  &lt;p&gt;[C]linical summaries [for patients] should [be] three business days and the threshold requirement should be 20 (not 50) percent. The measure should also be based on unique patients seen during the EHR reporting period, and not based on every office visit to minimize reporting burdens. In addition, a physician &lt;span class="bgy"&gt;should have the flexibility to include only the information that the physician believes to be relevant for the summary&lt;/span&gt;.&lt;/p&gt;

    &lt;p&gt;Bioethics has long approached cost containment under the heading of &amp;ldquo;allocation of scarce resources.&amp;rdquo; Having thus named the nail, bioethics has whacked away at it with the theoretical hammer of distributive justice. But in the United States, ethical debate is now shifting from rationing to the avoidance of waste.&lt;/p&gt;
    &lt;p&gt;&amp;hellip;&lt;/p&gt;
    &lt;p&gt;The main ethical objection to rationing is that physicians owe an absolute duty of fidelity to each individual patient, regardless of cost. This objection fails, however, because when resources are exhausted, the patients who are deprived of care are real people and not statistics. Physicians collectively owe loyalty to those patients too.&lt;/p&gt;

    &lt;p&gt;The two principal ethical arguments for waste avoidance are first, that we should not deprive any patient of useful medical services, even if they're expensive, so long as money is being wasted on useless interventions, and second, that useless tests and treatments cause harm. Treatments that won't help patients can cause complications. Diagnostic tests that won&amp;rsquo;t help patients produce false positive results that in turn lead to more tests and complications. &lt;span class="scapb"&gt;Primum non nocere&lt;/span&gt;  becomes the strongest argument for eliminating nonbeneficial medicine.&lt;/p&gt;

&lt;p&gt;An interesting problem with &lt;i&gt;association&lt;/i&gt; in this study&amp;mdash;what about the Hawthorne effect? Does the physician change his/her game because &amp;ldquo;when we know more, we do less&amp;rdquo;? If the outcome is decreased cost (marker), then is there an implicit assumption that decreased cost tracks improved quality of care?&lt;/p&gt;

&lt;p&gt;Another aspect that comes into play is not just the availability of information from an HIE, but is the relevant information from the HIE delivered JIT. The cost may go up if information is not JIT when you factor in that &amp;ldquo;without information, testing occurs&amp;rdquo; with longer dwell times in the ED (LOS). Then add in the cost from lost business, where LOS backs up the ED and affects overall throughput (increases LWBS).&lt;/p&gt;

No national estimates are available for the incidence of maternal opiate use at the time of delivery or NAS.&lt;/p&gt;
    &lt;p&gt;&lt;span class="scapb"&gt;Conclusion&lt;/span&gt;. Between 2000 and 2009, a substantial increase in the incidence of NAS and maternal opiate use in the United States was observed, as well as hospital charges related to NAS.&lt;/p&gt;

&lt;p&gt;The Administration’s framework consists of four key elements:&lt;/p&gt;

            &lt;li&gt;&lt;strong&gt;Transparency&lt;/strong&gt;:  Consumers have a right to easily understandable information about privacy and security practices.&lt;/li&gt;
            &lt;li&gt;&lt;strong&gt;Respect for Context&lt;/strong&gt;:  Consumers have a right to expect that organizations will collect, use, and disclose personal data in ways that are consistent with the context in which consumers provide the data.&lt;/li&gt;
            &lt;li&gt;&lt;strong&gt;Security&lt;/strong&gt;:  Consumers have a right to secure and responsible handling of personal data.&lt;/li&gt;
            &lt;li&gt;&lt;strong&gt;Access and Accuracy&lt;/strong&gt;:  Consumers have a right to access and correct personal data in usable formats, in a manner that is appropriate to the sensitivity of the data and the risk of adverse consequences to consumers if the data are inaccurate.&lt;/li&gt;
            &lt;li&gt;&lt;strong&gt;Focused Collection&lt;/strong&gt;:  Consumers have a right to reasonable limits on the personal data that companies collect and retain.&lt;/li&gt;
            &lt;li&gt;&lt;strong&gt;Accountability&lt;/strong&gt;:  Consumers have a right to have personal data handled by companies with appropriate measures in place to assure they adhere to the Consumer Privacy Bill of Rights.&lt;/li&gt;

Have we seen the entrenchment of a whole industry (satisfaction medicine) that is both detrimental to health care generally and to the individual specifically?&lt;/p&gt;

&lt;p&gt;Opinions will vary on the cost and value of &amp;ldquo;defensive medicine&amp;rdquo;&amp;mdash;so institutionalized in medicine. We now have the institutionalization of &amp;ldquo;satisfaction medicine&amp;rdquo;&amp;mdash;the cost and value are? I&amp;rsquo;ll bet, when scrutinized, that satisfaction medicine will dwarf both the cost and the &amp;ldquo;perceived&amp;rdquo; value of defensive medicine. US Healthcare probably deserves its own entry (if not chapter(s)) under the topic of &amp;ldquo;unintended consequences&amp;rdquo; (and &amp;ldquo;unintended incentivizations&amp;rdquo;). Seems silly and trivial, but &lt;a href="http://en.wikipedia.org/wiki/If_You_Give_a_Mouse_a_Cookie" title="Numeroff"&gt;Numeroff&lt;/a&gt; really has nailed what has happened so well &amp;mdash;&amp;ldquo;if&amp;rdquo; is never followed by a singular &amp;ldquo;then&amp;rdquo;. We find ourselves trapped by the superficially vetted, that now is so entrenched.&lt;/p&gt;

    &lt;p&gt;&lt;span class="scapb"&gt;Background&lt;/span&gt;. Patient satisfaction is a widely used health care quality metric. However, the relationship between patient satisfaction and health care utilization, expenditures, and outcomes remains ill defined.&lt;/p&gt;
    &lt;p&gt;&lt;span class="scapb"&gt;Conclusion&lt;/span&gt;. In a nationally representative sample, higher patient satisfaction was associated with less emergency department use but with greater inpatient use, higher overall health care and prescription drug expenditures, and increased mortality.&lt;/p&gt;

&lt;p&gt;Suggested changes:&lt;/p&gt;

&lt;ol&gt;
 &lt;li&gt;a mechanism for users to make informed choices about how their information is used and shared on the site, including in relation to third party apps;&lt;/li&gt;
 &lt;li&gt;increased transparency and controls over how personal data is used for advertising purposes;&lt;/li&gt;
 &lt;li&gt;transparency and control for users via the provision of all personal data held to them on request and as part of their everyday interaction with the site;&lt;/li&gt;
 &lt;li&gt;the deletion of information held on users and non-users via what are known as social plug-ins, and more generally the deletion of data held from user interactions with the site much sooner than at present;&lt;/li&gt;
 &lt;li&gt;an update to its data use policy/privacy policy to take account of recommendations as to where the information provided to users could be further improved;&lt;/li&gt;
 &lt;li&gt;an additional form of notification for users in relation to facial recognition/"tag suggest" that, it is considered, will ensure Facebook Ireland is meeting best practice in this area from an Irish law perspective;&lt;/li&gt;
 &lt;li&gt;an enhanced ability for users to control tagging and posting on other user profiles; and&lt;/li&gt;
 &lt;li&gt;an enhanced ability for users to control their addition to groups by friends.&lt;/li&gt;
