Some more great photos sent in this week my a reader. The float must have caught on fire, and then set fire to its cargo, a million-dollar bomb truck. This incident happend on the 3rd May, at Brockman.

Mingo Train Load-Out Derailment and Collapse

Wharncliffe -- Tuesday afternoon investigators were still sifting through the rubble, trying to find out why a train derailed at the Black Bear Preparation Plant in the Wharncliffe community of Mingo County.

That derailment knocked out the support beams of the "Load Out" at the coal mine, causing the structure to collapse. The Wharncliffe Volunteer Fire Department was one of the first on the scene.

"It's about a 70 foot building. It's just flat on the ground, debris everywhere, metal axles from the train cars laying beside the rail and stuff," said Joshua Vance of the Wharncliffe Volunteer Fire Department.

Vance says three were trapped in the debris-two coal miners and the train's conductor. They were taken to the hospital, one with serious injuries, two others with minor injuries. He adds that five train cars came off the tracks to cause the building collapse, creating a sight many say they'll never forget.

"Ain't never happened around here before. It ain't never happened. It's bad," said J.R Cline of Wharncliffe.

The mine is owned by Alpha Natural Resources of Virginia. The Office of Miner's Health, Safety and Training is investigating the incident.

These amazing pictures were sent in last week by a reader. They come from the Barrick gold mine, at Porgera in PNG.

From the incident report: At approximately 1300hrs on the 13th March, a light vehicle (LV) was run over by a 789 truck on Stage 5B of the Open Pit.

There was a driver and two passengers in the LV at the time of the incident. Neither of the passengers appear to have been injured. The driver of the LV appears to have suffered a dislocated collar bone.
The two passengers were able to remove themselves from the LV unassisted. The driver of the LV required assistance to be removed from the vehicle as the steering wheel was holding him in the seat.

Statements are currently being obtained and the investigation has commenced, however early indications are that the driver of the LV approached the truck from the right side and was unsighted by the truck driver. The truck was in motion and within approximately 50m of a Shovel, it is likely he was about to turn to line up to receive a load from the shovel.

The truck stopped leaving the LV at approximately the mid point of the truck.

Reclaimer Pushed By Cyclone

These great photos came in this week from a new reader of the blog.

RC702 first Long Travel

'New' FMG reclaimer first movement (not no-load commissioning). This occurred during the recent cyclone.

Instructions were to:

· Ensure all LT and Slew brakes were engaged

· 10t sling restraining each corner of the machine

· Wedges to be placed on LT wheels, and

· Boom to be luffed down and secured to crane counter weights

What happened:

· 2 out of the 12 LT brakes were engaged

· 3t come-alongs were used

· Wedges were not put in correctly, and

· The sling securing the boom came off in the high winds

Result: the machine moved 204m up hill on the rail and only stopped when the 3 leading bogies fell off the end of the rail, and the yard hold down roller frame was tipped over and jammed into the impact table. There were no end buffers in place so these hold down bolts were ran over.

Cost: 3-4 weeks extra delays in finishing reclaimer, and damage to 200m of belt and structure.

This incident comes from the Eaglefield mine, in the Bowen Basin. It looks like this shot was being loaded at the time of the failure.

Oxy-Acetelyene Explosion

From the MSHA report:
This incident happened on Monday the 29th of September 2003. A fitter with a work van left an E size Ox and Acetylene cylinder on the back seat of a Toyota dual cab over the weekend. The Acetylene cylinder v was not fully closed and a leak occurred.

Over the weekend the Acetylene accumulated in the van. On Monday morning the fitter approached the va opened the door, a large explosion took place. The ignition may have been caused by either the circuitry, the automatic door control, mobile phone which was on the front seat of the van, or lighting a cigarette. The fitter was also a smoker. He has damage to his ear drums and facial damage. As you can see by the att photo he was very lucky. Don't let safety be something you learn by accident.

Downshifting in a 793

Lots of people send it photos to this blog. (Thank you!). Normally, I check them to see if I already have them in my collection, and then put them into the rotation. Sometimes, the photos are so amazing that I just have to put them up on the site right away.

This is one of those times. These photos were sent in this week by a reader. In it, he says that the operator of this 793 downshifted from sixth gear to first. The engine didn't like it. Some rebuilding of the engine may be necessary.
