फार्मासिस्ट😄: जी हाँ ! बताइये आप को कौन सी दवा चाहिए ?
मुन्ना😶: अरे भाई क्या बताऊँ, कल नये साल की पार्टी में कुछ ज्यादा हो गया !अगर आप के पास पेट साफ़ करने के लिए कुछ है तो दीजिये !डल्कोफ्लेक्स है क्या ?
फार्मासिस्ट😄 : लेकिन इस दवा के लिए आप के पास डॉक्टर का पर्चा होना चाहिए !
मुन्ना😶 :हैरान और परेशान होते हुए बोले,"अरे भाई क्या आप जानते हैं , टी वी पर डल्कोफ्लेक्स की ऐड दिन भर आती रहती है ! और हमें आप बोल रहें हैं कि डॉक्टर का पर्चा ले कर आयें ! यह ऐड हमारे लिए तो आते हैं ! अगर इस दवाई के लिए पर्चे कि ज़रुरत है तो इस कि ऐड टी वी पर क्यों आती है"?
मुन्ना😶 : क्या आप इन फार्मा कंपनियों से ज्यादा स्मार्ट हैं ?
फार्मासिस्ट😄 : नहीं नहीं मैं इन कंपनियों के आगे कहीं नहीं ठहरता ! और इन की स्मार्टनेस को कोई नहीं चैलेंज कर सकता है, क्यों कि इंडिया में सबसे स्मार्ट वो ही होता है जो सब से पहले नियमों को तोड़ता है और फिर गलत काम करता है और फिर वो अपने ग्रुप के लोगों के बीच इतराता है !
मुन्ना😶 : क्या मतलब ?
फार्मासिस्ट😄 : पहले तो ये कंपनियां ओ टी सी ड्रग्स की आड़ में गलत तरीके से अपनी ऐड (विज्ञापन) देते हैं और इसके बाद जब इनकी सेल बढ़ जाती है ,तो ये कंपनियां मिल कर बड़े बड़े होटलों में जश्न मनाते हैं !
फार्मासिस्ट😄 : इन कंपनियों को इस से कोई फरक नहीं पड़ता कि इनकी ऐड के कारण इन दवाईओं से किसी को साइड इफेक्ट्स , ओवर डोस या ड्रग रिएक्शन से नुक्सान से हो सकता है !
मुन्ना😶 : अच्छा तो भाई फिर ऐसा करो , मुझे कोई आयुर्वेदिक दवा दे दो , क्योंकि आयुर्वेदिक दवाईयाँ सेफ होती हैं ! पेट सफा है तो दे दो !
फार्मासिस्ट😄 :मुस्कराते हुए बोले, "भाई साहेब,

आयुर्वेदिक दवाईयाँ सेफ तो हो सकती हैं , लेकिन इसका मतलब यह नहीं कि आयुर्वेदिक दवाइयों के साइड इफेक्ट्स नहीं होते !"

अधिकतर पेट साफ़ करने वाली आयुर्वैदिक दवाईयाँ, आतों का तरल (fluids) पदार्थ तथा उसके साथ मिनरल्स (पोषक तत्वों) को सोख कर मल (stool) को बड़ा बनाती हैं और फिर इसके कारण मल त्याग की इच्छा होती हैं !
जब हम टी वी पर इनकी ऐड देख कर , बिना किसी डॉक्टर के परामर्श के इन दवाईयों को लगातार बार बार प्रयोग करते हैं तो ये दवाईयां हमें इस तरह से नुक्सान पहुंचाती हैं !

हमारी मास पेशियाँ , कोलन (colon) , हृदय, नस {nerves) आदि को सही प्रकार से काम करने के लिए सोडियम , पोटैशियम , मैग्नीशियम , फॉस्फोरस , एलेक्ट्रोलाईटस , मिनरल्स आदि एक balanced मात्रा में उपलब्ध होना चाहिए ! अगर ये तत्व हमारे शरीर से अधिक मात्रा में मल के ज़रिये बाहर निकल जायेंगे तो हमारे अंग सही प्रकार से काम नहीं करें गे ! जब हमारे शारीर से मल के साथ ज्यादा पानी निकल जाता है तो हमारे दूसरे अंगों के साथ हमारी किडनी पर भी इस का बहुत बुरा प्रभाव पड़ता है !

