Norwill Fragoso: Familia, lo que parece un retoquito sencillo, puede convertirse realmente en una pesadilla, porque cada vez son más las personas que sufren complicaciones por inyectarse fillers con manos inexpertas y hoy te contamos los riesgos que debes conocer antes de hacerlo, así que recibimos al Oculoplástico, el doctor José Raúl Montes. ¡Bienvenido!

Dr. Montes: ¡Gracias Norwill! Para mí es un placer y buenos días a todos ustedes, también.

Norwill Fragoso: José Raúl, hablemos de esto. ¿Quiénes realmente están capacitados para poner los fillers? Por ejemplo, muchas personas pueden ven los fillers como algo puede ser sencillo. ¿O sea, eso se puede convertir en una situación catastrófica?

Dr. Montes: Eso es así, Norwill. Hoy voy a ser portador de las noticias malas. Las sustancias de relleno, cuando se inyectan en la cara, sobre todo —y en cualquier parte del cuerpo— no hay una zona facial que no te predisponga ti a un émbolo a la circulación de la retina y del ojo. Lo que estoy diciendo en palabras más claras es que estas partículas pueden atravesar a través de las arterias de la cara y llegar al ojo, llegar a la órbita … y en ese momento el paciente va a sentir dolor, va a decir que ve borroso, se va a marrear. Si usted no está preparado para hacer un examen visual en ese momento, ese paciente puede perder su visión totalmente.

Norwill Fragoso: Okay, ¿entonces cuáles son las complicaciones más serias que usted ha visto que pueden ocurrir con estas inyecciones?

Dr. Montes: Pues mira, yo he visto esta complicación. A mí me han llamado a la oficina (de repente) una persona que ha estado inyectando y me dice, doctor, tengo una paciente que le he inyectado esto, la paciente se mareo, esta vomitando y dice que ahora ve borroso. Inmediatamente, la transportamos a las clínicas, le hacemos un examen visual (que incluye fluoresceína del fondo del ojo, examen de la parte de atrás del ojo hasta una resonancia magnética, para estar seguro de que no hay una sustancia, un stroke), que esto puede ocasionar también accidentes cerebro vasculares. Esta paciente, pues hay que tratarla inmediatamente y tratara de disolver esa sustancia de relleno para que no siga ocluyendo la arteria, o sea, estamos hablando de que estos tratamientos a veces se minimizan, a veces se trivializan, pero tienen riesgos grandes y complicaciones que pueden ser catastróficas. Yo espero que aquí podamos ver algunas fotos que yo sometí para que ustedes vean cómo se ven. El paciente a veces es obvio. En la piel, el paciente sufre lo que se llama necrosis* (que necrosis es que de momento la piel pierde el flujo vascular y se pone oscura, cambia de color, la piel se pone blancuzca), o sea, pierde es color y poco a poco esa piel se empieza a desgastar, se empieza a infectar (también) y puede haber cicatrices permanentes.

Norwill Fragoso: Hay personas que han tenido efectos secundarios o que dicen que les dio una reacción alérgica. ¿Eso suele suceder?

Dr. Montes: Eso también puede suceder. Hay personas que son alérgicas a los componentes de estos inyectables y las alergias (tú sabes que) pueden ocurrir inmediatamente como que también pueden ser crónicas. Pacientes que tienen las respuestas rápidas pues se van a hinchar de momento exageradamente y tú tienes que estar dispuesto a intervenir para que ese paciente no se comprometa, hasta respiratoriamente. Hay otros que pacientes que desarrollan estas reacciones, lo que se llama delayed o retardada. Con el paso del tiempo yo he visto pacientes que se han inyectado sustancias de relleno —sobre todo en el área alrededor de los ojos— y las complicaciones se han visto un año después, seis meses después, cinco meses después. Lo que aquí es importante es que el médico hace esto tiene que ser un experto en poder diagnosticar cuál es la complicación y poder intervenir para que esta complicación se resuelva.

Norwill Fragoso: Así que usted no se puede poner en manos de cualquiera. Usted tiene que ir a un médico que sea experto, porque esto genuinamente ocasiona problemas reales de salud.

Dr. Montes: … muchas veces irreversible. Si u paciente se queja de que no está viendo bien, tienes que hacerle un examen visual. Si el paciente dice que no ve luz, que no ve movimiento de la mano, porque es tan sencillo como hacer esas pruebas, posiblemente la pérdida de visión sea irreversible.

Norwill Fragoso: What might seem like a simple little touch-up can turn into an actual nightmare. This is due to an increasing number of people suffering complications from having fillers injected by inexperienced hands. Today, we are going to discuss the risks you need to be aware of before undergoing such a procedure. So, let’s welcome our guest: Oculoplastic Surgeon, Dr. José Raúl Montes. Welcome!

Dr. Montes: Thank you, Norwill! It is a pleasure to be here, and good morning to all of you as well.

Norwill Fragoso: José Raúl, let’s discuss. Who is qualified to administer fillers? For instance, many people might view fillers as something relatively simple. I mean, can this really turn into a catastrophic situation?

Dr. Montes: That is absolutely the case, Norwill. I am sorry to be the bearer of bad news today. When dermal fillers are injected into the face —or indeed anywhere else on the body— there is not a single facial zone that does not carry the risk of triggering an embolism that affects the circulation of the retina and eye. Basically, these particles can travel through the facial arteries and reach the eye —or even the eye socket. At that moment, the patient will experience pain, report blurred vision and dizziness. If you are not prepared to perform a visual examination right then and there, that patient could lose their sight completely.

Norwill Fragoso: Okay, so what are the most serious complications occurring with these injections which you have personally witnessed?

