On a

He was part of a team offering treatment for malaria and diarrhea, and was also setting up an HIV prevention program. He sat in the clinic examining a 5-year-old girl with malaria who, after three days of standard chloroquine treatment, was still vomiting and running a fever. “Her malaria was clearly resistant to the medicine. Drug resistance is a natural phenomenon, and all infectious diseases eventually develop resistance. New drugs must constantly be found,” Orbinski recounted in his book, "An Imperfect Offering." “Every year, worldwide, almost two million people die of malaria; 90 percent of these deaths occur in Africa, and because children have more delicate immune systems, 800,000 of those who die are children,” he wrote.

When Orbinski gave a lecture at Concordia University in Canada on “Global Health and Humanitarianism” on Sept. 25, 2008, he spoke about his experiences as president of DWB in Rwanda and about his work on HIV/AIDS, encouraging the audience to take action.

After DWB was awarded the Nobel Peace Prize in 1999, Orbinski dedicated the money toward the development of drugs for the "neglected" diseases. He announced the release of two new anti-malaria drugs.

In an interview, he explained that because malaria develops drug resistance, it is not simply cured with one treatment. In 1999, a public-private partnership, the Medicines for Malaria Venture, was set up to develop new drugs for malaria, but the process could take up to 10 years. “In the short term, there is an enormous need for drugs,” he said. “500 million people can’t wait for what may come.”

In his book, he explained that the Neglected Diseases Working group thought that by combining two existing anti-malaria drugs into one tablet, they might be able to develop an effective treatment in the short term.

“The problem was real and immediate, but there seemed to be no way to get anyone to do anything about it,” he said. “With no viable alternatives on the landscape, we decided to do it ourselves.”

In Oslo, when he gave his Nobel Prize address in 1999, he spoke of injustice outside the context of war. “Ninety percent of all death and suffering from infectious diseases occurred in the developing world,” he wrote in his book. “Around the world, two billion people had no access to essential medicines. Poverty is a driving force behind poor health, and that was not a problem we could solve. But as doctors, the lack of access to medicines was an injustice we could do something about.”

The fight for essential medicines was DWB’s next challenge. A global campaign was launched, drawing in more than 100 different non-governmental organizations as well as the World Health Organization. It was led by Dr. Bernard Pecoul, former general director of DWB in Paris, and Daniel Berman, a former marketing executive in the pharmaceutical industry.

In 2001, when Orbinski finished his term as DWB president, he worked as chairman of the Neglected Diseases Working Group. The question was, why was there so little drug research for diseases that affect those in the developing world?

“Returns on investment are not good enough for pharmaceutical industry and they are driven by profit,” he wrote in his book. “Governments have failed to ensure that wealth created by patent monopolies is directed towards global health needs.”

Thus DNDi was born. It was launched in 2003 as a not-for-profit pharmaceutical research and development organization that focused on four neglected diseases: sleeping sickness, chagas disease, leishmaniasis and malaria, all of which are neglected because their manifestation occurs in developing countries, where patients are too poor to attract investment for drug research.

Neglected diseases can be cured. The DNDi Web site defines malaria as the single largest cause of death for children under 5 in Africa, killing one child every 30 seconds, or 3,000 children every day.

A group of 40 scientists, drug policy analysts and drug development specialists as well as the WHO got involved in the DNDi. They surveyed the top 20 pharmaceutical companies in the world with Harvard University, and found that among the 11 companies that responded, eight spent nothing on research and development for diseases like African sleeping sickness.

They also found that only 0.2 percent of the $60 billion spent globally every year on drug research and development went towards tuberculosis, malaria and other diseases, which account for 18 percent of global mortality from all diseases.

Over the course of two years, they established a network of 15 African research centers linked with DWB, the Pasteur Institute and medical research councils in India, Malaysia, Brazil and South Africa.

In May 2001, when Orbinski met with World Trade Organization Director General Mike Moore to ask him for funding opportunities for DNDi, Moore told him that he was committed to finding the "right balance" among other the issues.

But “there is no right balance between life and death when life is possible,” Orbinski reflects in his book. “There is no right balance between justice and injustice when justice is possible.”

At the Kananaskis G8 summit in 2002, as well as at meetings with other potential donor governments, Orbinski asked for money for the DNDi. He returned empty-handed.

“This is why the start-up money had to come in from DWB because if we didn’t, no one would,” Orbinski explained in his book. “It is the responsibility of governments to provide the means for public healthcare,” he said in an interview.

As Orbinski concludes in his book, “we acted not to assume responsibility for the problem but to practically demonstrate that effective change and just alternatives are possible.”

Previously Published in UPI.com

http://www.upi.com/Features/Public_Health/Health_Industry/2009/03/16/Dr_Orbinskis_fight_for_access_to_medicine_the_DNDi_Initiative/12370750126462/

From time to time, you can see people working on the fields, with their rice-peak-hats, looking up and down on a typical humid, hot and sunny day.

But you can also see people taking a break. Outside their houses, they are laying down in hammocks, some sleeping, some talking or relaxing.

Kids are everywhere. On their bicycles. Playing, jumping around or racing.

The bus stopped and we got off at the Cu Chi District's Vietcong Tunnel site for tourists.

What I had seen through the windows now felt now real: It was humid, hot and sunny.

Humid, hot and sunny...

