dissemination of violative promotional materials for Seroquel and Seroquel XR such as those described above. Please submit a written response to this letter on or before December 15, 2008, stating whether you intend to comply with this request, listing all promotional materials (with the 2253 submission date) in use for Seroquel and Seroquel XR as of the date of this letter, identifying which of these materials contain violations such as those described above, and explaining your plan for discontinuing use of such violative materials. In truth, the big generic hit will start this quarter as the generics start shipping. Last quarter for Risperdal was a decline simply because the wholesellers quit building inventory. Invega is another story. Well, it just sucks. According to Dominic Caruso, Vice-President, Finance and Chief Financial Officer, "Invega is continuing to improve quarter after quarter but I would say consistent with our prior comments &lt;strong&gt;it’s not where we had expected it to be.&lt;/strong&gt; &lt;strong&gt;It’s not the launch we had hoped for&lt;/strong&gt; as we had commented before but we do see steady improvement with Invega. We believe it’s a viable alternative for patients and when used physicians comment very favorably about the effect with patients." PhRMA Bans Physician Gifts</title><content type="html">Oh what a sorrowful day it is as I mourn the loss of the ubiquitous pharmaceutical sales rep tchotchke.  Today, &lt;a href="http://www.phrma.org/news_room/press_releases/phrma_code_reinforces_commitment_to_responsible_interactions_with_healthcare_professionals/"&gt;PhRMA announced a voluntary ban&lt;/a&gt; on the pens, pads, clocks, clipboards, shirts and anything else you can brand that reps have been dangling to docs for years as added incentive to prescribe their product.  And, let me tell you, they did it because it worked.  So, if you're a rep, your bag just got a little lighter, but your job just got a lot harder.  If you don't have new clinical information for a physician then why in the world would he/she meet with you now - just drop those samples off at the front and move on to the next office. This stands in stark contrast to &lt;a href="http://atypicalanti-psychotic.blogspot.com/2008/07/first-generic-atypical-antipsychotic.html"&gt;yesterday's post on Risperdal &lt;/a&gt;which just went generic. This is good news for all the branded atypicals as now there is just risperidone as the sole generic. Had Seroquel gone generic it would've turned the atypical market upside down. Seroquel and Seroquel XR as branded products make up over a 30% share of all atypical antipsychotics as it is and with juggernaut new indications coming in Major Depressive Disorder and Generalized Anxiety Disorder, it's not going to get any smaller anytime soon. We all knew this was coming.  Teva was ready with product to ship and JNJ was ready with their own in-house generic version in hopes of cannibalizing some Teva sales at the expense of their former cash cow. &lt;br /&gt;&lt;br /&gt;What does this mean for the common man? &lt;br /&gt;&lt;br /&gt;Well if you're schizophrenic, bipolar, autistic, or an off-label user, it means that the price you pay just got a whole lot cheaper AND after Teva's 180 days of exclusivity as the sole competitor allowed to sell risperidone, the price will drop even lower as more competition enters the market. &lt;br /&gt;&lt;br /&gt;If you're a sale rep. for Invega (the bastard child of Risperdal), your sales goals just got a wee bit more difficult now didn't they because who wants to buy Risperdal-lite when you can get the original for pennies on the dollar.  Wonder what that already stellar *saracasm* Invega uptake curve is gonna look like now.&lt;br /&gt;&lt;br /&gt;And what if you're Abilify, Geodon, Seroquel or Zyprexa?  Well, you hope and pray that your marketing and sales folks have done a good job differentiating their product from the competition because the price differential between you and the generic just went exponential.&lt;br /&gt;&lt;br /&gt;And what of the insurers you may ask.  Can you say fail first? It continues to grow year over year and now has the pediatric bipolar and adolescent schizophrenia indications in its pocket to be the kid-friendly atypical anti-psychotic. I meant how many filings does it take for Seroquel to get an MDD indication? 7? Well, maybe. Let's see we've got 4 for monotherapy, 2 for adjuctive therapy to ongoing anti-depressant therapy (like Abilify), and 1 more for maintenance. That's quite a shotgun effect. How exactly to you plan positioning for a product in MDD with this many possible outcomes?