Dr. Montes: Well, I have indeed seen this complication firsthand. I have received urgent calls at my office from someone who was administering injections, saying: «Doctor, I have a patient here whom I just injected with this product; she is feeling dizzy, she is vomiting, and now she says her vision is blurred.» We immediately transport her to the clinic and perform a visual examination —which ranges from fluorescein angiography of the fundus (an examination of the back of the eye) all the way to an MRI scan— to ensure there is no foreign substance present or evidence of a stroke, as these procedures can, in fact, trigger cerebrovascular accidents. This patient requires immediate treatment; we must attempt to dissolve the filler substance to prevent it from continuing to occlude the artery. In other words, we are talking about treatments that are sometimes downplayed or trivialized yet carry significant risks and potential complications that can be catastrophic. I hope we can display some of the photos I submitted here so you can see what these cases look like. Sometimes, the condition is immediately obvious on the patient’s skin. The patient suffers from what is known as necrosis (a sudden loss of vascular blood flow to the skin, causing it to darken or change color; the skin may turn whitish) —essentially losing its natural hue—and gradually begins to break down and become infected, potentially resulting in permanent scarring.

Norwill Fragoso: There are people who have experienced side effects or claim to have suffered an allergic reaction. Does that happen often?

Dr. Montes: That can certainly happen as well. Some individuals are allergic to the specific components found in these injectable fillers. As you know, allergic reactions can manifest immediately or develop into chronic conditions over time. Patients who experience rapid-onset reactions may suddenly develop severe, exaggerated swelling; the physician must be prepared to intervene immediately to ensure the patient’s condition does not become compromised —potentially even affecting their ability to breathe. Other patients develop what are known as «delayed» reactions. Over the years, I have seen patients who got filler injections —particularly in the area around the eyes— where complications did not manifest until a year later, six months later, or even five months later. The crucial point here is that the physician performing this procedure must be an expert —capable of accurately diagnosing the specific complication and intervening effectively to resolve it.

Norwill Fragoso: So, you cannot simply put yourself in just anyone’s hands. You must go to a doctor who is an expert, because this can genuinely cause health problems.

Dr. Montes: … often irreversible ones. If a patient complains that they are not seeing well, you must perform a visual examination. If the patient says they cannot see light —or cannot even see hand movements (because it really is as simple as conducting those tests)— then the vision loss may very well be irreversible.

Pamela Noa: … Y el doctor habla claro, o sea, si no funciona, pues o funciona.

Dr. Montes: Exactamente.

Norwill Fragoso: Entonces, ¿qué es lo que hace particular esta técnica?

Dr. Montes: Esta técnica, yo creo que podemos ver un video, porque no tenemos tanto tiempo para que vean cómo funcionan y voy a hablar sobre este vídeo. Desde hace veinte años, se están hablando de láseres que actúan básicamente creando calor y energía sobre la piel. Estos láseres de CO2 se conocen como láseres ablativos (es decir, queman, producen un gran efecto,) pero la desventaja es que la recuperación de esto es larga y tediosa y dolorosa para el paciente. Ha pasado. Me ha pasado y yo he utilizado este laser de CO2 por veinte, veinticinco años. Sin embargo, cuando yo veo un láser que es capaz de reproducir resultados con una eficacia aún mayor, increíble, donde el paciente apenas siente dolor, la recuperación es rápida y se hace en corto tiempo. Esto se llama Ultra Clear, que es una fibra.

Pamela Noa: Okay Doctor, tenemos muchas preguntas.

Este paciente, miren este paciente en un antes y después (cinco meses después), la recuperación de este paciente. El propósito para este paciente era para mejorar, recoger parte del tejido periocular que tiene pérdida de elasticidad, pero este laser lo que tiene es que es bien versátil. Tú quieres hoy día —y esto tú lo entiendes muy bien, Pamela y Norwill también— porque lo hemos hablado. Si hoy día el rejuvenecimiento facial no se trata de tratar de estar inyectándose cosas en la cara y estar creciendo en la cara y no verse natural. Se trata de verse natural y esto es lo que se conoce como prejuvenation. ¿Por qué? Porque lo principal —y todos los sabemos— es la piel. Ahí está la clave de verse bien a través de todas las décadas de tu edad.

Norwill Fragoso: Una pregunta, ¿a qué edad es recomendable someterse a este tratamiento?

Dr. Montes: Okay, tremenda pregunta. Si tú lo que quieres es mejorar la textura de la piel (tú tienes un problema como éste), tú puedes hacerte este láser desde los veinte, treinta años. Si tú eres una persona aún más joven que tiene una cicatriz en la piel por algún accidente, se puede hacer el láser también una edad más joven.

Pamela Noa: ¿Y entonces Doctor? Voy a traer mi caso porque sé que probablemente hay muchas personas que nos están viendo les pasa lo mismo. En mi caso, me piel es ultra sensitiva (es rosácea), o sea, que ya de por sí se me hincha, se me seca, etcétera. Una persona como yo es candidata para hacerse un láser.

Dr. Montes: Tú lees mentes, porque de eso iba a hablar yo ahora. Esto es otra de las maravillas de este laser. Nosotros todos tenemos fototipos de piel. Tenemos de I a V —para que tengan una idea— y los puertorriqueños aquí somos tipo III, tipo IV. Eso quiere decir que, si vamos a la playa, pues nos vamos a broncear un poquito. Tal vez nos quemamos un poco. Tipo IV se broncea mas fácil, se quema menos. El tipo V de piel casi nunca se quema y se broncea y el VI siempre se broncea (es decir, son las pieles más oscuras). Antes los láseres estaban prohibidos para pieles que fueran superior a tipo III. Es decir, se crearon todas estas tecnologías para la población caucásica nórdica solamente y esa tecnología esta ideada para cubrir todos los tipos de piel, condiciones de piel inflamatorias (como la que tú tienes, que es una rosácea leve y muy bien controlada).