COLON
जब हम इन दवाईयों का अधिक देर और लगातार सेवन करते हैं , तो निर्देशित मात्रा की डोस (dose) पर कोलोन (colon) काम करना बंद कर देता है तथा कोलोन को काम करने के लिए पहले से अधिक मात्रा में दवा लेनी पड़ती है !
तथा कोलोन over stretched (कोलोन का साइज़ बढ़ जाना) हो जाने के कारण, कोलोन infection (कोलोन का संक्रमण) हो सकता है और साथ में लीवर damage होने का ख़तरा बन जाता है !
बहुत अत्याधिक तथा रोज़ाना इन आयुर्वेदिक दवाईयों के उपयोग से कोलोन (colon ) का कैंसर भी हो सकता है !
अगर ज़रुरत पड़ने पर हम इन आयुर्वेदिक दवाइयों का इस्तेमाल करते हैं तो शरीर के तरल पतार्थों और पानी की मात्रा की आपूर्ति के लिए हमें ज़्यादा मात्रा में पानी पीना चाहिए !
सबसे बेहतर यही हो गा कि आप अपने लम्बे समय के कब्ज़ के लिए अपने डॉक्टर से ज़रूर सम्पर्क करें , न कि विभिन्न प्रकार के विज्ञापनों पर निर्भर हो जायें !

The above quote proved right,when Charanjeet ma'am gave me a reason to smile and tie up with my work. She messaged me about fluids.Due to information provided by her , I got a chance to write this post on the topic of "FLUIDS".

So, I am very very thankful to madam,Charanjit Kaur. Because of the information provided by her, I am able to produce this article.

One thing I must like tell that,this is only the blog/post on whole of internet,where you can get the information about the various methods of preparing different kinds of fluids.

There are many types of pharmaceutical fluids which contain the dextrose and sodium chloride.Some of them are available readymade in the market , but some of them are not available in ready made form and hence they are made manually.

FLUID PREPARATIONS AVAILABLE READYMADE:

5% DEXTROSE
10% DEXTROSE
50% DEXTROSE
5% DEX N/2
5% DEX N/4

FLUIDS PREPARATIONS WHICH ARE TO BE PREPARED:

5% DEX N/6

Method ⟹ Discard 80 ml of 5% dex and 80 ml of N/S in 500ml.

2. 10% DEX N/2

Method ⟹ Discard 50 ml of 5% N/2 and add 50 ml of 50% dex in 500ml.

3. 10% DEX N/4

Method ⟹ Discard 50 ml dextrose of 5% N/4 and add 50 ml of 50% dex in 500ml.

4. 10% DEX N/6

Method ⟹ Discard 80 ml of 10% dex and add 80 ml of N/S.

5. 7.5% DEXTROSE

Generally , most of the people call me by the name of Paracetamol or Acetaminophen and they think that I am (paracetamol) is an NSAID (Non steroidal anti inflammatory drug). NSAID's are the analgesic drugs which reduces pain, fever, and inflammation.So, this is a wrong perception which many people have about me . Actually I am (pararcetamol) not an NSAID drug. The reason behind this is that I (Paracetamol) do not reduce inflammation. I relieve from fever , headache and pain but I never reduce inflammation.

Story of my birth (DISCOVERY OF PARACETAMOL)

Now listen to the story of my birth !

The name of my mother is Acetanilide. According to her I don't have any father. She says that my birth was not pre planned but rather I was born accidentely. About 100 years ago when my mother Acetanilide (a very similar molecule to Paracetamol ) was added to a patient's prescription , the pain killing properties of paracetamol was discovered.Since the nature of my mother Acetanilide is very toxic , even in moderate doses . Therefore, chemists modified the structure of my mother , the Acetanilide . They added an "OH" group to my mother.Thus a new compound was discovered and I was born and I was christened as Paracetamol. After taking the birth , I retained the analgesic(painkilling properties) of my mother , but now I was not as toxic as my mother , Acetanil

My self (PARACETAMOL)

As now you must have come to know that one of the main reasons for my development was the toxicity nature of my mother , Acetanilide.
Since I am not as toxic as my mother , so it does not means that you can take me as your wish.
Even my name is under O.T.C drugs. O.T.C stands for Over The Counter. Over The Counter are those medicines which do not require a doctor's prescription for it's availability from the pharmacy.
There fore I am freely available. You can find me in any pharmacy and everywhere. You can even find me in the pharmacies which are situated in super markets, bus stands and railway stations.
Now a medicine which is so free available in the market , it may be misused knowingly or unknowingly.

