&lt;p&gt;Under the Affordable Care Act there&amp;rsquo;s a new emphasis on keeping people well rather than treating them when they&amp;rsquo;re sick. There will be new financial incentives for doctors to reach out to patients. For example doctors may be rewarded for making sure patients with chronic diseases are managing their conditions.&lt;/p&gt;

&lt;p&gt;Dr. Dennis Gingrich says the PCMH model puts the patient at the center &amp;ndash; and partners them with their primary care doctor. They&amp;rsquo;re supported by a care team &amp;ndash; which is made up of nurses &amp;ndash; a pharmacist whatever the patient needs.&lt;/p&gt;
