For the last half of 2009, workers could expect to see perhaps  $13 a week less withheld from their paychecks starting about June. Millions of  Americans who don"t make enough money to pay federal income taxes could file  returns next year and receive checks. Individuals making more than $75,000 and  couples making more than $150,000 would receive reduced amounts. &lt;br/&gt; &lt;/li&gt;&lt;li&gt;About $70 billion to spare about 24 million taxpayers from being hit with  the alternative minimum tax in 2009. The change would save a family of four an  average of $2,300. The tax was designed to make sure wealthy taxpayers can"t use  credits and deductions to avoid paying any taxes. But it was never indexed to  inflation, so families making as little as $45,000 could get significant  increases without the change. Congress addresses it each year, usually in the  fall. &lt;br/&gt; &lt;/li&gt;&lt;li&gt;About $13 billion to provide a $2,500 expanded tax credit for college  tuition and related expenses for 2009 and 2010. But it's one of the most far-reaching and controversial proposals for making health insurance more affordable and more widely available.&lt;br /&gt;&lt;br /&gt;Until now the government has guaranteed access to health care only for seniors and the disabled through Medicare and for the poor mainly through Medicaid. Under the proposals being advanced by Sens. Clinton, of New York, and Obama, of Illinois, and former Sen. Edwards, of North Carolina, the government would offer coverage for middle-class workers and their families, with comprehensive benefits comparable to those now provided for federal employees and members of Congress.&lt;br /&gt;&lt;br /&gt;Participation in the government alternative would be voluntary, but the approach sparks widely differing reactions.&lt;br /&gt;&lt;br /&gt;Mark B. McClellan, the former Medicare administrator in the Bush administration, called it a risky departure from the state-based reforms that the Democratic candidates have cited as their models.&lt;br /&gt;&lt;br /&gt;What McClellan and other critics say they fear is that the government plan could underbid private insurers and, if large numbers of people sign up, it could eventually replace private coverage, including the employer-sponsored plans that now serve most middle-class Americans.&lt;br /&gt;&lt;br /&gt;Despite these concerns, other respected experts say that giving people the option of joining a government plan might make for a sensible experiment.&lt;br /&gt;&lt;br /&gt;"It could be a really fair way to test the market, both in terms of people's preferences and to see what kind of plan can be more successful over time," said health economist Marilyn Moon, a trustee of Medicare and Social Security during the Clinton administration. "It depends on the details, as these things always do."&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;For example, Moon suggested that a government plan could be used to test many of the latest ideas for reducing waste and improving quality, such as cutting payments for treatments of dubious value and rewarding hospitals and doctors that can document improved results in patient care.&lt;br /&gt;&lt;br /&gt;"You could allow a government option as a way of setting a standard, as opposed to issuing lots of regulations that would apply to every insurer," she said.&lt;br /&gt;&lt;br /&gt;Republican presidential candidates also have health care plans, but – in keeping with conservative principles – they rely on market forces, economic incentives and the private sector to increase access to medical insurance and try to rein in costs. None favors creation of a government alternative to private health-care programs.&lt;br /&gt;&lt;br /&gt;GOP opposition is not the only political obstacle to the Democratic idea.&lt;br /&gt;&lt;br /&gt;The insurance industry, one of the nation's most powerful lobbies, is against it. "It is a move to significantly expand the role of government and move people off private coverage and to public coverage," Karen Ignagni, president of America's Health Insurance Plans and the industry's top lobbyist in Washington, said.&lt;br /&gt;&lt;br /&gt;Another critic is GOP presidential candidate Mitt Romney, who as governor of Massachusetts signed bipartisan legislation to provide coverage for all state residents – a model for the Democratic candidates.&lt;br /&gt;&lt;br /&gt;"You can call (the government plan) an option, but if you don't put any restrictions on who can enroll, and you give the government leverage in terms of ability to negotiate prices, you create a situation that leads to a government-run system," said Lanhee J. Chen, Romney's domestic-policy advisor.&lt;br /&gt;&lt;br /&gt;Clinton, Obama and Edwards dismiss the charge that they are trying to introduce the camel's nose of government-run health care under the tent of health reform.&lt;br /&gt;&lt;br /&gt;"Hillary never supported a single-payer system, not even in the early 1990s," said Neera Tanden, Clinton's health-policy adviser. "She has always supported a plan to build on what we have in our healthcare system." (Only one Democratic candidate, Ohio Rep. Dennis J. Kucinich, favors switching to a full-blown government-run system.)