(1) The internet is a new medium.
At least compared to print, it is. A website is a waste if it simply re-hashes something which could easily be put into print. Don't have the site be just an online brochure. Put up features which take advantage of the internet as a medium of communication. Filter information for them. Provide search capability. Provide interactivity with features like forums, quizzes and tools. Web visitors like to interact.

(2) Treat the Customer's Time as Valuable.
When a person visits your website, you have their attention for that point in time. You either need to use it or you will lose it - fast. Most visitors have short attention spans, what you need to design your site homepage so that it grabs their attention and provides what they are looking for right away. Its like walking into a restaurant. If you walk in and just stand there and nobody comes to greet you, you might wonder what is happening. But, if the hostess comes and greets you right away and walks you to a table, then you will be there for awhile and eat. The same analogy goes for websites. When posting links to other websites, don't just send your visitors somewhere else. They may never return. Provide them an exit page. Give them a pop-up when they try to leave your site. Or at the very least make external links open in a new window.

Pay attention to your site's statistics and react accordingly. What are people reading? How are they finding you? Do they just come and leave right from your homepage? How long as they are on your website? Do they return? This data is immensely valuable in fine-tuning your website based on customer needs and wants. Marrow transplant may have cured man’s AIDS

BERLIN - An American man who suffered from AIDS appears to have been cured of the disease 20 months after receiving a targeted bone marrow transplant normally used to fight leukemia, his doctors said Wednesday.

While researchers — and the doctors themselves — caution that the case might be no more than a fluke, others say it may inspire a greater interest in gene therapy to fight the disease that claims 2 million lives each year. The virus has infected 33 million people worldwide.

Dr. Gero Huetter said his 42-year-old patient, an American living in Berlin who was not identified, had been infected with the AIDS virus for more than a decade. But 20 months after undergoing a transplant of genetically selected bone marrow, he no longer shows signs of carrying the virus.“We waited every day for a bad reading,” Huetter said.

It has not come. Researchers at Berlin’s Charite hospital and medical school say tests on his bone marrow, blood and other organ tissues have all been clean.

However, Dr. Andrew Badley, director of the HIV and immunology research lab at the Mayo Clinic in Rochester, Minn., said those tests have probably not been extensive enough.

“A lot more scrutiny from a lot of different biological samples would be required to say it’s not present,” Badley said.

Apparent eradication
This isn’t the first time marrow transplants have been attempted for treating AIDS or HIV infection. In 1999, an article in the journal Medical Hypotheses reviewed the results of 32 attempts reported between 1982 and 1996. In two cases, HIV was apparently eradicated, the review reported.

Huetter’s patient was under treatment at Charite for both AIDS and leukemia, which developed unrelated to HIV.As Huetter — who is a hematologist, not an HIV specialist — prepared to treat the patient’s leukemia with a bone marrow transplant, he recalled that some people carry a genetic mutation that seems to make them resistant to HIV infection. If the mutation, called Delta 32, is inherited from both parents, it prevents HIV from attaching itself to cells by blocking CCR5, a receptor that acts as a kind of gateway.

“I read it in 1996, coincidentally,” Huetter told reporters at the medical school. “I remembered it and thought it might work.”

Roughly one in 1,000 Europeans and Americans have inherited the mutation from both parents, and Huetter set out to find one such person among donors that matched the patient’s marrow type. Out of a pool of 80 suitable donors, the 61st person tested carried the proper mutation.

Sometimes fatal treatment
Before the transplant, the patient endured powerful drugs and radiation to kill off his own infected bone marrow cells and disable his immune system — a treatment fatal to between 20 and 30 percent of recipients.

He was also taken off the potent drugs used to treat his AIDS. Huetter’s team feared that the drugs might interfere with the new marrow cells’ survival. They risked lowering his defenses in the hopes that the new, mutated cells would reject the virus on their own.Anthony Fauci, director of the National Institute of Allergy and Infections Diseases in the U.S., said the procedure was too costly and too dangerous to employ as a firstline cure. But he said it could inspire researchers to pursue gene therapy as a means to block or suppress HIV.

“It helps prove the concept that if somehow you can block the expression of CCR5, maybe by gene therapy, you might be able to inhibit the ability of the virus to replicate,” Fauci said.

David Roth, a professor of epidemiology and international public health at the London School of Hygiene and Tropical Medicine, said gene therapy as cheap and effective as current drug treatments is in very early stages of development.That’s a long way down the line because there may be other negative things that go with that mutation that we don’t know about.”

Even for the patient in Berlin, the lack of a clear understanding of exactly why his AIDS has disappeared means his future is far from certain.

“The virus is wily,” Huetter said. “There could always be a resurgence.”

Global AIDS crisis overblown?

LONDON - As World AIDS Day is marked on Monday, some experts are growing more outspoken in complaining that AIDS is eating up funding at the expense of more pressing health needs.

