Analyze inpatient and outpatient medication orders for completeness and appropriateness as it relates to the 6 rights: person, medication, dose, route, schedule, and documentation.
Supervise the work of the pharmacy technicians and provide support help as needed.
Ensure appropriate distribution of medications for both inpatients and outpatients. This includes but is not limited to sterile compounding, bar code scanning, and automated dispensing devices.
Analyze lab values to perform pharmacokinetic dosing, renal dosing, and therapeutic drug monitoring according to established protocols and current guidelines.
Document therapeutic drug monitoring decisions and clinical interventions into the electronic medical record.
Ensure appropriate medication histories are gathered for medication reconciliation.
Dispense medications and maintain appropriate records according to state and federal regulations.
Answer questions and provide information to clinicians and patients regarding drug interactions, side effects, dosages, and storage of pharmaceuticals.
Respond to emergency response codes in the hospital and maintain ACLS certification.
Provide assistance to other healthcare practitioners in coordinating nutrition support (including total parenteral nutrition).
Assists with development of drug protocols and standing orders.
Participates in Quality Assurance and Drug Use Evaluation programs and completes drug utilization reviews for the Pharmacy and Therapeutics Committee.
Performs other related duties as assigned.
          
Required Education and Experience
Doctorate in Pharmacy (Pharm.D.) preferred and 2 years’ experience in a hospital or residency.
Required Licenses, Certifications and/or Registrations
Current license as a registered Pharmacist in the State

Assists the pharmacist(s) with the dispensing, compounding, and delivery activities.

Checks for expired/unusable products and dispose of them properly.
Documents all activities appropriately.
Answers and initiates phone calls related to providing pharmaceutical care Prepares delivery tickets.
Assists the patient delivery representative in orders, receiving, maintaining and tracking all infusion related equipment.
Fulfills the duties of the patient delivery representative when required.
Maximizes the efficiency and productivity of the pharmacy and delivery operations.
Strives to achieve cost effectiveness in all areas (including purchasing/inventory control, staffing delivery services)