Pamela Noa: Gracias. Estamos trabajando en ella.

Dr. Montes: Well, we are going to talk about a laser, and I have a confession to make. You know that whenever we talk about new technologies, there are tons of them on the market, and as you know (thanks to social media), people get absolutely flooded with information. One claims to work, the other one does not, but often, they just do not work. I am someone who conducts exhaustive research before incorporating anything into my practice. I have been tracking this specific technology since it first launched two or three years ago, and I have seen the results firsthand. I served as an evaluator during the clinical trials, and I remember telling myself: «This is exactly what I want for my practice —because I want it for myself, too.» In other words, it is one of those things you incorporate because you want to use it on yourself.

Pamela Noa: … And the doctor speaks candidly: either it works, or it does not.

Dr. Montes: Exactly.

Norwill Fragoso: So, what is it that makes this specific technique so unique?

Dr. Montes: Regarding this technique —I think we can roll a video now, since we do not have a lot of time— so you can see how it actually works. I will provide commentary over the footage. For the past twenty years, the conversation around lasers has centered on devices that function primarily by generating heat and energy within the skin. These CO2 lasers are known as “ablative” lasers —meaning they essentially burn the tissue to produce a dramatic effect— but the major downside is that the recovery process is long, tedious, and painful for the patient. That has been the reality until now. It has happened to me, and I have been using this CO2 laser for twenty, twenty-five years. However, when I see a laser capable of reproducing results with even greater efficacy, incredible, where the patient feels hardly any pain, recovery is quick, and the procedure itself is completed in a short amount of time. This is called Ultra Clear, which utilizes a fiber-optic delivery system.

Pamela Noa: Okay, Doctor, we have a lot of questions.

Dr. Montes: These are micro-channels.

Pamela Noa: Look at the difference.

Norwill Fragoso: It is incredible.

Pamela Noa: Yes. Okay, Doc. When you say the recovery is faster, roughly how long are we talking about? I imagine it depends on the individual, but just a ballpark figure.

Dr. Montes: I perform CO2 laser treatments every week, at least twice a week, on various patients, and they achieve incredible results. However, recovery takes four to six months for the skin tone to return to what it was before. Take this patient, for instance —look at this patient in this «before and after» comparison (five months later)— look at this patient’s recovery. The objective for this specific patient was to improve and tighten some of the periocular tissue —the skin around the eyes— which had lost its elasticity. The great thing about this laser is that it is incredibly versatile. Nowadays —and you understand this very well, Pamela, as does Norwill— because we have discussed it before … nowadays, facial rejuvenation is not about constantly injecting things into your face, making your features look unnaturally inflated, or failing to look natural. It is about looking natural, and this is what is known as «prejuvenation.» Why? Because the most important factor —as we all know— is the skin itself. That is the key to looking your best throughout every decade of your life.

Norwill Fragoso: One quick question: at what age is it recommended to undergo this treatment?

Dr. Montes: Okay, excellent question. If your goal is simply to improve your skin texture —if you have a specific issue like the one shown here— you can undergo this laser treatment starting in your twenties or thirties. If you are a much younger person who has a scar resulting from an accident, laser treatment can also be performed at a younger age.

Pamela Noa: So, Doctor? I am using my own situation as an example, because I know that many people watching us are probably dealing with the same thing. In my case, my skin is ultra-sensitive (rosacea) which means that, by its very nature, it tends to swell, dry out, and so on. Would a person like me be a candidate for laser treatment?

Dr. Montes: You must be a mind reader, because that is exactly what I was about to discuss. This is yet another one of the wonders of this laser. We all have specific skin phototypes. They range from I to V —just to give you an idea— and here in Puerto Rico, we tend to be type III or type IV. That means that if we go to the beach, we will likely get a bit of a tan. We might get a little sunburned, too. Type IV skin tans more easily and burns less often. Type V skin almost never burns —it just tans— and type VI skin always tans (meaning, these are the darkest skin tones). In the past, lasers were off-limits for skin types higher than type III. In other words, all these technologies were originally created exclusively for the Nordic Caucasian population; however, this specific technology is designed to treat all skin types, as well as inflammatory skin conditions (such as the one you have—mild, very well-controlled rosacea).

Pamela Noa: Thank you. We are working on it!

Dr. Montes: So, in summary, we finally have a technology —the Ultra Clear laser— that can deliver incredible results through a comfortable, relatively painless treatment with a short recovery time.

Jasond Calderón: Si antes pensábamos que buscar un aspecto más joven era principalmente una preocupación exclusivamente femenina, pues es hora de desterrar ese mito, porque es un tema que cada vez más los hombres están hablando es de esto.

Carlos McConnie: Para guiarnos a través de lo que ellos realmente buscan cuando se trata de (Jasond: ellos, tú, ellos tú,) yo, los hombres (nosotros), de lo que buscan en general los hombres cuando se trata verse y sentirse mejor, pues recibimos al Oculoplástico, el doctor José Raúl Montes. Buenos días. ¿Qué es lo que están buscando los hombres cuando vienen a este tipo de tratamiento?

Dr. Montes: Buenos días (Carlos, Jason) y buenos días a todos ustedes. Pues, mire, antes se pensaba que los hombres eran de martes y las mujeres de venus. La realidad es que, con los procedimientos estéticos de hoy día, estos dos planetas están ya casi uno al lado de otro. La realidad es que los hombres cuando vienen a la oficina lo principal que buscan verse refrescados (natural) de acuerdo a su edad. Lo que los aleja de los procedimientos estéticos es el tiempo de recuperación. Por eso es que los tratamientos que son mínimamente invasivos son los que mayor popularidad entre esta población. Veamos los ejemplos. Este tratamiento de neurotoxina o Botox® sigue siendo el número uno. Tú me estabas preguntando, Jasond.