They argue that the world has entered a post-AIDS era in which the disease's spread has largely been curbed in much of the world, Africa excepted.
"AIDS is a terrible humanitarian tragedy, but it's just one of many terrible humanitarian tragedies," said Jeremy Shiffman, who studies health spending at Syracuse University.Roger England of Health Systems Workshop, a think tank based in the Caribbean island of Grenada, goes further. He argues that UNAIDS, the U.N. agency leading the fight against the disease, has outlived its purpose and should be disbanded.

"The global HIV industry is too big and out of control. We have created a monster with too many vested interests and reputations at stake, ... too many relatively well paid HIV staff in affected countries, and too many rock stars with AIDS support as a fashion accessory," he wrote in the British Medical Journal in May.

Paul de Lay, a director at UNAIDS, disagrees. It's valid to question AIDS' place in the world's priorities, he says, but insists the turnaround is very recent and it would be wrong to think the epidemic is under control.

"We have an epidemic that has caused between 55 million and 60 million infections," de Lay said. "To suddenly pull the rug out from underneath that would be disastrous."

U.N. officials roughly estimate that about 33 million people worldwide have HIV, the virus that causes AIDS. Scientists say infections peaked in the late 1990s and are unlikely to spark big epidemics beyond Africa.

In developed countries, AIDS drugs have turned the once-fatal disease into a manageable illness.

England argues that closing UNAIDS would free up its $200 million annual budget for other health problems such as pneumonia, which kills more children every year than AIDS, malaria and measles combined.

"By putting more money into AIDS, we are implicitly saying it's OK for more kids to die of pneumonia," England said.

His comments touch on the bigger complaint: that AIDS hogs money and may damage other health programs.

By 2006, AIDS funding accounted for 80 percent of all American aid for health and population issues, according to the Global Health Council.

In Ethiopia, Rwanda, Uganda and elsewhere, donations for HIV projects routinely outstrip the entire national health budgets.

'Gross misallocation of resources'
In a 2006 report, Rwandan officials noted a "gross misallocation of resources" in health: $47 million went to HIV, $18 million went to malaria, the country's biggest killer, and $1 million went to childhood illnesses.

"There needs to be a rational system for how to apportion scarce funds," said Helen Epstein, an AIDS expert who has consulted for UNICEF, the World Bank, and others.

AIDS advocates say their projects do more than curb the virus; their efforts strengthen other health programs by providing basic health services.

But across Africa, about 1.5 million doctors and nurses are still needed, and hospitals regularly run out of basic medicines.Diarrhea kills more kids
Experts working on other health problems struggle to attract money and attention when competing with AIDS.

"Diarrhea kills five times as many kids as AIDS," said John Oldfield, executive vice president of Water Advocates, a Washington, D.C.-based organization that promotes clean water and sanitation.

"Everybody talks about AIDS at cocktail parties," Oldfield said. "But nobody wants to hear about diarrhea," he said.

These competing claims on public money are likely to grow louder as the world financial meltdown threatens to deplete health dollars.

"We cannot afford, in this time of crisis, to squander our investments," Dr. Margaret Chan, WHO's director-general, said in a recent statement.

Some experts ask whether it makes sense to have UNAIDS, WHO, UNICEF, the World Bank, the Global Fund plus countless other AIDS organizations, all serving the same cause.

"I do not want to see the cause of AIDS harmed," said Shiffman of Syracuse University. But "For AIDS to crowd out other issues is ethically unjust."De Lay argues that the solution is not to reshuffle resources but to boost them.

"To take money away from AIDS and give it to diarrheal diseases or onchocerciasis (river blindness) or leishmaniasis (disfiguring parasites) doesn't make any sense," he said. "We'd just be doing a worse job in everything else."

HIV/AIDS Affecting Formerly Low-Risk Populations in Indonesian Province

Formerly low-risk groups such as married women and young children are contracting HIV/AIDS at higher rates in Indonesia's West Nusa Tenggara province during the past year, the Jakarta Post reports. According to the province's AIDS Eradication Commission, or KPAD, 25 married women and six young children contracted HIV last year. As of November 2008, 10 people had died from the disease, Rohmi Khoiriyati, KPAD secretary, said. KPAD as of November 2008 also recorded 25 new HIV cases, including three among people who had developed AIDS.
Rohmi said the province's HIV prevalence is "a matter of grave concern," particularly because the virus has affected married women and young children "despite the fact that they are not included in the high-risk category." According to KPAD, more married women contracted HIV in 2008 than people from populations considered at "high-risk" for the disease, including injection drug users and commercial sex workers. However, the actual HIV prevalence in the province could be greater than the reports indicate, Rohmi said. According to KPAD, it is difficult to know how many people are living with HIV in the country because many people are unaware of their status and there are a limited number of voluntary counseling and testing centers. Four sites in West Nusa Tenggara currently provide HIV testing, the Post reports. Rohmi said that encouraging people to seek medical attention for HIV will require a concerted effort to raise awareness and combat stigma associated with the disease.

West Nusa Tenggara province has reported 137 HIV cases and 70 AIDS cases since 2001, the Post reports. Since that time, 49 people have died from AIDS-related illnesses, Rohmi said. She added that people living with HIV in the province are able to receive medical treatment (Panca, Jakarta Post, 1/6).
