Los especialistas destacan el creciente uso de sustancias bioestimulantes de relleno, como el ácido poliláctico,
que induce la generación de colágeno de las propias células del organismo.

El Dr. José Raúl Montes, uno de los grandes referentes internacionales de cirugía oculoplástica ha mostrado esta mañana ante 200 colegas de todo el mundo algunas de las últimas tendencias y técnicas cosméticas poco invasivas aplicables en los casos de rejuvenecimiento o restauración facial, en la segunda y última jornada del curso de oculplástica que ha tenido lugar en el Instituto de Microcirugía Ocular de Barcelona.

En una conferencia previa al taller práctico, el Dr. Montes ha destacado el creciente uso de productos bioestimulantes de relleno, como el ácido poliláctico (PLA), una sustancia líquida que induce la generación de colágeno por parte de las propias células del organismo, restableciendo o restaurando de esta manera aquellas zonas donde se ha perdido soporte. Según este especialista, esta tendencia es fruto de un nuevo concepto de envejecimiento facial. “Mientras que antes los cirujanos plásticos centraban casi exclusivamente su trabajo en el estiramiento de la piel, ahora se busca restablecer o restaurar mediante la inyección de volumen a través de técnicas menos invasivas. Ya no se tratan las arrugas como algo aislado, sino que se abordan las necesidades estéticas del paciente de forma más integral.

En el caso de los rellenos, se tiende a inyectar en un mayor número de zonas sustancias seguras que den soporte, que levanten allí donde haya pérdida de volumen”, asegura el Dr. Montes.
Inicialmente, este relleno dérmico se empezó a utilizar en Europa como tratamiento en pacientes con lipodistrofia o pérdida de grasa facial asociadas al VIH. En estos casos, la mejoría era considerable y en 2008 fue aprobado por la FDA (Food and Drug Administration) para su uso cosmético en la restauración de volumen facial perdido, el establecimiento de soporte, así como la mejora de contornos, estructura y calidad de la piel. Según el Dr. Montes, “el ácido poliláctico actúa de forma progresiva y requiere entre 3 y 4 sesiones para poder ver los cambios, cuyos efectos duran de 2 a 3 años. “Frente al resto de sustancias de relleno (ácido hialurónico, hidroxiapatita de calcio…), el PLA ofrece al paciente unos resultados faciales mucho más naturales”, concluye.

Toxina botulínica, la sustancia más demandada
El taller teórico-práctico sobre la aplicación de inyectables perioculares también ha tratado el uso de la toxina botulínica, sustancia indicada tanto en patologías de tipo funcional (exoftalmos, blefaroespasmos, etc.) como estética (corrección arrugas de expresión (patas de gallo, líneas frontales y líneas interciliares). Aprobada para su uso estético en 2002, la toxina botulínica es actualmente el tratamiento cosmético mínimamente invasivo más solicitado en todo el mundo. Según el Dr. Montes, “se trata de una sustancia cien por cien segura, pero que siempre debe ser administrada por manos expertas, como la de los oftalmólogos especializados en cirugía oculoplástica”.

Más allá del uso de una u otra sustancia de relleno periocular, el Dr. Montes ha querido transmitir a sus colegas la importancia de saber mirar y evaluar a cada paciente, no solo desde el punto de vista estético. Según el especialista portorriqueño, “hablar y observar a la persona es clave a la hora de decidir qué tratamiento o producto es el más indicado para su caso en particular. Una evaluación que no debe terminar en la cara, sino que ha de profundizar en la personalidad, el carácter y las motivaciones del paciente, en definitiva, aprender a mirar más en su interior”.

Con este taller, se cierra la segunda jornada del III Curso Internacional Barcelona Oculoplastics, organizado por la Fundación IMO, que ayer ofreció ocho intervenciones de cirugía palpebral en directo. Entre ellas destacó especialmente el lifting mediofacial transconjuntival con injerto de tejido proveniente de la mucosa del paladar para corregir la retracción del párpado inferior asociada a una operación de blefaroplastia. Esta compleja intervención, pionera en Europa, fue realizada con éxito por el Dr. Ramón Medel, especialista en cirugía oculoplástica del IMO y director del curso.

As a director of the course, I expect that the two days we shared in IMO could be an encouragement to organize more events like this one. From my point of view, these events are fundamental to improve the specialty.

During the course in Barcelona, we were lucky to benefit from your participation and the presence of other recognised oculoplastic experts.
