You at the threshold of your adult life – turning 22 in just 6 weeks, about to graduate and to walk down the aisle with your betrothed. The next thing you know, you’re in recovery being told you have a spinal cord injury at C5 and C6 and you’re paralyzed. What do you do?

Jim was ‘that guy’. This is Jim’s story. He spent 16 months in rehabilitation, and then went home to tie the knot with the woman he’d wanted to marry 16 months before. I asked Jim a fully loaded question – what did you do? He said, he grieved a bit – then he got on with life. Wow! did he ever! I could write an epic about all of Jim’s accomplishments – they are so many, and they’re all vital. But, his most important accomplishment (besides a devoted, and happy, wife and a daughter attending college, which are huge accomplishments in and of themselves) is the development of The PUMP.

As a tetraplegic, Jim knew that to combat urinary tract infections (UTIs) and also combat hypotension, he needed to drink a substantial amount of water. But, as a tetraplegic, he couldn’t use the toilets or urinals on his own. He relied completely on a urinary bag to contain what his body couldn’t hold. This is where the problem came in. He needed to consume a lot of water, but he was not able to empty his urinary leg bag on his own. Put yourself in his shoes – can you imagine, or do you have first-hand knowledge, of how embarrassing it is to ask a caregiver (or a co-worker, a neighbour or a friend) to empty your urinary leg bag for you all the time?

It was extremely difficult and expensive to find a professional caregiver or aid who could come on demand to empty his leg bag. When he was at work, he had to rely on other employees to empty his urine bag. It wasn’t just anyone who could empty it – they had to be shown how to do it. Jim recalls those days with embarrassment. When he was at home he’d have to telephone his wife when he needed his leg bag emptied, and she would have to leave her job to come tend to his urine bag. Jim knew first-hand how inconvenient and embarrassing it was to have to rely on a caregiver, a co-worker or his wife to empty his urinary leg bag.

Jim and Robert are the co-founders of The PUMP leg bag emptier. They met while they worked together for a major communications company. Incontinence: More widespread than you think

You’ve seen those commercials with the frequent bladder urges, and you’ve been hearing the phrase “over-active bladder.” So it seems that the once-taboo topic, urinary incontinence, is finally out of the closet. Over 25 million Americans suffer from some degree of incontinence— 75-80 percent of them women. Yet for all the attention this subject is receiving, “many continue assuming it’s part of the normal aging process—so they don’t bother discussing it with their doctor,” says the Albuquerque, New Mexico, urogynecologist Rebecca Rogers, M.D. In fact, incontinence, or loss of bladder control, is an eminently treatable medical condition—or rather several conditions.

It comes in varieties. Stress incontinence (associated with coughing, sneezing, and laughing), is caused by a weak sphincter muscle in the bladder. Urge incontinence (a.k.a overactive bladder), by contrast, is a sudden, uncontrolled need to urinate. This is due to bladder irritation, bowel problems, or neurological damage.

Causes/risk factors

In women, anything that places stress on the pelvic floor—pregnancy, childbirth, menopausal hormone loss—can cause incontinence.

In men, it tends to result from blockage from an enlarged prostate or from prostate surgery, says Dr. Rogers, professor of Obstetric Gynecology at the University of New Mexico Health Sciences Center.

Other triggers include obesity, smoking, caffeine, spicy or acidic food, carbonated beverages, diuretics and cardiac and blood pressure mediations, dehydration, and overhydration.

Decrease your risk

To minimize or avert incontinence, maintain a healthy weight, exercise frequently, eat plenty of fiber, avoid smoking, and practice behavioral training. For example, when you feel the urge to urinate, wait 10 minutes before heading to the bathroom. Gradually increase the delay to several hours.

Treatments Many treatment options are now available, “which can be tailored to suit the goals and expectations (of ) each patient,” says Dr. Rogers.

For stress incontinence:

Urethral inserts—to prevent leakage in women.
Radiofrequency therapy—to tighten and strengthen the urethral muscles.
Hormone therapy—topical estrogen, to help restore tissues in the urethra and vagina; testosterone supplements, to help strengthen the urethral muscles in women.

For urge incontinence:

Oral medications—to tighten or strengthen the urethral and pelvic floor muscles, or to relax overactive bladder muscles.
Botox—injected into the bladder muscles to deaden contraction producing nerves. It offers success in the 70 percent range, says Dr. Kobashi.
Electrostimulation, biofeedback, and pelvic-muscle exercises. Management Tools

Yet many women, including myself, don't know how to improve or prevent problems, where to go to for advice, or what treatment options are available.

Recently I read Hold It Sister, by Mary O'Dwyer. Mary is a 'Down Under Physical Therapist' from Down Under. She specializes in Women's Health and has helped thousands of women reconnect with their pelvic floor function and manage their continence, control prolapse and regain their orgasm.

Here are a few hints to help you get started:

1 - If you are traveling by plane, train or bus, try to always make certain that you pick an aisle seat, close to the washroom, so that you can simply access it when nature calls.

As a reminder, buses, trains & planes have limited trash bins. So, plan ahead for this concern & use an adult brief that has a higher absorbency rating (for example an overnight rating), so that you won't have to change as frequently. This also works best for those who don't feel any warning signs of an impending accident, or if the warning signs don't occur early enough to get to the bathroom.

When looking for adult diapers, it is necessary to use hospital grade products. Hospital grade incontinence products have other features, for about the equivalent cost. Hospital quality products offer enhanced odor control, more discreet fit, elastic legs, etc. When compared to store bought products, hospital grade products win hands down. Hospital quality incontinence products are ideal for lengthy trips.

This tip also applies to those who journey by vehicle for their holidays. In the US, rest stops can be found very easily on the key highways of the nation, but many other countries don't proffer this escape route. 2 - Pack all of your incontinence products into a bag (aka: suitable travel kit). Further underwear & outer clothes
Travel soap & disposable wash cloths (flannels) or wipes
Plastic bags for hygienic & easy disposal (you should be able to find 'nappy bags' at your neighborhood dollar store in the baby section, or then again, you can use 'poop bags' for dogs if you can't find diaper disposal bags).
Barrier cream - for any rashes or other skin conditions owing to incontinence.

3 - Watch what you scoff and gulp. If you are traveling for extensive periods (for example flights, trains, bus trips), limit your liquid intake preceding to your trip & throughout your trip. However, do not permit yourself to become dehydrated - doing so will trigger headaches, nausea, etc. So, a short list of those bits and pieces to avoid are:

Something with caffeine (ie: tea, coffee & pop)
Salty refreshments & drinks; such as chips (crisps), peanuts or other nut mixtures, soft drinks, etc,
Alcohol

4 - Plan Ahead. Make the most of your trip. If you arrange on visiting museums, parks, etc, make sure to have a map (usually found at the reception area or visitor's counter). Distinguish where the public restrooms are for instant & stress-free admission. Don't overlook to carry your "travel kit" (diaper bag) so you won't be short of supplies & left in an awkward position.

A splendid place to map out bathrooms, that we found, is the NAFC (The National Association for Continence). They have an interactive plan on their site that shows you where the washrooms are!

Always arrange for a bathroom stop about 30 minutes after meals, when on the road. Also, don't make any strong changes to your diet when on rest, as this could have extreme consequences. Always bring water with you, so you won't be tempted with caffeinated/salty drinks.

5 - Daily routines. It's very effortless when on holiday to overlook your natural practices. With incontinence, that cannot happen. Always make time for your pelvic floor exercises.