Jasond Calderón: Claro, que sí. Voy para el Botox®. Voy para el Botox®.

Dr. Montes: Sí, tú eres candidato, tú eres un buen candidato. Como ves aquí, el propósito de esto es simplemente (podemos pasar a la próxima de este paciente) que se vea el paciente más refrescado. No cambió en la posición de las cejas, porque, sino, lo feminizas y también (podemos pasar a la otra gráfica para que vean), cuando tú te sonríes, tú necesitas expresión. Así que esto solamente se modula para suavizarlo, pero tienes que tener alguna maraca de que están vivo y de que están contento. Así es que (volvemos a lo próximo), la frente es un área difícil de tratar en el hombre porque uno trata de (aquí suavizamos), pero sabes que el paciente puede mover sus cejas, puede tener expresiones.

Jasond Calderón: Es mucho lo que se cambia.

Dr. Montes: Esto es muy importante para la gente. Tú, que eres actor, ¿cómo te voy a dejar sin movimiento en las cejas?

Carlos McConnie: Deja ver.

Jasond Calderón: ¿Mis cejas?

Dr. Montes: Vamos a las próximas. El otro tratamiento que es bien popular y aquí ven cómo se marca al paciente. Yo siempre marco para saber los lugares de inyección. Éste es el antes y el después para que tú veas como el paciente preserva esos movimientos y esas expresiones. Aquí lo es hablando y gesticulando tres semanas después del tratamiento. Vamos a la próxima para que tú vea. Aquí es ‘casa del herrero’, digo, ‘en casa del herrero no hay cuchillo de palo’. Ese soy yo antes y después del tratamiento para que veas (bueno y ahora mismo lo tengo puesto) y mis expresiones yo creo que se ven naturales y están intactas. Vamos a lo próximo. Esto es cirugía que es también lo segundo más importante (por lo menos en mi oficina). Esto es una persona de 90 años. También tenemos que pensar que en el hombre no necesariamente hay un límite de edad para verse bien y sentirse bien — cirugía en párpado superior y en párpado inferior y rejuvenecido, ya casi diez o veinte años. Si pasamos a las siguientes gráficas, vamos a ver un tratamiento. Esto es también es un paciente. Esto es un médico que vino, porque tenía los ojos demasiado cerrados y ya estaba interfiriendo con su visión y su función. Ven aquí, estos son los tratamientos en el párpado de abajo, que son corrección de las bolsas, también con cirugía. Esto también en algunos pacientes se puede hacer sin cirugía y vamos a ver lo que está hoy más frecuentemente utilizado. Estos son ácidos hialurónicos y cómo ven aquí, esto es difícil.

Jasond Calderón: ¿Los ácidos para que son? ¿Qué hacen esos ácidos?

Dr. Montes: Esto es una sustancia de relleno y aquí es difícil que ustedes vean dónde está colocado, pero miren dónde está: en el párpado superior. Este paciente tiene un hundimiento natural en el párpado superior, pérdida de grasa, hace mucho ejercicio, es fisiculturista y ahí uno puede sutilmente recoger esto.

Carlos McConnie: Entonces casi todos los tratamientos que hemos visto son entre los ojos y la frente. ¿Eso mayormente es lo están buscando los hombres?

Dr. Montes: Buen punto, Carlos. Eso es así. Esa es el área más importante. Como en este caso que tú ves, esas han sido disimuladas con sustancias de relleno y no con cirugía. Dr. Montes: Saludos. Buenos días, Carlos, Jackie. ¡Qué bueno estar aquí! Saludos a todos ustedes.

Carlos McConnie: Bueno, ¿listos para hablar de medicina regenerativa? ¿Qué significa esto?

Dr. Montes: Medicina regenerativa es el tema del momento en la medicina convencional yo también en el mundo estético. La medicina regenerativa no es otra cosa que utilizar el mecanismo que todos tenemos en el cuerpo natural de repararnos ante un trauma, ante una enfermedad, pero positivamente para uso cosmético. En este caso vamos a hablar del uso cosmético, porque esto aplica también a enfermedades. ¿Qué tiene que ver la medicina regenerativa? Vamos a empezar a ver estas gráficas para poder explicar (explicarles a ustedes) los agentes bioestimulantes; hablamos de agentes bioestimulantes. Los agentes bioestimulantes son aquellas cosas que se inyectan principalmente y que produce una reacción controlada en el cuerpo donde surge colágeno nuevo.

Jacky Fontánez: ¡Eso es bello!

Dr. Montes: Elastina. Exacto. Todas estas células que son importantes para que la piel, los tejidos, tengan una apariencia más joven y mejor. Aquí estamos viendo unos artículos que son bien importantes. Ustedes no los van a ver en pantalla, pero lo que dicen se los voy a decir, simplemente. Todo el mundo sabe lo que son los fillers: los ácidos hialurónicos. Los ácidos hialurónicos tienen poca capacidad regenerativa o bioestimulantes. Eso es lo que dicen estos artículos. Por eso los agentes bioestimulantes (que vamos a ver aquí cuáles son estos agentes): uno de ellos es el ácido poliláctico, conocido como Sculptra® (que está muy de moda hoy), el calcio hidroxilapataia (que es el Radiesse®) y también ustedes han oído de algo plasma rico en plaquetas (PRP, PRF) —todo esto— la gente que llama el lifting de vampiro (donde te sacan sangre, te la ponen en la cara), etcétera. Esos son todos procedimientos y técnicas que se usan para estimular en el cuerpo la producción de colágeno, factores de crecimiento

Carlos McConnie: ¿Todo esto está considerado dentro de la medicina regenerativa?

