      <title>Addressing Health Disparities Through Organizational Change - Evaluation Report (2012)</title>
      <description>&lt;p&gt;In 2005, The Colorado Trust funded Equality in Health, a five-year $13.1 million initiative to decrease health disparities in Colorado by increasing cultural competency of health care organizations serving racial and ethnic minorities. Community Science, a&amp;nbsp;social research and evaluation firm, conducted an evaluation of the first cohort of grantees. The results of this evaluation, as well as lessons learned, can be found in this report.&lt;/p&gt;