&lt;br /&gt;&lt;br /&gt;"The charge that this is socialized medicine is ridiculous," said Austan Goolsbee, a University of Chicago economist advising Obama. "This would remain one choice among many, and you don't have to pick it. You can pick a private option, or insurance through your employer."&lt;br /&gt;&lt;br /&gt;The candidates' plans differ in some important provisions, and certain key details are not yet spelled out. But Clinton, Obama and Edwards follow the same basic framework.&lt;br /&gt;&lt;br /&gt;As a starting point, they would create new insurance-purchasing pools through which individuals, families and businesses would have the option of buying coverage from a broad range of commercial insurers – or the new government plan.&lt;br /&gt;&lt;br /&gt;Private insurers who wished to participate in the pools would not be allowed to turn people down because of previous health problems, and the insurers would be limited in how much they could raise premiums to account for a person's age and other factors that can affect health. All the plans in the pool would have to offer a minimum standard benefit package, which would likely be keyed to the government plan.&lt;br /&gt;&lt;br /&gt;Individuals and families eligible for government subsidies through the candidates' health-reform plans would be able to use that assistance to buy either a private plan or the government coverage.&lt;br /&gt;&lt;br /&gt;Businesses would be allowed to participate in the purchasing pools and enroll employees in the government plan. However, in Obama's plan, participation would be limited to small companies and individuals who don't have access to employer coverage.&lt;br /&gt;&lt;br /&gt;(Begin optional trim)&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Most health-care costs are incurred by the sickest patients. In any given year, about 10 percent of the population accounts for more than 60 percent of health-care costs – a statistic that partly explains why insurers go to great lengths to avoid issuing policies to people with medical problems.&lt;br /&gt;&lt;br /&gt;Edwards' policy director, James Kvaal, said he would insist on regulations to prevent private carriers from dumping their sickest patients into the government's care. "We will stop private insurance companies from competing by weeding out the sickest people," said Kvaal.&lt;br /&gt;&lt;br /&gt;But what if those patients – weary of dealing with private insurers – sought out the government plan as a haven? Rules for insurers might not protect the government plan in such a situation. Instead, lawmakers might have to wade into the complex business of trying to adjust premiums between the government plan and the private policies.&lt;br /&gt;&lt;br /&gt;(End optional trim)&lt;br /&gt;&lt;br /&gt;The candidates acknowledge that their plans are broad brush strokes for now, and important details would have to be decided with Congress. But some people in the gay community &lt;br /&gt;feel their invitation may have gotten lost in the mail. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;"There's been an unintended erasure of blacks in the Castro," says Bill &lt;br /&gt;Doggett. "It's created a sense that there's nothing here for us." Now that's &lt;br /&gt;changing, thanks to a new program of mixers Doggett has launched at Magnet, a &lt;br /&gt;neighborhood health services office. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Doggett, a 52-year-old commercial photographer originally from Los &lt;br /&gt;Angeles, used to party at the Pendulum in the Castro, a lively bar near 18th &lt;br /&gt;and Collingwood that closed down for remodeling two years ago. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;The shuttering came in the midst of one of the neighborhood's messier &lt;br /&gt;sagas. A group of activists called And Castro for All accused Les Natali, the &lt;br /&gt;owner of the Pendulum and the Badlands, the popular club across 18th Street, of &lt;br /&gt;discriminating against black patrons  --  accusations that Natali strongly &lt;br /&gt;denied.  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;In May 2005, the San Francisco Human Rights Commission ruled that Natali &lt;br /&gt;had referred to African Americans as "non-Badlands customers" and required &lt;br /&gt;blacks to offer multiple forms of identification before entering  --  claims &lt;br /&gt;that Natali denied. After the commission's findings and community outrage, he &lt;br /&gt;closed the Pendulum two months later for remodeling  --  an act that was viewed &lt;br /&gt;by black leaders as retaliation for the original complaints. Subsequently, &lt;br /&gt;however, the state's Department of Alcoholic Beverage Control investigated the &lt;br /&gt;complaints, said they could not be substantiated and dismissed them.  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Natali did not return phone calls left at his residence and office. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;But the Badlands incident, along with the extended closure of the &lt;br /&gt;Pendulum, have left a distinct void in the city's gay community. At a time when &lt;br /&gt;the city's African American population is in decline, the loss of the Pendulum &lt;br /&gt;only highlights the dwindling presence of African Americans in the Castro.  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;In response, five months ago Doggett co-founded the monthly mixer for &lt;br /&gt;African American men at Magnet. Also, the Edge, a club across the street from &lt;br /&gt;the Pendulum that usually cranks out oldies and party music, recently started &lt;br /&gt;Soulful Thursdays, a night meant to attract more African Americans. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;John Newsome, a co-founder of And Castro for All, said that because his &lt;br /&gt;group reached a confidential settlement with the club owner in 2006, he was &lt;br /&gt;unable to discuss the status of the Pendulum.  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;But anecdotally, Newsome observed, "My sense is that fewer African &lt;br /&gt;Americans are coming into the community as the number of black-identified &lt;br /&gt;spaces has decreased." &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Newsome recalled that in the '80s, the neighborhood had several clubs that &lt;br /&gt;catered to black patrons, but that has changed over the years, and the Pendulum &lt;br /&gt;represented the last club available. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Newsome, who is African American, called Doggett's mixers "a critical need &lt;br /&gt;in the community."  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;"It begs the question," Newsome said, "if we're left to using social &lt;br /&gt;spaces that are on loan and shared, isn't it clear we don't have any permanent &lt;br /&gt;spaces?" &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Doggett said an average of 60 people have attended the mixers, which are &lt;br /&gt;held on the third Saturday of every month and are a mixture of networking, &lt;br /&gt;dance party and potluck dinners. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;It's also a chance to mentor recent arrivals in the area's African &lt;br /&gt;American gay community. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;"Many are attracted to the 'gay mecca' ideal," Doggett said, "where &lt;br /&gt;everyone will be welcoming and understanding. I had a different experience and &lt;br /&gt;so do a lot of people of color; we help to integrate them to the reality." &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Vincent Fuqua, co-founder of SFBrothas, an outreach program for black gay &lt;br /&gt;men in San Francisco, said, "Losing a bar where African Americans went, that &lt;br /&gt;hurt," he said. "There was not a huge presence to begin with, and now there's &lt;br /&gt;none."  &lt;br /&gt;&lt;/p&gt;&lt;p&gt;David Wheeler, a manager at the Edge, said the Soulful Thursdays night, &lt;br /&gt;still in a trial phase, was added to the club's weekly schedule after talks &lt;br /&gt;with Doggett. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;"I don't want to say it's an attempt to cater to African Americans," &lt;br /&gt;Wheeler said, "but it's a night that, hopefully, will make it more appealing &lt;br /&gt;for a diverse crowd." During this weekend's series of Pride events, a Friday &lt;br /&gt;night reception was held for the African American grand marshals at Levende &lt;br /&gt;Lounge in the Mission, and today a Black Pride stage in the Castro will host &lt;br /&gt;readings and music acts focused on African Americans. &lt;br /&gt;&lt;/p&gt;&lt;p&gt;Fuqua, the SFBrothas co-founder, said of the closed Pendulum, "Hopefully &lt;br /&gt;there will be something like that in the Castro again. "Plaintiffs were illegally strip-searched by the defendant officers noted herein without any just provocation or probable cause."&lt;br /&gt;&lt;br /&gt;Erica Harrold, spokeswoman for Oakland City Attorney John Russo, said the allegations "are very serious" and reserved further comment until city officials had a chance to review the complaint.&lt;br /&gt;&lt;br /&gt;The suit was filed by Oakland civil rights attorneys John Burris and Michael Haddad, who have filed five other similar suits on behalf of eight other men.&lt;br /&gt;&lt;br /&gt;"Oakland is the only police department of which we, and the OPD, are aware that has a written policy allowing officers to conduct strip searches on the street," Haddad said Thursday. "We contend that such searches are clearly unconstitutional."&lt;br /&gt;&lt;br /&gt;"These humiliating searches are made worse" by the apparent singling out by police of African Americans that are searched, Haddad said. All but one of the plaintiffs in this suit are African American.&lt;br /&gt;&lt;br /&gt;Burris said the practice "undermines the relationship" between police and the community.