Dr. Montes: Todo está dentro de la medicina regenerativa. Por ejemplo, vamos a hablar de los agentes bioestimulantes en específico. Miren este rostro. La gente piensa que los ácidos hialurónicos resuelven todo. En las áreas rosadas ustedes van a ver dónde yo inyectaría ácido hialurónico. Son en zonas bien específicas de la cara y bien pequeñas específicas. Sin embargo, los agentes bioestimulantes, que es el área amarilla, es lo que yo inyectará en el rostro, en todo el marco de la cara e internamente. ¿Para qué? Para darle al rostro una mejor proporción interna y natural, porque esto viene de regeneración interna y se ve externamente.

Jacky Fontánez: ¿Cuánto tiempo tarda en notarse ese cambio interno a externo?

Dr. Montes: Buena pregunta. Mira, por ejemplo, en este caso. En este joven que ustedes ven estos cambios (que la gente se queja) en la transición del párpado inferior y la mejilla, que uno empieza a verse como las ojeras, cansado, pérdida de volumen, esto es la inyección de un agente bioestimulante, en este caso: PLLA o Sculptra®. Esta inyección está bien precisamente colocada cerca de los huesos que están debajo del ojo de la órbita para regenerar el colágeno. Dr. Montes: Bueno, pero depresión es una enfermedad y yo he tenido personas que han estado en la silla de mi oficina (jóvenes, sobre todo) con depresión, porque han visto de momento que el pelo que tenían antes lo pierden rápidamente.

Jacky Fontánez: Ya que usted dice jóvenes, hay también que resaltar desde qué edad se puede realizar este tratamiento, porque sabemos que esto puede ocurrir en cualquier etapa. Sin embargo, tiene que haber un patrón estable en lo que es la calvicie.

Dr. Montes: Exactamente. Bueno, vamos a hablar de SmartGraft®, que es lo que tenemos aquí. Es una solución permanente para la pérdida de cabello en hombres y en mujeres. También se puede utilizar para reemplazar los vellos o el pelo de las cejas en las mujeres. Ese cabello que es resistente, que se va a quedar, moverlo a las partes que son susceptibles a la caída de cabello, que es la parte frontal y la parte de la coronilla. Aquí ven el cambio en las cejas de una persona que se hizo trasplante en las cejas.

Dr. Montes: Así que esto es un procedimiento para hombres y mujeres. Tú estabas preguntando (Jackie) la edad. De hecho, se han hecho fotos comparativas de ella cuando estaba en el 2018 a ahora que es un cambio, o sea, ella volvió a nacer.

Pamela Noa: Yo creo que está más joven que cuando hizo Mean Girls.

Jacky Fontánez: Exacto. Se ve tan bella. Entonces, queremos preguntarle a un experto que nos diga los códigos

Okay, se le ha afinado la cara, a esa edad sería raro hacer un facelift, pero su cara está bien reposicionada, así que es posible que haya tenido un facelift, muy bien hecho. A mí me llama mucho la atención la parte superior de sus cejas, porque ella tiene ahora espacio tarsal. Espacio tarsal es que tiene un poquito de espacio entre las pestaña y las cejas. Eso ocurre cuando tú haces un forehead lift, levantar la frente, o haces un Botox® o neurotoxinas de una manera muy bien para que las cejas suban.

Pamela Noa: Se había dicho, a través de las redes sociales que ella se había vuelto u poco adicta a los fillers y esto es lo que se dice en las redes. Usted corrige lo que sea un disparate y lo que no. Entonces, que luego ella mandó a sacarse los fillers o a derretirse o lo que sea y entonces luego de eso fue que hizo los procedimientos. ¿Eso es correcto? ¿Eso es posible, viable?

Dr. Montes: Bueno, tiene sentido, porque en algunas de las fotos he visto volúmenes exageradamente en una cara joven, con poca definición, teniendo ella una buena estructura ósea que, empezando por ahí, ya tiene todo lo de ganar. Es posible que ella, primero ella paró de hacerse fillers, que es lo que hay que hacer y hablamos de eso en el último programa.

Jacky Fontánez: Sí.

Dr. Montes: Overfilled syndrome, parar de hacerse fillers, disolver los que se puedan disolver y entonces si se hizo un facelift, usar agentes bioestimulantes y no usar ácido hialurónico. Los agentes bioestimulantes (hemos hablando aquí anteriormente) son los que hacen que tu propio cuerpo cree colágeno.

Si usted quiere una cara rejuvenecida en poco tiempo, usted mira a los ojos y mira en la parte inferior, los labios. Definitivamente los labios de ella y la dentadura tienen una mejoría notable (Jacky: Sí.) desde hace tiempo, digo comparando fotos anteriores. Tiene volumen en los labios (que eso es ácido hialurónico) y tiene una dentadura ya restaurada.

Pamela Noa: Okay. Tengo una pregunta: ¿cuánto tiempo puede haber tardado el tener esta nueva cara, que se ve espectacular?

Dr. Montes: Bueno, a mí me parece, que es una de las cosas que no podemos pasar por alto (Pamela, Jacky) es que ella cambió su estilo de vida.

Jasond Calderón: Bolsas en los ojos son un problema estético común que afecta a muchas personas, especialmente con el paso del tiempo

Grenda Rivera: Sin embargo, ¿qué las causa realmente y cuáles son las opciones de tratamiento para disminuirlas o quizás eliminarlas del todo? Tenemos aquí a al experto. Recibimos a nuestro Oculoplástico, el Doctor José Raúl Montes. ¡Bienvenido! ¡Buenos días!