&lt;/div&gt;&lt;div&gt;&lt;/div&gt;&lt;br /&gt;&lt;div&gt;The answer is it doesn't matter. Because regardless of the indication you still have to deal with the sedative, weight, and metabolic issues attached to your product and the class.&lt;/div&gt;&lt;div&gt;&lt;/div&gt;&lt;div&gt;I wonder how many they'll bring for Generalized Anxiety Disorder (GAD) in the next couple of months. Since Abilify's launch in late 2002, Astrazeneca, Jansen, Eli Lilly, BMS/Otsuka, and Pfizer have had free reign over the atypical anti-psychotic market. Starting with schizophrenia but gradually expanding to bipolar disorder, autism,  major depression, and generalized anxiety disorder coming soon, these products are infiltrating every corner of the mental health market in all age groups.  I don't have a lot to say here, just that's interesting how the increasingly more restrictive FDA is crushing potential competition through its tenor.  bifeprunox is dead.  asenapine is alive and may actually get the bipolar mania label. Vanda still has no sales force for iloperidone, Wyeth still has no chance with bifeprunox, and Seroquel still dominates the marketplace.  The only thing new is that Abilify is making a foray into the PCP arena now that they have the adjunctive depression label.  Risperdal and Zyprexa continue to lose share. Join us there!&lt;br /&gt;&lt;br /&gt;OK, OK, one brief comment on the use of atypicals in elder care facilities.  I think it's pretty unethical to be basically sedating non-psychotic grandmothers simply because the nursing staff doesn't want to come up with behavioral alternatives or are too understaffed to properly care for their patients. Perhaps others can help. Please feel free to comment in your products. A few off the top of my head are:&lt;br /&gt;&lt;br /&gt;&lt;a href="http://www.drugdevelopment-technology.com/projects/bifeprunox/"&gt;&lt;strong&gt;bifeprunox&lt;/strong&gt; &lt;/a&gt;- Wyeth/Solvay - got an approvable letter from FDA this year that basically puts any launch on hold until at least 2009. This product has a nice side effect profile especially with regards to weight; BMS/Otsuka and Pfizer are loving that this little piggie might not be coming to market since it might've munched on their share.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;iloperidone&lt;/strong&gt; - Vanda - this product has been around a long time; Vanda is the latest owner and they say they will commercialize it on their own, but would still consider partnering. Um, yeah. OK, this product's little claim to fame is that you can do a blood test to see if your patient is likely to suffer bad side effects from the drug and maybe learn about possible efficacy too. That's swell. Know many psychiatrists who like to do blood tests before tossing a product into this week's cocktail? Yeah me neither. What we've got in iloperidone is a product with little time left on its patent, from a company currently with ZERO CNS sales reps, and a story that requires a change in physician behavior. Good luck with that and your &lt;a href="http://today.reuters.com/news/articleinvesting.aspx?type=bondsNews&amp;amp;storyID=2007-11-01T165223Z_01_N01437725_RTRIDST_0_VANDA-STOCK-UPDATE-1.XML"&gt;latest debt offering too&lt;/a&gt;. Ouch.  Glad I sold that one.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;asenapine&lt;/strong&gt; - Pfizer/Organon - Oops, I meant just Organon because Pfizer saw the trial data and walked away. Now that's a ringing endorsement. This one actually may have the best chance of making it to market though. Despite an overall positive call and decent antipsychotic numbers, the news on Invega is little changed from what I suggested earlier. In today's &lt;a href="http://seekingalpha.com/article/50096-johnson-johnson-q3-2007-earnings-call-transcript"&gt;conference call&lt;/a&gt; Dominic Caruso, JNJ's CFO stated "With respect to Invega, we still continue to see in Invega as a promising product for the treatment of schizophrenia. I would tell you that we continue to see restrictions in the use of Invega based on formulary status, so whether it is prior authorization, et cetera, we continue to see that. It is a very difficult environment for new products in a new reimbursement arena. Those pressures are difficult to overcome without additional data on the product, especially additional data comparing it other products." In other words, uptake of Invega is still a trickle and whoever was handling payor negotiations has probably been replaced by now.