Now if you are misusing me knowingly , then only God can help you. But if you are ignorant about me , then you should know about me and my properties,benefits and demerits.

Never use me(paracetamol) during hang overs for relieving from pain or headache. Instead you can use NSAID'S like ibuprofen , aspirin and naproxen .
I can be more fatal when you take me after consuming alcohol , because the liver which is already working more to metabolize the the alcohol , can cause the lever damage when I get combined with the liver .

Always ask your doctor or a pharmacist for a proper dosage form of mine so that I (paracetamol) may not prove harmful for you. Generally adult dose must not exceed 4 grams or 4000 m.g of paracetamol per day in divided doses. The dose of children must be fixed according to their weight calculations.Taking excess doses of paracetamol can cause serious liver diseases. For fever , do not consume me for more than 3 days until directed by the doctor. For pain, do not consume me for more than 10 days .This is for adults. But children can take me only for 5 days during pain.

Use me (paracetamol) , to treat moderate pain and fever. You can use me for mild to moderate pains during headaches , toothaches , backaches , menstrual periods or during cold or flu aches.

There are many OTC or non prescription / prescription drugs in which I am already present in combination form , so do not take me along with those medicines without consulting your doctor or pharmacist.
Always try to check the label of your medicine to ensure that whether it contains the paracetamol / acetaminophen or not. Many medications of pain , fever , cough and cold may contain paracetamol already.

A PRESCRIPTION IS A PHYSICIAN'S WRITTEN LEGAL INSTRUCTION FOR PREPARATION AND ADMINISTRATION OF DRUGS OR MEDICAL DEVICE FOR AN INDIVIDUAL PATIENT.

Literally , the word prescription contains two words. The first word is PRE , which means before and the second word is SCRIPT which means writing or written and hence prescription is an order which must be written before the preparation of a drug.

A prescription consists of following parts:

SUPERSCRIPTION : A superscription is a type of heading which is denoted by a symbol Rx and this symbol is written above the inscription. It is a Latin word which means to take.
INSCRIPTION :Inscription is the body of the prescription. The names of ingredients of a prescription are written in the inscription area. The dose of a medicine and it's dosage forms are written in this area of the prescription.
SUBSCRIPTION : Subscription is a thing of past now. How ever in subscription area specific directions were used to written for preparing and compounding of medicines to the pharmacist. FOR EXAMPLE- "fill the 150 m.g of powder in 15 capsules and dispense". OR "make a suspension". In today's time doctor just writes the name of the syrup or capsule or tablet and it is readily available in the pharmacy.
SIGNATURA : The other name of signatura is (TRANSCRIPTION). The signatura or transcription is the direction how to take the drug , when to take the drug , for how many days a patient has to take the drug. LATIN TERMS are used for indicating these directions.

PRESCRIPTION DRUGS AND NON PRESCRIPTION DRUGS:

A prescription drug is a medicine which is prescribed by a doctor for a specific patient only and that drug must be bought from a legally authorized pharmacy only. A prescription drug is regulated through a Govt. agency of that country for approval , marketing , labeling of the drug.

A non prescription drug is also known as the OVER THE COUNTER (O.T.C) drug also. An OTC drug does not require doctor's prescription and it can be bought off-the-shelf in stores. An OTC drug can be marketed without the clearance of the Govt. agency.

MISUSE OF PRESCRIPTION DRUGS

Generally the prescription drugs are the strong medicines which are written by the well qualified physicians and doctors only. Every medicine has benefits as well as there are some risks also associated to it. Before prescribing a medication , doctor considers both benefit and risk to the patient , but when these medicines are self used because of their benefits without the consultation of any doctor , it is known as misuse of prescription drugs.

Manners in which the misuse of prescription drugs is done

One of the manners in which the misuse of a prescription is done buy acquiring some body's else legitimate medical prescription which contains strong pain killers or narcotic drugs.

Second way is that a person acquires the legitimate medical prescription by telling false complaints of pain or psychiatric problems to the doctor.

Who are misusers of prescription drugs?

Teen agers (both males and females ) , to get high or to feel euphoria , often are the misusers of prescription drugs.
In India , alcohol abusers also misuse the prescription drugs as the alcohol is much more costlier in India than pills.
In India , many low wage hard worker people also try to get the prescription drug in order to relieve from muscular pain and body aches and to have euphoria feelings.