Dr. Montes: ¡Buenos días, Grenda! Buenos días, Jason! Pues mira, las bolsas de los ojos es un tema súper importante, porque la mayoría de la gente que las padece la queja es que se sienten y se ven cansados. Ustedes hablaron con el tiempo, no necesariamente la edad es un factor aquí para que las personas tengan bolsas en los ojos. Esto puede ser causas congénitas, es decir, personas jóvenes que pueden tener ya estas bolsas y también en una mujer joven, siempre hay que descartar condiciones como la enfermedad de tiroides.

Grenda Rivera: Oh, ¿la tiroides puede ser una causa, entonces, de las bolsas?

Dr. Montes: Sí, la tiroides puede ser y por eso yo en todas, por ejemplo, la mujer siempre le hago un cernimiento de tiroides. Aquí ven a una paciente que tiene sus bolsas bien demarcadas y después de la cirugía. Podemos pasar a otros pacientes para que ustedes vean también en el paciente varón, lo mismo.

Jasond Calderón: ¿Eso tiene que ver con el sueño, no dormir bien?

Dr. Montes: No. Ese es otro tipo de bolsas que son mas bien las de edema o hinchazón.

Grenda Rivera: ¿No necesariamente?

Dr. Montes: Estas simplemente son las bolsas que son grasa, que están naturalmente en la órbita —alrededor del ojo— y con el paso del tiempo y en algunas personas por razones congénitas se mueven hacia al frente. Entonces, ustedes ven esto. Vamos a ver otra foto para que veamos todo tipo de condiciones y aquí ustedes ver en los cambios en la persona. Lo que hace es que la persona se ve descansada (Grenda: exacto), relajada y se le quitan como cinco años de edad.

Grenda Rivera: Si, definitivamente.

Dr. Montes: Podemos pasar a la próxima también.

Grenda Rivera: Mientras vemos más fotografías (doctor) en estos casos, ¿usted lo que hizo fue cirugía?

Dr. Montes: En todos estos casos son de cirugía porque todos estos casos son grasa, en el párpado, prolapso de graso, movimiento de la grasa hacia al frente. Claramente, ustedes ven aquí. Ahorita Jasond me preguntaba fuera de cámara, que si algunos de estos pacientes se podían tratar de una manera menos invasiva con relleno. Sí, hay casos de pacientes que la bolsa es bien pequeña y lo que se ve es la demarcación inferior. Esos son los casos dónde se puede la sustancia de relleno para disimularla, pero tú no estás corrigiendo la bolsa. Básicamente, la estás disimulando.

Jasond Calderón: ¿El rostro se pone más hinchado entonces, o sea, si tienes esa bolsita más rellenas el surco que le forma abajo, no se ve la cara un poco más gordita?

Dr. Montes: Pues mira, yo entiendo lo que tú me estás preguntando. Eso es uno de los problemas que hay, que hay personas que tienen la bolsa bien definida (tienen grasa) y el médico se confunde y piensa que puede hacer algo menos invasivo y rellena. ¿Qué pasa? Que lo que se ve ahí es todavía más hinchada la cara. El relleno tiene una indicación bien específica. La cantidad de grasa en el parpado tiene que ser mínima, tiene que haber una línea de demarcación clara y el paciente tiene que tener una piel que sea gruesa, que sostenga el relleno, porque si no, esto se vuelve como una hinchazón crónica. Aquí lo importante, lo que nosotros hemos visto aquí, son pacientes que tienen bolsas en los parpadas y esto es un problema quirúrgico o una condición quirúrgica; cirugía es la mejor alternativa.

Grenda Rivera: ¿La recuperación es rápida?

Dr. Montes: La recuperación de esto: yo diría que en una semana el paciente puede presentarse y hacer sus funciones diarias, como las hace anteriormente, excepto por el ejercicio o fuerza física que siempre le pedimos que por lo menos tome de dos a tres semanas.

Grenda Rivera: ¿Pudiera volver a aparecer con el tiempo?

Dr. Montes: Muy buena pregunta.

Jasond Calderón: Exactamente.

Dr. Montes: La mayoría de las veces, esto es una cirugía que el paciente se hace una sola vez. Es decir, el paciente se operó, no importa la edad (si tiene menos de 40 o 40 años o 50) y si la cirugía se hace de la manera adecuada (se quita el exceso de grasa, la que uno quiere la reacomoda) y sobre todo si la cirugía se hace de la manera que lo hacemos los especialistas del ojo, que es internamente, esta cirugía debe hacerse y durar todo el tiempo.

Jasond Calderón: Como esto es algo que no importa la edad, una vez se muestren las bolsas o haya señales que uno tiene bolsitas en los ojos, ya uno sabe que ese es el tamaño de la bolsa y ¿me puedo operar o tengo que esperar un poco más a ver si crece? Como la edad no es importante —digamos— todavía no tengo bolsitas en los ojos, pero me salen en tres años.

Dr. Montes: Bueno, cuando tú tienes mucha experiencia en esto —por ejemplo— yo te puedo hacer ahora mismo a ti una evaluación es muy difícil que tú desarrolles bolsas en los ojos.

Jasond Calderón:¡Gracias, Dios mío!

Dr. Montes: Ya lo estoy viendo. No sé si es natural o si es el maquillaje de hoy, pero definitivamente no creo que tú vas a ser un candidato para esto.

Grenda Rivera: Quizás para otras cosas.

Dr. Montes: Porque esto yo lo veo ya a distancia (cómo se dice) en algunos pacientes, tienen rasgos específicos dónde tú puedes predecir que esto va a ocurrir.