&lt;br /&gt;&lt;br /&gt;Oh, and he also touted the study I said they'd tout in the call..."Now you may know that we have just recently, this past weekend, had a session on the head to head trial of Invega and Seroquel so it is recent data. That trial showed that Invega performed very nicely against the Seroquel placebo in terms efficacy and is very well-tolerated and in fact, the dropout rate for Invega patients is far lower than either Seroquel or even placebo." Um, maybe so, because everyone already knows patients (esp. severe acute schiz. pts.) drop out when the product they're taking doesn't work and Seroquel isn't much better than a placebo in short-term study of acute schizophrenia so yes, Invega had a lower dropout rate. Perhaps Dominic would also like to comment on the incidence of acute AE's in the study...I guess not.&lt;br /&gt;&lt;br /&gt;This is probably the best quote though: "We have taken a realistic view of Invega in both our guidance for this year and in the cost improvement programs that were previously announced." We here at JNJ accept that our product has failed to launch successfully, but we'll just cut some jobs and take what we can get until Risperdal goes generic. So, big shocker...in a trial of 400 patients with acute exacerbations of their schizophrenia symptoms who were willing to submit to hospitalization for at least 10 days, Invega showed better efficacy scores on the &lt;a href="http://www.ncbi.nlm.nih.gov/sites/entrez?cmd=Retrieve&amp;amp;db=pubmed&amp;amp;dopt=AbstractPlus&amp;amp;list_uids=3616518"&gt;PANSS &lt;/a&gt;than did Seroquel *yawn*. Anyone care to guess what percentage of the US population meets those criteria? An even better question is what percentage of Seroquel's patient population is at risk? It's got to be at least 2 maybe 3 patients. Why even bother with this study? OK, and I'm reaching here, perhaps Janssen is considering a re-launch with a focus on more severe patients representing a new willingness to cannibalize its own Risperdal share and perhaps steal some share from the perennial metabolic whipping boy Zyprexa. At least now JNJ will have something to tout on their conference call.&lt;br /&gt;&lt;br /&gt;One point I failed to mention in my last post on Invega is mentioned in the article. With regards to the slow uptake it notes "Invega sales got off to a slow start earlier this year partly because managed-care plans didn't give the drug as favorable a position on their preferred-drug lists as J&amp;amp;J, New Brunswick, N.J., would have liked." There's little doubt that this is true. JNJ was trying to suggest Invega was at price parity with Risperdal when in fact a one-day supply was about twice the cost. MCO's saw through it and either didn't add them to formularies or placed them on a different tier than JNJ had forecasted. Invega is the "newest" atypical antipsychotic on the market. It was launched at the beginning of 2007 by Janssen. Janssen also carries &lt;a href="http://www.risperdal.com/"&gt;Risperdal &lt;/a&gt;and wouldn't you know it, Risperdal is going to lose patent protection in the middle of 2008. How convenient that a new product would arrive to fill the sales reps bag and the physicians' sample closets just as the old product becomes generic. Perhaps a little too convenient. You see Invega is really not that different from Risperdal. In fact, it's actually a metabolite of Risperdal so the chemical structure is virtually &lt;a href="http://en.wikipedia.org/wiki/Paliperidone"&gt;identical&lt;/a&gt;. One would think Janssen would be spending the next year trying to convert as many patients as possible over to Invega since Risperdal revenue will dry up midway through next year when multiple generic versions of risperidone hit the market. However, it appears that rather than have a conversion strategy, they instead are trying to pass Invega off as a truly new and unique product. This would be the correct strategy if they could actually convince doctors of its veracity. But, given the uptake to date, they're not really winning the docs over. It's so bad that when &lt;a href="http://www.jnj.com/"&gt;Johnson and Johnson&lt;/a&gt; report &lt;a href="http://www.investor.jnj.com/"&gt;earnings&lt;/a&gt;, they combine Risperdal and Invega revenues together to avoid showcasing Invega's launch numbers. At least that's what they've done for the first 2 quarters. Quarter 3 numbers come out on 10/16 and although prescriptions have probably risen, it's unlikely that there's anything Janssen can do to change the path of the product's uptake at this time.