I know , you must be knowing me by the name of BRUFEN or COMBIFLAM (combination of ibuprofen and paracetamol ). Basically I am an NSAID drug and I have a different chemical structure to steroids and I am not as much toxic. I am used to treat pain , fever and inflammation and some times I am also used in arthritis. I block the production of some natural substances known as prostaglandins , which are responsible for causing inflammation. Due to my this function , swelling , pain or fever is decreased.

IBUPROFEN

I was discovered by Stewart Adams in 1950. Thus I was born in 1950 and the name of my father is Stewart Adams.

When my father was at the age of 16 years , he left the school. At that time he had no future plans in his mind.

My father started an apprenticeship in retail pharmacy at Boots and later joined the degree programme in pharmacy at Nottingham University and after that he completed his PhD at Leeds University. After that he joined the research department of the Pure Drug Company Ltd.

Before my birth , aspirin was a prevalent drug used to be as a painkiller. But the problem associated with aspirin was that, it had to be taken in very high doses. This led to to risk of allergic reactions , indigestion , bleeding and various other allergic reactions and side-effects. This was the main reason why my father had a great quest for my birth.

My ( ibuprofen ) usefulness in daily life :

I relieve from muscle aches
I relieve pain from headache
I relieve from dental pain
I can be used in menstrual cramps
I am also useful in arthritis
I am also used to reduce fever and minor aches due to common cold and flu

Dose of Ibuprofen

At a time you can consume 800 m.g of me ( Ibuprofen ) , if you are an adult . In whole day , you can have 3200 m.g of ibuprofen in divided doses .

PRECAUTION

If you use me ( ibuprofen ) for a long time or in high doses , I can increase your risk of fatal heart attack or stroke. So , don't take me ( ibuprofen ) , if you are suffering with heart diseases.

Even people without heart disease or with risk factors could have a stroke or heart attack while taking ibuprofen.

DO'S and DONT'S

DO'S

Take me ( ibuprofen ) after every 4 to 6 hours with a full glass of water.
While taking me (ibuprofen ) , if you feel stomach upset , take me with food , milk or any antacid.

DONT'S

After taking me ( ibuprofen ) , do not lie down for at least 10 minutes.
If you are taking aspirin , to prevent heart attack or stroke , then don't take me ( ibuprofen ) because I can make aspirin less effective . Try to avoid me if you are a heart patient.But in any case , if the doctor suggests to take me ( ibuprofen ) along with the aspirin then , take me ( ibuprofen ) at least 8 hours before or 30 minutes after you take the aspirin ( non enteric coated form of aspirin ) .

The two main laws under which the medical products in India governed are :

The DRUGS AND COSMETICS ACT, 1940 :
The DRUGS AND MAGIC REMEDIES (objectionable advertisements ) Act, 1954 :

Let's see what are the importance of above two laws and what do they say .

The drugs and cosmetics act,1940, regulates the manufacture , import , distribution and sale of drugs and cosmetics.

The drugs and magic remedies act, 1954 , allows the assessments of advertisements or promotional material of any kind that tries to sell medicines and remedies on the basis of magic and miracle.

According to SCHEDULE (J) of DRUGS AND COSMETCS ACT, no medicines will be made to claim to prevent or cure the list of 51 diseases which are mentioned in the list.

Despite of having legislations meant for protecting the common man and consumers against the misleading and false advertisements, still there are advertisements with false claims and without any evidence.

A study was conducted to analyse the advertisements on medicines and treatments appearing in the newspapers of India. The truthfulness and rationality of the various advertisements in different newspapers were checked in these newspapers . The violation of the laws were also asessed.

In this study , a cross sectional methodology was employed , in which advertisements appearing from September 2012 to November 2012 were taken for study. These advertisements were taken from 10 English and Hindi daily newspapers . These daily newspapers covers Northern and Southern India .The advertisements data was collected and analysed on the basis of two legislation .

When the analysis of 82 advertisements were done it revealed shocking results that those advertisements were about sexual pleasure , menstrual problems , arthritis , hair problems , slimming , weight loss , infertility in males , enlargement of breasts , height gain and leucoderma .

Out of these 82 advertisements , 77 violated drugs and magic remedies act and provisions of schedule (j) of drugs and cosmetics act.

Hence , it was concluded that,"drugs advertised are false claims without having any evidence".

Those diseases which are listed in schedule (j) of the drugs and cosmetics act for which no one should claim to cure or advertise a cure.

The press and media and some times even the celebrities endorse the misleading advertised products. So, the government should take appropriate action against such types of advertisements which plays with the health of innocent people.