Dr. Montes: I understand what you are asking me. That is a problem, there are people who have a well-defined bag (they have fat) and the doctor gets confused and thinks that he can do something less invasive such as inject fillers. What’s happens? You end up with an even more swollen face. The filler has a very specific indication. The amount of fat in the eyelid has to be minimal, there has to be a clear demarcation line and the patient has to have a skin that is thick, that supports the filler, because if not, it becomes a chronic swelling. What is important is that we have seen patients who have bags on their eyelids. Being a surgical problem or condition, obviously surgery is the best alternative.

Grenda Rivera: Is the recovery quick?

Dr. Montes: The recovery: I would say that in a week the patient can resume daily functions, as before, except for exercise or anything requiring physical strength; we always ask the patient to wait at least two to three weeks.

Grenda Rivera: Could it reappear over time?

Dr. Montes: Very good question.

Jasond Calderón: Exactly.

Dr. Montes: Most of the time, this is a one-time surgery. The patient has the surgery, no matter the age (under 40, 40 or 50 years old) and if the surgery is done correctly (the excess fat is removed, the one you want is rearranged), and especially if the surgery is done like eye specialists do it, which is internally, this surgery is done once and will last forever.

Jacky Fontánez: Bueno, señores, los rellenos faciales han cobrado popularidad en los últimos años por el aspecto que le dan a nuestra piel: una piel rejuvenecida y tersa.

Jasond Calderón: Para no verte así porque (¿Qué pasa?) cuando abusamos de estos tratamientos podemos experimentar el tan conocido ‘facial overfill syndrome’. Recibimos al Oculoplástico, el Doctor José Raúl Montes, para que nos explique. ¡Buenos días!

Jacky Fontánez: ¡Buenos días!

Dr. Montes: ¡Buenos días, Jacky y Jasond! De verdad, me encanta. Ustedes son el mejor ejemplo de lo que voy a hablar hoy.

Jacky Fontánez: Cuéntanos cómo hacer para no vernos así.

Dr. Montes: Bueno, ¿tú sabes lo que es el ‘overfill syndrome’? Tú vas caminando por la calle y te encuentras de repente a estas personas que tú conocías y que de repente están irreconocibles. Tú dices: ¿qué le pasó a esta persona? Hay gente que dice: es cirugía, es el Botox®? No. La mayoría de las veces son los rellenos. ¿Por qué? Porque los rellenos que llegaron al mercado aprobados por la FDA hace veinte años al principio se usaban para áreas selectivas de la cara, como los pliegues nasolabiales, etc. Después se popularizó y la gente dijo: Ay los rellenos deben ser el arma para el antienvejecimiento y para levantar estructuras y debe ser un tratamiento regular, de mantenimiento. Entonces, los doctores —al principio— no se dieron cuenta de lo que estaban haciendo, que era que cada vez estaban llenando y llenando más las caras. Las personas tenían a veces menos arrugas, pero definitivamente las personas estaban desapareciendo en ese mar de ácido hialurónico. Ése es el problema.

Jasond Calderón: Entonces, todos se ven iguales. Esa (voy a decir deformidad, porque no es algo normal y natural lo que tú ves en un rostro de una persona) tú dices, se ve raro y todo el mundo se ve igual.

Jacky Fontánez: Algo está pasando.

Jasond Calderón: Muchas personas se ven similares y uno ve que también tienen rellenos.

Dr. Montes: Esa es de las partes peores y si nosotros vemos las gráficas que yo traje hoy para que ustedes entiendan. Por ejemplo, tenemos que esto es un fenómeno dónde el hombre no está exento.

Jasond Calderón: No, lo que pasa es que él era boxeador. Entonces él, cuando era Rocky, le dieron para de puños.

Dr. Montes: Sí, pero aparte de eso, los médicos le hicieron muchas cosas más.

De repente, la gente tenía tantos cachetes que parecían chipmunks, las ardillitas esas que se comen las avellanas. ¿Cómo usted se da cuenta de esto? Porque la gente se ve como otras personas y también porque los ojos de las personas se empiezan a ver pequeñitos.

Jasond Calderón: Le suben los cachetes, así como Kiko.

Dr. Montes: Exactamente. Empiezas a parecerte a Kiko. Los ojos se ven pequeños. Si vemos fotos, lamentablemente esto en un fenómeno que no está exento a la mayoría de figuras públicas.

También hay una cosa que quería hablarles a ustedes (Jacky y Jasond) que se llama ‘perception thrift’, que es como la percepción del individuo se cambia. Está viendo todo el tiempo, estos ‘celebrities’, estas, figuras con labios grandes, pómulos grandes, nalgas grandes y dicen: “Ah, eso es lo que está de moda y eso yo lo quiero.”

Jacky Fontánez: Eso es lo que quiero.

Dr. Montes: Entonces, mientas más grandes son estas cosas, ellos lo normalizan y empiezan a pedir más. ¿Qué pasa? Hoy día —y tenemos varias fotos todavía. Aquí hay una mezcla de cosas desafortunadamente. Esto es un ejemplo típico de un ‘overfill syndrome’ de momento a otro. Dr. Montes: Eso es de lo que iba a hablar ahora. ¿Cómo esto se trata? Primero que nada, yo le digo a la gente que tiene estar en lo que se llama un ‘washout period’, un periodo donde no se inyectan nada. Yo en mi oficina –por ejemplo—les dijo: usted me va a firmar aquí y me va a prometer …

Dr. Montes: … que usted no se va a volver a inyectar ácidos hialurónicos en su cara …

Grenda Rivera: Hablamos de un asuntito que ocupa a hombres o mujeres, porque más allá de la estética, los párpados caídos pueden afectar nuestra salud visual.

Ivonne Orsini: La pregunta sería: ¿cuándo deben ser operados y cuáles son los beneficios? Sobre esto hablamos con nuestro Oculoplástico [el experto], el Doctor José Raúl Montes.

Grenda Rivera: ¡Buenos días! ¡Hola!