How did the Viet Cong soldiers survive the Vietnam War living in this Tunnel City?

Imagine you are the enemy, walking through the forest with your uniform on.

You look down at your boots, and you catch between the leaves a small, square wooden entrance.

This is how the Viet Cong soldiers entered their tunnels:

Viet Cong soldiers recycled rubber from the enemy's boots to make themselves sandals.

They also recycled the material of the napalm bombs and shells that were dropped around to make the traps. The enemy falling into their own traps...

prescriptive technologies are designs or compliance. When working within such
designs, a workforce becomes acculturated into a milieu in which external
control and internal compliance are seen as normal and necessary. Eventually,
there is only one way of doing something. The Chinese could probably not have
imagined making bronze in any other manner, just as we can’t imagine cars being

‘Silk market” in Beijing: After visiting the Great Wall of China, I went back to the capital for some shopping. I wasn't done reflecting about what it meant to have created a common cause for people to unite in order to build a national border. With this in mind, I went through the doors. I wasn't exactly sure what I wanted to buy yet. At first, I was only curious to explore what I would be offered at first sight.

It was overwhelming.

Chinese men and women approached me with their calculators ready to set a price to negotiate. Prada bags, wallets or belts? Gucci, Dolce & Gabana, or their favorite: Louis Vuitton???

"No, no thanks. fact, not only can such a wall not protect, but the structure itself is in
constant danger. Those parts of the wall left standing abandoned in
particular regions could easily be destroyed again and again by the nomads,
especially by those back then who, worried about the building of the wall,
changed their place of residence with incredible speed, like grasshoppers, and

whom was the great wall to provide protection? Against the people of the
north. I come from south-east China. No northern people can
threaten us there. We read about them in the books of the ancients.
The atrocities which their nature prompts them to commit make us heave a sigh
on our peaceful porches. In the faithfully accurate pictures of artists we
see the faces of this damnation, with their mouths flung open, the sharp
pointed teeth stuck in their jaws, their straining eyes, which seem to be
squinting for someone to seize, whom their jaws will crush and rip to
pieces. When children are naughty, we hold up these pictures in front of
them, and they immediately burst into tears and run into our arms. But we
know nothing else about these northern lands. We have never seen them,
and if we remain in our village, we never will see them, even if they charge
straight at us and hunt us on their wild horses. The land is so huge, it
would not permit them to reach us, and they would lose themselves in empty air."

 	“So
if things are like this, why do we leave our homes, the river and bridges, our
mothers and fathers, our crying wives, our children in need of education, and
go to school in the distant city, with our thoughts on the wall to the north,
even further away? Why? Ask the leadership. They know
us. As they mull over their immense concerns, they know about us,
understand our small worries, see us all sitting together in our humble huts,
and approve or disapprove of the prayer which the father of the house says in
the evening surrounded by his family."

	“Our
land is so huge, that no fairy tale can adequately deal with its size.
Heaven hardly covers it all. And Peking is only a point, the imperial
palace only a tiny dot. It’s true that, by contrast, throughout all the
different levels of the world the emperor, as emperor, is great. But the
living emperor, a man like us, lies on a peaceful bed, just as we do. It
is, no doubt, of ample proportions, but it could be merely narrow and
short. Like us, he sometime stretches out his limbs and, if he is very
tired, yawns with his delicately delineated mouth. But how are we to know
about that thousands of miles to the south, where we almost border on the
Tibetan highlands?"

	"Around the
emperor the glittering and yet mysterious court throngs—malice and enmity
clothed as servants and friends, the counterbalance to the imperial power, with
their poisoned arrows always trying to shoot the emperor down from his side of
the balance scales. The empire is immortal, but the individual emperor
falls and collapses. Even entire dynasties finally sink down and breathe their
one last death rattle. The people will never know anything about these
struggles and sufferings. Like those who have come too late, like
strangers to the city, they stand at the end of the thickly populated side
alleyways, quietly living off the provisions they have brought with them, while
far off in the market place right in the middle foreground the execution of
their master is taking place."

	"If
one wanted to conclude from such phenomena that we basically have no emperor at
all, one would not be far from the truth. I need to say it again and
again: There is perhaps no people more faithful to the emperor than we are in
the south, but the emperor derives no benefits from our loyalty. It’s
true that on the way out of our village there stands on a little pillar the
sacred dragon, which, for as long as men can remember, has paid tribute by
blowing its fiery breath straight in the direction of Peking. But for the
people in the village Peking itself is much stranger than living in the next
world. Could there really be a village where houses stand right beside each
other covering the fields and reaching further than the view from our hills, with
men standing shoulder to shoulder between these houses day and night?
Rather than imagining such a city, it’s easier for us to believe that Peking
and its emperor are one, something like a cloud, peacefully moving along under
the sun as the ages pass."

"It’s true that in the main things the blame rests with the government, which in
the oldest empire on earth right up to the present day has not been able or
has, among other things, neglected to cultivate the institution of empire
sufficiently clearly so that it is immediately and ceaselessly effective right
up to the most remote frontiers of the empire. On the other hand,
however, there is in this also a weakness in the people’s power of imagining or
believing, which has not succeeded in pulling the empire out of its deep
contemplative state in Peking and making it something fully vital and present
in the hearts of subjects, who nonetheless want nothing better than to feel its
touch once and then die from the experience."