Dr. Montes: ¡Buenos días! ¡Qué bueno estar con ustedes, Ivonne y Grenda [igualmente] y con todos ustedes!

Grenda Rivera: Claro que sí.

Dr. Montes: Pues mira, lo primero que voy a decir es que la cirugía de párpados, sobre todo estábamos hablando de la cirugía del párpado superior, es la cirugía de las más efectuadas en el mundo, para hombres y para mujeres. Esta cirugía es bien importante porque en esta cirugía la función se manifiesta. Es importante la función, ver mejor, pero también la estética. La persona se debe ver mejor. No debe parecer que el cirujano estuvo ahí. Por ejemplo, aquí tenemos un antes y después de una persona cuyo párpado [¡Ay, qué diferencia!] … ¿verdad que ahí no se ven las pestañas? [No.] Ni siquiera , si tuviera pestañas postizas tampoco se verían, porque ese párpado está demasiado caído; tiene demasiado piel redundante. Después de esto, uno libera el párpado para que por lo menos entre las pestañas y lo que es el párpado haya un espacio.

Ivonne Orsini: Doctor, ¿ahí hay una función más allá de la estética? O sea, porque veo el ojo bastante más abierto.

Grenda Rivera: Porque la visión periferal {sic} (yo creo, no sé) con ese párpado tan caído podía verse afectada.

Dr. Montes: Definitivamente, lo que tú decías Grenda. En un párpado como éste (la parte lateral), esta persona no puede ver mucho más de lado. Solamente ve de frente e inclusive el campo superior de visión también está afectado. Se usan pruebas específicas como ésta. Esto es bien técnico, así que esto es para que sepan que para probar que te afecta visualmente y tú pedirle al plan que te ayude con esta cirugía, hay que hacer pruebas específicas y el médico lo tienen que certificar. Si vemos también aquí, esto no eso es una cirugía que tiene que ver con el género. Hombres y mujeres se benefician de esto. Aquí vemos a un caballero que también tiene los mismo. La piel casi ya le cubre las pestañas.

Grenda Rivera: Apenas se le ven los ojos.

Dr. Montes: Exactamente. Tiene déficit en el campo superior y éste es el después. Podemos seguir viendo ejemplos.

Ivonne Orsini: ¿Cómo se hace la operación? ¿Se corta parte de la piel? ¿Cómo se hace para que se suba?

Dr. Montes: Bueno, hay varias técnicas de cirugía, dependiendo del paciente. Es que la gente a veces se olvida que no es el párpado, es la frente y las cejas las que se caen. Entonces, cómo tú dices (Ivonne) la cirugía es totalmente distinta. Si estamos hablando del párpado, se interviene directamente en esta piel que está aquí. Si estamos hablando de las cejas y la frente, es otro tipo de cirugía. [Okay.] Podemos seguir viendo ejemplos para que vean.

Grenda Rivera: Ella es joven.

Dr. Montes: Por ejemplo, una persona joven, mira la simetría en el antes, en un párpado tiene piel extra (redundante); en el otro no tiene tanto, pero tú quieres que se vean simétricos, que se van iguales. Pero, ¿cuál es el concepto que es importante aquí? No solo se trata de remoción de piel. Se trata también de conservar la grasa del párpado. En el pasado, la mayoría de las personas que hacían esto, no tenían el concepto adecuado y ¿qué hacían?, que quitaban no solamente la piel, sino los paquetes grasos que tienen el párpado. Les voy a dar a ustedes ahora una perla para que siempre se acuerden. [Ahá.] ¿Saben lo que da juventud a la cara?

Cuando las maquillan a ustedes, tratan de hacer esto un poquito más corto [sí] y por eso usan sombra [allá arriba] …exactamente y usan sombra color marrón en vez de negra, porque la negra te puede hacer ver el ojo un poco hundido. La marrón te da esa profundidad, pero te hace crear un poquito de volumen [sí]. Así es que ése es otro tip, es práctico, en vez de usar eyeliner negro [claro], úsenlo marrón. Ivonne Orsini: ¡Qué bien!

Grenda Rivera: Doctor, háblenos sobre la recuperación y bueno, este tipo de cirugía en el párpado, específicamente, esto debe ser ambulatorio. ¿Correcto?

Dr. Montes: Es ambulatorio. Yo personalmente hago estas cirugías en mi oficina que tengo una sala de operaciones. La recuperación de la cirugía del párpado superior es rápida. Cuando yo digo rápido es que la persona inmediatamente sale de la cirugía con los ojos descubiertos y la persona puede hacer sus funciones normales, siempre que no haya esfuerzo, fuerza física.

Dr. Montes: Well, the first thing I am going to say is that eyelid surgery, especially upper eyelid surgery, is one of the most performed surgeries in the world, for men and women. This one is very important because in this surgery function is manifested. Function is important, seeing better, but aesthetics is also important. The person should look better. No one should notice that there was as surgery performed. For example, here we can see a before and after of a person whose eyelid [Oh, what a difference!] … Can you see the eyelashes? Dr. Montes: For example, in a young person, it may be symmetry as you can see here in the before picture, one eyelid has extra skin (redundant); in the other one it does not have as much. The goal is for them to look symmetrical, that they look the same. What concept is important here? It is not just about skin removal. It is also about preserving eyelid fat. In the past, most people who did this did not have the right concept. What would they do? They removed not only the skin, but the fatty packets in the eyelid. Dr. Montes: It is ambulatory. I personally do these surgeries in my office where I have an operating room. Recovery from upper eyelid surgery is quick. When I say quick, I mean, immediately coming out of surgery with eyes uncovered the person can perform all normal functions, as long as there is no physical effort.

Dr. Montes: Exactly, the person can return to all daily functions almost immediately.
